ABSTRACT

OBJECTIVE@#To conclude of the technical notes of percutaneous transforaminal endoscope-assisted lumbar interbody fusion (PT-Endo-LIF), and to investigate its safety and efficacy for treatment of degenerative lumbar disease.@*METHODS@#Twenty-four patients were treated by PT-Endo-LIF combined with posterior percutaneous pedicle screws fixation from October 2017 to April 2018. There were 16 males and 8 females, ranging in age from 39 to 72 years old, with a mean of (59.6±9.5) years old. There were 15 cases diagnosed with lumbar intervertebral disc herniation combined with degenerative disc, the other 9 cases were diagnosed as low level lumbar spondylolistheses w/o segmental instability. Single segmental fusion was performed for 22 cases(one for L₂,₃, 3 for L₃,₄ and 18 for L₄,₅) and 2 segmental fusion was performed for the other 2 cases (both for L₃,₄ and L₄,₅). PT-Endo-LIF was performed under local anesthesia with conscious sedation, followed by decompression through endoscopic technics. After that, end-plate preparation and autogenous bone and expandable cage implantation were performed. Finally, percutaneous screws and rod instrumentation were used. The visual analogue scale (VAS) and Oswestry Disability Index (ODI) were used to evaluate the clinical efficacy. The operation time, intraoperative bleeding volume, intraoperative and postoperative complications were recorded. All patients underwent X-ray, CT plain scan, three-dimensional reconstruction and MRI examination to evaluate the stability of the implants and fusion rate before 3 days and 1, 3, 6, 12 and 18 months after operation.@*RESULTS@#All patients were followed up, and the duration ranged from 12 to 18 months. The operation time of single-segment fusion was (192.3±22.7) min, and that of double-segment fusion was (272.5±24.7) min. The estimated intraoperative bleeding volume was less than 50 ml per segment, and no blood transfusion was performed in all patients. The VAS improved from preoperative 7.4±1.1 to postoperative 2.3±0.8 (=-19.65, <0.000 5). The ODI improved from preoperative (41.2±3.3)% to the final follow-up (12.3±2.5)%(=-35.76, <0.000 5). Postoperative complications occurred in 4 cases, and contralateral radicular symptoms occurred in 2 cases. After contralateral foraminoscopic decompression, the symptoms were completely alleviated. One case had neurological symptoms related to percutaneous screw placement, and the symptoms were alleviated after removal of the lateral screw rod internal fixation. The other cases had surgical incision infection and improved after debridement and suture. At the latest follow-up, no displacement or loosening of the fusion cage and screw rod system occurred in all patients, and 14 cases showed signs of fusion.@*CONCLUSIONS@#PT-Endo-LIF is a minimal invasive, safe and efficient surgical procedure for treatment of degenerative lumbar disease. Nevertheless, the long-term results still need to be confirmed by a multi-center and lagre sample follow-up study.

ABSTRACT

Objective: To investigate the inhibitory effect and mechanism of the extracts from fresh Lycii Fructus (LBL) on hepatoma HepG2 cell-induced cachexia in mouse.Method: The human hepatoma cell line HepG2 was injected into BALB/C mice to establish the cachexia model.Then the LBL was fed to the models respectively in low dose (5 mg·kg-1) or high dose (25 mg·kg-1).After 28 days of continuous feeding,the mice's body weight was detected.The expression levels of creatine kinase (CK),interleukin-1β (IL-1β),interleukin-6(IL-6) and tumor necrosis factor-α(TNF-α) were evaluated by enzyme linked immunosorbent assay (ELISA).The muscle degradation and the expression of p38 mitogen-activated protein kinase (p38 MAPK) were detected by immunohistochemistry staining.The expression levels of phospho-p38 mitogen-activated protein kinase (p-p38 MAPK) and nuclear factor-κB (NF-κB) were determined by Western blot.Result: Compared with cachexia group,the loss of body weight and muscle decomposition were significantly inhibited both in the low dose LBL group and high dose LBL group (P0.05,PPβ,IL-6 and TNF-α in both the low-dose LBL group and the high-dose LBL group (PPPκB were inhibited both in the low-dose LBL group and the high-dose LBL group (PConclusion: LBL could inhibit the cachexia induced by hepatoma HepG2 cell line in mouse,suggesting LBL could reduce major cytokine and plasma inflammatory factors through p-p38 MAPK pathway.

