ABSTRACT

Currently, the research or publications related to the clinical comprehensive evaluation of Chinese patent medicine are increasing, which attracts the broad attention of all circles. According to the completed clinical evaluation report on Chinese patent medicine, there are still practical problems and technical difficulties such as unclear responsibility of the evaluation organization, unclear evaluation subject, miscellaneous evaluation objects, and incomplete and nonstandard evaluation process. In terms of evaluation standards and specifications, there are different types of specifications or guidelines with different emphases issued by different academic groups or relevant institutions. The professional guideline is required to guide the standardized and efficient clinical comprehensive evaluation of Chinese patent medicine and further improve the authority and quality of evaluation. In combination with the characteristics of Chinese patent medicine and the latest research achievement at home and abroad, the detailed specifications were formulated from six aspects including design, theme selection, content and index, outcome, application and appraisal, and quality control. The guideline was developed based on the guideline development requirements of China Assoication of Chinese medicine. After several rounds of expert consensus and public consultation, the current version of the guideline has been developed.

ABSTRACT

INTRODUCTION@#Three doses of SARS-CoV-2 mRNA vaccines have been recommended for cancer patients to reduce the risk of severe disease. Anti-neoplastic treatment, such as chemotherapy, may affect long-term vaccine immunogenicity.@*METHOD@#Patients with solid or haematological cancer were recruited from 2 hospitals between July 2021 and March 2022. Humoral response was evaluated using GenScript cPASS surrogate virus neutralisation assays. Clinical outcomes were obtained from medical records and national mandatory-reporting databases.@*RESULTS@#A total of 273 patients were recruited, with 40 having haematological malignancies and the rest solid tumours. Among the participants, 204 (74.7%) were receiving active cancer therapy, including 98 (35.9%) undergoing systemic chemotherapy and the rest targeted therapy or immunotherapy. All patients were seronegative at baseline. Seroconversion rates after receiving 1, 2 and 3 doses of SARS-CoV-2 mRNA vaccination were 35.2%, 79.4% and 92.4%, respectively. After 3 doses, patients on active treatment for haematological malignancies had lower antibodies (57.3%±46.2) when compared to patients on immunotherapy (94.1%±9.56, P<0.05) and chemotherapy (92.8%±18.1, P<0.05). SARS-CoV-2 infection was reported in 77 (28.2%) patients, of which 18 were severe. No patient receiving a third dose within 90 days of the second dose experienced severe infection.@*CONCLUSION@#This study demonstrates the benefit of early administration of the third dose among cancer patients.

ABSTRACT

@#This study aims to compare flexural strength and flexural modulus of different resin-based composites (RBCs) and to determine the impact of dietary solvents on flexural properties. Forty specimens (12x2x2mm) for each of two conventional (Aura Easy [AE]; Harmonize [HN]) and one bulk fill (Sonic Fill 2 [SF2]) were fabricated using customised plastic moulds. Specimens were light-cured, measured and randomly divided into four groups. The groups (n=10) were conditioned for 7 days at 37°C: in one of media: air (control), artificial saliva (SAGF), 0.02N citric acid and 50% ethanol–water solution. After conditioning, the specimens subjected to flexural testing. Two-way ANOVA and one-way ANOVA (post hoc: Tukey’s or Dunnett T3 tests) were used at =0.05. Significant differences in flexural properties were observed between materials and conditioning media. Flexural strength and modulus values ranged from 124.85MPa to 51.25MPa; and 6.76GPa to 4.03GPa, respectively. The highest flexural properties were obtained with conditioning in air. Exposure to aqueous solutions generally reduced flexural properties. In conclusion, the effect of dietary solvents on flexural properties were material and medium dependent. For functional longevity of restorations, patients’ alcohol intake should be considered during material selection. Dietary advice (reduce alcohol consumption) should be given to patients post operatively.

