ABSTRACT

Objective To establish a method and study the pharmacokinetics for concentration determination of effective components in Xiakucao Xiaoliu mixture in Normal Rat Plasma By LC-MS/MS. Methods The mobile phase was methanol-water (0.1% formic acid) system under the positive ion mode of C18 chromatographic column, gradient elution was adopted, and the flow rate was 0.3 ml/min. In the negative ion mode, the mobile phase was acetonitrile-water (0.1% formic acid) system, gradient elution, with a flow rate of 0.4 ml/min. Caffeic acid, rosmarinic acid, syringic acid, rutin in positive ion mode and Atractylodes lactone II and Atractylodes lactone III in negative ion mode were respectively determined. Normal rats were intragastrically given Xiakucao Xiaoliu Mixture 7.8 ml/kg, and blood was taken from the orbit at different time points after the administration. The blood concentration was determined by the validated LC-MS/MS method and the non-standard DAS2.0 software was used. The pharmacokinetic parameters of rats after administration were calculated by the compartment model. Results The pharmacokinetic parameters belonged to non atrioventricular model. The pharmacokinetic characteristics of the four main anti-cancer active ingredients of Caffeic acid, Rosmarinic acid, Syringic acid and Atractylodes Ⅲ in rats after administration of Xiakucao Xiaoliu Mixture were significantly different from those reported in the literature after the administration of monomers. Conclusion The established method was simple, accurate and sensitive, which could be suitable for the content determination of effective components in Xiakucao Xiaoliu mixture in Normal Rat Plasma, which would be a valuable information for the study on main anticancer active substances.

ABSTRACT

Objective To study the curative effects of traditional Chinese medicine paste combined with Baduanjin in treatment of osteoporotic vertebral compression fracture (OVCF) after percutaneous vertebroplasty (PVP). Methods 120 OVCF patients treated with PVP in our hospital from January 2016 to September 2017 were divided into the observation group (60 cases) and the control group (60 cases) according to the random number table method. The control group was given calcium carbonate D3 chewable tablets orally with routine guidance. In addition to the same treatment as the control group, the observation group received the traditional Chinese medicine paste orally with Baduanjin exercise. Both groups were treated for 6 months and followed-up for 3 years. The curative effects in the two groups after 6 months treatment and the low back pain after 1, 3 and 6 months of treatment were recorded. The changes of bone mineral density (BMD), kyphosis angle (Cobb angle), anterior wall height of vertebral body (AVBH) and level of bone metabolism indexes in the two groups were compared before and after treatment for 6 months. The follow-up times and the incidences of push-back fracture after PVP during follow-up were recorded. Results After 6 months of treatment, the clinical cure rate of the observation group was 73.33%, which was higher than 53.33% of the control group(P<0.05). Compared with pretreatment, the scores of visual analogue scale (VAS) in the two groups gradually decreased after 3 and 6 months of treatment, and the observation group had a lower scores than the control group (P<0.05). After 6 months treatment, BMD and AVBH of lumbar vertebrae and femoral neck in both groups increased, and the observation group was higher than that in the control group. The Cobb angle and serum levels of Type I procollagen degradation products (β-Cross I), the n-terminal middle osteocalcin (N-MID Ost) and parathyroid hormone (PTH) decreased in both groups, and the observation group was lower than those in the control group (P<0.05). There was no significant difference in fracture incidence after PVP in the year 1, year 1 to 3 follow up between the two groups (P>0.05). During the 3 years follow-up, the incidence of push-body fracture after PVP in the observation group was 3.33%, which was lower than that in the control group 20.00%( P<0.05). Conclusion Traditional Chinese medicine paste combined with Baduanjin reduced the serum levels of β-Cross I, N-MID Ost and PTH, regulated bone metabolism, improved BMD and AVBH of lumbar vertebrae and femoral neck, reduced Cobb angle, promoted the recovery of lumbar function, alleviated patients' back pain, lowered the incidence of push-body fracture after PVP. The curative effects were remarkable.

The concept of Shanghai regional culture, Shanghai style Chinese medicine and Shanghai herbal paste were introduced by the combination of literature search and practical experience. The historical developments of Shanghai herbal paste were summarized in order to understand Shanghai herbal paste comprehensively and provide a historical and empirical basis for the inheritance and innovation of Shanghai herbal paste.

