ABSTRACT

BACKGROUND: The risk factors for the development of plantar fasciitis (PF) have been associated with the medial longitudinal arch (MLA), rearfoot alignment and calcaneal overload. However, the relationships between the biomechanical variables have yet to be determined. OBJECTIVE: The goal of this study was to investigate the relationships between the MLA, rearfoot alignment, and dynamic plantar loads in runners with unilateral PF in acute and chronic phases. METHOD: Cross-sectional study which thirty-five runners with unilateral PF were evaluated: 20 in the acute phase (with pain) and 15 with previous chronic PF (without pain). The MLA index and rearfoot alignment were calculated using digital images. The contact area, maximum force, peak pressure, and force-time integral over three plantar areas were acquired with Pedar X insoles while running at 12 km/h, and the loading rates were calculated from the vertical forces. RESULTS: The multiple regression analyses indicated that both the force-time integral (R2=0.15 for acute phase PF; R2=0.17 for chronic PF) and maximum force (R2=0.35 for chronic PF) over the forefoot were predicted by an elevated MLA index. The rearfoot valgus alignment predicted the maximum force over the rearfoot in both PF groups: acute (R2=0.18) and chronic (R2=0.45). The rearfoot valgus alignment also predicted higher loading rates in the PF groups: acute (R2=0.19) and chronic (R2=0.40). CONCLUSION: The MLA index and the rearfoot alignment were good predictors of plantar loads over the forefoot and rearfoot areas in runners with PF. However, rearfoot valgus was demonstrated to be an important clinical measure, since it was able to predict the maximum force and both loading rates over the rearfoot.

ABSTRACT

This study compares surface electromyographic activity of the internal oblique, rectus abdominis, multifidus, iliocostalis, anterior deltoids during the pull-up on a lower and on a higher difficulty level. We assessed nine adults with previous experience in Pilates. The root mean square (RMS) values were normalized by maximum isometric contraction for each participant. During the ascent phase, the low spring position showed a significantly higher RMS than the high spring position of 8.9% for deltoid, 17.2% for internal oblique, 22.3% for rectus abdominis, 4.1% for iliocostalis, and 5.6% for multifidus, and in the descent phase, the RMS in the lower spring exceeded significantly the high spring position in 1.6% for the deltoid, 10% for internal oblique, 31.4% for rectus abdominis and 11.4% for iliocostalis. There was no predominance of abdominal muscles over the shoulder muscle in any spring position. Este estudo compara a atividade eletromiográfica de superfície dos músculos oblíquo interno, reto abdominal, multífidos, iliocostal e deltóide anterior durante o pull-up em dois níveis de dificuldade (mola alta e mola baixa). Foram avaliadas nove adultos com experiência anterior em Pilates. Os valores RMS foram normalizados pela contração isométrica máxima. Durante a fase de subida, a posição de mola baixa mostrou RMS significativamente maiores em relação a alta de 8,9% para deltóide, 17,2% para o oblíquo interno, 22,3% para o reto abdominal, 4,1% para iliocostal, e 5,6% para o multífido, e na fase de descida, em 1,6% para o deltóide, 10% para oblíquo interno, 31,4% para o reto abdominal e 11,4% para o iliocostal. Não houve predomínio dos músculos abdominais sob o músculo do ombro em qualquer posição de mola. Este estudio compara la EMG superficial de los músculos recto del abdomen, oblicuo interno, multifidos, ilio-costal y deltoides anterior durante el ejercicio pull-up en dos niveles de dificultad. Se evaluaron a nueve adultos experimentados en Pilates. Los valores de RMS se normalizaron por la contracción isométrica máxima. Durante la fase de ascenso, la posición baja del resorte mostró valores significativamente majores de RMS que la posición alta de 8,9% para lo deltoides, 17,2% para oblicuo interno, 22,3% para recto abdominal, 4,1% para ilio-costalis, y 5,6% para multifidos. En la fase de descenso, el RMS, en el muelle inferior, excede significativamente la posición alta del resorte en 1,6% para el deltoides, 10% para oblicuo interno, 31,4% para recto abdominal y 11,4% para ilio-costalis. BACKGROUND: The progression of diabetes and the challenge of daily tasks may result in changes in biomechanical strategies. Descending stairs is a common task that patients have to deal with, however it still has not been properly studied in this population. OBJECTIVES: We describe and compare the net joint moments and kinematics of the lower limbs in diabetic individuals with and without peripheral neuropathy and healthy controls during stair descent. METHOD: Forty-two adults were assessed: control group (13), diabetic group (14), and neuropathic diabetic group (15). The flexor and extensor net moment peaks and joint angles of the hip, knee, and ankle were described and compared in terms of effect size and ANOVAs (p<0.05). RESULTS: Both diabetic groups presented greater dorsiflexion [large effect size] and a smaller hip extensor moment [large effect size] in the weight acceptance phase. In the propulsion phase, diabetics with and without neuropathy showed a greater hip flexor moment [large effect size] and smaller ankle extension [large effect size]. CONCLUSION: Diabetic patients, even without neuropathy, revealed poor eccentric control in the weight acceptance phase, and in the propulsion phase, they showed a different hip strategy, where they chose to take the leg off the ground using more flexion torque at the hip instead of using a proper ankle extension function.


