ABSTRACT

Abstract With the implementation of perinatal care strategies, the rate of vertical transmission of human immunodeficiency virus (HIV) has decreased considerably worldwide. Despite the absence of viral loads, infants exposed to HIV not infected during gestation have higher morbidity and mortality rates. This is found to be related to infectious diseases by opportunistic microorganisms and lower response to vaccines in their first months of life compared to non-HIV exposed infants, suggesting alterations in their immune system. In this review we address different evidence of alterations in the innate and adaptive immune responses of HIV exposed infants that could explain their immune dysfunctionality. Additionally, this knowledge helps to understand how the immune system develops from the early stages of gestation and will serve to find management alternatives and therapies for the welfare of the infants with this condition.

ABSTRACT

Abstract Objective: To analyse the mortality trends in children under five years old in Brazil from 2017 to 2020 and the influence of COVID-19 in 2020. Methods: A retrospective study employing secondary data from the Brazilian Mortality Information System. Deaths according to cause were extracted and disaggregated into early, late, post-neonatal, and 1 to 4-year-old periods. Corrected mortality rates per 1, 000 live births and relative risk ratio for the cause of death were calculated. Results: There were 34, 070 deaths, being 417 (1.2%) from COVID-19 in 2020. COVID-19 mortality was 0.17 per 1000 live births, reaching 0.006 in the early neonatal period, 0.007 in the late neonatal, 0.09 in the postneonatal, and 0.06 in 1 to 4-year-old. Mortality decreased mostly for some diseases that originated in the perinatal period, congenital anomalies, diseases of the respiratory system and external causes, in this order. In 2020, the highest rate was in the early neonatal period, with a fall from 7.2 to 6.5, followed by the postneonatal (3.9 to 3.4) and late neonatal (2.3 to 2.1). Among children aged 1 to 4-year-old, external causes had the highest proportional rate, and diseases of the respiratory system showed the highest decline. Conclusion: The mortality rate declined from 2017 to 2020, and this variation was higher in the early neonatal period. The risk of death from COVID-19 was 14 times higher in the postneonatal period and 10 times higher in children aged 1 to 4 year-old compared to the early neonatal period.

ABSTRACT

ABSTRACT Introduction: Telomere length (TL) is a biomarker of cellular proliferative history. In healthy individuals, leukocyte TL shortens with age and associates with the lifespan of men and women. However, most of studies had used linear regression models to address the association of the TL attrition, aging and sex. Methods: We evaluated the association between the TL, aging and sex in a cohort of 180 healthy subjects by quantile regression. The TL of nucleated blood cells was measured by fluorescent in situ hypridization (flow-FISH) in a cohort of 89 men, 81 women, and 10 umbilical cord samples. The results were validated by quantitative polymerase chain reaction (qPCR) and compared to a linear regression analysis. Results: By quantile regression, telomere dynamics slightly differed between sexes with aging: women had longer telomeres at birth and slower attrition rate than men until the sixth decade of life; after that, TL eroded faster and became shorter than that in men. These differences were not observed by linear regression analysis, as the overall telomere attrition rates in women and men were similar (42 pb per year, p < 0.0001 vs. 45 pb kb per year, p < 0.0001). Also, qPCR did not recapitulate flow-FISH findings, as the telomere dynamics by qPCR followed a linear model. Conclusion: The quantile regression analysis accurately reproduced a third-orderpolynomial TL attrition rate in both women and men, but it depended on the technique applied to measure TL. The Flow-FISH reproduced the expected telomere dynamics through life and, differently from the qPCR, was able to detect the subtle TL variations associated with sex and aging.

ABSTRACT

ABSTRACT SARS-CoV-2 has a high risk of outbreak in long-term skilled nursing facilities (SNF). Coronavirus disease (COVID-19) has high mortality rates among the elderly with chronic health conditions. Following identification of COVID-19 index case in a SNF, serial point-prevalence was implemented with reverse transcription-polymerase chain reaction (RT-PCR) and immunochromatographic assays. Active surveillance and early isolation of infected patients were implemented. Out of 23 SNF residents and 26 healthcare workers (HCW), 18 (78%) and 12 (46%) tested positive for SARS-CoV-2, respectively. High proportion (38%) of positive patients were asymptomatic and RT-PCR was positive up to six days before symptoms. Five (21.74%) residents were hospitalized with COVID-19, and 2 (9%) died; only 1 (4%) HCW needed to be hospitalized and no staff members died. Active surveillance helped COVID-19 control and management in a SNF. Testing symptomatic individuals only may fail to identify and isolate all persons contributing to transmission. In high-risk elderly, only symptoms screening may not be enough for outbreak control.

