ABSTRACT

ObjectiveTo explore the effect of Qinggan Zishen prescription on metabolic disorders in obesity-related hypertension (OBH) patients and analyze the potential pharmacological mechanism based on network pharmacology. MethodA total of 85 eligible OBH patients who were treated in the outpatient or wards of Jiangsu Province Hospital of Chinese medicine from September 2018 to January 2020 were selected and randomized into the observation group (45 cases) and control group (40 cases). All patients were treated with western medicine during a four-week introduction period, and then the observation group was treated with Qinggan Zishen prescription on the basis of western medicine. The study lasted 6 months, and indicators, such as triglycerides (TG), total cholesterol (TC), high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol (LDL-C), glycosylated hemoglobin (HbA1c), fasting blood glucose (FBG), fasting insulin (FINS), waist circumference (W), hip circumference (H) were detected and homeostasis model assessment of insulin resistance (HOMA-IR),body mass index (BMI), waist-hip ratio (WHR) were calculated before and after intervention. At the same time, the regulation network of the Qinggan Zishen prescription was visualized and the protein-protein interaction (PPI) network was constructed. The core targets of the network were obtained for Gene Ontology (GO) term enrichment and Kyoto Encyclopedia of Genes and Genomes (KEGG) pathway enrichment analysis. ResultAfter intervention for 6 months, the levels of W, H, WHR, FINS, and HOMA-IR in the observation group were reduced as compared with those in the control group (P<0.05, P<0.01). According to network pharmacology, the main components of Qinggan Zishen prescription in treating OBH were luteolin, quercetin, and berberine and the key targets were amyloid precursor protein (APP), vascular endothelial growth factor A (VEGFA), and estrogen receptor 1 (ESR1). Moreover, the key biological pathway was advanced glycation end product (AGE)/advanced glycation end product receptor (RAGE) signaling pathway. ConclusionQinggan Zishen prescription can improve the metabolic disorder of OBH patients through multiple components, multiple targets, and multiple pathways, which provides new mindset for follow-up studies.

