ABSTRACT

Diversos estudios demuestran que la inclusión de polinizadores, como las abejas Apis mellifera, generan efectos positivos sobre la productividad de cultivos de durazno (Prunus persica). Esto lleva a probar metodologías que estimulen a las abejas para visitar, con mayor frecuencia, el cultivo de interés, sugiriendo el uso de la técnica de osmoguiado, como herramienta para potencializar el efecto de la polinización. Este estudio valoró un protocolo de osmoguiado, para estimular híbridos africanizados de Apis mellifera, a recolectar polen de flores de durazno, utilizando, como indicador indirecto, el análisis palinológico en muestras de polen. Los tratamientos utilizados fueron con osmoguiado y sin osmoguiado. Cada tratamiento contó con tres colmenas y el periodo de evaluación fue de cinco semanas, coincidiendo con el mayor periodo de floración del durazno. Los resultados evidenciaron que este protocolo no logró estimular a las abejas a visitar la flor de durazno para la búsqueda de polen. La disponibilidad permanente de otras plantas productoras de polen, reportadas previamente como importantes en el aporte de este recurso, pudo influir en la selección de fuentes por parte de las abejas. El polen obtenido provino, principalmente, de plantas de las familias Fabaceae, Asteraceae, Malvaceae y Passifloraceae.


Several studies show that the inclusion of pollinators such as honeybee Apis mellifera generates positive effects in the productivity of peach (Prunus persica) crops. It led to test methodologies that stimulate bees to improve the visit of crops of interest, suggesting the use of the osmoguiding technique, as a tool to potentiate the effect of pollination. This study evaluated an osmoguided protocol to stimulate Africanized hybrids of Apis mellifera to collect pollen from peach flowers, using palynological analysis of pollen samples as an indirect indicator. The treatments used were with osmoguided and without osmoguided. Each treatment had three hives, and the evaluation period was five weeks, coinciding with the peak flowering period of the peach. The results showed that this protocol did not stimulate the bees to visit the peach blossom to search pollen. The permanent availability of other pollen-producing plants, previously reported as significant in the contribution of this resource, could influence bee source selection. The pollen obtained came mainly from plants of the families Fabaceae, Asteraceae, Malvaceae, and Passifloraceae.

Asiatic black bear is present in variety of habitats like broad-leaves and coniferous forests, extending form sea level to 4300m elevation and change their habitat for food purpose seasonally. The present study was conducted at Kaghan and Siran Valleys, District Mansehra, Khyber Pakhtunkhwa, Pakistan to assess habitat of black bear. Line transect method was used for observation of bear signs. Twelve meter circular radius plots were selected for the concern vegetation’s i.e. (trees, shrubs and herbs) and three to six plots were placed in each transect. At the result of sign survey, thirteen different categories of bear signs were recorded and encounter rate was calculated for each sign. A total of 1858 signs were observed during field surveys. Total (81%) coniferous species were recoded among trees, with the highest appearance of Pinus wallichiana (34.22%) and Spruce spp (27.76%), similarly broad leaves trees (18.56%) were also recoded from habitat plots. Most of the signs were encountered in bushy areas, whereas high number of Viburnum Spp (60.29%) was present. It is indicated that black bear prefers blend of Coniferous Trees, Viburnum and Ferns Species; probably because these plants provide enough food, protection, and meticulous shelter because more than 80% of habitat composed of these three species. Currently habitat destruction and increase in human population are the up-growing issues for wild animals (especially Asiatic black bear), which is highly sensitive to such problems. High levels of conservation efforts are recommended for the protection of black bear habitat and to avoid human interference in their territory.


