ABSTRACT

Introducción: Staphylococcus aureus es considerado uno de los patógenos humanos más importantes a nivel mundial y sus niveles de resistencia a meticilina han aumentado incluso en cepas aisladas de personas sin factores de riesgo nosocomial, por lo que la tipificación genética de los clones circulantes es fundamental para comprender los patrones de diseminación. Objetivo: Obtener la tipificación de SARM que causaron infecciones invasivas a niños mediante el empleo de la técnica de análisis multi-locus de número variable de repeticiones en tándem (MLVA) automatizada. Materiales y Métodos: Estudio observacional, descriptivo de corte transverso. Resultados: Se analizaron 25 cepas SARM que representan más de 700 aislamientos de S. aureus colectados en los años 2010, 2012 y 2013 de 4 hospitales de referencia nacional. La automatización de la técnica MLVA incluyó la tipificación del 88% (22/25) de los aislamientos en estudio, resultando 3 perfiles diferentes, cada uno asociado a un "spa tipo" distinto, siendo el perfil 1-t019 el predominante (86%), seguido por el perfil 3-t002 (9%), arrojando 100% de concordancia con el método MLVA manual, así como una alta concordancia con el método estándar de oro, PFGE. Conclusiones: La inclusión de un método de análisis de fragmentos automatizado permitió llevar a cabo la caracterización de aislamientos mejorando el tiempo de respuesta y manteniendo una alta sensibilidad en comparación con el método manual.


Introduction: Staphylococcus aureusis considered one of the most critical human pathogens worldwide, and its levels of methicillin resistance have increased even in strains isolated from people without nosocomial risk factors. Therefore the genetic typing of circulating clones is essential to understand dissemination patterns. Objective: Obtain the MRSA typing that caused invasive infections in children by using the automated multi-locus variable number of tandem repeats (MLVA) analysis technique. Materials and methods: Observational, descriptive, cross-sectional. Results: 25 strains MRSA representing more than 700S. aureusisolates collected in 2010, 2012, and 2013 from 4 national reference hospitals were analyzed. The MLVA automation included the typing of 88% (22/25) isolates, resulting in 3 different profiles, each one associated with a different spa type, being the 1-t019 the predominant (86%), followed by the 3-t002 profile (9%), yielding 100% concordance with the MLVA manual, as well as high concordance with the standard gold method, PFGE. Conclusions: The inclusion of an automated fragment analysis method led to the characterization of isolates, improving response time, and maintaining high sensitivity compared to the manual process.

Introducción: La capacidad de S. aureus para adherirse a superficies y a otras células bacterianas, para conformar un biofilm, es una de las principales causas de la persistencia de las infecciones por este patógeno. Objetivo: Analizar la capacidad de formación de biofilm, mediante el ensayo in vitro en placa de poliestireno, de cepas de S. aureus. Material y métodos: Estudio descriptivo de corte trasverso Se estudiaron 10 cepas resistentes a meticilina, con caracterización molecular de factores de virulencia y representativas de los grupos clonales detectados en un total de 347 cepas que causaron infecciones de piel y partes blandas e invasivas en niños en el 2012. La estandarización del ensayo in vitro fue exitosa, permitiendo clasificar a las cepas en 3 categorías según la intensidad de formación del biofilm. Resultados: El 100% de las cepas analizadas fue formadora de biofilm y se identificó dentro de la categoría de formadores moderados. Conclusiones: La detección de la formación de biofilms bacterianos, sobre todo en infecciones crónicas, es un tema crucial, ya que de ello depende la elección de estrategias especiales para su eliminación, como la remoción quirúrgica, dada la inefectividad de las terapias antibióticas tradicionales.


Introduction: The capacity of S. aureus to adhere to surfaces and other bacterial cells by forming a biofilm is one of the main causes of the persistence of infections by this pathogen. Objective: To analyze the capacity of S. aureus strains to form biofilm, using in vitro polystyrene plate culture techniques. Materials and methods: This was a descriptive, cross-sectional study. We studied and performed molecular characterization of viruence factors in ten strains of S. aureus, which were representative of the clonal groups detected in a total of 347 strains that caused skin and soft tissue invasive infections in children in 2012. The standardization of the in vitro assay was successful, allowing to classify the strains into 3 categories according to the intensity of biofilm formation. Results: 100% of the analyzed strains were biofilm formers and were categorized as moderate formers. Conclusions: The detection of the formation of bacterial biofilms, especially in chronic infections, is crucial, since it affects the choice of special strategies, such as surgical removal, for their elimination, given the ineffectiveness of traditional antibiotic therapies.

ABSTRACT

Background: Lack of adherence with medications is the main cause of antihypertensive treatment failure. Aim: To assess adherence to antihypertensive drugs and its determinants. Material and Methods: The Morinsky-Green questionnaire to determine treatment adherence was applied to 310 hypertensive patients from primary care centers, aged 60 ± 10 years (65% females) in treatment for 4 ± 1 months. Socio-demographic features, use of medications and quality of life using EQ5D questionnaire were also assessed. Results: Twenty percent of patients were diabetic and 19% were smokers. Fifty four percent were adherent to therapy. A higher age and being unemployed were associated with a higher compliance. The main reasons to justify the lack of adherence were forgetting to take the pills in 67% and adverse effects in 10%. Only diastolic pressure was lower in adherent patients, compared with their non-adherent counterparts (78 ± 12 and 81 ± 17 mmHg, respectively p < 0.01). Conclusions: Only half of hypertensive patients comply with their antihypertensive therapy.

ABSTRACT

Background: The prevalence of Chlamydia pneumoniae infection varies according to the population and geografic area studied. Aim: To evaluate the incidence of Chlamydia pneumoniae infection in Chilean subjects with community acquired pneumonia. Patients and methods: Between 1995 and 1997, patients with community acquired pneumonia attending two emergency rooms in Santiago, were studied. The diagnosis of Chlamydia pneumoniae infection was based on the detection of Chlamydia pneumoniae specific IgG antibody in samples from both the acute and convalescent phase, using an indirect microimmunofluorescent technique. Evidence of present infection was defined as seroconversion, a significant increase in the titer of the second sample and an initial titer equal or greater than 1/512. Results: During the study period, 160 patients consulted and seven (six male), aged 54ñ27 years old, complied with the diagnostic criteria of present Chlamydia pneumoniae infection. These patients had no special clinical or radiological features. Five of seven patients improved without any specific treatment. Conclusions: Eight percent of this sample of patients with community acquired pneumonia had Chlamydia pneumoniae infection. This agent should be included in the design of empiric treatment schemes, although our results cast doubt on the pathogenic role of Chlamydia pneumoniae in pneumonia
