ABSTRACT

Abstract Alterations of ecosystems have deep effects on the distribution of parasites. Big cities of Argentina present structural features that favor the presence of synanthropic species, acting as source of zoonotic diseases, for example in urban rodents: the Norway rat (Rattus norvegicus) and the black rat (R. rattus). One of the important zoonotic pathogens related are the RNA virus Hantavirus, with high prevalence rates in South America. The aim of this study was to explore and identify the presence of Hantavirus in urban rodents from Gran La Plata, Argentina. The presence of anti-hantavirus IgG antibodies was determined by the Enzyme-Linked Immunosorbent Assay. Six samples turned out positive for Seoul virus (SEOV, p= 14.3%). These are the first records of SEOV in urban rodents in Gran La Plata. It represents the first report in R. rattus in Argentina, and in America. This situation underscores the inequality and historical forgetfulness of a portion of society, calling for urgent action to be taken in this regard.

ABSTRACT

RESUMEN Objetivo: Diversos estudios realizados sobre el linfoma de Hodgkin (LH) han determinado la expresión de la proteína de muerte programada (PD-L1) en las células de Reed-Sternberg-Hodgkin (HRS), con resultados variables. Esta proteína ha adquirido relevancia por su papel en la respuesta inmunitaria en las neoplasias malignas. El objetivo de este trabajo fue determinar la expresión inmunohistoquímica de PD-L1 en las HRS y su relación con la edad, sexo, estadio clínico y supervivencia global en una cohorte de pacientes peruanos. Materiales y métodos: Se evaluaron 25 biopsias de pacientes con diagnóstico de LH, lo que permitió determinar la expresión inmunohistoquímica de PD-L1 en las HRS en asociación con la sobrevida global (SG) y los datos clínicos de los pacientes. Resultados: Todos los casos mostraron expresión de PD-L1 en más del 1 %. No hubo diferencia estadística significativa en la SG cuando se comparó dos grupos de acuerdo con la expresión de PD-L1 con punto de corte en 50 %, ni tampoco con el estadio clínico (EC), la edad y el sexo. Conclusiones: Se ha encontrado una alta expresión de PD-L1 en los tumores con LH previos al tratamiento. No se ha encontrado asociación entre la expresión de PD-L1, la SG, la edad, el sexo o el EC. Son necesarios otros estudios con mayor cantidad de pacientes para reevaluar el impacto pronóstico de la expresión de esta proteína en LH.


ABSTRACT Objective: Various studies on Hodgkin's lymphoma (HL) have determined programmed death-ligand 1 (PD-L1) expression in Hodgkin and Reed-Sternberg (HRS) cells, with variable results. This protein has gained relevance for its role in the immune response in malignant neoplasms. This work aimed to determine PD-L1 immunohistochemical expression in HRS cells and its relationship with age, sex, clinical stage and overall survival (OS) in a cohort of Peruvian patients. Materials and methods: Twenty-five (25) biopsies from patients diagnosed with HL were assessed, which allowed determining PD-L1 immunohistochemical expression in HRS cells in relation to OS and clinical data of the patients. Results: All cases showed PD-L1 expression in more than 1 % of tumor cells. There was no statistically significant difference in OS when two groups were compared in terms of PD-L1 expression with a cut-off point of 50 %, clinical stage (CS), age and sex. Conclusions: High PD-L1 expression was found in pre-treatment HL tumors. No association was found between PD-L1 expression, OS, age, sex or CS. Further studies with a larger number of patients are necessary to reassess the prognostic impact of the expression of this protein in HL.

ABSTRACT

NTRODUCCIÓN: Los roedores sinantrópicos, representados por el ratón doméstico (Mus musculus), la rata parda (Rattus norvegicus) y la rata negra (Rattus rattus), representan un riesgo importante para la salud. En Sudamérica, la fragmentación socioeconómica se refleja en marcadas diferencias entre centros urbanos y áreas periféricas, y se asocia a un registro heterogéneo. El objetivo fue relevar datos por encuestas a los habitantes de dos barrios del Gran La Plata con características contrastantes para explorar, describir y evaluar la percepción en relación con la presencia de roedores en domicilio, peridomicilio y barrio como vehículos de transmisión de enfermedades. MÉTODOS: A partir de un diseño descriptivo exploratorio, se confeccionaron y realizaron encuestas siguiendo la técnica de muestreo estratificado. Se consideraron las variables género y grupo, de tal manera que la muestra tuviese la misma distribución. La información se transfirió a una base de datos y se analizó a través de IBM SPSS Statistics V25. RESULTADOS: Existe una preocupación común respecto al rol de los roedores urbanos como reservorios y fuentes de infección de patologías zoonóticas. En el barrio más vulnerable, la presencia de roedores fue más frecuente que en el centro de la ciudad. DISCUSIÓN: Este estudio provee un abordaje diferente en relación con roedores y patologías asociadas, considerando la percepción social y revelando su importancia para los programas de manejo y control.