ABSTRACT

Objective:To explore the application value of a novel ultrasonic scale in the evaluation of sub-massive pulmonary embolism (sub-PE).Methods:Retrospective analyses were conducted in 137 patients with acute pulmonary embolism confirmed by computed tomography pulmonary angiography (CTPA) in the second affiliated hospital of Harbin Medical University from September 2017 to June 2019. They were divided into experimental set (77 cases) and testing set(60 cases). According to the European Society of Cardiology (ESC) guidelines for pulmonary embolism in 2019, the patients were classified into sub-PE(71 cases) and small pulmonary embolism(small-PE, 66 cases). Parameters in the experimental set were screened by statistical methods to make an ultrasonic scale, and then the patients in the testing set were scored by the scale. ROC curve was plotted to calculate the diagnostic efficacy of the ultrasonic scale for sub-PE.Results:①The patients of sub-PE were older than small-PE patients [(61.57±1.45) years vs (56.31±1.59) years, P=0.016], but there was no significant difference between the two groups in other general data( P>0.05). ②Within the 77 pulmonary embolism patients in the experimental set, 41(53.25%) were classified as the sub-PE and 36(46.75%) as the small-PE. Compared with the small-PE group, right ventricular diameter, pulmonary artery trunk diameter, right ventricle/left ventricle ratio, tricuspid regurgitation velocity, the Tei index and the inferior vena cava diameter in the sub-PE group increased significantly ( P<0.05), and right ventricular wall motion amplitude, right atrium area rate, right ventricular area rate, tricuspid annulus systolic displacement, collapse rate of inferior vena cava and pulmonary artery blood flow acceleration time decreased significantly ( P<0.05); ③Fifteen ultrasonic parameters were used in the ultrasonic scale. The scale in sub-PE group of the testing set was significantly increased compared with that in small-PE group (11.63±3.87 vs 4.43±1.96, P<0.001). ROC showed that the AUC in diagnosing sub-PE by ultrasonic scale was 0.96. When the cut-off value was 6.5, the sensitivity and specificity were 90.00% and 83.33%, respectively. Conclusions:The novel ultrasonic scale can provide a comprehensive and feasible ultrasound imaging method for evaluation of sub-PE.

ABSTRACT

Increased serum urotensin II (UII) levels in human cirrhotic populations have been recently shown, but the long-term effects of UII receptor antagonist on the cirrhosis have not been investigated. To investigate the therapeutic effects of urotensin II receptor (UT) antagonist palosuran on rats with carbon tetrachloride (CCl)-induced cirrhosis, the hepatic and systemic hemodynamics, liver fibrosis, the metalloproteinase-13 (MMP-13)/tissue inhibitor of metalloproteinase-1 (TIMP-1) ratio, hepatic Rho-kinase activity, and the endothelial nitric oxide synthase (eNOS) activity are measured in CCl-cirrhotic rats treated with palosuran or vehicle for 4 weeks. Primary hepatic stellate cells (HSCs) are used to investigate the changes in UII/UT expression and the in vitro effect of palosuran. Compared with the vehicle-treated cirrhotic rats, treatment with palosuran can reduce the portal pressure (PP), decrease the risk of liver fibrosis and the level of α smooth muscle actin, collagen-I (COL-I), and transforming growth factor β expression. However, treatment with palosuran can increase MMP-13/TIMP-1, pvasodilator-stimulated phosphoprotein (p-VASP), and p-eNOS expression. Moreover, in vitro UII/UT mRNA expression increases during HSC activation. MMP-13/TIMP-1, COL-I, and p-VASP are inhibited after palosuran treatment. Our data indicate that long-term administration of palosuran can decrease PP in cirrhosis, which results from decreased hepatic fibrosis and enhanced eNOS-dependent HSC vasodilatation.

Excess fat mass accumulation can lead to a wide range of metabolic and cardiovascular complications.It includes immune cell infiltration and altered secretion of anti-and proinflammatory mediators,which contribute to systemic,low-grade inflammation.Bariatric surgery may reverse the altered secretory profile observed in the adipose tissue of severe obesity patients.This article will review available literature documenting the impact of surgeryinduced weight loss on tissue expression or circulating levels of a broad spectrum of inflammatory mediators.

ABSTRACT

Objective To assess the evidence-based support of short-term efficacy of sleeve gastrectomy and Roux-en-Y gastric bypass in obesity with metabolic syndrome by laparoscopy.Methods Electronic literature search was performed on PubMed,EMBASE,Cochrane Library,CNKI database for the efficacy of sleeve gostrectomy and Roux-en-Y gastric bypass in the treatment of obese patients with metabolic syndrome from Jan.2011 to Jun.2016.The inclusion and exclusion criteria were selected for quality evaluation,data extraction.Review Manager 5.3 software was used to conduct the statistical analysis.Results Thirteen articles were included in the Meta-analysis,including 5 randomized controlled trails,eight case-control studies with 18,850 cases,including 2,559 cases in the sleeve gastrectomy group and 16,291 cases in the Roux-en-Y gastric bypass group.The results of the Meta-analysis showed that there was no significant difference in the remission rate of type 2 diabetes rnellietus between sleeve gastrectomy and Roux-en-Y gastric bypass group (78.7% vs 83.0%;RR =0.94,95% CI:0.81 ~ 1.09,P >0.05),the reduction of body mass index was significantly higher in Roux-en-Y gastric bypass group than sleeve gastrectomy group from the short-term follow-up (MD =-2.03,95% CI:-3.25 ~ 0.81,P < 0.05).Both two surgeries can reduce the incidence of high blood lipids and hypertension,but there was no significant difference between them as well as type 2 diabetes mellitus remission.Conclusion Both sleeve gastrectomy and Roux-en-Y gastric bypass are effective in treating obesity complicated with metabolic syndrome,and can reduce the incidence of type 2 diabetes mellitus,hypertension and hyperlipidemia and reduce the body weight.Roux-en-Y gastric bypass is superior to sleeve gastrectomy in the treatment of obesity.