O urso-negro asiático está presente em uma variedade de habitats, como folhas largas e florestas de coníferas, estendendo-se do nível do mar até 4.300 m de altitude e mudando seu habitat para fins alimentares sazonalmente. O presente estudo foi conduzido nos vales Kaghan e Siran, distrito de Mansehra, Khyber Pakhtunkhwa, Paquistão, para avaliar o habitat do urso preto. O método de transecto linear foi usado para a observação de sinais do urso. Parcelas de raio circular de 12 m foram selecionadas para a vegetação de preocupação, ou seja, árvores, arbustos e ervas, e 3 a 6 parcelas foram colocadas em cada transecto. No resultado da pesquisa de sinais, 13 categorias diferentes de sinais de urso foram registradas e a taxa de encontro foi calculada para cada sinal. Um total de 1858 sinais foi observado durante os levantamentos de campo. O total (81%) de espécies de coníferas foi recodificado entre as árvores, com maior aparecimento de Pinus wallichiana (34,22%) e Spruce spp (27,76%); árvores de folhas largas (18,56%) também foram recodificadas em parcelas de habitat. A maioria dos sinais foi encontrada em áreas com arbustos, enquanto um alto número de Viburnum spp (60,29%) estava presente. É indicado que o urso-negro prefere a mistura de árvores coníferas, espécies de viburnos e samambaias; provavelmente porque essas plantas fornecem alimento suficiente, proteção e abrigo meticuloso porque mais de 80% do habitat é composto por essas três espécies. Atualmente, a destruição do habitat e o aumento da população humana são questões crescentes para os animais selvagens, especialmente o urso-negro asiático, que é altamente sensível a esses problemas. Altos níveis de esforços de conservação são recomendados para a proteção do habitat do urso-negro e para evitar a interferência humana em seu território.

Resumen: La colocación de una sonda enteral es un procedimiento invasivo rutinario que puede presentar complicaciones graves. Presentamos el caso de un hombre de 54 años de edad que presentó paro cardiorrespiratorio secundario a infarto de miocardio y que, después de la colocación de una sonda nasoyeyunal, desarrolló neumotórax, por lo que ameritó su retiro, la colocación de una sonda endopleural y la administración de antibióticos. El paciente mostró adecuada expansión pulmonar, lo cual permitió el retiro de la sonda endopleural a los seis días de su instalación, la extubación del enfermo y finalmente su egreso por mejoría. El neumotórax asociado a la colocación de la sonda enteral se presenta en 1.2% de los pacientes. Los factores de riesgo son: déficit neurológico, alteración de la deglución y del reflejo tusígeno. El tratamiento descrito en la literatura es el retiro de la sonda, el uso de antibióticos y el drenaje torácico.


Abstract: Enteral tube placement is a routine invasive procedure that can present serious complications. We present the case of a 54-year-old man who presented with cardiorespiratory arrest secondary to myocardial infarction and who, after placement of a nasojejunal tube, developed pneumothorax that required removal of the tube, placement of a chest tube and administration of antibiotics. The patient presented adequate lung expansion, which allowed removal of the chest tube six days after its installation, extubation and finally discharge due to improvement. Pneumothorax associated with enteral tube placement occurs in 1.2% of patients. Risk factors are neurological deficit, impaired swallowing and cough reflex. Treatment described in the literature is removal of the tube, use of antibiotics and chest drainage.


Resumo: A colocação de sonda enteral é um procedimento invasivo de rotina que pode apresentar complicações graves. Apresentamos o caso de um homem de 54 anos que apresentou parada cardiorrespiratória secundária a infarto do miocárdio e que, após colocação de sonda nasojejunal, desenvolveu pneumotórax que exigiu sua retirada, colocação de sonda endopleural e administração de antibióticos.O paciente apresentou expansão pulmonar adequada, o que permitiu a retirada do tubo endopleural 6 dias após sua instalação, a extubação do paciente e por fim sua alta por melhora. O pneumotórax associado à colocação de sonda enteral ocorre em 1.2% dos pacientes. Os fatores de risco são: déficit neurológico, deglutição prejudicada e reflexo da tosse. O tratamento descrito na literatura é a retirada da sonda, uso de antibióticos e drenagem torácica.