Resumen ANTECEDENTES: El trastorno hipertensivo del embarazo es una de las causas de mortalidad materna y perinatal. Se estima que hay complicaciones incluso hasta en 10% de los embarazos, con un aumento significativo en la incidencia mundial en las últimas dos décadas y alrededor de 50,000 a 60,000 muertes maternas anuales. CASO CLÍNICO: Paciente de 26 años, con 17 semanas de embarazo; acudió a Urgencias debido a un episodio de 20 minutos de evolución de cefalea severa, con posterior episodio convulsivo tónico-clónico generalizado, con tensión arterial al ingreso de 170-102 mmHg, leve palidez mucocutánea, útero grávido con altura uterina de 18 cm y frecuencia cardiaca fetal de 136 lpm. Los estudios paraclínicos confirmaron: síndrome de HELLP, sin duda una situación atípica para las semanas de embarazo en la que sucedió. CONCLUSIONES: Los casos atípicos de preeclampsia suelen ser raros, sobre todo cuando sobrevienen antes de las 20 semanas; cuando así sucede es importante que el proceso diagnóstico sea oportuno al igual que el tratamiento. Es indispensable estar alertas del posible surgimiento de nefritis lúpica, púrpura trombocitopénica trombótica y síndrome urémico hemolítico, embarazo molar y síndrome antifosfolipídico.


Abstract BACKGROUND: Preeclampsia before 20 weeks' gestation is exceptional; it is associated with antiphospholipid syndrome. Hypertensive disorder of pregnancy is the leading cause of maternal and perinatal mortality worldwide, represents a 10% complication of all the pregnancies worldwide, where there has been a significant increase in its incidence has been observed. In the last two decades, with an annual estimate of 50,000 to 60,000 maternal deaths in the world. CLINICAL CASE: 26-year-old woman in the 17th week of pregnancy, who was admitted to the emergency department for a 20-minute evolution of severe headache with subsequent generalized tonic-clonic seizure episode, admission blood pressure was 170/102 mmHg, she was also found with mucocutaneas pallor, gynecological examination found a gravid uterus with a uterine height of 18 cm, and a fetal heart rate of 136 bpm. The paraclinical tests confirmed a HELLP syndrome, being considered an atypical situation due to the gestational age in which she presented. CONCLUSIONS: Atypical cases of preeclampsia are usually rare cases, especially when they appear before week 20; however, in this population it is important to make a diagnosis process in a timely manner, to initiate rapid treatment. Additionally, caution should be exercised with disorders including lupus nephritis, thrombotic thrombocytopenic purpura, and hemolytic uremic syndrome, as well as molar pregnancy and antiphospholipid syndrome.

INTRODUCCIÓN: Los trastornos sensitivos genitales y su relación con el maltrato psicológico en la pareja es un tema poco tratado en la literatura y, sin embargo, presente en la entrevista médica y psicológica. Aplica para aquellos casos en que no se aprecia alteración en la exploración física suficiente como para atribuirlo a causas orgánicas. OBJETIVO: Recopilar e integrar diferentes visiones acerca del síntoma de insensibilidad genital durante las relaciones sexuales, definir si este síntoma corresponde a una disfunción sexual e indagar sobre su relación con la violencia psicológica en la relación de pareja. MÉTODO: Revisión bibliográfica. Resultados: Los trastornos sensitivos cuya base orgánica es descartada no corresponderían a una disfunción sexual, sino más bien a una alteración sensorial disociativa como respuesta a una situación de violencia psicológica acompañada de violencia sexual en todos los casos. CONCLUSIONES: La insensibilidad genital es un tema que debe ser considerado al hacer la anamnesis de una paciente, para así ser derivada a un especialista con la intención de abordarlo de forma interdisciplinaria y, por ende, evaluar y diferenciar los elementos orgánicos y funcionales del síntoma.


INTRODUCTION: Genital sensory disorders and how they relate to psychological abuse in the couple is a topic that is scarcely address in the literature, however present it might be in medical and psychological interviews. It is often found in cases where there is no perceived sufficient alteration in physical examinations in order to attribute it to organic causes. OBJECTIVE: To collect and integrate different views regarding the symptom of genital insensitivity during sexual intercourse, define whether this symptom relates to sexual dysfunction and inquire about its connection with psychological violence in the couple. METHOD: Bibliographic assessment. Results: Sensory disorders, whose organic basis is ruled out, do not relate to sexual dysfunction but rather to a dissociative sensory alteration in response to a situation of psychological violence accompanied, in all cases, by sexual violence. CONCLUSIONS: Genital insensitivity is an issue that must be taken into account at the time of taking the patients medical history. This way, it can be referred to a specialist with the intention of addressing it in an interdisciplinary way and, therefore, evaluate and differentiate the organic and functional elements of the symptom.

Resumen Aunque en la actualidad la lesión renal aguda en mujeres embarazadas es una complicación poco común, esta se correlaciona con peores desenlaces maternos y fetales. Sus causas son múltiples y varían según el trimestre de presentación. Los cambios fisiológicos renales durante el embarazo hacen que los criterios diagnósticos de lesión renal aguda en la población general no sean fácilmente extrapolables a las pacientes obstétricas, por lo que la sospecha temprana y el tratamiento oportuno son fundamentales para evitar la progresión del daño renal y sus complicaciones sistémicas asociadas, incluidas las metabólicas, urémicas e hidroelectrolíticas.


