Luxación subtalar medial: reporte de un caso clínico / Medial subtalar dislocation

Subtalar dislocation is the simultaneous dislocation of the distal articulations of the talus, of the level the talocalcaneal and talonavicular joints. They can happen in any direction and they produce deformity in the foot. Most common is the medial dislocation. Less common are lateral, anterior and posterior dislocations. These dislocations are associated with osteochondral fractures. Closed reduction and immobilization continue being the pillars of the treatment. The x- Rays and CT scan of the ankle and affected foot confirms the post-reduction stability of subtalar joints and exclude fracture. The RNM is a good alternative to evaluate avascular necrosis at level of the talus in the follow of these patients. The case of a medial subtalar dislocation is presented in a 20 year-old patient. Together with the revision of the literature, the diagnostic methods and the carried out treatment are exhibited, as well as their clinical results and imagens.

La luxación subtalar es una luxación simultánea de las articulaciones del talus, a nivel talocalcáneo y talonavicular. Pueden ocurrir en cualquier dirección y producen deformidad en el pie. La más común es la luxación medial. Menos comunes son la luxación lateral, anterior y posterior. Estas luxaciones están asociadas a fracturas osteocondrales. La reducción cerrada y la inmovilización siguen siendo los pilares del tratamiento. Las radiografías y el TAC del tobillo y pie afectados confirman la congruencia de la articulación subtalar post reducción y descartan fracturas. La RNM es una buena alternativa para evaluar necrosis vascular a nivel del talus en el seguimiento de estos pacientes. Se presenta el caso de una luxación subtalar medial en un paciente de 20 años. Junto con la revisión de la literatura, se exhiben los métodos diagnósticos y el tratamiento realizado, así como sus resultados clínicos e imagenológicos.
