ABSTRACT

Metastases to the skin from internal malignancy are very rare and have been reported to occur in 0.9% to 8% of patients with cancer. Particularly there are few cutaneous metastatic carcinomas from the urinary bladder and their incidence is 1% to 2% of the cutaneous metastasis. A 76-year-old man developed cutaneous metastasis six months later after the diagnosis of carcinoma of the bladder had been made. He was presented with asymptomatic erythematous indurated plaques on the left thigh and the inguinal area. This lesion was diagnosed histologically as poorly differentiated transitional cell carcinoma. We report a case of cutaneous metastasis from carcinoma of the urinary bladder showing the unusual clinical feature of an erythematous indurated plaque.

ABSTRACT

Speckled lentiginous nevus(SLN) is characterized by a tan background patch on which more darkly pigmented macules and papules are randomly distributed. The presence of agminated Spitz nevi with SLN is well known, but a solitary Spitz nevus arising on a SLN is rare. We herein report a case of SLN with two Spitz nevi developed individually and congenital melanocytic nevus in a 9-year-old boy.

ABSTRACT

Alopecia areata is a common cause of acquired circumscribed alopecia in childhood. In infancy alopecia areata is exceptional. No case of congenital alopecia areata has been documented yet. Thus, it is generally assumed that alopecia areata is acquired only postnatally, and it is believed that the presence of an alopecia at birth virtually excludes the diagnosis of alopecia areata. In this report, we document a case of congenitally developed alopecia areata that shows typical clinical and histological features of alopecia areata.

ABSTRACT

Erythromelalgia(Erythermalgia) is a syndrome of red, warm, congested, and burning painful extremities. Presently, there are three distinct clinicopathological entities: erythromelalgia, primary erythermalgia, and secondary erythermalgia. Secondary erythermalgia usually arises at an adult age either in association with a detectable underlying disorder or with the use of vasoactive drugs. Typically, it responds to proper treatment of the underlying disorder or discontinuation of the causative drugs. A seven-year-old boy presented with persisting burning pain, erythema, and warmth on both of his feet and lower legs, along with hypertension. He had had this condition for 20 months. Laboratory studies of the patient revealed increased urinary catecholamines and plasma renin activity. Various treatment modalities including aspirin, captopril, sodium nitroprusside, labetalol, epidural block, and stellate ganglion block yielded unfavorable results. Herein, we report an intractable case of secondary erythermalgia associated with hypertension in a child.

ABSTRACT

BACKGROUND: The majority of Staphylococcus aureus strains isolated from atopic eczema skin produce toxins like enterotoxin B or toxic shock syndrome toxin-1(TSST-1) as a superantigen and these toxins may contribute to the exacerbation of this skin disease. OBJECTIVE: We designed this study in order to investigate the effect of toxic shock syndrome toxin-1 on IL-4, interleukin-10(IL-10), interleukin-12(IL-12) and IFN- r production by peripheral blood mononuclear cells from normal and atopic patients. METHODS: Peripheral mononuclear cells obtained from ten patients with severe atopic dermatitis and ten healthy volunteers were stimulated with TSST-1 and cultured for 4 days. The amount of IL-4, IL-10, IL-12 and IFN- r in the culture supernatants were determined by a solid-phase enzyme- linked immunosorbant assay. RESULTS: l. After stimulation with TSST-1, IL-4, IL-10, IL-12 and INF- r, production of peripheral mononuclear cells from both atopic patients and healthy volunteers were increased. 2. After stimulation with TSST-1, the amount of IFN- r in the culture supernatants was significantly greater in the healthy controls than in the atopic patients. 3. After stimulation with TSST-1, the amount of IL-10 in the culture supernatants was significantly greater in the healthy controls than in the atopic patients. CONCLUSION: This data indicates that TSST-1 induces Thl-type and Th2-type cytokines associated with parallel Thl and Th2 stimulation in atopic patients and healthy donors. In addition, peripheral mononuclear cells from atopic patients have a diminished capacity to synthesize IFN- r and IL-10 in vitro in response to stimulation with TSST-1. Our results suggest that TSST-1 is not considered as a really important aggravating factor in atopic dermatitis.