Abstract Acute kidney injury in pregnant women is currently an uncommon complication, but it is correlated with worse maternal and fetal outcomes. Its causes are multiple and vary according to the quarter of presentation. Renal physiological changes during pregnancy mean that the diagnostic criteria for acute kidney injury in the general population are not easily extrapolated to obstetric patients. For this reason, early suspicion and prompt treatment is essential to avoid the progression of kidney damage and its associated systemic complications, including metabolic, uremic, and electrolyte complications.

Resumen La enfermedad renal crónica (ERC), definida como una alteración estructural o funcional renal que persiste por más de 3 meses con o sin deterioro de la función renal o como un filtrado glomerular (FG) <60 mL/min/1,73 m2 sin otros signos de enfermedad renal, es una condición prevalente en la población general; sin embargo, no es tan frecuente en mujeres embarazadas. A pesar de su baja frecuencia en gestantes, el antecedente de enfermedad renal se asocia con peores desenlaces maternos y fetales cuando se presenta ERC. Las fórmulas estándar (CKD-EPI, Cockroft-Gault y MDRD) utilizadas para determinar la tasa de filtrado glomerular en población no obstétrica tienen poca precisión en las embarazadas debido a que subestiman la función renal en aproximadamente un 20 %. Ante esto, el aclaramiento de creatinina medido utilizando recolecciones de orina de 24 horas y la estimación de creatinina sérica que se correlaciona estrechamente con el aclaramiento de inulina (estándar de oro) son opciones que pueden utilizarse durante el embarazo. Asimismo, el reconocimiento temprano de la enfermedad, la optimización de las estrategias de nefroprotección, el evitar las drogas nefrotóxicas y la instauración de un tratamiento específico para la etiología garantizan una mayor sobrevida y un menor número de complicaciones derivadas de la enfermedad renal.


Abstract Chronic Kidney Disease, defined as a renal structural or functional alteration that persists for more than 3 months, with or without deterioration of renal function, or a glomerular filtration rate (GFR) <60 ml / min / 1.73 m2 without other signs of Kidney disease is a prevalent condition in the general population, however, it is not so common to find it in pregnant women. However, a history of kidney disease is associated with worse maternal and fetal outcomes. The standard formulas (CKD-EPI, Cockroft-Gault, and MDRD) used in the non-obstetric population have poor precision in determining glomerular filtration rate, because they underestimate kidney function by approximately 20 %. Creatinine clearance measured using 24-hour urine collections and serum creatinine estimate closely correlates with inulin clearance ("gold standard") and can be used during pregnancy. Early recognition of the disease, optimization of nephroprotection strategies, avoiding nephrotoxic drugs and specific treatment of the etiology, will guarantee a longer survival and fewer complications derived from kidney disease.

Abstract Introduction: Alterations in the acid-base balance are studied in all medical specialties. Although most cases derive from a preexisting pathology, they can also manifest themselves in a primary context. The proper identification of the acid-base disorder allows the pathological process to be characterized. The correct interpretation of the blood gasometry as a technique for monitoring the ventilatory status, oxygenation and acid-base balance of a patient requires the integration of various physicochemical approaches in order to specify a diagnosis, quantify a therapeutic response, and monitor the severity or the progression of a pathological process. Methodology: A literature review was conducted in the PubMed, Scopus and Science Direct databases. The articles were selected according to the title and the abstract and sorted by topics relevant by pathophysiology, divergences, clinical approach, diagnosis, and management. Results: A guide the clinical correlation of the critical patient with the blood gasometry parameters to characterize the acid-base disorder through the proposition of a diagnostic algorithm. Conclusion: The incorporation of the three theories in a diagnostic algorithm facilitates a greater understanding of the pathophysiological mechanisms and allows us to identify a more precise therapeutic objective to correct the underlying disorder in the different clinical contexts of the patient.


Resumen Introducción: las alteraciones del equilibrio ácido-base se estudian en todas las especialidades médicas. Aunque la mayoría de los casos derivan de una patología preexistente, también pueden manifestarse en un contexto primario, por lo que la identificación adecuada del trastorno ácido-base permite caracterizar su proceso patológico. La correcta interpretación de la gasometría sanguínea como técnica para monitorizar el estado ventilatorio, la oxigenación y el equilibrio ácido-base de un paciente requiere la integración de varios enfoques fisicoquímicos para precisar un diagnóstico, cuantificar una respuesta terapéutica y monitorizar la gravedad o la progresión de un proceso patológico. Materiales y métodos: se realizó una revisión de la literatura en las bases de datos PubMed, Scopus y ScienceDirect. Los artículos fueron seleccionados según el título y el resumen, y ordenados por tópicos relevantes por fisiopatología, divergencias, abordaje clínico, diagnóstico y manejo. Resultados: en la literatura consultada se evidenció que el uso de un enfoque lógico y sistemático es necesario para interpretar adecuadamente los parámetros de la gasometría sanguínea e identificar oportunamente el trastorno ácido-base. Conclusión: la incorporación de las tres teorías en un algoritmo diagnóstico facilita una mayor comprensión de los mecanismos fisiopatológicos y permite identificar un objetivo terapéutico más preciso para corregir el trastorno de base en los diferentes contextos clínicos del paciente.