Resumen La enfermedad periodontal (gingivitis y periodontitis) es un proceso inflamatorio ocasionado por la actividad de bacterias patógenas y sus productos sobre el surco gingival, con la consecuente activación de la respuesta inmunitaria. La saliva y el fluido crevicular contienen una gran variedad de enzimas y factores antimicrobianos que están en contacto con la región supragingival y subgingival; entre ellos, las β-defensinas (hBDs). Las hBDs son péptidos catiónicos no glicosilados ricos en cisteína, producidos por las células epiteliales; tienen efecto antimicrobiano e inmunorregulador; de esta forma, contribuyen al mantenimiento de la homeostasis en los tejidos periodontales. Los cambios en la microbiota y en la respuesta inmunitaria de un periodonto sano a gingivitis y, finalmente, a periodontitis, es compleja. Su severidad depende de un equilibrio dinámico entre las bacterias asociadas a la placa, factores genéticos y ambientales. Los avances recientes han permitido comprender la implicación de las hBDs en la detección, el diagnóstico y la terapéutica de la enfermedad periodontal, así como la relación que hay entre la periodontitis y otras enfermedades inflamatorias. El objetivo de esta revisión es describir el efecto de las hBDs en la respuesta inmunitaria y su utilización como marcadores de la actividad inflamatoria de la enfermedad periodontal.


Abstract Periodontal disease (gingivitis and periodontitis) is an inflammatory process caused by the activity of pathogenic bacteria and their products on the gingival sulcus, with the consequent activation of the immune response. Saliva and crevicular fluid contain a wide variety of enzymes and antimicrobial factors that are in contact with the supragingival and subgingival region, including β-defensins (hBDs). hHBDs are non-glycosylated, cysteine-rich cationic peptides produced by epithelial cells with antimicrobial and immunoregulatory effects, thus contributing to maintaining homeostasis in periodontal tissues. The changes in the microbiota and the immune response from a healthy periodontium to gingivitis and, finally, to periodontitis are complex. Their severity depends on a dynamic balance between bacteria associated with plaque, genetic and environmental factors. Recent advances have made it possible to understand the implication of hBDs in the detection, diagnosis, and therapy of periodontal disease and the relationship between periodontitis and other inflammatory conditions. This review aims to describe the effect of hBDs on the immune response and its use as a possible marker of the inflammatory activity of the periodontal disease.

ABSTRACT

Background: Dengue fever is the most rapidly spreading mosquito borne viral disease worldwide. It is estimated that the incidence has increased 30-fold over the last five decades. Recently, the reappearance of dengue in the pediatric population is a matter of concern among pediatricians, especially due to the presence of atypical features, leading to delayed diagnosis. There are few studies regarding the changing trendof paediatric dengue patients due to under reporting of the cases.Material & Methods:This prospective study was conducted among forty-five paediatric dengue patients below 15 years of age(n=45)at MH Samorita Hospital & Medical College, Dhaka over a period of 6 months from July 2021 to December 2021. Neonates and patients with co-infection were not included in the study. All clinical and laboratory details were collected during their hospital stay in a structured questionare form.Results:Forty-five paediatric dengue patients below 15 years of age were enrolled in the study (n=45). Out of the 45 cases 35 patients were non-severe dengue (undifferentiated fever, dengue fever with warning signs, and dengue fever without warning signs) and 10 patients were of severe dengue (DHF and DSS) according to WHO guidelines. The male to female ratio was 1.6: 1 in our study group. The majority of the cases 25 (55.5%) were within 6 to 10 years of age. Patients having features of severe dengue were also seen mostly (17.7%) in this age group. The mean age of hospitalized patients was 8.9 years. In our study there were 16 patients (35.6%) with classical dengue fever, 23 (51.1%) patients with dengue hemorrhagic fever without shock and 6 patients (13.3%) with features of dengue shock syndrome. Atypical clinical features were seen in 17 cases (37.8%). The most common atypical features found in twelve children were acalculous cholecystitis (26.7%). Renal impairment and hepatitis were present in 17.8% and 13.3% of patients. Among neurological manifestations two patients (4.4%) developed impaired consciousness and one patient (2.2%) had seizure. Conclusion:Atypical manifestations of DF can be fatal in many paediatric patients. There should be a high index ofsuspicion among clinicians for timely detection and management of atypicalmanifestation of severe dengue infection to prevent mortality.