ABSTRACT

A quick HPLC-ESI-MS/MS method was established for simultaneous determination of four major diterpenoids in Rabdosia japonica var.glaucocalyx, including glaucocalyxin A, oridonin, hebeirubesensin and enmenol. Analysis was performed on an Agilent ZORBAX SB-C18(4.6 mm×250 mm, 5 μm ) column eluted in a gradient program with methanol and water. The flow rate was 0.8 mL•min⁻¹. Multiple reaction monitoring (MRM) scanning mode was performed in negative ion switching mode to apply for the quantitative determination. The calibration curves for the above four compounds were linear in corresponding injection amount. The average recoveries of the compounds ranged from 92.40% to 105.9%, with RSDs of 1.7%-6.5%. The method is simple, rapid, accurate with good repeatability, which can provide a reference for overcalling evaluation the quality of R. japonica var.glaucocalyx. The result of cluster analysis- showed that the quality of R. japonica glaucocalyx var. greatly varied between areas and parts.

ABSTRACT

BACKGROUND/AIMS: Peptic ulcer bleeding (PUB) is the most common cause of upper gastrointestinal bleeding in Korea but there has been no research done using big data. This study evaluates the optimal operational definition (OD) for big data research by analyzing clinical characteristics of PUB. METHODS: We reviewed the clinical characteristics of 92 patients with PUB confirmed on endoscopy in Wonkwang University Sanbon Hospital (January 2013 to December 2014). We calculated sensitivity and positive predictive value (PPV) to detect confirmed PUB patients using ODs developed by combining clinical features of patients with PUB. RESULTS: The mean patient age was 63 years. Men had higher prevalence of PUB than women. Bleeding gastric ulcer was proportionately common in the age range of 40s to 60s in men, while a significantly higher rate of bleeding occurred in women older than 70s. The rate of drug-induced ulcer was 28.2%, whereas the prevalence of Helicobacter pylori was 47.8%. Among the hospitalized patients with diagnostic code of PUB, we ruled out patients with endoscopic removal of gastric adenoma or peritonitis, and selected patients who had been administered intravenous proton pump inhibitor. The sensitivity in this setting was 82.6%, and PPV was 88.4%. CONCLUSIONS: PUB was more common in older patients, and there was a clear gender difference in gastric ulcer bleeding by age. With a proper OD using PUB diagnostic codes, we can identify true patients with sufficiently high sensitivity and PPV.

ABSTRACT

Objective: To establish HPLC-PDA fingerprint for study on Rabdosiae Rubescentis Herba (RRH) from different areas and to scientifically evaluate and effectively control the quality of RRH from different areas by HPLC fingerprint combined with principal component analysis (PCA). Methods: The fingerprint of RRH was established by HPLC-PDA and PCA and semblance analysis methods were applied to treating the experimental data in order to fine the similarity and difference among 18 batches of RRH from four different areas. The chromatographic procedure was carried out with DiamonsilTM C18 (250 mm × 4.6 mm, 5 μm) as an analytic column and a mixture consisting of acetonitrile-0.05% phosphoric acid in gradient as mobile phase, and the temperature of column was 30 ℃. The detection wavelength was set at 238 nm and the flow rate was 0.8 mL/min. Results: The common mode of HPLC characteristic chromatographic profile was set up with 19 common peaks, and four of them were identified as oridonin, ponicidin, lasiodonin, and rosmarinic acid. The similarities of 18 batches of RRH were between 0.421- 0.984. The result of PCA showed that the accumulative contribution rate of the first three factors was up to 87.3%, which were chosen to comprehensively evaluate the quality of RRH. Conclusion: The application of HPLC combined with PCA could quickly and objectively evaluate the quality difference of RRH in different batches.

