ABSTRACT

Objective To investigate the imbalance between T helper 17 (Th17) cells and CD4+CD25+ regulatory T (Treg) cells in the peripheral blood of patients with psoriasis vulgaris and its significance. Methods Peripheral blood were collected from 48 patients with psoriasis vulgaris and 32 normal human controls. Pasoriasis area and severity index (PASI) was used to assess the disease severity in these patients. Flow cytometry was performed to determine the percentage of Th17 and Treg cells in peripheral blood, enzyme linked immunosorbent assay (ELISA) to measure the levels of serum interleukin (IL) -17 and IL-10. Results There was a significant increase in the percentage of Th17 cells [(2.70 ± 1.43)% vs. (0.86 ± 0.25)%, P< 0.01] and serum IL-17 level (90.65 ± 29.61 ng/L vs. 48.82 ± 5.49 ng/L, P < 0.01), but a decrease in the percentage of Treg cells [(3.63 ± 1.14)% vs. (7.87 ± 1.26)%, P< 0.01] and serum IL-10 level (17.78 ± 4.09 ng/L vs. 23.76 ± 3.82 ng/L, P <0.01) in patients with psoriasis vulgaris compared with the normal controls. The ratios of Thl7 to Treg cells and serum IL-17 to IL-10 level were significantly higher (0.95 ± 0.76 vs. 0.12 ± 0.06, 5.78 ± 3.19 vs. 2.16 ±0.68, both P < 0.01) in the patients than in the normal controls. The PASI score in patients was positively correlated with the percentage of Th17, serum level of IL-17, Th17/Treg ratio and IL-17/IL-10 ratio (r = 0.97,0.93, 0.99 and 0.97, all P < 0.01), but negatively correlated with the percentage of Treg cells and serum IL-10 level (r = -0.87, -0.90, both P < 0.01). Conclusion The imbalance between Th17 and Treg cells may play an important role in the pathogenesis of psoriasis vulgaris.

ABSTRACT

Objective To investigate the expression of heparanase in non-melanoma skin cancer including cutaneous squamous cell carcinoma (CSCC) and basal cell carcinoma (BCC) and the relationship among heparanase expression and pathologic grading and lymph nodes metastasis of CSCC. Methods The expressions of heparanase were detected by RT-PCR assay and immunohistochemical staining in 38 cases of CSCC, 33 BCC and 19 normal human skin tissues. Then the relationship between heparanase expression and CSCC clinical pathological features was explored. Results The positive rate ( 68.4%, 60. 5% ) of heparanase mRNA and heparanase in CSCC (24. 2%, 18. 2% ) was significantly higher than those of BCC and normal skin tissue ( 10. 5% ,5.3% ) ( P <0. 01 ) . Heparanase expression rate in CSCC with lymph nodes metastasis (92. 3% ,84.6% ) and Ⅲ~Ⅳ(Broders) (92. 3% ,84. 6% ) was higher than those in tumor without lymph nodes metastasis (56. 0% ,48.0% ) and Ⅰ~Ⅱ (Broders) (56. 0% ,48. 0% ) ( P <0. 05). Conclusions Heparanase was over-expressed in CSCC compared with BCC and normal skin tissues. Expression of heparanase was correlated to pathologic grade and lymph nodes metastasis of CSCC.

ABSTRACT

[Objective]To observe radiographic morphology of femoral head at skeletal maturity of the Salter innominate osteotomy for treatment of patients with Legg-Perthes disease and evaluate sphericity of the femoral head and congruity of the hip.Based on system review of literatures,thes attempted to determine the indications and methods of treatment.[Method]Sixteen patients of Legg-Perthes disease were treated by Salter innominate osteotomy before 1994.The mean age of patients was six years and seven months at operation.Of them,only were 12 patients followed up 10 years and ten months avergely and mean ages of patients were 17 years and 2 months at last follow-up.Growth of plate of femoral head and Y cartilage of acetabulum was closed,indicating skeletal maturity.[Result]According to Stulberg classification of outcome,there were four cases in Stulberg class Ⅰ and six patient in class Ⅱ and one patient in class Ⅲ and one patients in class Ⅳ and no case in class Ⅴ.Lower limb of shortening was averagely 1.5 cm(ranged 0.8~2.0 cm)and the range of motion of hips was almost normal and no abnormal gait was observed.[Conclusion]The Salter innominate osteotomy could keep sphericity of the femoral head and congruity with acetabulum of the hip for children with Legg-Perthes disease and also could be in aid of preventing premature arthritis of hip.

ABSTRACT

Objective To investigate the expression of nerve growth factor receptors, P75 neurotrophin receptor (P75~(NTR)) and tyrosine kinase A (TrkA), and to explore their role in the pathogenesis of psoriasis vulgaris. Methods In situ hydridization and immunohistochemical technique were used respectively to detect the expression of P75~(NTH) and TrkA mRNA and proteins in lesional and non-lesional skins of 38 patients with psoriasis vulgaris (21 cases in progressive stage, 17 cases in static stage), and in normal skins of 10 healthy controls. ABSTRACT

Objective Congenital dislocation of the patella is rare and produces varying degrees of knee disability with the growth of the child. It should be managed as soon as the diagnosis is made. The purpose of this paper is to describe the procedure and to assess the results of congenital dislocation of patella treated with medial transfer of patellar tendon and lateral transfer of medial fascio muscular flap. Methods Five children with congenital dislocation of patella were treated with medial transfer of patellar tendon and lateral transfer of medial fascio muscular flap between June 1994 and June 1998. There were two boys and three girls. The age of the patients at the surgery varied from 4 to 10 years. Four of them were unilateral and one was bilateral without any other deformities. An S-shaped incision is made to expose the distended medial fascio muscular flap, dislocated patella and well developed vastus medialis. The patellar tendon is detached from its insertion and released from the surrounding retinacula. A hole corresponding to the width of the patella is made in the membrane and another one is made through the synovial membrane and the fibrous capsule. The patellar tendon was sutured into the hole in the tibial metaphysis through the two membrane holes. The iliotibial band was lengthened and the biceps tendon was turned over the abundant medial retinaculum with a part of the vastus medialis to the lateral side of the synovial membrane. The patient was kept in a long leg cast and immobilized for 6 weeks. Results The average period of follow up was three years(range, one year and six months to five years). Four cases were able to extend the knee completely and one case had minus ten degrees in extention. Knee valgus in two patients disappeared after operation and one had a lower patella, but the function of knee was normal without any symptoms and signs. There were no complications in this series. Conclusion The patients with congenital dislocation of patella which is permanent and irreducible must be interfered with operation. Five patients were successfully treated with medial transfer of patellar tendon and lateral transfer of medial fascio muscular flap with less invasive and no complications, therefor it is worthwhile to be recommended for the treatment of congenital dislocation of patella.

ABSTRACT

Objective To investigate the molecular mechanism of mood disorders in psoriatic patients.Methods The expression of nerve growth factor receptor p75(NGFR p75)and p140trkA were detected by immunohistochemical technique.The patients' mood was measured with self-rating anxiety scale(SAS)and self-rating depression scale(SDS).Results There is a positive correlation of the expressions of NGFR p75and p140trkA with the patients' mood disorders in psoriatic patients.Conclusion NGF and its receptors are the important substance served as a mediator of the mood disorders affecting psoriatic patients' condition.
