ABSTRACT

Proponents of laparoscopic appendectomy emphasize the advantages of laparoscopic operation decreased hospitalization, less operating time and minimal complication. this study includes comparison between laparoscopic vs open appendectomy. 50 patients in each group were included in the study during the period of November 2015 to June 2017.There were 60 male patients while female patients were 40. Of the 60 male patients, 34 patients underwent LA while 26 patients underwent OA. 16 female patients underwent LA, while 24 female patients underwent OA. All patients presented with complaint pain. Nausea was present in 54% of the patients and vomiting in 81%. Fever was present in 58%. Per-operative diagnosis were slightly different than USG findings . 19% patients had enlarged appendix with dilated bowel loop, 4% patients had adhesions of bowel, 61% patients had acute appendicitis with omental adhesion, 8 % patients had RIF collection, 2% patients had only inflamed appendix, 3%patients had gangrenous perforated appendix with flakes, 3% patients had enlarged dilated appendix with dilated bowel loop. In our study most common anatomical position was retrocaecal 72%, 24% had pelvic postion,1% had preileal and 3% had subcaecalposition. Operating time was longer in the OA group compared to LA group and was statistically significant. The median operation time for the LA was 20-30 minutes while that of OA was 40-50 minutes. Patients who underwent LA had a shorter hospital stay compared to OA group. The average duration of hospital stay for LA group in the study was 3-4 days while for the OA group was 5-6 days. In the LA group, only 1 patient (2%) developed caecal perforation, while 3 (6%) patients developed port site infections . For those undergoing OA, 16% had complications. The most common being wound sepsis, which occurred, in 14% of the patients. One patient had developed small bowel obstruction which was diagnosed 7 days after the surgery.

Introduction-Leprosy is a chronic infectious disease which still remains a serious public health problem due to its ability to cause disability. The prevention of leprosy ultimately lies in the early diagnosis and treatment of the individ-uals having leprosy. Changing leprosy scenario has led to requirement of leprosy education. However the knowledge of medical college students on leprosy remains unknown. Aim: To assess the post-sensitization knowledge and awareness of medical students after state leprosy sensitization program. Materials and methods: A questionnaire based, cross sectional study was conducted among 107 medical students of different years starting from 2ndyear and onwards in the month of July 2017 after state leprosy sensitization program. Results: Among the medical students 70 (75%) were male students and 37(25%) were female students coming from different years of their admission in med-icalcollege.Regardingthebasicknowledgeofleprosy100%of3ndyear and final year students were having knowledge about the cause of leprosy and 100% of final year and 3rdyear medical students know about other name of Leprosy (Hansen,s disease) respectively.100% of final year ,3rdyear and 2ndyear medical students had knowledge about multidrug therapy (MDT) in in-depth knowledge of leprosy respectively. Regarding the virulence of bacilli 87%, 60% and 63% of final, 3rd and 2nd year medical students had apt knowledge respectively. Conclusion-These observa-tions suggest that the basic and in-depth knowledge of medical students about cause, types, prevention and treat-ment etc. of leprosy shows increasing trend owing to the part of their curriculum. However there is still a need to organize sensitization programs at regular intervals to impart basic and in-depth knowledge to new medical stu-dents, as well as to update the knowledge of those who already had such sensitization programs.

ABSTRACT

Punctate palmoplantar keratoderma (Brauer-Buschke-Fischer syndrome) is a rare entity. Among punctate keratoderma, the linear presentation is much rarer, and exact incidence is not known. Unilateral linear punctate palmoplantar keratoderma is not yet reported in the literature. Here we report a case 12-year-old child presented with asymptomatic linear punctate plaque on the left sole and hand; histology revealed hyperkeratotic epidermis without columns of parakeratosis or cornoid lamella.

ABSTRACT

We report a case of a 58-year-old female patient who presented with multiple, asymptomatic, slowly-growing, raised pigmented lesions all over her body for the past 10 years with ulceration in one of the lesions on the trunk for the past five months. Histopathology of the lesion revealed features consistent with melanoacanthoma. Here, we report the first case of cutaneous melanoacanthoma presenting with an ulcerated plaque and the third case of cutaneous melanoacanthoma with multiple lesions. To the best of our knowledge, ulceration has not yet been reported as a feature of cutaneous melanoacanthoma in the medical literature.

ABSTRACT

The competence of clinical and procedural skills possessed by medical students on graduation have received considerable importance in recent years. This questionnaire-based study was conducted at B.P. Koirala Institute of Health Sciences, Dharan, Nepal with an aim to assess the perception about clinical and procedural skills attainment by final-year medical students in a new medical school of Nepal. Fifty-nine final year MBBS students were interviewed using a set of questionnaire related to the experience gained by the students in relations eight clinical and twenty procedural skills. The perception about the level of attainment was good for most of the clinical skills and for few procedural skills, for example, using an auroscope and ophthalmoscope. The level of attainment was very poor in inserting the flatus tube, aspirating pleural and ascitic fluids, performing lumbar puncture, administering enema and giving subcutaneous injection. The perceived confidence in giving intravenous injection, inserting venflons, catheterizing urinary bladder, performing ambu bag and mask ventilation, proctoscopy and suturing of superficial wound was greater than the level of practical experience. The practical experience was more in doing venesection but the perceived confidence was poor in it. The final year medical students had mastered most of the clinical skills but not all procedural skills equally. Those skills in which they are not competent must be acquired during internship. Training in these skills needs to be reinforced by establishing a skill laboratory.