ABSTRACT

Objective:To observe the effects of Fushengong prescription on secretory glycoprotein (Wnt)/β-serial protein (β-catenin) signaling pathway in kidney of rats with chronic renal failure (CRF),and to further explore its mechanism of releasing the aggregation of extracellular matrix(ECM),inhibiting renal tubule interstitial fibrosis (TIF) and prolonging the progression of CRF. Method:A total of 55 SD male rats were randomly divided into the normal group, the model group, and the low, medium and high dose groups of Fushengong prescription, with 11 rats in each group. The normal group was routinely reared and the other 4 groups of rats were used to establish CRF model with 0.5% adenine fodder, fed them continuously for 21 d. After successful modeling, all model rats were switched to conventional feed. Normal saline (NS) was given the normal group and the model group by 20 mL·kg-1·d-1, the low, middle and high dose groups rats of Fushengong prescription were given intragastric administration Fushengong prescription according to the body weight of 4, 8, 16 g·kg-1,once a day, continuous gavage for 30 d. After the experiment, the pathomorphism change of renal tissues of rats was measured by Masson staining, the expression of Wnt4 and β-catenin mRNA in the kidney tissues were observed by the method of Real-time fluorescence quantitative polymerase chain reaction(Real-time PCR), the expression of Wnt4,β-catenin and matrix metalloproteinase-7(MMP-7) protein of renal tissues were detected by the methods of Western blot. The expression of Wnt4, β-catenin protein of renal tissues were detected by the methods of immunohistochemistry (IHC). Result:Compared with normal group, renal tubule interstitial fibrosis of renal tissues increased distinctly and the expression of Wnt4,β-catenin and MMP-7 protein increased significantly in the model group. Wnt4 and β-catenin mRNA also increased significantly in model group(P<0.01). Compared with model group, the expression of Wnt4, β-catenin and MMP-7 protein in the Fushengong prescription groups decreased obviously (P<0.05). The expression of Wnt4 and β-catenin mRNA in Fushengong prescription groups also decreased obviously. Conclusion:The mechanism of Fushengong prescription can release the aggregation of ECM,inhibit TIF and delay the progression of CRF,which may be related with the activation of Wnt/β-catenin signal pathway.

ABSTRACT

Bone defects caused by trauma, tumour resection, infection and congenital deformities, together with articular cartilage defects and cartilage-subchondral bone complex defects caused by trauma and degenerative diseases, remain great challenges for clinicians. Novel strategies utilising cell sheet technology to enhance bone and cartilage regeneration are being developed. The cell sheet technology has shown great clinical potential in regenerative medicine due to its effective preservation of cell-cell connections and extracellular matrix and its scaffold-free nature. This review will first introduce several widely used cell sheet preparation systems, including traditional approaches and recent improvements, as well as their advantages and shortcomings. Recent advances in utilising cell sheet technology to regenerate bone or cartilage defects and bone-cartilage complex defects will be reviewed. The key challenges and future research directions for the application of cell sheet technology in bone and cartilage regeneration will also be discussed.

ABSTRACT

Aims@#A simple in vitro model system was applied in this study assessing the dynamics of the microbial community associated with the shrimp gut system to understand the changes that influence dietary variables. @*Methodology and results@#The diversity and abundance of microbiome were monitored within two different treatment slurries inoculated with shrimp faecal samples as to mimic the effect of diet manipulation, and 16S rRNA gene of MiSeq Illumina-based sequencing was applied. The different diets tested were a commercial standard diet and a prodigiosin added diet. There was very clear separation between the commercial standard diet and prodigiosin added diet as revealed by the total viable counts (TVC) and sequencing data. It suggested that the microbial community of the shrimp gut system exhibited a dynamic response with the treatments and allochthonous bacterial present. The prodigiosin added diet was clearly separated from the commercial standard diet serving as a potential shrimp feed additive. The sequencing data analysis showed that members of the genera Vibrio, Shigella and Photobacterium became predominant on the commercial standard diet treatment. The prodigiosin-added diet treatments indicated an abundance of members of the genera Micrococcus, Arthrobacter, and Shigella. @*Conclusion, significance and impact of study@#In vitro model system-based testing of diets could be a useful method to determine the potential effect of diet manipulation on shrimp gut system microbiome members.