CONTEXTUALIZAÇÃO: A progressão do Diabetes Mellito e as atividades desafiadoras do dia a dia podem resultar em mudanças da estratégia biomecânica adotada. Descer escadas é uma tarefa comum do dia a dia, vivenciada pelos pacientes, mas ainda não foi satisfatoriamente estudada nessa população. OBJECTIVOS: Descrever e comparar os momentos articulares e a cinemática de membros inferiores em indivíduos diabéticos com e sem a neuropatia periférica e controles saudáveis durante o descer escadas. MÉTODO: Quarenta e dois adultos foram avaliados: grupo controle (13), grupo diabético (15) e grupo de diabéticos neuropatas (14). Os picos flexores e extensores dos momentos articulares e os ângulos articulares de quadril, joelho e tornozelo foram comparados e descritos por análise do tamanho do efeito e ANOVAs (p<0,05). RESULTADOS: Na fase de aceitação do peso, ambos os grupos diabéticos apresentaram maior ângulo de dorsiflexão de tornozelo [tamanho de efeito grande] e menor momento extensor de quadril [tamanho de efeito grande]. Na fase de propulsão, diabéticos com e sem a neuropatia apresentaram maior momento flexor de quadril [tamanho de efeito grande] e menor ângulo de extensão de tornozelo [tamanho de efeito grande]. CONCLUSÃO: Pacientes diabéticos, mesmo antes da neuropatia instalada, revelaram um pobre controle excêntrico na fase de aceitação do peso e, na fase de propulsão, esses pacientes mostraram uma estratégia diferente ao levar o membro inferior à frente a partir de um maior torque flexor de quadril ao invés de usar uma função extensora apropriada de tornozelo.

ABSTRACT

OBJECTIVE: This study proposes a new approach that considers uncertainty in predicting and quantifying the presence and severity of diabetic peripheral neuropathy. METHODS: A rule-based fuzzy expert system was designed by four experts in diabetic neuropathy. The model variables were used to classify neuropathy in diabetic patients, defining it as mild, moderate, or severe. System performance was evaluated by means of the Kappa agreement measure, comparing the results of the model with those generated by the experts in an assessment of 50 patients. Accuracy was evaluated by an ROC curve analysis obtained based on 50 other cases; the results of those clinical assessments were considered to be the gold standard. RESULTS: According to the Kappa analysis, the model was in moderate agreement with expert opinions. The ROC analysis (evaluation of accuracy) determined an area under the curve equal to 0.91, demonstrating very good consistency in classifying patients with diabetic neuropathy. CONCLUSION: The model efficiently classified diabetic patients with different degrees of neuropathy severity. In addition, the model provides a way to quantify diabetic neuropathy severity and allows a more accurate patient condition assessment.

