ABSTRACT

Instrumentos de avaliação clínica foram criados a partir de experiências prévias de cuidados para outras condições crônicas complexas da infância, de modo a planejar e sistematizar ações. Contudo, é importante que tais instrumentos tenham validação externa e contemplem o atual conceito de saúde biopsicossocial. Uma interlocução com a Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF) poderia dar subsídio teórico e validação aos instrumentos criados. Objetivo: Identificar o conteúdo comum da CIF com o instrumento de avaliação clínica aplicado em população exposta ao vírus Zika em um ambulatório de doenças infecciosas em pediatria de um hospital de referência no Estado do Rio de Janeiro. Métodos: A Ligação com a CIF foi realizada por dois revisores independentes, segundo a proposta de Cieza e colaboradores. O coeficiente Kappa foi utilizado para análise da concordância interobservadores. Resultados: O instrumento de avaliação clínica utilizado no ambulatório de referência é composto principalmente por categorias da CIF de estruturas do corpo (46,4%). Observou-se poucas categorias relacionadas aos fatores contextuais (13,1%). E, não foram encontrados itens relacionados às categorias de atividade e participação. Conclusão: A ferramenta de avaliação apresenta principalmente informações sobre as funções e estruturas do corpo, voltando-se a um olhar puramente restrito as funções fisiológicas e as estruturas anatômicas. A inexistência de categorias de atividade e participação pode comprometer a percepção das experiências vividas pelas crianças que foram expostas ao vírus Zika


The Zika epidemic and the emergence of a new health condition in Brazil, imposed a rapid organization on services to meet the current demand for care. Clinical assessment instruments were created, based on previous care experiences for other complex chronic conditions in order to plan and systematize actions. However, it is important that these instruments have external validation and include the current concept of health, biopsychosocial. An interlocution with the International Classification of Functioning, Disability and Health (CIF) could provide theoretical support and validation to the instruments created. Objective: To identify the common content among the clinical evaluation instrument applied to the population exposed to the Zika virus in a pediatric infectious disease outpatient clinic in a reference hospital in the State of Rio de Janeiro with the ICF. Methods: The link with the ICF was carried out by two independent reviewers, according to Cieza's proposal and the Kappa coefficient was used for interobserver analysis. Results: It was identified that the instrument is mainly composed of items of body structure (39; 46.4%). There were few categories related to contextual factors (11; 13.1%). And there were no items related to the categories of activity and participation. Conclusion: The assessment tool presents mainly domains of function and structure of the body, turning to a biomedical look. The lack of categories of activity and participation can compromise the perception of health status. A line of care in the light of the biopsychosocial model provides advantages for health services planning and actions

Abstract Introduction: Rare health conditions as mucopolysaccharidoses (MPS) can directly influence functioning experiences. Mobility restriction, osteoarticular alterations, leads to delayed neuropsychomotor development are some of the negative impacts of MPS. Aims: The purpose of this study is to evaluate the functioning of children with MPS, from the International Classification of Functioning, Disability, and Health (ICF) perspective. Methodology: It is a case series study with a sample of 15 children and adolescents with MPS with a median age of 12 years, followed in a tertiary hospital in Rio de Janeiro, Brazil. Results: The patients were assessed by the model ICF and results were as following: regarding body functions, most categories presented slight impairment. For mobility of joints and gait, the impairment was severe. Activity and participation with most significant limitations were "learning to read/write", "read/write", "listening" and "performing multiple tasks." In self-care, the main limitations were in "drinking", "taking care of body parts" and "taking care of one's health." Also, there were restrictions on "doing household tasks", "basic economic transactions", "community living" and "religion and spirituality". Conclusion: MPS can have a significant impact in different body systems which act as limiting activities that require body mobility.

ABSTRACT

ABSTRACT Purpose: The aim of this study was to identify the most typical and relevant categories of the International Classification of Functioning, Disability and Health (ICF) for patients with 22q11.2 Deletion Syndrome. Methods: Based on the Delphi technique an expert survey through e-mail was performed among health professionals' specialists in the 22q11.2DS. Data were collected in 2 rounds. Answers were analysed for the degree of consensus. Results: 7 Experts recruited through e-mail distribution lists of professional organizations and personal networks participated in the study. Categories in all ICF components that were considered typical and/or relevant by at least 80% of the responders were added to a pilot ICF instrument for children with 22q11.2DS, with a total of 145 ICF categories. Conclusion: a list of ICF categories that are considered relevant and typical for 22q11.2DS condition by international experts was created. This is an important step towards identifying ICF Core Sets for chronic paediatric conditions in Brazil.