ABSTRACT

A quick HPLC-ESI-MS/MS method was established for simultaneous determination of three chemical compositions in Usnea, including usnic acid, diffractaic acid, and ramalic acid. The separation was performed on a chromatographic column of Agilent ZORBAX SB-C, (4.6 mm x 250 mm, 5 µm), and the mobile phase was methanol (0.05% formic acid)-0.05% formic acid solution (4 mmol ammonium acetate), with an isocratic elution at a flow rate of 0.8 ml · min⁻¹. Multiple reaction monitoring scanning mode (MRM) was performed combined with the ion switching technology in positive and negative ion switching mode to apply for the quantitative determination. The calibration curves for the above three compounds were linear in corresponding injection amount. Their average recoveries were 95.0%-105.1%, with RSDs of 1.1%-5.2%. The method was simple, rapid, accurate with high repeatability, which could provide a reference for overcalling evaluation the quality of Usnea.

ABSTRACT

Objective To observe the effects of Marigold lutein on the cardiovascular system, the nervous system and the respiratory system of experimental animals. Methods The influence on blood pressure, heart rate, electrocardiogram, breathing flow and frequency in anesthetic dogs were observed after duodenum euonymus, and the impact on spontaneous activity, coordinated movement, and the sleep situation of threshold dose and sunthreshold dose pentobarbital sodium in mice were observed by intragastric administration of Marigold lutein. Results Marigold lutein had no side effects on breathing flow and frequency, blood pressure, heart rate, electrocardiogram in anesthetic dog, and had no obvious influence on spontaneous activity and coordinated movement in mice. Marigold lutein did not evidently change the number and the time of sleeping in mice. Conclusion Marigold lutein has no evident effects on respiratory system, cardiovascular system and central nervous system of experimental animals.

ABSTRACT

Objective To observe the antioxidative dosage-effect relationship of Marigold lutein, and provide experimental data for clinical use. Methods The mice were randomly divided into seven groups:blank control group, model control group, 1 mg/kg lutein group, 5 mg/kg lutein group, 25 mg/kg lutein group, 125 mg/kg lutein group and 625 mg/kg lutein group. The mice in blank control group were dealt with saline solution by intraperitoneal injection, the others were dealt with D-galactose (120 mg/kg) by intraperitoneal injection for seven weeks to make oxidative damage model, meanwhile the mice were given corresponding dose of the drug. Subsequently, the level of malondialdehyde (MDA) and activity of superoxide dismutase (SOD) in the serum were measured, and the antioxidative dosage-effect relationship was observed. Results The 1, 5, 25 mg/kg lutein reduced the MDA level and increased SOD activity, and the 125, 625 mg/kg dose of lutein did not show significant antioxidant activity. Conclusion Lutein has significant antioxidant activity in mouse dealt with D-galactose within the dose range of 1-25 mg/kg. The results suggest that the clinical dosage range of lutein should be kept within reasonable limits.

ABSTRACT

Objective To observe the antioxidative dosage-effect relationship of grape seed proanthocyanidin extract (GSPE). Methods The mice were randomly divided into seven groups:blank group, model group, 5 mg/kg GSPE group, 15 mg/kg GSPE group, 45 mg/kg GSPE group, 135 mg/kg GSPE group and 405 mg/kg GSPE group. The mice in blank group were dealt with saline solution by intraperitoneal injection, the others were dealt with D-galactose (120 mg/kg) by intraperitoneal injection for seven weeks to make oxidative damage model. Meanwhile, the mice were given corresponding dose of the drug. Subsequently the level of malondialdehyde (MDA) and activity of superoxide dismutase (SOD) in the serum were measured to observe the antioxidative dosage-effect relationship of GSPE. Results The 45, 135, 405 mg/kg GSPE group reduced the MDA level, and the 15, 45, 135, 405 mg/kg GSPE group increased the SOD activity. Conclusion GSPE has significant antioxidant activity on mice dealt with D-galactose above the dose of 15 mg/kg, suggesting that the clinical use of GSPE should guarantee a certain dose to play a good antioxidant effect.