ABSTRACT

Although psychological treatments for depressive disorders are available, they are often expensive or inaccessible for many. Web-based interventions that require minimal or no contact with therapists have been shown effective. To the best of our knowledge, no study using this treatment format has been conducted in Brazil. The Deprexis program was designed using empirically established principles of cognitive-behavioral therapy to reduce depressive symptoms. The objective of this study was to evaluate the effectiveness of Deprexis in Brazil. This randomized controlled trial will include 128 Brazilians with clinically significant depression symptoms or who have been diagnosed with depressive disorder (major depressive disorder or dysthymia), recruited over the internet (Brazilian forums, social networks, or e-mail lists). Individuals with other psychiatric diagnoses that require significant attention (e.g., bipolar disorder, psychosis) will not be included in the trial. Participants will be randomly assigned to 1) treatment as usual plus immediate access to Deprexis or 2) treatment as usual plus delayed access to Deprexis (after 8 weeks). Participants will be able to obtain other treatment types in addition to the online intervention. If found effective, this web-based intervention would increase the evidence-based care options for depression treatment in Brazil. Clinical trial registration: RBR-6kk3bx, UTN U1111-1212-8998

ABSTRACT

Objective: A large proportion of psychotherapy patients remain untreated, mostly because they drop out. This study compares the short- and long-term outcomes of patients who dropped out of psychotherapy to those of therapy completers. Methods: The sample included 63 patients (23 dropouts and 40 completers) from a controlled clinical trial, which compared narrative therapy vs. cognitive-behavioral therapy for major depressive disorder. Patients were assessed at the eighth session, post-treatment, and at 31-month follow-up. Results: Dropouts improved less than completers by the last session attended, but continued to improve significantly more than completers during the follow-up period. Some dropout patients improved with a small dose of therapy (17% achieved a clinically significant change before abandoning treatment), while others only achieved clinically significant change after a longer period (62% at 31-month follow-up). Conclusion: These results emphasize the importance of dealing effectively with patients at risk of dropping out of therapy.Patients who dropped out also reported improvement of depressive symptoms without therapy, but took much longer to improve than did patients who completed therapy. This might be attributable to natural remission of depression. Further research should use a larger patient database, ideally gathered by meta-analysis.

ABSTRACT

ABSTRACT The lagerpetid Dromomeron romeri and the theropod Tawa hallae are two dinosauromorphs from the Norian (Upper Triassic) of the Chinle Formation, situated in New Mexico, USA. However, a recent study suggests the inclusion of the holotype of D. romeri (GR 218) and paratype (GR 155) and referred (GR 235) specimens of T. hallae in an ontogenetic series of a single species. The specimens GR 218 and GR 155 include just an isolated femur, while GR235 includes femora, pelvis and tail. The inclusion of the specimens in an unique ontogenetic series relies on the putative immature condition and plastic deformation of the specimen GR 218. However, as observed here, the disparity between the femora of D. romeri and T. hallae is considerably higher than those expected from the ontogenetic variance in dinosauromorphs. In addition, D. romeri shares an unique suite of traits with Dromomeron gigas, a species known from a mature specimen. Therefore, the high disparity between D. romeri and T. hallae, lack of traits shared solely between the three femora, and a suite of traits shared between D. romeri and D. gigas, precludes the inclusion of the three femora from Hayden Quarry in a growth series of a single taxon.

ABSTRACT

ABSTRACT Background: Telomeres, the ends of linear chromosomes, shorten during mitotic cell division and erosion may be aggravated by inflammation or proliferative and oxidative stress. As the bone marrow is under hyperproliferative pressure in sickle cell disease and several tissues are submitted to chronic inflammation, this study sought to determine the telomere length of patients with sickle cell disease. Methods: The mean telomere length was measured in peripheral blood leukocytes by quantitative polymerase chain reaction. The age-adjusted telomere to single copy gene ratio was compared between 91 adult sickle cell disease patients and 188 controls. Results: Sickle cell disease patients had significantly shorter telomeres than the controls (p-value < 0.0001). Moreover, among sickle cell disease genotypes, Hb SS patients had significantly shorter telomeres compared to Hb SC and Hb Sβ patients (p-value < 0.0001). Patients on hydroxyurea also had shorter telomeres in comparison to those off the drug (p-value = 0.02). A positive correlation was observed between telomere length and hemoglobin level (r = 0.3; p-value = 0.004), whereas negative correlations were detected between telomere length and lymphocyte count (r = -0.3; p-value = 0.005) and interleukin-8 serum levels (r = -0.4; p-value = 0.02). Conclusions: The findings of this study indicate that telomeres are short in sickle cell disease patients and that telomere erosion directly correlates with disease genotype, inflammation markers, and the use of hydroxyurea.