BACKGROUND: Patellofemoral Pain Syndrome is one of the most common knee disorders among physically active young women. Despite its high incidence, the multifactorial etiology of this disorder is not fully understood. OBJECTIVES: To investigate the influence of Patellofemoral Pain Syndrome on plantar pressure distribution during the foot rollover process (i.e., the initial heel contact, midstance and propulsion phases) of the gait. MATERIALS AND METHODS: Fifty-seven young adults, including 22 subjects with Patellofemoral Pain Syndrome (30 ± 7 years, 165 ± 9 cm, 63 ± 12 kg) and 35 control subjects (29 ± 7 years, 164 ± 8 cm, 60 ± 11 kg), volunteered for the study. The contact area and peak pressure were evaluated using the Pedar-X system (Novel, Germany) synchronized with ankle sagittal kinematics. RESULTS: Subjects with Patellofemoral Pain Syndrome showed a larger contact area over the medial (p = 0.004) and central (p = 0.002) rearfoot at the initial contact phase and a lower peak pressure over the medial forefoot (p = 0.033) during propulsion when compared with control subjects. CONCLUSIONS: Patellofemoral Pain Syndrome is related to a foot rollover pattern that is medially directed at the rearfoot during initial heel contact and laterally directed at the forefoot during propulsion. These detected alterations in the foot rollover process during gait may be used to develop clinical interventions using insoles, taping and therapeutic exercise to rehabilitate this dysfunction.

ABSTRACT

Objetivos: Correlacionar a postura dos pés com o arco plantar de adolescentes usuárias e não usuárias de calçados de salto alto. MÉTODOS: Foram selecionadas 36 adolescentes, 16 no grupo de não usuárias e 20 no grupo de usuárias, com idade entre 13 e 20 anos. A postura do pé foi analisada por fotos nas condições descalça e com calçado de salto alto tipo Anabella, previamente padronizado, após terem permanecido com ele por uma hora. Sua análise foi realizada pelo software SAPO. A impressão plantar foi realizada descalça e, a partir dela, calculado o Índice de Chipaux - Smirak para classificação do arco plantar. Para análise estatística, utilizou-se o teste t pareado para verificar igualdade entre lados direito e esquerdo. Foi realizado o teste de aderência Shapiro Wilk e, então, a análise inferencial por meio dos testes não paramétricos de Wilcoxon, o teste de Mann-Whitney e a correlação de Spearman. O nível de significância adotado foi de 0,05. RESULTADOS: Não foi encontrada correlação entre o tipo de arco plantar e a postura do pé das adolescentes estudadas. Porém, o ângulo do retropé se mostrou significativamente diferente, apresentando varo de retropé após a colocação do calçado em ambos os grupos, e o arco plantar do grupo de usuárias apresentou valores menores quanto ao Índice Chipaux - Smirak. CONCLUSÕES: Não existe correlação entre a postura do pé e o tipo de arco plantar, embora essas variáveis tenham sofrido influência do calçado de salto alto. Objectives: To investigate the relationship between foot posture and plantar arch among adolescent wearers and non-wearers of high-heeled shoes. METHODS: Thirty-six female adolescents aged 13 to 20 years were selected and grouped as 16 high-heel non-wearers and 20 high-heel wearers. Foot posture was analyzed using photos, firstly barefoot and, secondly, after wearing previously standardized high-heeled platform shoes for an hour. The analysis was performed using the software SAPo. Barefoot impressions were taken, and the Chipaux-Smirak Index was calculated to classify the plantar arch of the foot. For statistical analyses, the paired t test was used to investigate equality between the right and left sides. The Shapiro-Wilk adherence test was performed, followed by inferential analysis using the non-parametric Wilcoxon test, the Mann-Whitney test and Spearman's correlation coefficient. The significance level used was 0.05. RESULTS: There was no correlation between the type of plantar arch and foot posture among the female adolescents studied. However, the rearfoot angle was significantly different, with rearfoot varus after wearing the shoes in both groups. The plantar arch in the wearers group presented smaller values for the Chipaux-Smirak Index. CONCLUSIONS: There is no correlation between foot posture and the type of plantar arch, although these variables are influenced by high-heeled shoes. Na prática fisioterapêutica, pressupõe-se que alterações posturais do membro inferior influenciem a biomecânica e função dos demais complexos durante o movimento. No entanto, a literatura sobre a relação entre desalinhamentos estáticos do joelho e possíveis alterações dinâmicas ainda é escassa e inconclusiva. Assim, buscou-se avaliar o efeito do alinhamento frontal estático do joelho sobre a distribuição da pressão plantar durante a marcha. Foram avaliados inicialmente 44 adultos jovens assintomáticos. Por fotogrametria digital, mediu-se o ângulo frontal do joelho, classificado como normal (170° a 175°), valgo <170º e varo >175°. Dado o baixo número de valgos, foram analisados dois grupos: de joelhos normais (n=18) e de joelhos varos (n=23). A distribuição da pressão plantar foi avaliada durante a marcha em cinco áreas. Os grupos mostraram-se estatistica-mente semelhantes em todas as variáveis cinéticas avaliadas em todas as áreas plantares. Joelhos normais apresentaram significativa correlação com o tempo de contato no antepé lateral e médio-pé; e os varos, correlação com a área e tempo de contato em duas e três áreas plantares, e com a pressão integral no antepé lateral. Os resultados mostraram que o desalinhamento frontal de 3° do joelho, embora com moderada correlação, não influencia a distribuição de cargas na superfície plantar durante a marcha. Sugere-se pois que a avaliação clínica não se limite à avaliação articular estática do joelho, mas inclua atividades dinâmicas...