ABSTRACT

Resumen La gastrostomía endoscópica percutánea (GEP), descrita por primera vez en la década de 1980, es un procedimiento universalmente utilizado para asegurar la alimentación enteral en pacientes que conservan la función del tracto digestivo, pero que por alguna condición médica o quirúrgica no pueden ingerir alimentos sólidos o líquidos. La realización de una GEP en pacientes con diálisis peritoneal (DP) sigue siendo controvertida, ya que muchos autores la consideran una contraindicación absoluta debido al alto riesgo de morbimortalidad asociada a infección intraperitoneal. Se presenta el caso de un paciente de 70 años, con antecedente de enfermedad renal crónica en estadio terminal y portador de catéter de DP, quien requirió GEP. Se socializa la estrategia utilizada en este caso particular.


Abstract Percutaneous endoscopic gastrostomy (PEG), described in the 1980s, is a procedure universally used to ensure enteral feeding in patients who retain digestive tract function, but who cannot eat solid or liquid foods due to any medical or surgical condition. The performance of PEG in patients with peritoneal dialysis (PD) remains controversial, being considered an absolute contraindication for many authors, taking into account the high risk of morbidity and mortality associated with intraperitoneal infection. We present the case of a 70-year-old patient, with a history of chronic end-stage renal disease with a peritoneal dialysis catheter, who required PEG; and we propose a management protocol for this type of patient.

Resumen Introducción: el COVID-19 es una enfermedad causada por un nuevo beta coronavirus (SARS-CoV-2), cuyo espectro incluye casos severos con neumonía y complicaciones sistémicas que se dan como consecuencia de una liberación exagerada de mediadores inflamatorios conocida como "tormenta de citoquinas". En este tipo de pacientes las terapias de purificación sanguínea, incluyendo la hemoadsorción y la terapia plasmática podrían tener un beneficio clínico importante y evitar las complicaciones como disfunción multiorgánica y muerte. Objetivo: revisar toda la literatura disponible acerca de las terapias de purificación sanguínea y sus beneficios en los pacientes con COVID-19. Materiales y métodos: se realizó una búsqueda de la literatura en las bases de datos ClinicalKey, Embase, PubMed y Ovid con los términos "COVID-19", "SARS-CoV-2", "Tormenta de citoquinas", "terapias de purificación sanguínea", "sepsis", "hemoadsorción" y "CytoSorb". Resultados: se encontraron 246 referencias y luego de aplicar los filtros, se seleccionaron 61 publicaciones con contenido relevante para la presente revisión. Conclusión: teniendo en cuenta su gran evidencia científica en el manejo de la tormenta de citoquinas en otros escenarios, las terapias de purificación sanguínea, que incluyen técnicas difusivas, convectivas, de hemoadsorción y terapia plasmática, pueden ser herramientas de tratamiento muy prometedoras en pacientes con COVID-19.


Abstract Introduction: COVID-19 is a disease caused by a new beta coronavirus (SARS-CoV-2), whose spectrum includes severe cases with pneumonia and systemic complications that occur as a consequence of an exaggerated release of inflammatory mediators known as "cytokine storm". In this type of patients, blood purification therapies, including hemoadsorption and plasma therapy, could have an important clinical benefit and avoid complications such as multiple organ dysfunction and death. Objective: to review all available literature about blood purification therapies and their benefits in patients with COVID-19. Materials and methods: a literature search was conducted in the ClinicalKey, Embase, PubMed and Ovid databases using the terms "COVID-19", "SARS-CoV-2", "Tormenta de citoquinas", "terapias de purificación sanguínea", "sepsis", "hemoadsorción" and "CytoSorb". Results: 246 references were found and after applying filters, 61 publications with relevant content for the present review were selected. Conclusion: taking into account the great scientific evidence in the management of the cytokine storm in other scenarios, blood purification therapies, including diffusive, convective, hemoadsorption and plasma therapy techniques, could be very promising treatment tools for patients with COVID-19.