ABSTRACT

@#AIM: To Study the anti-fungal effect of corneal collagen cross-linking combined with natamycin <i>in vivo</i> and <i>in vitro</i>, so as to provide the treatment and experimental basis for the treatment of clinical fungal keratitis. <p>METHODS: Three common pathogenic fungi(<i>Aspergillus flavus</i>, <i>Fusarium Solani</i> and <i>Candida albicans</i>)were used. The experimental group was divided into cross-linking combined natamycin group, natamycin combined riboflavin group, natamycin combined UVA irradiation group, cross-linking group and natamycin group as the control group. The drug was added to the center of the Sabouraud dextrose agar(SDA)plate coated with liquid with each fungal spores with the same maid turbidity of 1.5. Ten minutes later, it was irradiated with collagen cross-linking instrument for 10min and cultured at 28℃ for 36h, and then the inhibition zone size was measured and analyzed statistically. The rabbit model of <i>Fusarium Solani</i> corneal infection was prepared. The model rabbits were randomly divided into model control group, cross-linking treatment group, natamycin treatment group, cross-linking combined natamycin group, 5 rabbits in each group. And another 5 normal rabbits were taken as control, and five rabbits were irradiated in accordance with corneal collagen cross-linking therapy. The results were observed by anterior segment photography, corneal scraping and confocal microscopy, and the ultra micro structural changes of the corneas were observed by electron microscope after the treatment.<p>RESULTS: Corneal collagen cross-linking alone had shown no effect on each fungus <i>in vitro</i>. Corneal collagen cross-linking combined with natamycin produced significant anti-fungal effect(<i>P</i><0.05). However, the anti-fungal effect of natamycin combined riboflavin group and natamycin combined ultraviolet light group showed no statistical difference(<i>P</i>>0.05)comparing with the control group. For the model of rabbit fungal infection, the course of disease was about 14d in the natamycin group and CXL combined with natamycin group, and it was about 21d in CXL group. After the treatment, all the groups healed. There were no defects in the corneal epithelium, no mycelium in the corneas, except for more corneal neovascularization. The results of the anterior segment photography showed that the treatment effect of the cross-linking combined natamycin group was better than other groups, with fewer scar tissue, better corneal healing and relatively short course of disease.<p>CONCLUSION: Corneal collagen cross-linking combined with natamycin treatment is able to enhance anti-fungal effect, promote corneal healing, and shorten the course of disease. So it is a promising therapeutic technique for the clinical treatment of fungal keratitis.

ABSTRACT

Objective: To establish the composition-activity relationship (CAR) model based on study of chemical composition and relating proliferation inhibitory rate of Corydalis soxicola and recognize the anti-hepatic fibrosis active compounds of C. soxicola. Methods: Nine orthogonal C. soxicola extract were analyzed by HPLC, and 21 characteristic peaks were profiled. Anti-hepatic fibrosis activity was investigated by MTT assays on HSC-T6, and the potential active components were identified by scores plot and variable importance in projection (VIP) values by means of orthogonal partial least squares (OPLS) analysis, and the activities of identified components were verified by MTT and flow cytometry. LDH kit was used to detect the effect of dehydrocavidine, palmatine, and berberine on LDH activity. Results: The results showed that six peaks including peaks 13, 14, 16, 18, 19, and 20 were significantly related to anti-hepatic fibrosis activity among nine orthogonal extract from C. soxicola. Peaks 18, 19, and 20 were characterized as dehydrocavidine, palmatine, and berberine, respectively. MTT assay showed that dehydrocavidine, palmatine, and berberine with various concentration significantly inhibited the proliferation of HSC-T6 cells. It was also found in flow cytometry that the apoptotic rates of dehydrocavidine, palmatine, and berberine (0.10 mg/mL) on HSC-T6 were 42.12%, 42.22%, and 36.73%, respectively, which were obviously higher than that of control (1.69%, P < 0.01). No obvious cytotoxic effect was found when the final concentration of dehydrocavidine, palmatine, and berberine were less than 0.15, 0.10, and 0.10 mg/mL, respectively. Conclusion: In this study, it was for the first time found that dehydrocavidine, palmatine, and berberine in C. soxicola extract can effectively inhibit the proliferation and induce apoptosis of HSC-T6 based on composition-activity relationship, and there was no obvious cytotoxic effect of the effective concentration, which showed that they may be the active components with potential anti-hepatic fibrosis effect and have a certain security in the application in C. soxicola. At the same time, it also suggests that the research ideas based on CAR can provide an effective method for the identification of active components in natural plants.