ABSTRACT

Background: Histopathological patterns help to define the orientation as well as the duration of disease. The findings identified through this tool aids in the identification and selection of treatment modality as well as provides a prognostic assessment of the condition. Methods: A prospective study design was opted for this research. A total of 221 patients from either gender, ages between 31 to 38 years, undergoing different types of surgical procedures were en rolled from the Faisal and Masroor Base Hospital, Karachi Pakistan. After receiving consent, a biopsy specimen was taken and transferred to the laboratory for staining and further processing for histopathological examination. Results: The major findings i solated among specimens constituted of samples from the Appendix; 67.87% (150/221) followed by stomach; 18.10% (40/221). Among the appendix specimens, 82% (41/221) patients were suffering from acute appendicitis and the remaining had peri - appendicitis. Ami d the stomach specimens 65% (26/221) patients were diagnosed with (Mild) Chronic Superficial Gastritis, followed by 20% (8/221) patients with H - Pylori associated (Mild) Chronic Superficial Gastritis and the remaining were normal gastric antrum and chronic gastritis with partial villous atrophy. Conclusion: By using histopathological examination, the outcome of many diseases can be determined at an earlier stage thereby assisting in lowering the levels of morbidity and mortality among the population. In our research we were able to distinguish a variety of infections among different areas/ organs of the body.

ABSTRACT

To study the physical properties of biomaterials like bony hard tissues through the propagation of elastic waves is one of the most successful method.With the help of internal friction measurement, we can study the processes such as recrystallization, annealing, quenching and tempering, plastic deformation and strain ageing. In the present investigation we calculate and -1 reported the internal friction loss (Q ) for all the six types of bovine compact bones for the first time in a single experiment by using the ultrasonic piezoelectric composite oscillator technique with the help of X-cut crystal in different physiological conditions. Significant variation is observed from bone to bone. This may be attributed to composition and mineral content of the bones. Peak temperature data also is also determined. Temperature variation of logarithmic decrement in X-cut quartz transducer is also verified with mounting of bovine compact bar shaped bone -1 samples. Change of internal friction (Q ) with temperature is also calculated to predict the mechanism of acoustic losses and phase transitions in these bony hard tissues. Internal friction measurements are also useful to obtain the information about imperfections in bony hard tissues. The present investigation constitutes a step towards the application of natural bovine bone ceramic materials for the transducer applications

The contribution of the perpetuation of atrial fibrillation is caused by electrical remodeling in which calcium, sodium and potassium channels could refer to changes in the ion channel protein expression, development of fibrosis, gene transcription and ion channel redistribution. Calcium and magnesium could influence the risk of atrial fibrillation which is the leading cause of cardiac death, heart failure and ischemic stroke. The elevated serum concentration of calcium had a higher range of in-patient’s mortality, increased total cost of hospitalization and increased length of hospital stay as compared to those without hypercalcemia in atrial fibrillation patients. Moreover, chloride channels could affect homeostasis, atrial myocardial metabolism which may participate in the development of atrial fibrillation.Up to a 50% risk of incidence of AF are higher in which left ventricular hypertrophy, sudden cardiovascular death and overall mortality relate to a low serum magnesium level. Additionally, magnesium prevents the occurrence of AF after cardiac surgery, whereas greater levels of serum phosphorus in the large population-based study and the related calcium–phosphorus products were linked with a greater incidence of AF. Numerous clinical studies had shown the high preoperative risk of AF that is linked with lower serum potassium levels. The conventional risk factor of increased risk of new onset of AF events could independently link with high dietary sodium intake which enhances the fibrosis and inflammation in the atrium but the mechanism remains unknown. Many drugs were used to maintain the electrolyte imbalance in AF patients.

