CIE-10 y DSM-III-R: Semejanzas y diferencias / ICD-10 & DSM-III-R: Similarities and differences

Se presentan las categorías diagnósticas de la décima revisión de la Clasificación Internacional de Enfermedades (CIE-10) y la tercera edición revisada del Manual Diagnóstico y Estadístico de Trastornos Mentales de la Asociación Psiquiátrica Americana (DSM-III-R), para pacientes adultos y geriátricos, señalando sus semejanzas y diferencias. Asimismo, se comentan las bondades y deficiencias de cada una de ellas. Finalmente, se resumen las diferencias entre la CIE-10 y el DSM-III-R.

The diagnostic categories corresponding to adult and geriatric patients of the International Classification of Diseases, tenth Revision (ICD-10), and the American Psychiatric AssociationÆs Diagnostic and Statistical Manual of Mental Disorders, third edition revised (DSM-III-R) are presented. The similarities and differences are pointed out. Likewise, the goodness and deficiencies of each classification are commented. Finally, the differences between ICD-10 and DSM-III-R are summarized.

Proposición de un modelo de historia clínica semiestructurada para dependientes de drogas / Proposition of a model of semi-structured clinical history of drug dependent

En base a la experiencia clínica acumulada en el Departamento de Farmacodependencia del Instituto Nacional de Salud de Mental Honorio Delgado-Hideyo Noguchi, se elabora un modelo holístico de historia clínica semiestructurada, flexible, para dependientes de drogas, con el objeto de aportar un procedimiento instrumentado de estudio sistemático e integral de la adicción química. La historia clínica semiestructurada viabiliza una entrevista estandarizada programada así como la evaluación comprehensiva de la adicción con fines diagnósticos, terapéuticos, de rehabilitación, y de reeducación integral del paciente y su entorno familiar y ambiental. Ello posibilita el registro de información homogénea orientada a la creación del banco de datos. La formulación diagnóstica se basa en las categorías clínicas de la CIE-10 y DSM III-R. El formato también cubre aspectos terapéuticos, de seguimiento y alta. La prueba piloto se efectuó en 50 pacientes adictos a múltiples drogas lícitas e ilícitas. El instrumento contiene 23 secciones estructuradas siguiendo. Mayormente, el orden de la historia clínicas convencional. Se aplica en tiempos sucesivos, conforme evoluciona el paciente hasta el alta. Pata efectos de elaborar el formato final, próximamente, el instrumento será sometido al juicio de expertos.

Based on the accumulated clinical experience of the Pharmacodependence Department of the National Institute of Mental Health Honorio Delgado-Hideyo Noguchi, a holistic, flexible, semistructured clinical history was produced to study drug dependent persons with the aim of developing an instrumental procedure for the systematic and comprehensive study of chemical addiction. The semi-structured clinical history permits a standardized interview faciliting the comprehensive evaluation of the addiction disorder with diagnostic, rehabilitative and integral re-education aims for the patient within his family setting and environment. Recollection of homogeneous clinical information will be used for the creation of a data bank. Diagnostic formulation is based on CIE-10 and DSM-III-R. The format also covers areas of therapeutic details, follow-up and discharge. The clinical instrument contains 23 sections, mostly following the order of a conventional clinical history; the pilot test was conducted on addictive patients who have used multiple drugs, licit and illicit ones. The instrument was applied at successive times recording the evolution and progress of the patient until his discharge. For purposed of producing and final format, the instrument will be finally submitted to the opinion of experts.