Resumen Introducción: El COVID-19, es una neumonía ocasionada por el nuevo coronavirus SARS-CoV-2, que se puede presentar con cuadros severos, distrés respiratorio agudo (SDRA), disfunción orgánica múltiple y muerte; como consecuencia de una respuesta inflamatoria alterada denominada "tormenta de citoquinas". La plasmaféresis se propone como una estrategia de tratamiento prometedora para el manejo de este tipo de complicaciones. Objetivo: nuestro objetivo principal es mostrar toda la bibliografía disponible, referente a la utilidad de la plasmaféresis en el manejo de la tormenta de citoquinas en pacientes con COVID-19 grave; evaluar su posible beneficio y proponer realizar nuevos ensayos clínicos que avalen el uso rutinario de esta terapia. Metodología: Se realizó una búsqueda avanzada con los términos DeSC "Infecciones por Coronavirus; SARS-CoV; Plasmaféresis; Recambio plasmático; Disfunción orgánica múltiple; Sepsis; Síndrome de respuesta inflamatoria sistémica; Lesión renal aguda". A través de los motores de búsqueda Clinical Key, Embase, PubMed y Ovid, obteniendo un total de 156 resultados, entre artículos originales, reportes de casos, series de casos y revisiones de la literatura, se seleccionaron un total de 54 artículos que fueron utilizados para la elaboración de la presente revisión de la literatura. Conclusiones: La terapia de recambio plasmático se podría utilizar como tratamiento complementario, con el objetivo de reducir carga inflamatoria y viral, reduciendo así el daño de órgano blanco. Sin embargo, hace falta la realización de ensayos clínicos controlados y con buenos diseños metodológicos, que nos ayuden a demostrar la efectividad de este tipo de terapias en pacientes con COVID-19 grave.


Abstract Introduction: COVID-19 is a pneumonia caused by the new SARS-CoV-2 coronavirus, which can present with severe symptoms, acute respiratory distress (ARDS), multiple organ dysfunction and death; as a consequence of a dysregulated inflammatory response called "cytokine storm". Plasmapheresis is proposed as a promising treatment strategy for the management of this type of complications. Objective: our main objective is to show all the available bibliography, referring to the utility of plasmapheresis in the management of cytokine storm in patients with severe COVID-19; evaluate its possible benefit and propose new clinical trials to support the routine use of this therapy. Methodology: An advanced search was performed with the DeSC terms "Coronavirus Infections; SARS-CoV; Plasmapheresis; Plasma exchange; Multiple organic dysfunction; Sepsis; Systemic inflammatory response syndrome; Acute kidney injury ". Through the search engines Clinical Key, Embase, PubMed and Ovid, obtaining a total of 156 results, among original articles, case reports, case series and literature reviews, a total of 54 articles were selected and used for the preparation of this literature review. Conclusions: Plasma replacement therapy could be used as a complementary treatment, with the aim of reducing inflammatory and viral loads, thus reducing target organ damage. However, it is necessary to carry out controlled clinical trials with good methodological designs that help us demonstrate the effectiveness of this type of therapy in patients with severe COVID-19.

ABSTRACT Background: The left atrial appendage (LAAp) resection is an effective treatment approach to reduce the risk of thromboembolism in patients with atrial fibrillation. Objective: To study was to study the impact of removing atrial appendages in the production of natriuretic peptides (NPs) in conditions of volume overload and to develop an experimental model of LAAp resection. Materials and Methods: In a swine model of ischemic heart failure (HF), serum NP levels were measured before (Basal-1A) and after (Basal-1B) a fluid overload. Animals were grouped as follows: (0) preserved appendages, (1) resected LAAp, and (2) both atrial appendages resected. Levels of NP were measured before (2A) and after a fluid overload (2B). Results: Furin levels were higher in Group 0-2A than in Group 2-2A, and a significant increase was found in Group 0-2B compared to Groups 1-2B and 2-2B. Corin levels increased in Basal-1B versus Basal-1A. Atrial NP (ANP) decreased in Basal-1B compared to Basal-1A. After HF induction, ANP increased in Groups 2-2A and 2-2B. Conclusions: Resection of atrial appendages drastically modifies the natriuretic mechanisms of cardiac homeostasis, especially after a fluid overload challenge. Herein, we describe the face and predictive validation of an animal model of atrial appendage resection useful to investigations in translational medicine.

BACKGROUND Intermediate hosts are key organisms in maintaining parasite life cycles, because they can act as amplifiers in the transmission from natural reservoirs to humans. One of the most important groups of intermediate hosts for zoonotic nematode infections are gastropods,slugs and snails. These are essential organisms in the larval development of Angiostrongylus species. OBJECTIVES The objective of this paper is to review reports of Angiostrongylus spp. in naturally infected gastropods from the Americas, taking into account the diagnostic methods used in their identification, to be able to provide more accurate list of their intermediate hosts. We also discuss the factors that aid the dispersion of Angiostrongylus spp. in the Americas. METHODS This study reviews scientific publications and book sections on Angiostrongylus spp. in the Americas, including original works assessing larvae of Angiostrongylus in intermediate hosts. The eligible reports were classified accordingly to their geographical location, year of first record, and the larvae identification methodologies used. Digital repositories were used for the search. The bioecological characteristics of the main intermediate hosts are summarised. FINDINGS A total of 29 gastropod species that are naturally infected with Angiostrongylus spp. have been reported as intermediate hosts, 16 of which are land snails, two are freshwater snails, and 11 land slugs. MAIN CONCLUSIONS This study highlights the importance of integrative studies, analysing both the etiological agent and its transmission dynamic in the environment, the biological and ecological characteristics of the hosts, and the impact on host populations. It is necessary to increase interdisciplinary studies to determine the potential epidemiological health risk of angiostrongyliasis in the Americas, and thus be able to establish prevention, monitoring and contingency strategies in the region.