Background@#Insulin resistance is associated with metabolic disorders including diabetes, obesity, hypertension, and inflammation which are the risk factors for Atrial Fibrillation. Many studies have reported that type 2 diabetes and AF are related and also their prevalence is increasing globally. Moreover, insulin resistance begins the type 2 diabetes.Main body: This review explains the pathophysiological aspects of insulin resistance in AF patients and discusses the drugs that are used to manage insulin resistance including Biguanides (metformin), thiazolidinediones (TZDs) [Pioglitazone, rosiglitazone], Sodium-glucose cotransporter 2 (SGLT2) inhibitors, Concentrated Insulin Products, Dipeptidyl peptidase-4 (DPP-4) Inhibitors, Glucagon-like peptide 1 (GLP-1) receptor Agonists, Pramlintide, Sulfonylureas, Meglitinides, α-Glucosidase Inhibitors, Colesevelam, Bromocriptine. This review will highlight a few major drugs that played a significant role in AF patients. For this purpose, many databases were used for reviewing the literature and keywords are used such as Insulin Resistance, Pathophysiology, Atrial Fibrillation, and Drugs. @*Conclusion@#This review article concludes that insulin resistance is related to AF. It also provides an outlook on the recent pathophysiological aspects of insulin resistance in AF; however, more studies are needed to clarify the management of insulin resistance in AF patients to prevent the development of type 2 diabetes.

This review present Gingko biloba (GB) interactions, based on clinical and pre-clinical presentations. Literature was retrieved using databases; ScienceDirect, PubMed, Google scholar, Web of Science, Scopus etc. 14/45 interactions were found with clinical presentations. More interactions (80%) were reported with drugs followed by herbs (11.1%), and nutraceuticals (6.7%) with major mechanisms of interaction observed as; inhibition of Cytochrome metabolizing enzymes (44.4%) and platelet-activating factor (PAF) i.e. 15.6%. Major clinical features were; increased bleeding (eye, parietal), hematomas (subdural), and seizures as well as increased blood pressure, priapism, loss of infection/antiviral failure, and coma. Drugs with major interactions belonged to anti-platelet/anti-coagulant and NSAIDs. Synergistic effects were observed for GB vs herbs (except cannabis which showed rhabdomyolysis), foods, and nutraceuticals (except pyridoxine where neurotoxicity was seen). GB use should be monitored and the patient may seek proper advice from a healthcare professional.


Esta revisión presenta las interacciones de Gingko biloba (GB), basadas en presentaciones clínicas y preclínicas. La literatura se recuperó utilizando bases de datos; ScienceDirect, PubMed, Google Scholar, Web of Science, Scopus, etc. Se encontraron 14/45 interacciones con presentaciones clínicas. Se informaron más interacciones (80%) con fármacos seguidos de hierbas (11,1%) y nutracéuticos (6,7%) con los principales mecanismos de interacción observados como; inhibición de las enzimas metabolizadoras del citocromo (44,4%) y factor activador de plaquetas (PAF), es decir, 15,6%. Las principales características clínicas fueron; aumento de sangrado (ojo, parietal), hematomas (subdural) y convulsiones, así como aumento de la presión arterial, priapismo, pérdida de infección / insuficiencia antiviral y coma. Los fármacos con interacciones importantes pertenecían a los antiplaquetarios/anticoagulantes y los AINE. Se observaron efectos sinérgicos para GB frente a hierbas (excepto cannabis que mostró rabdomiólisis), alimentos y nutracéuticos (excepto piridoxina donde se observó neurotoxicidad). Se debe controlar el uso de GB y el paciente puede buscar el asesoramiento adecuado de un profesional de la salud.

Antecedentes: Las personas que sienten miedo y ansiedad muestran peores condiciones de salud bucal, postergando las citas odontológicas hasta tal punto de evitarlas. Objetivo: Determinar los factores asociados al miedo y la ansiedad en pacientes atendidos en las clínicas de la Facultad de Odontología de la Universidad Cooperativa de Colombia. Materiales y métodos: Se realizó un estudio cuantitativo tipo descriptivo transversal con 198 pacientes atendidos en las clínicas de la facultad de Odontología. Se aplicaron las encuestas: Dental Fear Survey (DFS), Escala de Ansiedad Dental de Corah Modificada (MDAS) y una tercera encuesta para identificar: características sociodemográficas y elementos, situaciones y sensaciones asociados a miedo a la ansiedad durante la consulta. Resultados: El 92,4% de los participantes presentaron algún grado de miedo en la consulta odontológica. El 48,7% presento ansiedad en algún grado. La presencia del miedo y ansiedad se encontró asociada con la evaluación de las experiencias previas en la consulta odontológica (Vp <0,001), evidenciado en la mayor proporción de individuos con miedo alto y ansiedad severa cuya experiencia fue negativa en comparación con las frecuencias observadas para aquellos con clasificación de ansiedad leve o moderada y miedo nulo o bajo. Conclusiones: Las experiencias traumáticas previas están relacionadas con el miedo y la ansiedad dental y pueden influir en el adecuado desarrollo de la consulta odontológica, por lo que se recomienda indagar al paciente acerca de estas antes de iniciar atención odontológica.


