ABSTRACT

Between 1986 and 1990, external rotation osteotomy of the proximal humeruswas performed in 30 patients with shoulder disability following obstetric paralysis, in the Children Hospital Cairo University. The youngest patient was 2 years old the oldest was 10. The osteotomy was performed between the humeral attachment of the pectoralis major and deltoid muscles. Internal fixation of the osteotomy was carried out using plates, the type of which depended upon the age of the patient. The follow-up of these patients ranged from 7 to 26 months with an average of 15.4 months. The operation improved the function as well as the appearance of the limb. It also restored a better functional position of the extremity so that elevation of the hand was possible by flexion of the elbow and minimal forward flexion at the shoulder. Sew compilations were encountered including limitation of internal rotation of the shoulder, malunion and radial palsy

ABSTRACT

Congenital dislocation of the hip is an important condition that has attracted a large share of attention of the orthopaedic surgeons. The results of treatment of 73 hips in 62 patients are analysed. The pre-operative the operative and post-operative scheme of treatment is discussed. Good results were obtained in 56 hips [76.7%] and bad results in 17 hips [23.3%]. The most important causes of the poor results were avascular necrosis of the femoral capital epiphysis, degenerative arthritis and recurrence of the subluxation. The treatment of congenital dislocation of the joint includes:Relaxation of the tight soft tissues around the hip, reduction of the hip and stabilizing operations to maintain the reduce position