Abstract Toxoplasma gondii and Neospora caninum are closely related coccidian parasites (phylum Apicomplexa). This is the first study from urban synanthropic rodent species that involved serological and molecular diagnosis of T. gondii and N. caninum infection, and genotyping of T. gondii in Argentina. A total of 127 rodent samples were trap captured: Mus musculus (n = 78), Rattus norvegicus (n = 26) and Rattus rattus (n = 23). Antibodies against T. gondii and N. caninum were detected by IFAT in 32.8% (40/122) and 0.8% (1/122) of rodent samples, respectively, demonstrating contact with these protozoans. Additionally, T. gondii DNA was detected in 3.3% (4/123) of rodent central nervous system samples and 2 samples were genotyped by multilocus nPCR-RFLP. Neospora caninum DNA was not detected by PCR. The 2 genotyped samples were type III allele for all markers except for SAG-1 (type I for Rat1Arg and type II/III for Rat2Arg) and were identified as #48 and #2 (likely) according to the allele combinations reported on Toxo DB (Toxo-DB). The results of the present study revealed a wide distribution of T. gondii and less for N. caninum, in synanthropic rats and mice in the studied area.


Resumo Toxoplasma gondii e Neospora caninum são parasitas coccídeos intimamente relacionados (filo Apicomplexa). Este é o primeiro estudo de espécies de roedores sinantrópicos urbanos, o qual envolveu diagnósticos sorológicos e moleculares da infecção por T. gondii e N. caninum e genotipagem de T. gondii na Argentina. Um total de 127 amostras de roedores foram obtidas: Mus musculus (n = 78), Rattus norvegicus (n = 26) e Rattus rattus (n = 23). Anticorpos contra T. gondii e N. caninum foram detectados pela IFAT em 32,8% (40/122) e 0,8% (1/122) das amostras de roedores, respectivamente, demonstrando contato com esses protozoários. Adicionalmente, o DNA de T. gondii foi detectado em 3,3% (4/123) das amostras do sistema nervoso central de roedores e duas amostras foram genotipadas por nPCR-RFLP multilocus. O DNA de N. caninum não foi detectado por PCR. As 2 amostras genotipadas eram do tipo III para todos os marcadores, exceto para SAG-1 (tipo I para Rat1Arg e tipo II / III para Rat2Arg) e foram identificadas como # 48 e # 2 (provavelmente) de acordo com as combinações de alelos relatadas no Toxo DB (Toxo-DB). Os resultados do presente estudo indicam uma ampla distribuição de T. gondii e menor para N. caninum , em ratos e camundongos sinantrópicos na área estudada.

Laleishmaniasis cutánea (LC) es una zoonosis endémica y un problema de salud pública en el Ecuador. En los últimos años, Manabí, es la provincia más incidente de Leishmania como consecuencia de los desastres naturales ocurridos y la migración de locales a las áreas rurales.El objetivo del estudio es determinarlos aspectos clínicos yepidemiológicosde la Leishmania.El estudio de casos de datos secundarios, reclutó 4 pacientes registrados en el hospital regional de Portoviejo en Manabí, en el 2017. La determinación clínica (características de las lesiones) y epidemiológica de la leishmaniasis, así como el diagnóstico complementario de las muestras (polo benigno) con frotis directo fueron evaluados. La determinación taxonómica de Leishmaniano se prescribe en entidades públicas gubernamentales.El análisis descriptivo se realizó en M. Excel, determinándose la LC en el 100% (N=4) de las mujeres, de entre 1 a 43 años, 3 (75%) de ellas <16 años. Solo 1 (25%) paciente acudió a la consulta hospitalaria por las lesiones dérmicas mientras que el resto [n=3 (75%)], fueron determinadas como un hallazgo fortuito durante la hospitalización por otras entidades clínicas emergentes. El 25% (n=1) de ellas, recibió tratamiento hospitalario con glucantime, mientras que el 75% (n=3) de ellas fueron derivados a hospitales de segundo nivel. La necesidad urgente de capitación del equipo médico de la atención primaria es inherente para la identificación oportuna de Leishmaniasis comunitaria para mejorar la supervisión epidemiológica de la zoonosis. Aún existen reportes clínicos diagnósticos erróneos sobre las lesiones dérmicas, sin considerar los aspectos epidemiológicos y de laboratorio que pueden ayudar a determinarLC.