Background: People who feel fear and anxiety show worse oral health conditions than those who not, thus delaying dentist appointments to the point of cancelling all together. Aim: Determine the degree of fear, anxiety and associated factors in patients seen in the clinics of the Faculty of Dentistry of the Universidad Cooperativa de Colombia. Materials and methods: A quantitative cross-sectional descriptive study was conducted with patients treated in clinics of the Faculty of Dentistry, for a universe of 941, a sample of 198 participants were calculated, not randomly selected. Three surveys were applied by the DFS (Dental Fear Survey), the MDAS (Anxiety Scale Modified Corah Dental) and a third survey to identify sociodemographic variables and factors associated with fear and anxiety. Results: The 92.4% of the participants presented some degree of fear in the dental practice. 48.7% have anxiety to some degree. The presence of fear and anxiety was found associated with the evaluation of previous experiences in dental practice (Vp <0.001), evidenced in the highest proportion of individuals with high fear and anxiety whose experience was negative compared to the frequencies observed for those with mild or moderate anxiety and zero or low fear. Conclusion: Previous traumatic experiences are related to fear and dental anxiety and can influence the proper development of the dental appointments, so it is recommended to inquire about these before starting with a dental treatment.

Background@#The adipokines, secreted from adipose tissue or body fats, are also called adipocytokines which are cytokines, cell signaling proteins or cell–cell communication. However, AF is a common cardiac arrhythmia in which the heart beats so fast by abnormal beating and is a serious public health disease associated with increased heart failure, systemic thromboembolism, and death. Adipokines are cardiovascular disease (CVD) mediators or biomarkers that affect the heart as well as blood vessels, by increasing the cardiac contractility and action potential duration, which result in the extent of left ventricular and atrial remodeling. @*Methods@#(Main body): Google Scholar, PubMed, and science direct were used to review the literature. Many keywords were used for searching the literature such as Adipokines, Leptin, Apelin, Adiponectin, Omentin-1, Chemerin, CTRP3, TNF-α, IL-6, IL-10, and AF. According to the literature, much more data are available for numerous adipokines, but this review article only has taken few major adipokines which played their major role in Atrial Fibrillation. The review article did not limit the time frame. @*Conclusion@#In conclusion, adipokines play a significant role in the development and progress of atrial fibrillation.Also, there are major adipokines such as adiponectin, apelin, C1q/TNF-Related Protein 3 (CTRP3), Chemerin, Omentin-1, interleukin-6, Leptin, TNF-α, resistin, and interleukin-10, which played their pathophysiological role in atrial fibrillation by causing cardiac hypertrophy, increasing the cardiac contractility and action potential duration, atrial fibrosis, electrical and structural remodeling of atrial tissue.