In physical therapy practice, it is assumed that lower-limb posture changes may influence other complexes’ biomechanics or function during movement. However, literature on the relationship between static knee alignment and possible dynamic changes is still scarce and inconclusive. This study assessed the effect of static frontal knee alignment on plantar pressure distribution during gait. At first 44 young asymptomatic adults were evaluated. Front knee angles were measured by digital photogrammetry and classified as normal (170° to 175°), valgus <170°, or varus >175°. Given the low number of valgus found, two groups were analysed: normal knees (n=18) and varus knees (n=23). Plantar pressure distribution was measured during gait in five plantar surface areas. Groups were found to be statistically similar in all kinetic variables assessed in all plantar surface areas. Normal knees showed significant correlation with the contact time at lateral forefoot and midfoot. Varus knees showed correlation with contact area and time at respectively two and three plantar areas, and with full pressure at lateral forefoot. Results thus show that a 3° frontal knee malalignment, though with moderate correlation, does not influence pressure distribution on plantar surface during gait. It is hence suggested that clinical evaluation should not be limited to static knee assessment, but rather include dynamic activities.

ABSTRACT

OBJECTIVE: To investigate and compare the influence of a previous history of foot ulcers on plantar pressure variables during gait of patients with diabetic neuropathy. INTRODUCTION: Foot ulcers may be an indicator of worsening diabetic neuropathy. However, the behavior of plantar pressure patterns over time and during the progression of neuropathy, especially in patients who have a clinical history of foot ulcers, is still unclear. METHODS: Subjects were divided into the following groups: control group, 20 subjects; diabetic neuropathy patients without foot ulcers, 17 subjects; and diabetic neuropathy patients with at least one healed foot ulcer within the last year, 10 subjects. Plantar pressure distribution was recorded during barefoot gait using the Pedar-X system. RESULTS: Neuropathic subjects from both the diabetic neuropathy and DNU groups showed higher plantar pressure than control subjects. At midfoot, the peak pressure was significantly different among all groups: control group (139.4±76.4 kPa), diabetic neuropathy (205.3±118.6 kPa) and DNU (290.7±151.5 kPa) (p=0.008). The pressure-time integral was significantly higher in the ulcerated neuropathic groups at midfoot (CG: 37.3±11.4 kPa.s; DN: 43.3±9.1 kPa.s; DNU: 68.7±36.5 kPa.s; p=0.002) and rearfoot (CG: 83.3±21.2 kPa.s; DN: 94.9±29.4 kPa.s; DNU: 102.5±37.9 kPa.s; p=0.048). CONCLUSION: A history of foot ulcers in the clinical history of diabetic neuropathy subjects influenced plantar pressure distribution, resulting in an increased load under the midfoot and rearfoot and an increase in the variability of plantar pressure during barefoot gait. The progression of diabetic neuropathy was not found to influence plantar pressure distribution.