The cutaneous leishmaniasis (CL) is an endemic zoonosis and a public health problem in Ecuador. In recent years, Manabí is the most incident province with Leishmania as a result of recent natural disasters and the migration of locals to rural areas. The Aim of this study is to determine the clinical and epidemiological aspects of Leishmania. The case study used secondary data cases and recruited 4 patients, registered at a tertiary hospital in Portoviejo, Manabí, in 2017. The clinical (characteristics of the lesions) and epidemiological determination of CL, as well as the complementary diagnosis of skin samples (benign pole) with direct smear were evaluated. The taxonomic determination of Leishmania is not prescribed in public health institutions. The descriptive analysis was performed in M. Excel. The CL was determined in 100% (N=4) of women between 1 to 43 years, 3 (75%) of them <16 years. Only 1 (25%) patient attended the hospital checking out for skin lesions while the rest [n=3 (75%)] were determined as a fortuitous finding during hospitalization by other emerging clinical entities. Only 25% (n=1) of them received treatment with glucantime at hospital, while 75% (n=3) of them were referred to second level hospitals. The urgent need for training of health workers at primary care level is inherent to the timely identification of community leishmaniasis, to improve epidemiological surveillance upon the zoonoses. Some mistakes in diagnosis of skin lesions, without considering the epidemiological and laboratory aspects, in order to determine cutaneous leishmaniasis, have been reported.

BACKGROUND Angiostrongyliasis is an infection caused by nematode worms of the genus Angiostrongylus. The adult worms inhabit the pulmonary arteries, heart, bronchioles of the lung, or mesenteric arteries of the caecum of definitive host. Of a total of 23 species of Angiostrongylus cited worldwide, only nine were registered in the American Continent. Two species, A. cantonensis and A. costaricensis, are considered zoonoses when the larvae accidentally parasitise man. OBJECTIVES In the present study, geographical and chronological distribution of definitive hosts of Angiostrongylus in the Americas is analysed in order to observe their relationship with disease reports. Moreover, the role of different definitive hosts as sentinels and dispersers of infective stages is discussed. METHODS The study area includes the Americas. First records of Angiostrongylus spp. in definitive or accidental hosts were compiled from the literature. Data were included in tables and figures and were matched to geographic information systems (GIS). FINDINGS Most geographical records of Angiostrongylus spp. both for definitive and accidental hosts belong to tropical areas, mainly equatorial zone. In relation to those species of human health importance, as A. cantonensis and A. costaricensis, most disease cases indicate a coincidence between the finding of definitive host and disease record. However, in some geographic site there are gaps between report of definitive host and disease record. In many areas, human populations have invaded natural environments and their socioeconomic conditions do not allow adequate medical care. MAIN CONCLUSIONS Consequently, many cases for angiostrongyliasis could have gone unreported or unrecognised throughout history and in the nowadays. Moreover, the population expansion and the climatic changes invite to make broader and more complete range of observation on the species that involve possible epidemiological risks. This paper integrates and shows the current distribution of Angiostrongylus species in America, being this information very relevant for establishing prevention, monitoring and contingency strategies in the region.

To date, 21 species of the genus Angiostrongylus (Nematoda: Angiostrongylidae) have been reported around the world, 15 of which are parasites of rodents. In this study, new host, geographic records, and histopathologic studies of Angiostrongylus spp in sigmodontine rodents from Argentina, with an updated summary of records from rodent hosts and host specificity assessment, are provided. Records of Angiostrongylus costaricensis from Akodon montensis andAngiostrongylus morerai from six new hosts and geographical localities in Argentina are reported. The gross and histopathologic changes in the lungs of the host species due to angiostrongylosis are described. Published records of the genus Angiostrongylus from rodents and patterns of host specificity are presented. Individual Angiostrongylusspecies parasitise between one-19 different host species. The most frequent values of the specificity index (STD) were between 1-5.97. The elevated number of host species (n = 7) of A. morerai with a STD = 1.86 is a reflection of multiple systematic studies of parasites from sigmodontine rodents in the area of Cuenca del Plata, Argentina, showing that an increase in sampling effort can result in new findings. The combination of low host specificity and a wide geographic distribution of Angiostrongylus spp indicates a troubling epidemiological scenario although, as yet, no human cases have been reported.

ABSTRACT

Cartilage has poor regeneration capacity due to the scarcity of endogenous stem cells, its low metabolic activity and the avascular environment. Repair strategies vary widely, including microfracture, autologous or allogenic tissue implantation, and in vitro engineered tissues of autologous origin. However, unlike the advances that have been made over more than two decades with more complex organs, including vascular, cardiac or bone tissues, similar advances in tissue engineering for cartilage repair are lacking. Although the inherent characteristics of cartilage tissue, such as the lack of vascularity and low cellular diversity, suggest that it would be one of the more simple tissues to be engineered, its functional weight-bearing role and implant viability and adaptation make this type of repair more complex. Over the last decade several therapeutic approaches and innovative techniques show promise for lasting and functional regeneration of hyaline cartilage. Here we will analyze the main strategies for cartilage regeneration and discuss our experience.