ABSTRACT

Antecedentes: La prevalencia mundial de discapa- cidad en adultos mayores oscila entre 29.5% y 43.4% en países de ingreso alto/bajo (OMS 2011). Objetivo: Determinar la preva- lencia de discapacidad y factores asociados en adultos mayores, Honduras, 2014-2015. Métodos: Estudio descriptivo transversal con análisis de asociación. Estudiantes de último año, Carrera de Medicina, Universidad Nacional Autónoma de Honduras, realizaron búsqueda activa de 50 participantes ≥60 años de edad. Se aplicaron dos instrumentos, uno para búsqueda activa y otro para caracterizar al participante con/sin discapacidad permanente ≥1 año de evolu- ción. Se evaluó limitación de actividad y restricción de participación ≤30 días utilizando la Clasiicación Internacional del Funcionamien- to, Discapacidad y Salud (CIF) y cuestionario WHODAS 2.0 (OMS). Se realizó análisis univariado (frecuencias, porcentajes, prevalen- cias, IC95%) y análisis bivariado (diferencia de proporciones); valor p<0.05 se consideró signiicativo). Resultados: De un total de 5,126 participantes ≥60 años de edad, 3,017 (58.9%) mujeres, edad pro- medio 73.6 años (rango 60-106), la prevalencia de discapacidad fue 51.2% (2,627/5,126); discapacidad grado leve se presentó en 1,893 (72.1%). Las estructuras relacionadas con movimiento (75.5%) y función esquelética (72.6%) fueron las más afectadas. La limitación de moverse (93.4%) y la restricción en invertir dinero propio y fami- liar en su estado de salud (86.8%) fueron las más frecuentes. Los factores sexo femenino, edad>70 años, escolaridad <6 años, enfer- medades crónicas, trauma, accidente o violencia podrían estar aso- ciados a discapacidad; todos p<0.01. Discusión: La discapacidad en adultos mayores se asoció a algunas condiciones prevenibles. Es apremiante implementar programas de envejecimiento saludable en Honduras...(AU)

Dengue is a viral disease which is serious health concern from last few decades and the infection transmitted through mosquito bite into human. This study was conducted to carry out prevalence of dengue fever in District Swabi. A total of 196 blood sample were collected from patients with age ranges (0-80 years) having dengue fever on the basis of physical symptoms from Bacha Khan Medical Complex Swabi during August to October 2017. Serological test were performed for detection of IgM, IgG and NS1 (Non structural protein antigen of virus) against dengue. Out of total 196 confirmed dengue cases the most prone gender was male 123(62%) while 73(38%) were female. Among the age groups; 21-30 years group 62 (31.6%) was found the most predominated age group. The higly affected areas in district Swabi were Topi (40.8%) followed by Main Swabi (27%), Maneri (8.2%), Marghuz (6%), Shawa Ada (5.1%), Shah Mansoor (5.1%), Gohati (4.1%), and Chota Lahore (3.6%). Therefore, the health department should take actions by educating the public about basic cleanliness of the environment. The community should be encouraged to participate in the control of such vector based diseases/infections.


A dengue é uma doença viral que é um sério problema de saúde das últimas décadas e a infecção transmitida através da picada de mosquito em humanos. Este estudo foi realizado para realizar a prevalência da dengue no distrito de Swabi. Foram coletadas 196 amostras de sangue de pacientes com faixa etária (0 a 80 anos) com dengue com base nos sintomas físicos do Complexo Médico Bacha Khan de Swabi, no período de agosto a outubro de 2017. Foram realizados testes sorológicos para detecção de IgM, IgG e NS1 (antígeno proteico não estrutural do vírus) contra a dengue. Do total de 196 casos confirmados de dengue, o sexo mais propenso foi o masculino 123 (62%), enquanto 73 (38%) eram do sexo feminino. Entre as faixas etárias; A faixa etária de 21 a 30 anos 62 (31,6%) foi a faixa etária mais predominante. As áreas altamente afetadas no distrito de Swabi foram Topi (40,8%), seguidas por Main Swabi (27%), Maneri (8,2%), Marghuz (6%), Shawa Ada (5,1%), Shah Mansoor (5,1%), Gohati (4,1%) e Chota Lahore (3,6%). Portanto, o departamento de saúde deve tomar ações educando o público sobre a limpeza básica do meio ambiente. A comunidade deve ser incentivada a participar do controle de tais doenças / infecções baseadas em vetores.