ABSTRACT

A proposta deste estudo foi analisar por uma visão cinesiológica e biomecânica alguns exercícios do método Pilates e compará–los entre si para uma melhor descrição do método e dos benefícios desta atividade. Duas professoras do método foram fotografadas realizando tais exercícios nos aparelhos Mat, Cadillac, Chair e Reformer e, posteriormente os músculos trabalhados, de forma concêntrica e excêntrica, e os alongados foram comparados com os ângulos articulares mensurados no programa Corel Draw. Para o cálculo dos torques resistentes a partir do modelo ntropométrico de Dempster, foi aplicado o método segmentar nas fotografias digitalizadas para determinar os centros de gravidade dos segmentos do corpo. Concluímos que há uma grande variação dos torques resistentes em função do posicionamento dos membros superiores e inferiores, tronco e cabeça nos exercícios analisados e que a musculatura abdominal é o principal grupo muscular trabalhado.


The purpose of this study was to describe some of the Pilates technique’s exercises under biomechanical and kinesiological considerations. Two subjects (Pilates technique teachers) were photographed while performing the exercises in different equipments – Mat, Cadillac, Chair and Reformer. The relationship among the articular angles – analyzed in the Corel Draw – and the muscles working eccentrically or concentrically, or that were elongated has been established. The segmental model was applied on the digitalized photos in order to locate the gravity center and the resistant torques were calculated by Dempster´s anthropometric model. We concluded that there is a great variation of the resistant torques in relation to upper limbs, lower limbs, head and trunk positions in the exercises and the abdominal muscles are the main group functioning in the exercises analyzed.

ABSTRACT

A utilização de posturas equivocadas durante as atividades que realizamos freqüentemente pode levar a uma aceleração do processo de desgaste sofrido pelo aparelho locomotor. O estudo da relação entre a postura adotada e as estruturas anatômicas acometidas por diversas patologias é uma preocupação crescente de diversas áreas da ciência, sendo a Biomecânica importante representante deste grupo. Neste contexto, o objetivo deste trabalho foi aplicar um método de análise biomecânico em diferentes situações da vida diária, nas quais alterações posturais fossem registradas: em decúbito lateral, durante a realização de manobras massoterapêuticas em divãs com diferentes alturas, durante o suporte de mochila escolar e na utilização do salto alto. Foi utilizado o método da cinemetria, mediante fotos digitais, além do cálculo do centro de gravidade por meio do método segmentar, a determinação de ângulos articulares e torques externos nestas posturas. Com este protocolo, procurou-se estabelecer uma metodologia simples e objetiva de quantificação de alterações posturais em situações cotidianas e/ou ergonômicas. Foram discutidos aspectos cinesiológicos, complicações crônicas da adoção de tais posturas e possíveis intervenções fisioterapêuticas. Em todos os estudos, a metodologia mostrou-se válida para identificar as alterações posturais adotadas nas situações estudadas, e possibilitou a discussão das implicações biomecânicas e cinesiológicas de tais posturas para a fisioterapia.


The adoption of certain postures during our daily activities can lead to an acceleration of the damage process suffered by the motor system. The study of the relationship between daily posture and anatomical structures attacked by various pathologies is a growing concern among several areas related to Health, particulary to Biomechanics.Within this context, the purpose of this paper was to apply a biomechanical method to analysis to 4 different daily life situations in which postural alterations were registered: in lateral decumbency, during performance of therapeutic massage handling on couches at different heights, in the bearing of school backpacks and the wearing of high heel shoes. The method applied was kinemetry through use of digital pictures, besides calculation of the center of gravity through segmentary method and determination of articular angles and external torques in those postures. With this protocol, we sought to establish a simple and objective methodology to quantify postural alterations in daily and/ or ergonomic situations. Kinesiological aspects, chronic complications due to the adoption of those altered postures, and potential therapeutic interventions were discussed. In all studies the methodology appeared to be valid to identify postural alterations adopted in the 4 situations studied and allowed the discussion of biomechanical and kinesiological implications of those postures for physiotherapy.