ABSTRACT

Objective To discuss the clinical features of behavioral and psychological symptoms of demen-tia(BPSD)and the relation between different sub-clinical syndromes and cognition. Methods One hundred and sixteen dementia patients were assessed with neuropsychiatric inventory and mini-mental state examination (MMSE)and made factor analysis according to DSM-IV-R. Results Twelve common behavioral and psychological symptoms could be further divided into five sub-syndromes,including disinhibition behavior,psychosis,agitation, emotion and apathy factors. MMSE total score and years of education entered regression equation of apathy factor (P<0.05). Conclusion BPSD can be divided into five factors and apathy factor are related with cognitive function.

Objective:To establish the quality standard for Qinzhu Liangxue mixture. Methods:Seutellaria baiealensis, glyeyrrhizae and jobstears seed were identified by TLC. Baicalin and glycyrrhizic acid were determined by HPLC. Results:Seutellaria baiealensis,glyeyrrhizae and jobstears seed could be identified by TLC. The linear range of baicalin was 0. 030- 0. 971 mg·ml -1(r = 1. 000 0),and the average recovery was 97. 74%(RSD = 2. 76% ,n = 9). The linear range of glycyrrhizic acid was 0. 013- 0. 220 mg·ml -1(r = 1. 000 0),and the average recovery was 99. 02%(RSD = 1. 79% ,n = 9)Glycyrrhizic acid. Conclusion:The method is specific,simple and accurate. It can be used for the quality control of Qinzhu Liangxue mixture.

ABSTRACT

Objective To develop a novel skin microdialysis technology in vivo,and to determine the pharmacokinetic of ba-icalin after transdermal administration in rats. Methods An HPLC-MS/MS method used for the determination of baicalin in skin mi-crodialysis samples was established ,SD rats were pretreated with skin microdialysis operation under anesthesia , and then the baicalin gel was applied to the skin surface of probe in vivo.The baicalin concentration of skin microdialysates was determined , the time curve of baicalin concentration was drawn and the topical pharmacokinetics parameters of percutaneous absorption was calculated. Results Baicalin was optimized at the transitions m/z 447.3→271.2.The linearity correlation was good and the assay exhibited good precision and accuracy.The subcutaneous probe recovery of baicalin in vivo was(24.40 ±0.91)%and was stable over the 240 min study peri-od.Baicalin could be detected in the microdialysis samples after transdermal administration , and its concentration continued to rise in 8 h.AUC0-t in skin tissue was(50.04 ±34.17) mg· min· L-1. Conclusion The method of skin microdialysis in vivo could be used in the local pharmacokinetic research of baicalin.

ABSTRACT

Objective To establish a rat model of hyperlipidemia and analyze the effect of conjugated linoleic acid (CLA) on lipid metabolism and expression of visfatin gene. Method High fat diet was used to establish the hyperlipidemia rat model. Blood was taken at four weeks after high fat diet feeding to analyze the level of glucose (GLU), cholesterol (CHO) and triglyeride(TG). The rats were divided into experimental group and control group after the hyperlipidemia rat model was established successfully. The experimental group rats were treated with CLA(0.8 mL /0.1 kg)orally for four weeks. The food intake and body weight were recorded. The rats were sacrificed, and both body fat and serum lipid levels were measured. Semi-quantitative RT-PCR was used to measure the expression level of visfatin mRNA. Result The hyperlipidemic rat models were induced by high fat diet successfully. The body weight, food intake and body fat in the rats of CLA experiment group were significantly decreased compared with that of the control group (P< 0.05). The level of GLU, CHO, TG and LDL in the experimental group were significantly lower than that in the control group (P< 0.05), but the serum HDL-C was increased in the experimental group. Semi-quantitative RT-PCR demonstrated that the expression level of visfatin gene of the experimental group was lower than that in the control group. Conclusion CLA can reduce the expression of visfatin gene and improve the lipid metabolism in hyperlipidemic rats.
