ABSTRACT

Background: Authors define acute bronchitis as a self-limiting infection of large airways, which is characterized by cough without pneumonia. NSAIDS (Non-steroidal anti-inflammatory drugs) are prescribed in patients with Lower Respiratory Tract Infections (LRTI). In cough, fever and chest pain it is common practice to prescribe NSAIDS or antibiotics. Authors have conducted assessment of effect of medications in the resolution of cough of patients with uncomplicated acute bronchitis.Methods: It was a single blinded randomized clinical trial conducted in patients admitted to pulmonary medicine department of government medical college Shivpuri. Patients were from age group 18 to 70 years presenting with respiratory tract infection of less than one week’s duration, with cough as the predominant symptom and diagnosed with non-complicated acute bronchitis. Patients were randomized into two groups, group A and B according to medications.Results: Among 120 participants were randomized (60 to ibuprofen and 60 to antibiotic). The median number of days with frequent cough was slightly lower among patients of group A (12 days) compared with those receiving amoxicillin-clavulanic acid (14 days). No significant difference was found. Adverse effect was seen in 24 patients, which was most common in group B (15.25%) than group A (9.15%). p<0.05).Conclusions: No significant differences were observed in the number of days with cough between patients with uncomplicated acute bronchitis treated with ibuprofen or amoxicillin-clavulanic acid.

ABSTRACT

Background: Specialized female surgical clinics are the basic requirement in surgical department which should be addressed routinely. The management of surgical problems in female patients requires multidisciplinary approach mainly general surgery, obstetrics and gynaecology, general medicine, paediatrics, dermatology and psychiatry. If proper care is provided then this will uplift the follow up of same patients in such clinics. The objective of the study was to assess the success rate of female surgical clinics and to determine the follow up visits done by female patients once visited the female surgical clinics.Methods: Prospective educational interventional study was conducted. A total of 952 patients visiting the clinic were studied. Statistical analysis was done.Results: Out of 952 cases 944 (99.2%) cases were willing to continue visit to the clinic, while 809 (84.9%) cases revisited for follow up treatment in female surgical clinics within one year duration.Conclusions: It can be concluded that there is a requirement and need of a fully functional specialized female surgical clinic in our hospital. As the success rate for follow up visit by patients in this clinic was high.

ABSTRACT

The pulp and periodontium have embryonic, anatomic and functional inter-relationships. The simultaneous exist-ence of pulpal problems and inflammatory periodontal disease can complicate diagnosis and treatment planning. This case report evaluates the efficacy of G-Bone graft in the management of vertical bone loss associated with an endo-perio lesion in a left mandibular first molar and second molar. A 40 year-old male patient with an endo-perio lesion in the left mandibular first and second molars was initially treated with endodontic therapy. Following the en-dodontic treatment, the defect was treated using G-Bone graft. At the end of 6 months, there was a gain in the clini-cal attachment level and reduction in probing depth. Radiographic evidence showed that there was a significant bony fill.

ABSTRACT

Adrenal Myelolipoma is a rare tumor that is benign in nature and unilateral. It is usually asymptomatic with just a few millimeters in size but they can grow over 30 cms becoming a life threat to the patient. This tumor is generally considered as a type of hormonally inactive neoplasm which is composed of variable amounts of mature adipose tissue and normal hematopoeitic elements. These may not account for extramedullary sites of haemotopoiesis and are not associated with any specific hematologic disorder. Rarely, Adrenal Myelolipomas occur in association with Adrenocortical Adenoma or Adrenocortical Carcinoma. We report a case of Adrenocotical Adenoma with Myelolipoma in a 35 year old female who presented with pain in the right upper abdomen. The radiological appearance resembled adrenocortical carcinoma. This case emphasize the importance of this combination as a pitfall in the correct diagnosis and management of patient with Adrenal mass.

Meckel's cave tumors are uncommon intracranial tumours. Lipoma in meckel's cave is a rare entity. CT & MRI imaging are modialities used to evaluate meckel's cave lesion. We report a case of Meckel's cave lipoma presenting with trigeminal neuralgia.

ABSTRACT

One of the rare complications of choledochal cysts is rupture. In majority of the cases, the cause of rupture is unknown. Reconstructive surgery is the treatment of choice. We describe three patients with choledochal cyst rupture, who were admitted with acute abdomen. Diagnosis of biliary ascites with peritonitis was made in all the three patients. At surgery, two patients underwent T-tube placement, and definitive repair was done electively. One patient underwent definitive repair of ruptured choledochal cyst, but died due to septicemia. External bile drainage would be safer in emergency condition.