ABSTRACT

Osteoporosis is a rising problem, as the older age population is increasing due to prolongation of life. Genetic and environmental factors play key roles in bone metabolism, and diet is also an important factor. We investigated the relationship among factors affecting bone mineral density (BMD), including daily food intake and nutrient intake in men aged >50 years. Seventy-eight men, who visited the health promotion center at one of the university medical centers, were divided into normal and osteopenia groups according to their BMD. The body weight of the normal group was significantly higher than that of the osteopenia group. The osteopenia group showed significantly higher carbohydrate intake and lower calcium and vegetable calcium intake compared to those in the normal group. Lumbar spine BMD was negatively correlated with energy, fat, vitamin B1, and sodium intake in the normal group. Additionally, femoral neck BMD was negatively correlated with total animal protein, energy, protein, fat, phosphorous, iron, animal iron, potassium, vitamin B1, B2, B6, and niacin intake. Lumbar spine BMD was positively correlated with fruit, calcium, vegetable calcium, animal calcium, and vitamin C intake in the osteopenia group. Femoral neck BMD was negatively correlated with meat, dairy product, total animal protein, plant protein, animal protein, vitamin A, and cholesterol intake. A stepwise multiple regression analysis revealed that several dietary factors affected BMD, including energy, fat, vitamin B1, B2, B6, niacin, sodium, protein, iron, animal iron, phosphorous, potassium, and animal protein in the normal group and zinc, calcium, vegetable calcium, animal calcium, vitamin C, fruit, protein, animal protein, meat, dairy product, carbohydrates, cholesterol, vegetables, mushrooms, and seasonings in the osteopenia group. These results indicate that adequate nutrient intake plays an important role maintaining optimum bone health in middle aged men.

ABSTRACT

Mugwort leaf is the traditional herb being used for moxibustion. Although its place of production and the course of making Mugwort floss have been described in detail, there is no unified standard about the quality and processing of Mugwort floss until now, which seriously affect on the popularity and application of moxibustion. In accordance with the real herb of Mugwort leaf and floss as well as its processing and quality judgment, this paper thoroughly analyze the related data including ancient documents, modern research results and clinical applications. The authors consider that the Mugwort floss in the market is lack of regulations for its processing, and it needs to be discussed the way to judge the quality of Mugwort leaf by volatile oil, suggesting that the further study on the quality control of Mugwort leaf and floss has to be carried out from multiple aspects, such as physical attributes and chemical characteristics.

ABSTRACT

The article aims at providing theoretical foundation for security of moxibustion through analyzing chemical compositions of Artemisia Argyi of different years from Qichun County, Hubei Province, and moxa wool refined in different proportions. Artemisia Argyi from Qichun on 2007, 2008 and 2009 were taken as raw materials, and processed into moxa wool with the proportions of raw material and product as 3 : 1, 5 : 1, 8 : 1 and 15 : 1, respectively. Essential oils of Artemisia Argyi and the refined moxa wool were extracted by steam distillation. Their chemical compositions were identified by gas chromatography-mass spectrometry (GC-MS) and calculated with semiquantitative method. The result showed that chemical compositions of Artemisia Argyi of different years and moxa wool refined in different proportions were almost the same, but their contents were with obvious difference. The relative content of volatile substances decreased with the age prolonged and a rise in the proportion of the refined moxa wool, while the involatile material increased. Therefore it can be concluded that the essential oil of Artemisia Argyi from Qichun and the refined moxa wool is basically safe. Involatile substances such as Juniper camphor, Caryophyllene oxide and Caryophyllene etc. are the main contents of high proportional moxa wool of old year. And these substances may be the effective components in moxibustion treatment.

