ABSTRACT

Bile duct obstruction due to gall stone is the most common cause of biliary infection. Bacteria stabilize during the inflammation of gallbladder and it is confirmed that longer duration of inflammation is associated with more risk of post operative infection. In this study we sought to determine the biliary microbial flora in patients with acute and chronic cholecystitis in order to find the suitable antibiotic to treat preoperative infection by using antibiotic susceptibility test [antibiogram]. This was a descriptive cross sectional study. From all patients with acute and chronic cholecystitis due to stones that underwent cholecystectomy a sterile sample was taken during surgery and sent for laboratory analysis. It was examined for gram negative and gram positive, aerobic and anaerobic bacteria. Bile culture was performed within an hour after taking samples. The data were registered and analyzed by statistical SPSS software version 11.5. The majority of patients [73.1%] suffered from chronic cholecystitis and the stones obtained were 48.7% of cholestrolic type and 29.5% of mixed type. No significant relationship existed between stone type and bile culture results. Gram positive cocci [e.g. streptococcus] and gram negative bacili [e.g. citrobacterfrondei] were found in 11.5% and 17.9% of the patients, respectively. Most sensitivity to antibiotics was towards fluoroquinolones. Study results showed that bile fluid was not infected in most of the cases and in positive cases sensitivity to fluoroquinolones such as ciprofloxacin, ofloxacin and norfloxacin was detected. The relationship between stone type and culture results was not significant

ABSTRACT

Pre-eclampsia affects approximately 5-8% of pregnant women. The aim of this study was to compare the serum level of Lactate dehydrogenase [LDH], Homocystein, Hemoglubin and platelet in pregnant women diagnosed as pre-eclampsia and a normal group in Gorgan city, Northeastern Iran from 2007-2008. In this case control study, 50 cases of pre-eclampsia were compared with the control group women hospitalized in Dezyani hospital. Pre-eclampsia criteria were: Blood pressure more than or equal to 140/ 90 mm hg and Proteinuria greater or equal to 300 mg/ 24 hours urine sample in the third trimester. Hemoglobin, platelet, LDH and hemocystein were measured. Data were analyzed by the mean of SPSS-14 program and Chi-2 or t-student were used. The difference of BMI and family incomes was significant between two groups [P-value<0.01]. LDH level was not statistically different between healthy and pre-eclamptic individuals. Six cases [12%] in controls and 9 cases [18%] in pre-eclamptic group had thrombocytopenia [P-value>0.01]. Hemocystein level was more than normal range in five patients with pre-eclampsia [P-value<0.001]. In this study, hemocystein level was significantly higher in pre-eclampsia patients but LDH, hemoglobin and platelet level had no significant difference

ABSTRACT

Background and Purpose: P53 gene is a tumor suppressive gene and its product, P53 protein, is a protective factor against tumor formation that inhibits the extension of genetically damaged cells. P53 aggregation in tumoral cell nucleus is related with p53 gene mutations, which can be detected by immunohistochemical methods. This study was designed to investigate the protein aggregation in patients with gastric cancer in Gorgan, Iran


Methods and materials: All paraffin-embedded blocks of gastric cancer cases during the years 2000 to 2004 in the pathology ward of the 5th Azar Hospital in Gorgan, Iran [111 cases altogether] were evaluated and stained, for detecting microtome cutting. The obtained data were entered into SPSS and chisquare was used for analysis


Results: 111 cases with gastric cancer were investigated. The most frequent type of gastric tumor was adenocarcinoma [66.7%]. In 59.5% of the affected cases, P53 protein was detected. No significant relationship was observed between P53 expression and factors such as age, gender, and histological type of the tumor


Conclusions: In this study, P53 protein rate was high but it showed no significant relationship with other variables. We suggest further studies to discover the relationship between protein aggregation in tumor cells and other variables in affected cases

ABSTRACT

Tuberculosis is an important problem in the world and Iran. Tuberculosis is an ancient disease but because of different presentations is a mysterious disease too. A 20-year-old girl who complained of abdominal pain with peptic ulcer and weight loss was admitted in our hospital. Since 4 months ago before referring, she had been treated many times for peptic ulcer including treatment for helicobacter pylori. Then endoscopy and biopsy were done. In endoscopy one ulcer in antrum with malignant appearance was observed. At biopsy granulomatous inflammation compatible with tuberculosis reported. She was treated by anti-tuberculosis drug for six months. Endoscopy repeated after 2 months of therapy showed completely normal pattern and she had 5 kg weight gain and totally symptom free after completion of therapy. Gastric TB should be considered for differential diagnosis of patient with gastric symptoms in endemic regions

ABSTRACT

A 58-year-old female patient who had recurrent right temporal region cystic mass, eight months after the first unsuccessful operation is our Study case. CT scan of cranium and Brain was performed and a multilocular cystic mass with no Intra-orbital or sinus involvement was reported. The patient underwent exploration and hydatid cyst of soft tissue of the temporal region was identified. Cysto-pericystectomy and open drainage was done. Histopathologic report confirmed the diagnosis of hydatid cyst. The patient was discharged at the third post-operative day with no complications. The aim of this case study is the rarity of soft tissue hydatid cyst

ABSTRACT

A 70 year old male presented with right lower quadrant pain and anorexia for about 72 hours prior to hospital admission. He underwent laparotomy with impression of acute appendicitis. The operative finding was an inflamed tumoral appendix without appendicular base involvement, and appendectomy was done. The patient had an uneventful post -op course. Due to the pathology report of prostatic carcinoma he underwent cystoscopy and biopsy of prostate on the 4th post-op day. Histopathology of the appendix was consistent with metastatic adenocarcinoma of the appendix with prostatic origin. Histopathology of the prostatic specimen was consistent with high grade prostatic adenocarcinoma