ABSTRACT

The National Curriculum Parameters (NCPs) indicated directions for the teaching of physical education in schools. The NCPs showed didactic and methodological proposals and indicated curriculum contents to be studied in the different levels of education. The Federal Institutes of Science and Technology are part of a restructuring of vocational education in Brazil, which occurred in 2008 and had ideology of integral formation of the individual, about political, cultural and social awareness. It is observed divergence in the discourses and practices of teachers and school managers about the perception of Physical Education representation in these institutions. This study aimed to identify all the papers that discussed the physical education in technical schools, which were published in journals of the field since the publication of the LBG 9394/96, and to discuss exposed pedagogical actions. Searches were done in data bases Scielo, Capes Periodics, Google Scholar, and in all journals classified as A or B in Qualis Capes, that has the School Physical Education in their scope. Only seven studies discussed the topic since the publication of the LBG. The papers were categorized into "legitimacy of the Physical Education in the academic community", "the history of Physical Education at technical school", and "pedagogical actions of Physical Education teachers in technical education". The lack of studies published in journals of the field, which discusses Physical Education in technical education, exposes the needs that have to be explored to fill this scientific demand and to encourage the practice of teachers in this educational modality....(AU)

ABSTRACT

Introduction: The prevalence of cryptogenic cirrhosis ranges from 5% to 30% of cirrhotic patients in past series (CADWELL et al, 1999). Liver transplantation represents the only option to revert the hepatic insufficiency and its complications in cirrhotic patients in advanced stage. In Brazil, Mansonic Schistosomiasisis a public health problem and endemic disease, particularly in the Northeast of the country. Methods: Case report of a male patient, admitted to Hospital Geral de Fortaleza, Ceará, Brazil, with cryptogenic hepatic cirrhosis, classified as Child-Turcotte-Pugh C, with a MELD of 25, submitted to a liver transplantation, and found to have schistosomal hepatic disease on biopsy report. We reviewed the patient's medical history and physical examination on admission, prescription, results of laboratory tests and follow up data. Survey of the literature in national and international scientific journals helped incollecting information on this disease. Conclusions: Parasitic infections in solid organ transplant has increased in the recent years. It is very important to keep a strict control of the quality of the organs and tissues used in transplantations, as well as an improvement in diagnosis, treatment and prophylaxis techniques, especially in liver transplantation, in view of a high prevalence of parasitic infections in our country, in order to prevent the developmentof other comorbidities, and to increase the survival of transplanted patients. In endemic countries, potential donors or recipients who have active schistosomal infection should be preventively treated.

ABSTRACT

This is a prevalence study with the first specific description of drowning cases ina non-coastal area of Brazil, in the locality of Ribeirão Preto, State of São Paulo—a region with standards of living comparable to those in developed countries—regarding the establishment of preventative measures for this specific kind of injury. Methodology: An analysis of the forensic documentation of 89 drowning cases occurred between 2001 and 2004. The pattern of drowning fatalities was studied in relation to parameters like age, sex, socio-economic background and evaluation of alcohol consumption. Results: rate of 2.69 cases per 100,000 habitants per year was observed. The predominant profile among drowning victims was that of the Caucasian male of economically productive age (15 to 59 years), accidentally drowned in rivers and dams in rural areas during the summer and autumn, being impossible the evaluation of alcohol consumption. Conclusion and Relevance: the observation of the victim profile through the analysis of forensic reports allowed the development of a successful pilot program of drowning prevention that can be expanded to other non-coastal areas in Brazil, regarding the reduction of the number of victims by prevention. The difficulties in alcohol consumption analysis are discussed.

