ABSTRACT

ABSTRACT Background: Triatomines infest atypical public spaces in the Western Amazon. Methods: Frequent visitors to these spaces captured the insects in the state of Acre, Brazil (Rio Branco and Cruzeiro do Sul). Results: Six insects were found in a penitentiary, a church, a school, a university, a hospital, and a health center. Five of the insects were adults (three positive for Trypanosoma cruzi) and one was a nymph. Conclusions: This is the first report of triatomine occurrence in schools or churches. These data are important for implementing surveillance strategies and alerting individuals about possible changes in Chagas disease transmission dynamics.

ABSTRACT

ABSTRACT Background: Triatominae are vectors of the protozoan Trypanosoma cruzi, the causative agent of Chagas disease. This study aims to report an infestation on Triatominae colonies by a beetle, previously identified as a pest. Methods: The management of these colonies should be improved to maximize their usefulness, and factors that may cause harm to them should be avoided as much as possible. Results: This is the first report on a coleopteran infestation on living Triatominae colonies worldwide. Conclusions: The present record provides an important warning to researchers who maintain insectaries in general, especially those who rear triatomines, to carry protective measures against such invasions.

ABSTRACT

Abstract Objective To evaluate the postoperative clinical outcomes of the arthroscopic repair of rotator cuff injuries using a suture configuration we have developed based on a modification of the Suture Bridge (Arthrex, Naples, FL, United States). Methods A retrospective study with 28 male (41.2%) and 40 female (58.8%) subjects with a mean age of 60 years. All patients underwent rotator cuff repair with the modified Suture Bridge technique and follow-up for a minimum period of 18 months. The clinical assessment was performed using the University of California, Los Angeles (UCLA) Shoulder Score. Results The mean postoperative range of motion was of 134° (range: 110° to 140°) for elevation, 58° (range: 40° to 70°) for lateral rotation, and T10 (range: L4 to T7) for medial rotation. The mean increase was of 15° for elevation, 14° for lateral rotation, and 2 vertebral levels for medial rotation. The outcomes were excellent in 61 (83.6%) cases, good in 8 (10.9%), and regular in 4 cases (5.5%). Conclusion The modified Suture Bridge technique for the arthroscopic repair of rotator cuff injuries led to excellent or good postoperative clinical outcomes in most cases (69; 94.5%).

ABSTRACT

Abstract Objective The primary aim of the present study is to evaluate the functional results of a modification to the latissimus dorsi (LD) transfer around the shoulder for irreparable posterosuperior rotator cuff tears. The secondary aim is to evaluate variables that might influence the outcomes. Surgical Technique Through a single deltopectoral approach, the LD tendon is detached, reinforced, and elongated with a tendinous allograft, transferred around the humerus, and fixed superolaterally to the greater tuberosity and anteriorly to the subscapularis. Methods Retrospective functional evaluation of 16 cases. The average follow-up was 21 months (12-47). The postoperative results (at last follow-up) were compared with the preoperative ones, as well as to other pre, intra, and postoperative variables. Results All (but one) patients were satisfied. Average University of California, Los Angeles (UCLA) score increased from 11.6 (8-16) to 27.3 (17-30) (p< 0.001). Improvements of shoulder pain, function, and strength achieved statistical significance (p< 0.001). Nonetheless, normal strength was never restored. Average active range of motion improved as follows: forward elevation, from 106° (60-140°) to 145° (130-160°) (p< 0.001); external rotation from 30° (0° to 60°) to 54° (40-70°) (p< 0.001); and internal rotation from L1 (gluteus to T7) to T10 (T12-T3) (p< 0.05). No complication has occurred. Preoperative pseudoparesis was reverted in all the six cases in which it was present. None of the variables analyzed influenced the outcomes, including pseudoparesis. Conclusions At early follow-up, this technique is safe and effective at recovering from pseudoparesis and at improving shoulder pain, function, and strength.

A dor, a função e a força do ombro apresentaram melhora estatisticamente significativa (p< 0,001).

ABSTRACT

ABSTRACT The objectives of this study were to verify the influence of the procedures adopted to contain the Covid-19 pandemic on the levels of physical activity (PA) of children and adolescents, as well as to check if there are any differences between the sexes and age groups. A quick literature review was carried out by means of a search in journals available in the PubMed, Scopus and Web of Science databases, using the following keywords: Children, Adolescents, Physical Exercise, Physical Activity, and Covid-19. Of the 449 studies identified, 17 were selected by the criterion of containing data referring to before and during the social restriction period in the same study. With the exception of one article, the synthesis of results indicated a decrease in PA levels and an increase in time spent on screen activities during the social restriction period, in greater proportions for adolescents and for females. Results are similar among countries in the same region than comparing different continents, due to differences in the sociocultural environment in which young people live, mainly encouraged by their guardians. From a public health point of view, there should be a concern about the continuity of this pattern of behavior after the end of the pandemic, which would further increase the risk of already existing problems, such as delays in motor development, overweight, obesity, cardiovascular diseases and diabetes in adulthood.

