ABSTRACT
Fundamento: es indiscutible la necesidad de poseer conocimientos adecuados sobre las normas bioéticas para el manejo del paciente con trastornos mentales. Objetivo: identificar los aspectos bioéticos que se deben fomentar durante el proceso de formación de nuevos especialistas en Psiquiatría. Métodos: se realizó una investigación de desarrollo con enfoque cualitativo, en el Hospital Psiquiátrico Universitario "Dr. Luis San Juan Pérez" de Villa Clara, durante el primer semestre del curso 2016-2017. Se utilizaron métodos de los niveles teórico: analítico-sintético, inducción-deducción y sistémico-estructural; y empíricos: análisis documental del programa y las orientaciones metodológicas y una encuesta en forma de entrevista a profesores para explorar las vivencias del manejo del paciente con trastorno mental desde el enfoque bioético actual y los principales conflictos bioéticos. Resultados: se identificaron aspectos bioéticos que se consideran aplicables al contexto estudiado y necesarios de profundizar durante el proceso de formación de los nuevos especialistas, entre ellos los relacionados con las hospitalizaciones involuntarias, el accionar ante fenómenos como la multiculturalidad, religión, sociedad plural y respeto de la diversidad, los estigmas de las enfermedad mentales, la repercusión de las medicaciones, el papel del psiquiatra como perito judicial y en la toma de decisiones, la aplicación de restricciones mecánicas, la mediación en temas íntimos y su papel de confidente y la respuesta ante una agresión física. Conclusiones: los aspectos bioéticos identificados constituyen una herramienta de utilidad para evaluar, desde la educación en el trabajo, la práctica de una temática que permite un enfoque más humanista e integral a la especialidad.
Background: the need to have adequate knowledge about bioethical standards for the management of patients with mental disorders is indisputable. Objective: to identify the bioethical aspects that should be promoted during the training process of the new specialists in Psychiatry. Methods: a qualitative research was carried out in the Psychiatric Hospital "Dr. Luis San Juan Pérez" of Villa Clara, during the first semester of the academic year 2016-2017. Methods were used at the theoretical levels: analytical-synthetic, induction-deduction and systemic-structural; and empirical studies: documentary analysis of the program and methodological guidelines and a survey in the form of interviews with teachers to explore the experiences of the management of patients with mental disorders from the current bioethical approach and the main bioethical conflicts. Results: bioethical aspects that were considered applicable to the studied context and necessary to deepen during the process of formation of the new specialists, among them those related to involuntary hospitalizations, the action to phenomena such as multiculturalism, religion, plural society and respect for diversity, the stigma of mental illness, the impact of medications, the role of the psychiatrist as a judicial expert and in decision-making, the application of mechanical restraints, mediation in intimate matters and his role of confidant and response to physical aggression. Conclusions: the bioethical aspects identified constitute a useful tool to evaluate, through the in-service training, the practice of a thematic that allows a more humanistic and comprehensive approach to the specialty.