ABSTRACT

OBJECTIVE: To establish the parameters of intra- and interventricular synchrony in normal individuals and to compare them with patients with dilated cardiomyopathy with and without conduction disorders shown in the electrocardiogram (ECG) examination. METHODS: Three groups of patients were included in this study: 18 individuals (G1) with no cardiomyopathy and with a normal ECG (52±12 years, 29 percent male); 50 patients with dilated cardiomyopathy and severe left ventricular dysfunction, with 20 patients (G2) presenting QRS <120ms (51±10 years, 75 percent male) and 30 patients (G3) with QRS >120ms (57±12 years, 60 percent male). All patients underwent RV. Evaluation of left intraventricular dyssynchrony was carried out with the measurement of the phase histogram width and interventricular dyssynchrony was evaluated by the difference of the mean phase angle between the right and left ventricles (RLDif). RESULTS: Left ventricle ejection fractions (LVEF)s were: 62 ± 6 percent (G1), 27 ± 7 percent (G2) and 22 ± 8 percent (G3) and right ventricle ejection fractions were: 46 ± 5 percent (G1), 41 ± 6 percent(G2) and 38 ± 8 percent (G3). Evaluation of the phase histogram width was: 89 ± 18 ms (G1), 203 ± 54 ms (G2) and 312 ± 130 ms (G3), p<0.0001. The measurement of RLDif was: 14 ± 11 ms (G1), 39 ± 40 ms (G2) and 87 ± 49 ms (G3); comparing G1 vs. G2 and G1 vs. G3, p<0.0001 and G2 vs. G3, p=0.0007. CONCLUSION: The parameters analyzed discriminate the three groups of patients according to the ventricular synchrony degree. Patients with dilated cardiomyopathy and withno branch block in ECG (QRS <120 ms) may present dyssynchrony, but at a lower degree than patients with widened QRS.

ABSTRACT

We investigated the prevalence and the risk factors for infection with Helicobacter pylori in a randomly-selected population of adults from a low-income community in Northeastern Brazil. Helicobacter pylori infection was determined by ELISA. Risk factors were assessed using a structured interview. Two hundred and four individuals were included in the study, including 49 males and 155 females, ranging from 18 to 80 years old. Overall, 165 of 204 participants (80 percent) were H. pylori positive, without significant gender differences (p= 0.49). The infection rate was of 84.7 percent in subjects 18 to 30 years of age, increasing to 92 percent in subjects 46-60 years old. Above 60 years old, the prevalence decreased slightly. As a whole, the prevalence of infection did not increase significantly (p=0.147) with age. There were no significant differences in the prevalence of H. pylori infection, when patients were classified by age, smoking habit, educational level, alcohol consumption, the number of persons per room, the number of children per household, the number of adults per household, cup-sharing, household pets, toilet location, number of persons per bed and medical history of antibiotic and raw vegetable ingestion. In conclusion, no risk factors associated with infection was found in these adults, suggesting that the infection, even in a poor population, may be acquired predominantly during childhood; the relatively high prevalence that we observed may be more due to a cohort effect than to acquisition of infection during adulthood.

ABSTRACT

Post-prostatectomy urinary incontinence is an uncommon complication of adenomectomies, occurring in approximately 1 percent of cases and being more frequent following radical prostatectomies. There is a significant implication in the quality of life for these patients. The surgical techniques employed for its treatment are the implantation of an artificial sphincter, peri-urethral injections and suburethral slings. Considering the low efficacy of peri-urethral injections and the high cost of artificial sphincters, we present in this work a technical modification of the suburethral sling, whose preliminary results are satisfactory. The fundamental modification in this technique is due to the replacement of the synthetic material usually employed for making the sling for autologous tissue, constituted by an aponeurotic strip taken from the rectus muscle of abdomen. This modification aims to minimize risks of urethral erosion that, despite it was not described in this population due to the use of synthetic materials, is a possibility when facing the tension that is used over the bulbar urethra. In addition to such aspects the autologous aponeurosis does not have a cost except for a short prolongation of the surgical act.

ABSTRACT

OBJECTIVE: To evaluate prior mitral surgical commissurotomy and echocardiographic score influence on the outcomes and complications of percutaneous mitral balloon valvuloplasty. METHODS: We performed 459 complete mitral valvuloplasty procedures. Four hundred thirteen were primary valvuloplasty and 46 were in patients who had undergone prior surgical commissurotomy. The prior commissurotomy group was older, had higher echo scores, and a tendency toward a higher percentage of atrial fibrillation. RESULTS: When the groups were compared with each other, no differences were found in pre- and postprocedure mean pulmonary artery pressure, mean mitral gradient, mitral valve area, and mitral regurgitation . Because we found no significant differences, we subdivided the entire group based on echo scores, those with echo scores <=8 and those with echo scores >8 the mitral valve area being higher in the <=8 echo score group 2.06Ý0.42 versus 1.90Ý0.40cm² (p=0.0090) in the >8 echo score group. CONCLUSION: Dividing the groups based on echo score revealed that the higher echo score group had smaller mitral valve areas postvalvuloplasty