ABSTRACT

Objective: To compare the volatile constituents of folium artemisiae argyi in order to provide chemical evidence for quality standardization. Methods: Volatile oil was extracted from folium artemisiae argyi from five sources by vapour distillation and then detected by gas chromatography-mass spectrometer (GC-MS) to identify and semi-quantify its chemical constituents. Results: The folium artemisiae argyi in Qichun (Hubei Province) and Anguo (Hebei Province) contained many volatile constituents, with uniform and stable contents of these volatile constituents. Conclusion: The types and contents of volatile oil of folium artemisiae argyi are varied in different sources.

ABSTRACT

Type, mode of affiliating, mechanism of action of entrainer in Supercritical CO2 on extraction of Chinese traditional and herbal drugs were briefly reviewed. Application of entrainer in Supercritical CO2 on extraction of flavones, terpenes, sterols, and saponins in Chinese traditional and herbal drugs were recommended in particular. Some problems and directions in research of entrainer in Supercritical CO2 on extraction of Chinese traditional and herbal drugs were shown in this paper.

ABSTRACT

Lifestyle behaviors including dietary habits are well known to play key roles in bone metabolism. The purpose of this study was to investigate the relationship among the factors affecting bone mineral density (BMD) including age, anthropometric parameters, lifestyle behaviors, and dietary habits of men aged more than 50 years. Ninety-one men, who visited health promotion center at one of the university medical centers, were divided into two groups according to the BMD: normal and osteopenia. The BMD of femoral neck in the osteopenia group was significantly lower than that of the normal group (0.77 +/- 0.28 vs. 0.98 +/- 0.08 g/cm2). The proportion of the regular exercisers was significantly lower in the osteopenia group than in the normal group (p = 0.027). In the osteopenia group, the femoral neck BMD was significantly decreased in smokers and coffee drinkers compared to no-smokers and no-coffee drinkers. The femoral neck BMD was increased among those who consume breakfast and beans and bean products more frequently and those with a greater meal regularity. In the normal group, the lumbar spine BMD was significantly increased among those with frequent consumption of beans and bean products. The lumbar spine BMD was significantly correlated with exercise (r = 0.263), and the femoral neck BMD with weight (r = 0.284), BMI (r = 0.324), relative body weight (r= 0.294), exercise (r = 0.269) and frequency of beans and bean products consumption (r = 0.216). These results indicate that lifestyle behaviors and dietary habits play important roles in maintaining optimum bone health in the middle-aged men.

ABSTRACT

<p><b>OBJECTIVE</b>To discuss the protective effect of Niuhuang Shangqing pills (NSP) on the experimental cerebral ischemia and the influence of blood rheology in animals.</p><p><b>METHOD</b>Using the middle cerebral artery occlusion (MCAO), bilateral common carotid artery ligation and lipopolysaccharide (LPS) in combination with carrogeenin (Ca)-induced stagnation of blood model rats, we investigated the influence of NSP on the physical sign indexes, brain infarct size and the water content, the content of lactate(LD), glutathione peroxidase (GPx), catalase (CAT), malondialdehyde (MDA) and the blood rheology.</p><p><b>RESULT</b>The 3.00, 1.50 g x kg(-1) dosage groups of NSP decreased the neurosigns indexes, significantly reduced the brain infarct areas in MCAO rats, decreased the brain water content, increased the activities of GPx and CAT, decreased the content of LD and MDA in bilateral common carotid artery ligation rats. They improved the blood rheology in lipopolysaccharide (LPS) in combination with carrogeenin (Ca)-induced stagnation of blood model rats.</p><p><b>CONCLUSION</b>NSP has the function of protective effect on experimental experimental cerebral ischemia.</p>
