ABSTRACT

Cervicocogenic headache (CeH) is a relatively common disorder. Although no ideal treatment is available so far, blockades in different structures and nerves may be temporarily effective. We studied the effects of 1-2 mL 0.5 per cent bupivacaine injection at the ipsilateral greater occipital nerve (GON) in 41 CeH patients. The pain is significantly reduced both immediately and as long as 7 days after the blockade. The improvement is less marked during the first two days, a phenomenon we called "tilde pattern". GON blockades may reduce the pool of exaggerated sensory input and antagonize a putative "wind-up-like-effect" which may explain the headache improvement.

ABSTRACT

A doença de Behçet, afecçäo sistêmica de causa desconhecida que acomete mais freqüentemente o sexo masculino, iniciando-se na terceira década, tem como tríade clínica fundamental a presença de úlceras orais e genitais recorrentes, e uveítes. Entretanto, outros sinais e sintomas podem se associar ao quadro, sendo o sistema nervoso acometido em 10 a 49% dos casos. A meningoencefalite com sinais neurológicos focais é a manifestaçäo neurológica mais comum. Näo existem exames complementares patognomônicos. O tratamento com corticóides e/ou imunossupressores pode ser benéfico, sendo os casos com envolvimento ocular e neurológico os de pior diagnóstico. Nós apresentamos o caso de um paciente de 25 anos com hemiparesia esquerda, uveítes, úlceras orais e genitais recorrentes