INTRODUCCIÓN: La vía congénita es una de las principales formas de transmisión de la enfermedad de Chagas: afecta desde un 0,7 a un 17% de los niños nacidos de madres infectadas por Trypanosoma cruzi OBJETIVOS: Calcular la cantidad de infectados por T. cruzi entre niños nacidos de madres diagnosticadas con Chagas durante 2010 y sus hermanos. MÉTODOS: Se realizó un estudio transversal. Se definió como caso índice a toda mujer embarazada residente en el departamento chaqueño de San Fernando con diagnóstico de enfermedad de Chagas en 2010. Se efectuaron visitas en el domicilio de las mujeres y se extrajeron muestras a sus hijos, que fueron procesadas por la red pública de laboratorios mediante Hemaglutinación indirecta (HAI ) y Ensayo por inmunoabsorción ligado a enzimas (ELISA), definiendo las discordancias con Inmunofluorescencia indirecta (IFI). RESULTADOS: Se localizó a 71 madres que cumplían la definición de caso. Entre las encuestadas, 65 (92,9%) conocían su diagnóstico de infección por T. cruzi; de las cuales 4 (5,8%) realizaban controles cardiológicos anuales. Por cada tres puérperas con Chagas, se diagnosticó un infectado adicional. De 188 muestras de hijos de madres infectadas con el parásito, 23 resultaron positivas: la proporción de transmisión por vía congénita fue del 12,2% (IC 95%: 7,2-17,1). CONCLUSIONES: La proporción de infección congénita encontrada no difiere significativamente de las de la región y América Latina en general.


INTRODUCTION: The congenital transmission of Chagas disease is one of the most important, affecting between 0.7 to 17% of children born to Trypanosoma cruzi infected mothers. OBJECTIVES: To calculate the number of T. cruzi infected children among those born to mothers diagnosed with the infection in 2010 and their siblings. METHODS: A cross-sectional study was conducted. Anindex case was defined as a pregnant woman living in San Fernando Department (Province of Chaco) diagnosed with Chagas disease in 2010. The samples collected in children during home visits were processed by the network of public laboratories through indirect hemagglutination (IHA) and ELISA, resolving the discordance with indirect immunofluorescence (IIF) RESULTS: A total of 71 mothers met the case definition. 65 (92.9%) of the women surveyed knew their diagnosis, of which 4 (5.8%) had annual heart checks done. Among positive cases, 4 (5.8%) received an etiological treatment. There was an additional infection diagnosis for every three cases with Chagas. Out of a total of 188 samples of children from infected mothers. 23 were positive, which means that the rate of congenital transmission was 12.2% (CI 95%: 7.2-17.1). CONCLUSIONS: The rate of congenital transmission does not differ significantly from the ones in the region and Latin America in general.

Resident and migratory shorebirds inhabit different kinds of wetlands such as lagoons, rivers and seashores among others. In recent years, these areas have been importantly affected by urban, agriculture and touristic activities, such as the Barra de Navidad lagoon, for which little information is available to support conservation programs. The aim of this work was to describe shorebirds temporal and spatial distribution in Barra de Navidad lagoon during three non-breeding seasons (1999-2000, 2006-2007 and 2008-2009). For this, monthly censuses were performed from November-April with the purpose of registering all the shorebirds species. We were able to identify 19 shorebirds species (three residents and 16 winter visitors), of which Charadrius wilsonia, Limosa fedoa and Tringa semipalmata were the most abundant. The greater number of species was registered for November, December and March of the first and third seasons. The greater number of individuals was registered when birds were feeding during low tides, mainly in December, January and February of the first and third seasons. At low tide, there was a great number of species and individuals in zone C. This area had muddy substrates that were exposed during low tides and were used to feed. Barra de Navidad lagoon provided suitable habitats for feeding and resting for resident and migratory birds. Twelve of the 19 species were considered as priority within the Mexican bird conservation strategy. However, these habitats are threatened by human activities performed in the nearby areas of the lagoon that may have negative consequences for the distribution, abundance and conservation of these species.


Hay un escaso conocimiento de las aves playeras en los humedales costeros de Jalisco, y en particular en la laguna Barra de Navidad. El presente trabajo contribuye al conocimiento de este grupo de aves y describe su distribución temporal y espacial en la laguna Barra de Navidad durante tres temporadas no reproductivas (1999-2000, 2006-2007 y 2008-2009). Se realizaron censos mensuales de noviembre-abril en las tres temporadas con el fin de registrar todas las especies de aves playeras. Se identificaron 19 especies (tres residentes y 16 visitantes de invierno), de las cuales Charadrius wilsonia, Limosa fedoa y Tringa semipalmata presentaron la mayor abundancia. Doce especies son consideradas como prioritarias en la “Estrategia para la Conservación y Manejo de las Aves Playeras y su Hábitat en México”. El mayor número de especies fue registrado en noviembre, diciembre y marzo en la primera y tercera temporada. El mayor número de individuos fue registrado alimentándose en marea baja, principalmente en diciembre, enero y febrero de la primera y tercera temporada. En marea baja hubo un mayor número de especies e individuos alimentándose en la zona C. Esta zona se caracterizó por tener sustratos lodosos expuestos durante marea baja y que fueron aprovechados por las aves para alimentarse. La laguna Barra de Navidad proporcionó hábitats de alimentación y descanso para las aves residentes y migratorias. Sin embargo, estos hábitats se ven amenazados por las actividades humanas realizadas dentro de la laguna, que sin duda tendrán consecuencias negativas para la distribución y abundancia de las aves playeras.
