Daño miocárdico pospicadura de escorpión / Myocardial damage after scorpion sting

La picadura de escorpión es una entidad que afecta al mundo entero y llega a ser la segunda causa más importante de envenenamiento de animales en todo el mundo. En Perú, se ha reportado al escorpión Hadruroides charcasus que se encuentra distribuido en los departamentos de la costa norte del país. La fisiopatología que explica la lesión miocárdica no está completamente establecida. Se reporta el caso de un paciente que presento picadura de escorpión y que curse con injuria miocárdica documentada. Se presenta la secuencia de hechos con sus resultados y una revisión de la literatura.

The scorpion sting is an entity that affects the whole world becoming the second largest animal poisoning worldwide. In Peru, it has been reported scorpion Hadruroides charcasus that is distributed in the departments of the north coast. Explaining the pathophysiology of myocardial injury is not fully established. We report a patient who presented scorpion sting and documented myocardial injury. We present the sequence of events with their results and a review of the literature.

Endometriosis pulmonar parenquimal / Parenchymal pulmonary endometriosis

Paciente con hemoptisis a repetición sincrónica con el periodo menstrual. La radiografía y tomografía de tórax fueron normales. La fibrobroncoscopía solo mostró sangrado proveniente del lóbulo superior derecho, de donde se tomaron muestras de biopsia, lavado y cepillado broncoalveolar. El estudio histológico mostró tejido endometrial compatible con endometriosis pulmonar parenquimal. Se trató con un agonista de GnRH (danazol), con control de la hemoptisis. Seis meses más tarde, la paciente se encontraba asintomática.

A patient with recurrent hemoptysis which was synchronous with the menstrual period. The chest X-ray and thoracic CT scan were normal. A bronchoscopy only showed evidence of bleeding from the right upper lobe, where samples were taken for biopsy, bronchoalveolar lavage and brushing. The hystological study showed endometrial tissue compatible with parenchymal pulmonary endometriosis. It was treated with an GnRH agonist (danazol), with control of hemoptysis. Six months later, the patient was asymptomatic.