ABSTRACT

A Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF) apresenta uma abordagem inovadora ao funcionamento: o modelo biopsicossocial. Desde então, a estrutura da CIF e seu esquema explicativo têm sido utilizados em muitos contextos, incluindo saúde, previdência social e educação. Recentemente, surgiram algumas críticas e surgiram propostas a respeito de suas revisões. Objetivos: Este artigo discute propostas anteriores de revisões do esquema da CIF e apresenta sugestões de uma nova. Métodos: Descrevemos algumas das sugestões de esquemas de funcionamento alternativo da CIF e apresentamos algumas de suas características. Resultados: Também propomos nosso próprio esquema, destacando suas vantagens sobre os antecessores. Em nossa proposta, os componentes atuais do esquema são mantidos e dispostos equidistantemente ao longo de uma estrutura tridimensional de "casco de bola" conectada por setas de duas pontas. Nosso esquema é inovador, pois não apresenta nenhum componente como central, permitindo que o esquema seja mais adaptável à realidade de cada perfil funcional. Também é dinâmico, girando em 3 eixos, possibilitando o posicionamento central do componente mais importante. As esferas para cada componente podem ser ampliadas, demonstrando a magnitude de cada componente. Conclusão: Esperamos contribuir para a discussão atual sobre o esquema da CIF e sua revisão.


The International Classification of Functioning, Disability and Health (ICF) presents an innovative approach to functioning: the biopsychosocial model. Since then, the ICF framework and its explanatory scheme has been used in many settings, including health, social security, and education. Recently, some criticism has arisen and proposals have ensued regarding its revisions. Objective: This paper discusses previous proposals for revisions to the ICF scheme and present suggestions of a new one. Methods: We outline some of the suggestions of ICF alternative functioning schemes, and present some of their features. Results: We also propose our own scheme, highlighting its advantages over predecessors. In our proposal, current scheme components are kept and arranged equidistantly along a three dimension 'ball shell' structure connected by double-headed arrows. Our scheme is innovative in that it does not present any component as central, allowing the scheme to be more adaptable to the reality of each functioning profile. It is also dynamic, by rotating on 3 axes, making possible the central positioning of the most important component. The spheres for each component can be enlarged, demonstrating ​​the magnitude of each component. Conclusion: We hope to contribute to the current discussion on ICF scheme and its revision.

ABSTRACT

O câncer do colo do útero (CCU) é uma doença com elevada incidência mundial. Do total de casos, cerca de 80 por cento se deu em países em desenvolvimento. O tratamento oncológico relacionado ao câncer do colo do útero envolve efeitos deletérios diversos que podem afetar esferas biológicas, psicológicas e sociais de forma aguda ou tardia e dessa forma impactar de forma negativa na qualidade de vida das mulheres acometidas. Desde o ano de 2001, a Organização Mundial de Saúde (OMS) instituiu um programa robusto para classificar a funcionalidade e o estado de saúde com a introdução da Classificação Internacional de Funcionalidade, Incapacidade e Saúde (CIF). Tendo em vista que a CIF é uma classificação com um amplo número de categorias, foram propostas listas específicas resumidas para determinadas condições de saúde visando a disseminação da ferramenta. No campo da oncologia existem listas para o câncer de mama e de cabeça e pescoço, porém nenhuma lista até o momento foi criada voltada para a população de mulheres com câncer do colo do útero. Objetivos: Elaborar um modelo de lista resumida da CIF para mulheres com câncer do colo do útero. Metodologia: A proposta de construção do instrumento de avaliação da funcionalidade para mulheres com câncer do colo do útero foi baseada em quatro metodologias distintas: revisão sistemática,estudo observacional com pacientes com CCU, abordagem qualitativa realizado com pacientes com câncer do colo do útero e estudo com especialistas utilizando a metodologia Delphi. Introduction: cervical cancer is a disease with high incidence worldwide. Of the total cases, 80 percent occurred in developing countries. The cancer treatment related involves several deleterious effects that can affect biological, psychological and social aspects.This damage can occur in acute or late form and has a negatively impact in the quality of life of affected women. Since 2001, the World Health Organization (WHO) established arobust program to classify the functioning and state of health with the introduction of the International Classification of Functioning, Disability and Health (ICF). Considering that, the ICF is a classification with a large number of categories, were specific proposals summarized lists health conditions designed to disseminate the tool. In the oncology field,there are lists for breast cancer and head and neck, but so far, no list was developed toward the population of women with cervical cancer. Objectives: Develop a summary list of the ICF model for women with cervical cancer. Methodology: four distinct methodologies were used to create the list: a systematic review, observational study of patients with CCU, qualitative approach conducted with patients with cervical cancer and study with experts using the Delphi methodology. Conclusion: The resultsof the summary list proposal can serve as a starting point for documentation and evaluation of functional profiles in women with cervical cancer and has the distinction of surpassing the biomedical model of health-disease process approach and the use of biopsychosocial approach as a reference...