ABSTRACT

Dermatofibrosarcoma protuberans (DFSP) is a rare, distinctive cutaneous tumor, which consists of spindle shaped ceils arranged in densely packed interlacing bundles with the storiform or cartwheel pattern. Histologically, it resembles deep growing dermatofibroma, nodular fasciitis, neurofibroma and neural sheath tumors. DFSP is one of t.he connective tissue tumors which is difficult. to diagnose histologically as well as clinically. Recently, the immunochemical staining with a monoclonal antibody to CD34 is reported to give assistance in the clear differential diagnosis of DFSP from other fibrous or neural tumors. Herein, three cases of DFSP were stained by immunohistochemical staining with S-100 protein, vimentin, factor VIII and anti-CD34 antibody in order to assess the use of anti-CD34 in the differential diagnosis of DFSP.

ABSTRACT

BACKGROUND: The rhus has been known to be such a potent allergen that it causes severe contact dermatitis, and mary studies have tried to reveal the mechanism of allergic contact. dermatitis to urushiol. However, the mechanism of systemic contact dermatitis to urushiol is not, well-known, though the consumption of rhus as a health product and for desensitization is not unconmon in Korea. OBJECTIVE: The purpose of this study was to evaluate the niechanism of systernic contact dermatis, and we carried out irnmunohistochemical stainings un the two strands of dermatitis (systemic arid, llergic contact dermatitis). METHODS: The patients of systemir, contact dermatitis were divided into three groups by duration and severity of the illness according to the desired criteria. We perfor med immunohistor, hernical staining of the tissues from the patients with anti CD4, CD8, CDla, ICAM-1, IL-2R monoclorial antibodies. RESULTS: 1. The results were as follows. 1. The immunohistochemical staining showed that CD8 positive cell numbers in systemic contact dermatitis(18+/-10) were significantly lower than those of allergic contact der matitis(37 +/- 13) ( p0.05). CONCLUSION: The result suggest that lower numbers of CDH positive T cell in systemic contact dermatitis caused a reduation in the suppressor effect to the urushiol mediated immune rection and CD8 positive T cells play an important role in systemic contact dermatitis due to rhus.

ABSTRACT

Clear cell sarcoma of the tendon and aponeurosis was first described by Enzinger in 1965, and has become a well-accepted clinicopathological entity as malignant melanoma of soft parts. Although it posesses the ability to produce melanin, it is quite different from conventional melanoma in that, it is more deeply located, usually associated with tendon and aponeurosis, displays pale-staining round to oval-shaped tumor cells and has a longer clinical course and better prognosis.Several cases have been reported in Korea, and we experiened an additional case of this rare tumor that showed ulceration.

ABSTRACT

The epidermal nevus syndrome is a disorder characterized by epidermal nevi and associated neurologic, skeletal, and other abnormalities. A 15-year-old female patient presented with extensive, bilateral systematized, verrucous plaques, involving the face, trunk, both extremities, and anogenital area. The onset of the lesions were at the age of 2 months. Abdominal CT showed the absence of the left kidney, teratoma of the right ovary and left ovarian cyst. This is a case of epidermal nevus syndrome, which was composed of extensive epidermal nevi, congenital solitary kidney, left ovarian cyst and right ovarian teratoma.