ABSTRACT

Objective: The antimicrobial drugs especially Quinolones are the top most therapeutics class of drug prescribed in some country like Pakistan in primary and tertiary care hospitals and clinics. The objective of the study was to assess the irrational prescribing practice of quinolones in tertiary care setups of Karachi. Methodology: A prospective observational study was conducted for the period of 04 months from November 2019-February 2020, in Public and Private Sector Tertiary Care setups of Karachi. Data was collected from surgical, medical and emergency wards on the basis of World Health Organization prescribing pattern, to evaluate the prescriptions based on Culture sensitivity tests (CST), empirical, targeted, and/or prophylactic therapies. Results: Out of total 1000 patients 41.6% patients were from Public and 58.4% were from Private Sector Tertiary Care Hospital. 13.7% of the patients from Public and 24.8% from Private Sector Tertiary Hospitals were prescribed Quinolones after performing their Culture Sensitivity Test. However, 79.8% of the patients were prescribed Quinolones without Culture Sensitivity Test. 36.4% patients were prescribed with empirical therapy, 20.2% with De-Escalation therapy, while 43.4% were prescribed with prophylactic therapy of quinolones. Conclusion: Study concluded that some serious efforts are required to avoid the irrational prescribing practice and promote the rational prescribing practice of antibiotics particularly for Quinolones.

Antecedentes: La Revista Médica Hondureña (RMH) se ha caracterizado durante más de 90 años por ser el principal medio de difusión médico-científico en Honduras. Objetivo: Identificar si el manejo rehabilitador es considerado como opción terapéutica de acuerdo a la discapacidad de los casos clínicos publicados en la RMH en el periodo 1995-2019 y revisar algunas condiciones que generan discapacidad cuando no son atendidas oportunamente. Métodos: Revisión bibliométrica de los artículos de la sección Casos Clínicos, desde el Volumen 63 Número 1 Año 1995 al Volumen 87 Número 2 Año 2019, 98 números en total. En cada artículo se revisó si se había incluido la rehabilitación dentro del manejo terapéutico, y/o si había sido mencionado como parte integral del manejo de los pacientes. Resultados: De 206 artículos revisados, 11 (5.3%) describieron la rehabilitación como parte del manejo recibido; en 8 publicaciones el autor describió en la revisión de literatura que el manejo ideal en tales casos incluye la rehabilitación. De acuerdo a parámetros ya establecidos en Medicina Física y Rehabilitación, los casos informados en 106 (51.4%) artículos se habrían beneficiado de recibir manejo de rehabilitación. Discusión: A través de esta revisión bibliométrica de veinticinco años, se ha podido documentar que en Honduras aún no se reconoce el beneficio de la rehabilitación en las distintas patologías, tanto en el manejo hospitalario como ambulatorio. La Medicina Física y Rehabilitación contribuye a disminuir y/o evitar complicaciones en los pacientes, por lo que exhortamos a médicos generales y de otras especialidades a tomar conciencia de ello...(AU)

ABSTRACT

Background: The pathways that are involved for the duration of pregnancy and type of parturition are extremely complex, involving maternal as well as fetal systems. The objective of this study was to determine the frequency of types of parturition and their relationship with maternal characteristics among pregnant female patients.Methods: A cross-sectional survey using a non-probability convenient sampling technique was conducted among 195 healthy pregnant females at Obstetrics and Gynaecological Department of Hamdard Hospital, Karachi, from 1st March 2019 to 31st August 2019. After taking written informed consent from the participants, the relevant data were gathered with the help of a structured questionnaire designed specifically for the study. Statistical package for social sciences was used for data entry while the chi-square test was applied for inferential analysis. The duration of the study was six months. Data were entered and analyzed using Statistical Package for Social Sciences version 20.0. Descriptive analysis was performed by generating means and standard deviations for continuous variables while frequencies and percentages for categorical variables. A Chi-square test was applied to perform the inferential analysis while the significance level was set at 0.05.Results: A total of 195 pregnant females were included in the study, whose mean age was 29.29±5.22 years. The study results showed that BMI before pregnancy (p=0.021), rest is taken during pregnancy (p=0.034) and gravida status (p=0.047) were all significantly associated with the type of parturition among the study participants, but spacing in pregnancies and parity were not.Conclusions: Maternal characteristics were found to be significantly associated with the type of parturition among pregnant females. For gynecologists the maternal characteristics identified in this study may serve as a useful indicator of the type of parturition expected in their patients.
