ABSTRACT
El Clostridium perfringens es un bacilo grampositivo anaerobio con capacidad de formar esporas. Es uno de los patógenos bacterianos con mayor distribución en el medio ambiente. Se presenta el caso de una paciente que sufrió un accidente con un instrumento agrícola, con fractura expuesta de 1/3 proximal de la tibia con pérdida de la continuidad ósea en el miembro inferior derecho. Fue intervenida quirúrgicamente y se aplicó antibióticoterapia. Dos días después del alta hospitalaria acudió nuevamente refiriendo dolor intenso y secreción fétida en sitio de herida quirúrgica. Se ingresó con impresión diagnóstica de sepsis de herida quirúrgica por germen productor de gas. Se le detectó gangrena gaseosa por lo que le fue amputado el miembro. De la herida fue aislado el Clostridium perfringens. Por la importancia de tener en cuenta esta posibilidad de infección ante heridas con instrumentos potencialmente contaminados con gérmenes del medio ambiente, se decidió la presentación de este caso.
Clostridium perfringens is an anaerobic Gram-positive bacillus with spore-forming ability. It is one of the most widely distributed bacterial pathogens in the environment. A case report of a female patient who had an accident with an agricultural implement, suffering proximal third tibial fracture with loss of the continuity of the bone in the right leg is presented. She underwent surgery and antibiotic therapy. Two days after being discharged, the patient came back complaining of acute pain and foul-smelling discharge in the surgical wound site. She was hospitalized with diagnostic impression of surgical wound sepsis caused by gas-producing germ. Gas gangrene was diagnosed, resulting in member amputation. Clostridium perfringens was isolated from the wound. Because of the importance of taking into account this possibility of infection in wounds caused by instruments potentially contaminated with environmental germs, it was decided to report this case.