ABSTRACT

Abstract The chemical hydroxymethylation of the antimicrobial nitrofurazone leads to the prodrug NFOH, also increases the anti-T. cruzi activities (in vitro and in vivo), as well as showed non-genotoxic (Ames and micronucleus assays). In the present study, we assessed the anti-T. cruzi effect of the NFOH In vivo - in acute Swiss and C57Bl/6 experimental Chagas models. The treatment started at 5 days post-infection during 20 consecutive days (orally, once day, 150mg/kg), and the parasitaemia as well as histopathology analysis were performed. In both experimental murine models, NFOH was able to reduce parasitemia blood avoiding parasitic reactivation, during immunosuppression period (dexamethasone 5mg/kg, 14 days), in 100% of the mice, and decrease tissue parasite nests, demonstrating absence of amastigote forms in all organs (100%) analyzed, data similar to benznidazole (BZN). Therefore, the results shown here pointing to the NFOH as promising compound for further preclinical studies, being a high potential drug to effective and safe chemotherapy to Chagas disease.

ABSTRACT

Abstract INTRODUCTION This study confirms the occurrence of Psammolestes tertius Lent & Jurberg, 1965 (Hemiptera, Reduviidae, Triatominae) in the state of Paraná, Brazil. METHODS: In 2002, a male specimen of P. tertius was collected in the municipality of Porto Rico, Paraná, Brazil. RESULTS: This finding adds to the data on the geographical distribution of P. tertius from 14 to 15 known occurrences in Brazilian states and, therefore, reports the increase in the diversity of triatomines in Paraná. CONCLUSIONS: The presence of P. tertius in the state of Paraná demonstrates that the biodiversity of these insects may have been underestimated.

ABSTRACT

OBJECTIVES: Owing to the fact that obstructive sleep apnea (OSA) is an underreported disease, the strategy used for the diagnosis of OSA has been extensively dissected to devise a simplified process that can be accessed by the public health services. Polysomnography (PSG) type I, the gold standard for the diagnosis of OSA, is expensive and difficult to access by low-income populations. In this study, we aimed to verify the accuracy of the oxyhemoglobin desaturation index (ODI) in comparison to the apnea-hypopnea index (AHI) using a portable monitor. METHODS: We evaluated 94 type III PSG home test results of 65 elderly patients (69.21±6.94 years old), along with information, such as the body mass index (BMI) and sex, using data obtained from a clinical trial database. RESULTS: A significant linear positive correlation (r=0.93, p<0.05) was observed between ODI and AHI, without any interference from sex, BMI, and positional component. The sensitivity of ODI compared to that of AHI increased with an increase in the severity of OSA, while the specificity of ODI in comparison to that of AHI was high for all degrees of severity. The accuracy of ODI was 80.7% for distinguishing between patients with mild and moderate apnea and 84.4% for distinguishing between patients with moderate and severe apnea. CONCLUSION: The ODI values obtained in uncontrolled conditions exhibited high sensitivity for identifying severe apnea compared to the AHI values, and correctly identified the severity of OSA in more than 80% of the cases. Thus, oximetry is promising strategy for diagnosing OSA.

ABSTRACT

ABSTRACT Introduction: Circadian rhythms can impact athletes' sports performance, where the plateau occurs between 15 and 21 hours. Swimming is a peculiar case, as athletes perform training and final sessions in competitions at different times, as in the Rio2016 Olympic Games for example, where the semifinal and final competitions took place from ten o'clock at night. Objectives: (1) to present the protocol of an intervention performed with elite athletes of the Brazilian swimming team during the 2016 Olympic Games in Rio; (2) to find out whether the time at which the competitions were held affected the swimming performances of these athletes during the competition. Materials and Methods: Fourteen athletes of the Brazilian swimming team (males: n= 10; 71% and females: n= 4; 29%) participated in the study. They were followed up during two preparation periods (baseline and intervention) for the 2016 Olympic Games in Rio during June and July 2016. During the competition, we recorded the Reaction Time (RT) and Competition Time (CT) of each athlete in different modalities. The intervention strategies used were light therapy and sleep hygiene. The values of RT at the starting block and CT were registered and conferred with the official results. Results: The athletes showed a decrease in the total time awake (Δ = −13%; Effect size [ES] = 1.0) and sleep latency (Δ = −33%; ES = 0.7), and an increase in total sleep time (Δ = 13%; ES = 1.1; p = 0.04) between the baseline and the period of the intervention, pre-competition. We identified an improvement in the RT (Δ = −2.2% to −1.0%; ES = 0.2 to 0.5) during the competition only for the athletes who participated in the competition finals. Conclusion: We conclude that the intervention carried out was effective in mitigating any negative influence of competition time on the RT and CT of elite athletes of the Brazilian swimming team. Level of evidence II; Prospective comparative study.

