ABSTRACT

OBJECTIVE@#To analyze the distribution and drug resistance of pathogens in oral mucositis associated with chemotherapy in hospitalized patients with malignant hematopathy, so as to provide scientific evidences for rational selection of antibiotics and infection prevention and control.@*METHODS@#From July 2020 to June 2022, 167 patients with malignant hematopathy were treated with chemical drugs in the Department of Hematology, Hainan Hospital, and secretions from oral mucosal infected wounds were collected. VITEK2 COMPECT automatic microbial identification system (BioMerieux, France) and bacterial susceptibility card (BioMerieux) were used for bacterial identification and drug susceptibility tests.@*RESULTS@#A total of 352 strains of pathogens were isolated from 167 patients, among which 220 strains of Gram-positive bacteria, 118 strains of Gram-negative bacteria and 14 strains of fungi, accounted for 62.50%, 33.52% and 3.98%, respectively. The Gram-positive bacteria was mainly Staphylococcus and Streptococcus, while Gram-negative bacteria was mainly Klebsiella and Proteus. The resistance of main Gram-positive bacteria to vancomycin, ciprofloxacin and gentamicin was low, and the resistance to penicillin, cefuroxime, ampicillin, cefotaxime, erythromycin and levofloxacin was high. The main Gram-negative bacteria had low resistance to gentamicin, imipenem and penicillin, but high resistance to levofloxacin, cefotaxime, cefuroxime, ampicillin and vancomycin. The clinical data of oral mucositis patients with oral ulcer (severe) and without oral ulcer (mild) were compared, and it was found that there were statistically significant differences in poor oral hygiene, diabetes, sleep duration less than 8 hours per night between two groups (P<0.05).@*CONCLUSION@#Gram-positive bacteria is the main pathogen of oral mucositis in patients with malignant hematopathy after chemotherapy. It is sensitive to glycopeptide antibiotics and aminoglycosides antibiotics. Poor oral hygiene, diabetes and sleep duration less than 8 hours per night are risk factors for oral mucositis with oral ulcer (severe).

ABSTRACT

Objective To investigate and analyze consistency in attitudes toward advance directive and life sustaining treatments between cancer patients and their family members. Methods Totally 242 pairs of cancer patients and family members were involved in the research through convenience sampling from two tertiary hospitals. We investigated consistency in attitudes toward advance directive and life sustaining treatments between cancer patients and their family members using questionnaire of attitudes toward advance directive. Results After explanation of advance directive,51.65% of cancer patients would like to make advance directive. Most of the family members(89.26%) said they would like to follow patients' wishes if they made advance directive. The consistency in attitudes toward advance directive between cancer patients and their family members were "willing" accounting 48.35%,and "not willing" accounting for 1.65%,with weak consistency(Kappa value was 0.05,P>0.05).The consistency in attitudes toward life sustaining treatments between cancer patients and their family members were "willing" accounting for 16.94%~38.84%,"not willing" accounting for 1.65%~4.96%,and "never considered" accounting for 9.92%~17.77%,with weak consistency(Kappa value was 0.09-0.18,P<0.05).Conclusion Cancer patients and their family members show positive attitudes toward advance directive without too many differences.Family members' choices on life sustaining treatments cannot fully represent patients' wishes.

ABSTRACT

Objective To observe the clinical effects of painting therapy on nausea, vomiting and anxiety induced by chemotherapy in breast cancer patients. Methods This study used convenience sampling and before-and-after self-controlled design, and enrolled 200 patients with breast cancer undergoing EC (Epirubicin+Cyclophosphamide)chemotherapy regimen. All patients were given painting therapy on the first day of the second chemotherapy.Functional living index-emesis(FLIE) and self-rating anxiety scale(SAS) were used to compare the effects before and after intervention, and measured after the first chemotherapy and after implementation of painting therapy. Results The scores of FLIE were significantly different before and after implementation of painting therapy(P<0.05);scores in SAS for patients' subjective feelings(nervous, panic, headache, upset),and objective symptoms(hands trembling, numbness, indigestion)were significantly lower after intervention than those before intervention, and the differences were statistically significant(P<0.05).Conclusion Painting therapy can alleviate chemotherapy induced nausea and vomiting in breast cancer patients effectively.

ABSTRACT

We aimed to make comprehensive cancer care guidelines of China according to American guidelines of National Comprehensive Cancer Network,Ametican Society of Clinical Oncology and Oncology Nursing Society, and to guide clinical nurses to study and draw lessons from foreign latest guidelines, and combined with the actual situation in our country. The results of study and application were advantageous to the nursing profession, the security and interests of the patients. We should develop comprehensive cancer care guidelines of our country, eventually make nursing practice guide system of China constantly improve and perfect.

ABSTRACT

Objective To explore the effect of family constellation therapy on the spouses of female tumor patients . Methods Two hundred and two spouses of 202 female tumor patients were divided to the control group ( n = 43 ) and experiment group according to their wills(n=159):the former were intervened with health education and skills training and the latter trained in a workshop with family constellation by psychology training specialists in addition to health intervention and skill training intervention. After the intervention, the two groups were compared in terms of quality of life, family function and marriage status. Result The total scores of the experiment group on quality of life , social relationship and environment were all higher than those of the control group (all P<0.05) and the experiment group got higher score in trusting their spouses than the control group (P<0.05). Conclusion The family constellation can improve female tumor patients'quality of life and promote the trust in their spouses.
