ABSTRACT

Hepatocyte growth factor (HGF) is a potent angiogenic factor that can stimulate the production of blood vessels in ischemic tissue. We investigated whether gene therapy using HGF-expressing adenovirus could enhance skin flap survival. Sprague-Dawley rats were randomly divided into three groups. Rats were subdermally injected with HGF-expressing adenovirus (HGF virus group), recombinant HGF (rhHGF group), or phosphate buffered saline (PBS group) 2 days before and immediately after 3 x 9 cm caudal flap elevation. The survival area of the skin flap, the ratio of blood flow, CD31-positive vessels and, VEGF expression were examined. Skin flap viability was significantly increased in the HGF virus group compared to the rhHGF and PBS groups (71.4% +/- 5.9%, 63.8%+/- 6.4%, and 39.2% +/- 13.0%, respectively) (P = 0.025). Furthermore, the blood flow ratio was significantly increased in the HGF virus group. In the HGF virus group, the number of CD31-positive vessels and vascular endothelial growth factor (VEGF) expression were significantly increased. Gene therapy using HGF-expressing adenovirus increase VEGF expression, the number of viable capillaries, and blood flow to the flap, thereby improving skin flap survival.

ABSTRACT

Kimura disease is a relatively uncommon chronic inflammatory disorder of unknown etiology. Kimura disease occurs mainly in oriental young males. The majority of lesions occur in the head and neck and most patients have involvement of regional lymph nodes and major salivary glands. Although Kimura disease is similar to malignancy clinically and radiologically, it is benign disease showing good prognosis in only conservative management. So it is important to familiarize clinical, histological and radiological findings. We experienced a case of mass which developed on parotid gland of 40-year-old male. On impression of malignancy, total parotidectomy was performed. The Histological report was Kimura disease. There has been no recurrence during follow-up period of two years.

ABSTRACT

PURPOSE: To report the case of recurred malignant proliferating trichilemmal tumor on scalp METHODS: Case report and literature review RESULTS: A 40-year-old man presented with a recurred malignant proliferating trichilemmal tumor, which developed on the occipital area. It was 8 x 9 x 4.5 cm in size, protruded, firm and non-tender mass. This tumor was widely excised including normal skin margin and pericranium. Immediate reconstruction using free latissimus dorsi muscle flap and skin graft were done. Histopathologic examination revealed abrubt, compact trichilemmal keratinization in the central area of lobular epithelial proliferation. CONCLUSION: There has been no recurrence of tumor during follow-up period of nine months without adjuvant chemotherapy or radiotherapy.