ABSTRACT

BACKGROUND: The incidence of atopic of atopic dermatitis (AD) varies widely by different ages and it has been well-founded that the epidemiology of AD has been studied on admission of the first grade schoolchildren. AD is a common skin disease of childhood and tends to increase significantly during recent decades, but informations regarding the exact prevalence of AD, based on the large-scale general population study, are unavailable in korea. OBJECTIVE: The purpose of this study is to elucidate the exact prevalence of AD and to evaluate the diagnostic significance of some well-known minor clinical features associated with AD in the first grade school children in one of Kouean cities, Puchon, kyunggi-do. METHODS: Four thousands and eighteen first grde school children (boys ; 2072, girls ; 1946) participated inthis study. Two dermatologists determined the presence of skin lesions by through skin examination. Diagnosis of AD was made only when typical flexural and/or extensor and/or infraauricular fissure/eczema exist. The frequency as well as their diagnostic significance of ten minor features of AD (xerosis, ichthyosiform skin lesion, hyperlinear palm,)keratosis pilaris, non-specific hand/foot dermatitis, cheilitis, nipple eczema, perifollicular accentuation, pityriasis alba, Dennie-Morgan infraorbital fold) thought suitable items for field survey by authors, were also evaluated. RESULTS: The total prevalence of AD in the first grade school children of Puchon city was 3. 78%. The prevalence in boys was 3.37% and that in girls was 4.2 Associated minor features in individuals diagnosed as having AD were, in order of frequency, pityriasis alba (59.8%), perifollicular accentuation (57.8%), , keratosis pilaris (56.5%), hyperlinear palms (55.2%), xerosis (43.3%), Dennie-Morgan infraorbital fold (26.3%), etc. All but the ichthyosiform skin lesions and nipple eczema were significantly more frequent in individuals diagnosed as having AD (P<0.01). CONCLUSION: A relatively low prevalence shown in this study might be due to rather strict diagnostic criteria. Some helpful and diagnostically significant minor clinical features of AD, suitable for field survy, is suggested. Further studies are needed for accurate estimation of the prevalence of AD in korea including different regional and age population.

ABSTRACT

Granuloma gluteale infantum may be considered a complication oi the primary irritant type of diaper dermatitis and a modern disease due to the change in tae are of the napkin area. Most patients were infants who had worn plastic diapers and were treated with fluorinated topical corticasteroid cream for their diaper dermatitis. The lesions comorise one or several, soft and reddish brown or livid purple nodules on the napkin area. We report a typical case of granuloma gluteale infantum in a 13-month old female who had been treated with topical cortieosteroid crearn for her diaper derniat;lis. The granuloma regressed spontaneously after withdrawl of the topical corticosteroid and restricted use of the occlusive plastic diaper.

ABSTRACT

BACKGROUND: Substrates play an important role in the effectiveness of the indirect immunofluro-rescence test. The monkey eaophagus has been known to be a more reliable and reproducible substrate than any other animal substrates for detecting pemphigus vulgaris autoantibodies, but it is quite difficult to get the monkey esophagus in Korea, therefore, we tried to find other substrates which could be substituted for it. OBJECTIVE: We studied to select the substrate which can be the most sensitive, to compare the immunofluorescent sensitivity of various animal substrates and fores, cins for detection of pemphigus autoantibodies, and finally to evaluate the effectiveness of cultured hunan keratinocytes as a substrate for indirect immunofluorescerice. METHOD: With foreskin, guinea pigs, cats, rats, dogs, rabbits and cultured human keratinocytes as substrates, an indirect imrnunofluorescence test was performed to detect pemphigus vulgaris autoantibodies in seven sera from the active patients before treatment and diluted serially from 1:10 to 1:1280 RESULT: We could detect pernphigus autoantibodies in all animal subtrates used in this study except rats and obtain good immunofluorescent results with foreskin, guinea pigs, and cultured human keratinocytes. The results were moderate with dogs, cats, rabbits, and poor with rats. CONCLUSION: We confirm thut cultured human keratinocytes can be a good effective substrate. According to effectiveness, subtrates can be divided into three groups. Foreskin, guinea pigs, and cultured human keratinocytes are included in the good effective substrate group, dogs, cats, and rabbits in the modeate group and rats in the poor substrate group, but we can not find any superiority of substrates within each groups.

ABSTRACT

We report a case of epidermolysis bullosa acqumta with characteristic clinical features, subepidermal vesicles in histopathology, and deposits of IgG in basement membrane zone at routine direct immunofluorescent test. 1M NaCl-treated immunofluorescent test was performed in order to correctly diagnose our case. In this method, linear immunofluorescent deposits of IgG were found only at the dermal part of separation induced by 1M NaC1 treatment to skin specimen.