ABSTRACT

Abstract Introduction: The genus Rhodnius in the subfamily Triatominae comprises 20 species, which can transmit Trypanosoma cruzi and Trypanosoma rangeli. Due to the development of molecular techniques, Triatominae species can now be characterized by mitochondrial and nuclear markers, making it possible to verify and/or correct the existing data on these species. The results achieved in this study provide a more detailed and accurate differentiation of the Rhodnius species, helping the establishment of a more appropriate classification. Methods: Data collection was performed by DNA analysis, morphological and morphometric studies to distinguish four populations of R. neglectus and four of R. prolixus. Phylogenetic data were compared to morphological and morphometric data. Results: The analysis of Cytb fragments suggests that the four colonies designated to Rhodnius neglectus as well as those of R. prolixus were correctly identified. Conclusions: The morphological characters observed in the specimens of the colonies originally identified as R. prolixus and R. neglectus, such as the presence or absence of collar in the eggs, the patterns of the median process of the pygophore, and anterolateral angle, are consistent with the species. Geometric morphometrics also show an intraspecific variability in R. prolixus.

ABSTRACT

Fractures of ankles are very common injuries of the musculoskeletal system, causing major repercussions on their function and can cause joint disorders such as instability, limitation of motion, joint incongruity and secondary osteoarthritis (post-traumatic). Immobilization post fracture generates adverse effects such as joint contracture and muscle and bone atrophy. This study is a case report conducted at the Polyclinic and Aesthetic Center Duque de Caxias (Unigranrio School Clinic), with a male patient who suffered fracture of tibia and distal fibula in the right lower limb. We evaluated the degree of amplitude of tibiotalar joint, the degree of muscle strength, girth and pain intensity. The objective was to demonstrate the importance of holding the perimeter of ankle after fracture legs as a factor that predisposes to musculoskeletal complications, not being this factor, patient grievance. It was concluded that as many studies show the existence of post-fracture atrophy, measuring the perimeter becomes important factor in preventing possible compensatory dysfunctions of the musculoskeletal system. ABSTRACT

(AU)

There is a trend of progressive aging of population in Brazil and in the world, as well as the Social Anxiety Disorder, which compromises quality of life and becomes a risk factor for dementia processes and collaborates with functional limitation. This study aimed to describe possible impacts of this disorder in the aging process, seeking to know the future of this elderly population living with disabilities characteristics of age and possibly potentiated by social phobia. This study consisted of an integrative review of the literature, using articles that involve social phobia and depression in the elderly. The research of articles was carried out in databases of the Virtual Health Library and in the portals indexed as: Bireme, Lilacs, Scielo, and Pubmed, in the period from August 2002 to March 2018. We observed in literature that among the psychotherapy modalities, the cognitive-behavioral therapy is the most efficient treatment for social phobia; on the other hand the psychodynamic group psychotherapy show signs of being a viable treatment for individuals that experience generalized social phobia. We concluded that new studies will prove the deterioration of elderly functions with Social Anxiety Disorder. (AU)

ABSTRACT

Abstract INTRODUCTION This communication reports the occurrence of Rhodnius neglectus in the State of Acre, Brazil. METHODS An adult male R. neglectus specimen was collected from the Catuaba Experimental Reserve, which is located in the municipality of Senador Guiomard. RESULTS This increases the number of triatomine species reported from the State of Acre to eight and is also the first report of R. neglectus from the Brazilian Western Amazon Region. CONCLUSIONS The occurrence of R. neglectus is alarming because even though the species is wild, it can invade and colonize human dwellings and peridomiciles, thereby increasing Chagas disease transmission.

ABSTRACT

Abstract Chagas disease is a protozoan infection that was identified over a century ago. No drugs are available to treat the indeterminate and determinate chronic phases of the disease. Success of a drug design is dependent on correct biological evaluation. Concerning new drug designs for Chagas disease, it is essential to first identify the most effective, existing, experimental chronic protocols that can be used for comparison purposes. Here, we present a literature review regarding experimental models with chronic Chagas disease to evaluate the efficacy of benznidazole (BZN). We searched literature published in PubMed and Web of Science databases, using these keywords: animal model, BZN, Chagas disease, T. cruzi, and chronic phase, with no timeframe limitations. We excluded articles involving acute phase animal models and/or those without BZN treatment. The selected studies were conducted using different BZN concentrations (10mg-100mg) involving several different periods (5-70 days). Concentrations and durations of use are directly related to side effects, but do not prevent chronic tissue lesions. BZN use during the late/chronic phases of Chagas disease is unable to eliminate amastigote forms present in infected tissues. This study suggests the administration of a lower BZN concentration (<100mg/kg/day) during the chronic phase of the animal model, as this had been reported to result in fewer side effects.
