ABSTRACT

The thyroid is resistant to infection due to its anatomical and physiological characteristics. We present a rare case of invasive liver abscess with metastatic thyroid abscess and endogenous endophthalmitis in a previously healthy 55-year-old female patient without diabetes or other medical illness. This report raises an important question of the virulence of Klebsiella pneumoniae as an increasingly common causative agent of liver abscess.

ABSTRACT

A 49-year-old woman visited the clinic because of acute hepatitis and acute kidney injury with decreased urine output presenting microscopic hematuria and proteinuria. An abdominal computed tomography revealed a localized, hypoattenuated lesion in a hepatic lateral segment, and kidney biopsy showed oxalate crystal deposition with tubular necrosis. In addition, the patient's 24-hour urinary excretion of oxalate was increased. Her kidney and liver injury improved after sessions of hemodialysis, and urinary oxalate excretion was normalized. Major mutations in primary hyperoxaluria have not been proven. A full sequencing of target genes may be helpful to diagnose a rare form of primary hyperoxaluria.

ABSTRACT

Klippel-Trenaunay syndrome is a rare congenital mesodermal abnormality characterized by varicose veins, cutaneous hemangiomas, soft tissue and bony hypertrophy of limb. Potential complications such as deep venous thrombosis and pulmonary thromboembolism have not been reported in Korea to date. We demonstrate the case of a 48-year-old woman with Klippel-Trenaunay syndrome with extensive varicose veins on right lower limb, hypertrophy of left big toe and basilar artery tip aneurysm, complicated with acute submassive pulmonary thromboembolism treated successfully with intravenous thrombolytic therapy.

ABSTRACT

Quetiapine is an atypical antipsychotic that is frequently used to manage delirium in geriatric patients. Acute pancreatitis associated with quetiapine has rarely been reported. A 70-year-old male presented with severe abdominal pain a few hours after taking a dose of quetiapine prescribed for delirium. Despite the lack of risk factors of pancreatitis in his medical history, the patient had a slight increase of serum lipase and amylase levels. His general condition improved on discontinuation of quetiapine. A month later, quetiapine was readministered for the recurrence of delirium. Subsequently, the patient developed the same symptom with a significant increase in serum pancreatic enzyme levels, confirming that quetiapine induced the pancreatitis. We reported the first case of quetiapine-induced pancreatitis in Korea, together with a review of the literature.

ABSTRACT

Takotsubo cardiomyopathy (TC) is a transient form of acute heart failure that most often occurs in postmenopausal women, typically triggered by a preceding emotional or physical stressor. A 74-year-old woman who suffered from chronic psychiatric stress visited National Medical Center for dyspnea. Acute emotional or physical stress could not be identified despite careful history taking. An electrocardiogram showed diffuse T-wave inversions with prolonged QT interval, and the echocardiogram showed akinesia of mid and apical segments of the left ventricle with hyperkinesia at the base. There was no significant stenosis on coronary angiography. Differing from the typical case of TC, which follows acute stress triggers, our case indicates that underlying chronic psychiatric illness exacerbation can lead to TC. We suggest that cardiologists and psychiatrists be aware of this predisposition to TC, especially in the circumstances of acute heart failure.

ABSTRACT

Recently, the incidence of pulmonary cryptococcosis is gradually increasing in rheumatoid arthritis (RA) patients. Pulmonary rheumatoid nodules (PRN) are rare manifestations of RA. Eighteen months ago, a 65-year old woman was admitted to hospital due to multiple nodules (2.5x2.1x2 cm) with cavitations in the right lower lobe. She was diagnosed with RA three year ago. She had been taking methotrexate, leflunomide, and triamcinolone. A video-assisted thoracoscopic surgery biopsy was performed and PRN was diagnosed. However, a newly growing huge opacity with cavitation was detected in the same site. Pulmonary cryptococcal infection was diagnosed through a transthoracic computed tomograpy guided needle biopsy. Cryptococcus antigen was detected in serum but not in cerebrospinal fluid. The patient was treated with oral fluconazole which resulted clinical improvement and regression of the nodule on a series of radiography. Herein, we report the case of pulmonary cryptococcosis occurring in the same location as that of the PRN.

