ABSTRACT

Objective:To investigate the clinical efficacy of intraspinal analgesia, pudendal nerve block anesthesia and modified perineal protection in combination in midwifery.Methods:A total of 160 primiparous women who were subjected to full-term vaginal delivery in Suqian Hospital of Nanjing Drum Tower Hospital Group between January 2019 and January 2021 were included in this study. They were randomly assigned to undergo either bilateral pudendal nerve block combined with modified perineal protection (control group, n = 80) or intraspinal analgesia, pudendal nerve block anesthesia and modified perineal protection in combination (observation group, n = 80). Each clinical index was compared between the control and observation groups. Results:Duration of labor in the observation group was significantly shorter than that in the control group [(7.23 ± 2.11) hours vs. (9.35 ± 3.79) hours, t = 4.27, P < 0.05). Cesarean section rate in the observation group was significantly lower than that in the control group [8.7% (7/80) vs. 52.5% (42/80), χ 2 = 17.18, P < 0.05]. Incidence of perineal tears in the observation group was significantly lower than that in the control group [8.7% (7/80) vs. 32.5% (26/80), χ 2 = 15.48, P < 0.05]. Third-degree perineal tears occurred in neither group. Time of postpartum off-bed ambulation and length of postpartum hospital stay in the observation group were (1.37 ± 0.13) days and (3.22 ± 0.31) days, respectively, which were significantly shorter than those in the control group [(2.52 ± 0.22) days, (5.23 ± 0.62) days, t = 25.90, 25.94, both P < 0.05). The incidence of complications in the observation group was significantly lower than that in the control group [7.5% (6/80) vs. 23.7% (19/80), χ 2 = 8.01, P < 0.05]. There were no significant differences in incidence of neonatal asphayxia, 2-hour postpartum hemorrhage and postpartum hemorrhage between the two groups (all P > 0.05). Conclusion:During midwifery of full-term vaginal delivery, combined application of intraspinal analgesia, bilateral pudendal nerve block anesthesia and modified perineal protection can shorten duration of labor and decrease cesarean section rate.

ABSTRACT

Objective To investigate the relationship between perinatal outcome and weight gain of pregnant women with gestational diabetes mellitus (GDM).Methods Ninety-two cases pregnant women with GDM were collected.The rate of weight increasing was recorded throughout the pregnancy and body mass index (BMI) was calculated.Based on the amplitude of packets of body weight increasing,24 cases was selected during pregnancy as insufficient weight gain group,suitable weight gain group of 42 cases,and excessive weight gain group of 26 cases.Their perinatal outcome were compared.Results In excessive weight gain group,the proportion of the gestational hypertension (19.23%) and postpartum hemorrhage (23.07%) were significantly higher than suitable weight gain group and insufficient weight gain group(7.14%,8.33% and 4.76%,4.17%),the differences were statistically significant (x2 =8.132,10.453,P < 0.001).Among the three groups,no significant difference was found in terms of the incidence of anemia (3.85%,4.76%,4.17%),preterm labor (7.69%,7.14%,8.33%),premature rupture of membranes (11.54%,11.90%,12.50%) in pregnancy (P2 =0.572,0.492,0.212,P=0.63 1,0.692,0.783).The huge birth weight children rate in excessive weight gain group(15.38%) was significantly higher than suitable weight gain group and insufficient weight gain group (4.76% and 4.17%).Among three groups,no significant difference was found in terms of the fetal distress (15.38%,16.67%,12.50%) and proportion of low birth weight children (3.85%,4.76%,4.17%) (P2 =1.034,0.572,P =0.310,0.631).Conclusion Excessive weight gain during pregnancy in GDM patients will significantly increase the gestational hypertension,the incidence of huge children and postpartum hemorrhage.Therefore,patients should be closely monitored during pregnancy weight and timely taken intervention.

ABSTRACT

Objective To investigate the effect of the courses of image diagnostics on clinical pro-bation period via the combination of PBL and multimedia network teaching model. Methods 126 under-graduates from the department of medical image (Grade 2011 and 2012) were selected as teaching subjects, and PBL combined with multimedia network teaching was adopted in the teaching of the course of medical imaging diagnosis. After class we implemented a questionnaire survey, and let the students write learning experience. Statistical analysis was conducted by using SSPS 13.0 to reflect the teaching participation in the students' questionnaire and a. summary analysis of the learning experience of the students was also made. Results 123 valid questionnaires were collected. 94.31%(116 people) of students were active participants in these courses, 90.25%(111 people) of the students thought they could actively communicate in teaching, and 43% (110 people) of the students thought they got fully prepared before class. 123 articles in accor-dance with the requirements of experience were received; most students took positive attitude to the active influence the teaching methods had brought; some student pointed out that the teaching method had the disadvantages such as too much time to prepare, too much information, and so on. Conclusions The com-bination of PBL and multimedia network teaching has preliminary results. It is a novel model that is available for teaching image diagnostics on clinical probation period.

ABSTRACT

Objective To investigate the impact of symbiotic probiotic fermentation on anti-tumor effect of Agaricus blazei polysaccharide. Methods Probiotic fermentation and non-fermentation of Agaricus blazei polysaccharide samples were divided into large (0.8 g/kg), middle (0.4 g/kg), small (0.2 g/kg) dose groups, and negative control group for a total of 7 group. The anti-tumor effect on tumor-bearing mice was observed. Results The highest inhibition rate of probiotic fermentation Agaricus blazei polysaccharides group was 53.71%, while that of non-fermatation of Agaricus blazei polysaccharide group was 38.90%. Conclusion Inhibition rate of probiotic fermentation samples of Agaricus blazei polysaccharide was significantly higher than non-fermentation samples. Probiotic fermentation can significantly improve the anti-tumor effect of the Agaricus blazei.