ABSTRACT

Hepatic disease is one of the high-prevalence diseases in China, of which gastrointestinal bleeding is a common complication treated by proton pump inhibitors. Vonoprazan is a novel proton pump inhibitor which acts better than lansoprazole in pharmacokinetics and pharmacodynamics. In this study, the pharmacokinetics of vonoprazan was compared between acute hepatic injury and normal condition in rats. Results showed that the exposure (AUC) of vonoprazan was significantly higher in rats with acute hepatic injury than in normal rats, and the metabolites formation rates of vonoprazan also slowed down, which might be due to the change of activity of enzymes and transporters. This find may provide a theoretical basis for the dose regulation of vonoprazan in patients with hepatic injury.

ABSTRACT

OBJECTIVE:To find out potential financial risk in the listed Chinese pharmaceutical companies and to provide rec-ommendations. METHODS:The Z-score established with Altman E was adopted to classiy and analyzed empirically 143 listed phar-maceutical companies in China during 1998-2013 by sub-sectors(medical devices, pharmaceutical commercial, biological products chemical raw medicine, traditional TCM). The effects of some data in annual reports on Z-score were validated by using stepwise regression. RESULTS:The result showed that the average Z-score values percentage of the best 3 companies were 90.95%,47.96%and 41.25%,respectively,from 1998-2013;while for the worst 3 companies, the values were 0.80%,1.14%and 1.21%,respective-ly. At the same time, the financial situations of chemical raw medicine companies were the worst among 5 sub-sectors,of which dur-ing 2004-2006,the constituent ratios of companies in“Distress”zones went over 50.00%,which were 60.00%,55.20% and 53.60%,respectively. If counted by calendar year, the market value per share of 10 years and float of 9 years significantly affected the Z-score values(P<0.05);if counted by listed year, the market value per share of 15 years significantly affected the Z-score val-ues. CONCLUSIONS:Although major pharmaceutical companies have good financial status, financial crisis still exists in some companies. Especially, the financial situations of chemical raw medicine companies should not be neglected. The government should pay more attention to share price in order to prevent financial crisis which was induced by stock bubbles;at the same time,supervi-sion system should be established to evaluate the financial situation of listed pharmaceutical companies, even for all listed companies.

Methods Sixty clinically stable MHD patients from the Hemodialysis Center were enrolled in the study . The patients were divided into exercise group and control group with 27 cases in each group by using the random digit number table. The patients in the exercise group underwent a 6-month intra-dialytic exercise, in which the patients conducted physical exercises supinely on a cycle ergometer each for 50 minutes, 3 times a week. The patients in the control group were treated without any exercise intervention. Serum interleukin-6(IL-6), C-reactive protein(CRP) and tumor necrosis factor-α(TNF-α) were determined after the study for 0 and 6 months. Results After the 6-month intradialytic exercise program, serum IL-6, CRP and TNF-α were all decreased in the exercise group (P 0.05). Conclusion The intra-dialytic exercises can alleviate micro-inflamatory state in maintenance hemodialysis patients.