ABSTRACT

We report a case of a 73-year-old patient with a 17 year-history of well-controlled primary hypertension with a single antihypertensive drug, which became uncontrolled since 9 months ago when he started on oral carbamazepine (CBZ) therapy for syringomyelia. On admission, the patient had a blood pressure of 200-215/95-104mmHg despite an antihypertensive combination therapy with five different drugs. Further investigations ruled out secondary hypertension such as primary aldosteronism, pheochromocytoma and renal artery stenosis. After the discontinuation of CBZ, the blood pressure profile became significantly improved. The rechallenge with CBZ aggravated his blood pressure profile. Therefore, we considered that resistant hypertension was induced by the oral CBZ therapy.

ABSTRACT

The purpose of this study was to compare surface roughness and bone formation around two types of threaded commercially pure titanium implants manufactured by two different companies. The test implants were manufactured by Sumin synthesis dental materials Co. (Avana, Busan, Korea), while the controls were manufactured by Nobel Biocare (MK II, Goteborg, Sweden). To compare bone formation adjacent to newly product with Branemark MK II implant, surface roughness was measured by Accurate 1500M and histomorphometric analysis was done. The results were as follows : 1. Measurement of surface roughness showed that Avana implant had a slightly more irregular surface compared with Branemark implant. 2. In the light microscopic studies, no infiltration of inflammatory cells nor the giant cells wee observed on both groups. 3. In th light and fluorescent microscopic studies, the amount of osseointegration and the extent and the timing of bone formation were similar. 4. There were no statistically difference between two groups in the average bone to implant contacts. Branemark implant ; 67%(SC 23%), Avana implant ; 70%(SD 16%). Comparing with Branemark implant, Avana implant made of CP grade II titanium showed similar good bone healing, formation and osseointegration.

ABSTRACT

Many techniques have been tried to avoid the adverse effect of succinylcholine administe-red for endotracheal intubation especially wish the complication of increased IOP, hyperk-alemia, aspirationl pneumonia and post operative muscle pain, One of these is that the prior administration of a small, subparalyzing dose(15 ug/kg) of non-depolarizing muscle relaxant would shorten the onset time of an intubating dose(80 ug/kg) of muscle relaxant. Intra-venous lidocaine has bean effective in attenuating the reflex intra-ocular response to laryngoscopy Therefore, we determined the effectiveness of this drug regimen with and without intra venous lidocaine to attenuate the IOP, blood pressure and heart rate response to laryngos-copy and endotracheal intubation. Forty patients were divided intro two groups. Group l (n=20) administered saline 5 ml. Group ll(n=70) administered 2% preservative free lidocaine(1.5 mg/kg) as pretreatment drug. The results are as follows : 1) There was no statistically significant difference of intubation condition between one and another group. Among the forer patients, Grade 1,2,3,4 are 8(20%), 19(47.5%), 13(32.5%),0, in orders. 2) In the Saline Group, IOP, BP, HR increased significantly after laryngoscopy compared wiith control value. (p<0.001, P<0.05/p<0,001, p<0.005) and maintained above control values to 4~5 minutes later. 3) In the Lidocaine Group, IOP, BP, HR increased slightly after laryngoscopy compared with control value, but thege changes were not statistically significant, and decreased below control values in 2 min, 2min, 3 min after laryngoscopy each to each. From the above results, it is suggested that combined method of pretreatment of 2% lidofaine(1.5 mg/tg) and divided dose of pancuronium is valuable in general anesthesia of ophthalmic patient who need to attenuate the IOP.

ABSTRACT

Pneumothorax was recognized as a potential hazard of mechanical ventilation after tracheal intubation. Excessive pressure the trachea during general anesthesia could produce lung rupture and pneumothorax. The incidence of pneumothorax has increased as a result of increased application of invasive diagnostic and therapeutic procedures. Emphysematous bleb rupture afterlaryngospasm, bronchospasm, or cough may be causes of pneumothorax. Tension pneumothorax during anesthesia is often difficult to diagnosis, but it needs prompt dignose and treatment. We experienced 3 cases of tension pneumothorax during and after general anesthesia.