ABSTRACT

OBJECTIVE@#To compare the clinical effect of microsurgery and endovascular embolization in the treatment of spinal dural arteriovenous fistula (SDAVF) by meta-analysis.@*METHODS@#A systematic review was performed to retrieve all relevant literature about surgical treatment or endovascular embolization of SDAVF up to December 2019 through PubMed, Embase, Web of Science, Cochrane Central Register of Controlled Trials Results, CNKI, Wanfang Data, and SinoMed. The Chinese and English key words included: "SDAVF", "spinal dural arteriovenous fistula", "spinal AVM", "spinal vascular malformation and treatment". The included studies were evaluated using the Newcastle-Ottawa scale. The early failure rate, long-term recurrence, neurological recovery, and complications were evaluated and the clinical effects of the two methods in the treatment of SDAVF were compared by using RevMan 5.3 software. And a further subgroup analysis of the therapeutic effect of endovascular embolization with different embolic agents was conducted.@*RESULTS@#A total of 46 studies involving 1 958 cases of SDAVF were included, in which 935 cases were treated by microsurgery and 1 023 cases were treated by endovascular embolization. The funnel plot demonstrated that there was no publication bias. The results of meta-analysis showed that the incidence of early surgical failure was lower than that of endovascular embolization (OR=0.20, 95%CI: 0.13-0.30, P < 0.05), and the long-term recurrence was also lower than that of endovascular embolization (OR=0.36, 95%CI: 0.22-0.58, P < 0.05). The improvement of neurological function in the surgical patients is significantly higher than that in the patients treated with endovascular embolization (OR=2.86, 95%CI: 1.36-5.99, P < 0.05). There was no significant difference in the occurrence of complications in these two groups (OR=1.52, 95%CI: 0.88-2.64, P=0.14). In the cases of endovascular embolization, the risk of treatment failure or recurrence was higher with Onyx glue than with n-butyl 2-cyanoacrylate (NBCA), and the difference was statistically significant (OR=4.70, 95%CI: 1.55-14.28, P < 0.05).@*CONCLUSION@#Although the treatment of dural arteriovenous fistulas by intravascular embolization has been widely used, the clinical effect of microsurgery is still better than that of endovascular embolization. Large scale and high-quality randomized controlled trials are required to validate the efficacy and safety of endovascular treatment in SDAVF patients.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the Epstein-Barr virus (EBV) infection, the expression of EBV latent membrane protein 1 ( LMPl) and oncogene bcl-2 in lung cancer patients.</p><p><b>METHODS</b>EBERI in 108 cases of lung cancer were detected with in situ hybridization. EBV positive and negative lung cancer tissues were analysed for the expression of LMP1 and Bcl-2 by immnohistochemistry. The average area (AA) and integral optical density (IA) of each sample was measured with the digital medical image analyzing system.</p><p><b>RESULTS</b>In 108 cases of lung cancer, 36 cases were EBER1 positive and 7 cases were LMP1 positive. The expression of Bcl-2 was higher in EBV positive lung cancer tissues than that in EBV negative. The AA value was 58014.23 +/- 6918.45 and 38156.22 +/- 4096.79, while the IA value was 11.00 +/- 1.48 and 8.03 +/- 0.78 respectively. No statistic difference was fund in the expression of Bcl-2 betwen LMP1 positive and negative lung cancer tisssues.</p><p><b>CONCLUSION</b>EBV infection in lung cancer increased the expression of bcl-2, which may play a role in the occurrence or development of lung cancer. The increased expression of Bcl-2 may not be induced by LMP1. The exact mechanism need further study.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare therapeutic effects of acupuncture at local points of eye and western medicine on paralytic strabismus.</p><p><b>METHODS</b>One hundred cases were randomly divided into an acupuncture group and a medication group. The acupuncture group (58 cases) was treated with acupuncture at local points of eye, Jingming (BL 1), Tongziliao (GB 1), Shangming (Extra) etc. were selected; and the medication group (42 cases) was treated with oral administration of Methycobal and Vitamin B1. After treatment of 5 courses, the therapeutic effects in the two groups were observed.</p><p><b>RESULTS</b>The total effective rate of 94.8% in the acupuncture group was superior to that of 85.7% in the medication group (P<0.01); the therapeutic effects of the acupuncture group for treatment of oculomotor nerve and abducent nerve were significantly better than that of the medication group (P<0.05); the acupuncture group was better than the medication group in synoptophore examination results and improvement of rima oculi and pupil (P<0.01, P<0.05), the acupuncture group was superior to the medication group in improvement of the function of paralysis eye muscle, including medial rectus and lateral rectus except superior oblique (P<0.01, P<0.05).</p><p><b>CONCLUSION</b>Acupuncture on local points of eye is an effective therapy for paralytic strabismus.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To examine the antagonization of phentolamine against the effects of norepinephrine (NE) on the activity of pain-related neurons in the parafascicular nucleus of morphine-dependent rats.</p><p><b>METHODS</b>Electric impulses were applied as nociceptive stimulus to the right sciatic nerve of morphine-dependent rats, and the discharges of the pain-related neurons in the parafascicular nucleus were recorded by extracellular recording method with glass microelectrodes.</p><p><b>RESULTS</b>Intracerebroventricular injection of norepinephrine resulted in the inhibition of evoked response of the pain-excited neurons as well as the excitation of evoked response of the pain-inhibiting neurons. Both the inhibitory effect on the electric discharges of the pain-excited neurons and the excitatory effect on the pain-inhibiting neurons of norepinephrine were almost completely blocked by intracerebroventricular administration of phentolamine.</p><p><b>CONCLUSION</b>Phentolamine antagonizes the inhibitory effect of norepinephrine on the activity of pain-related neurons in the parafascicular nucleus in morphine-dependent rats, and norepinephrine may play an important role in the integration of the pain signal through the alpha-receptors.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the genetic polymorphism of 9 short tandem repeats (STR) gene loci, namely CSFIPO, TPOX, TH01, D16S539, D7S820, D13S317, F13A01, FESFPS and vWA in Chinese Korean population in Mudajiang area.</p><p><b>METHODS</b>Amplified fragment length polymorphism (Amp-FLP) method was used to get the allele frequency distribution.</p><p><b>RESULTS</b>The genotype distributions of the 9 STR loci are conformed to Hardy-Weinberg equilibrium by chi(2) test analysis. The total accord frequency, the accumulated total discrimination power and the the accumulative excluding probability of paternity were calculated.</p><p><b>CONCLUSION</b>The result suggested that all 9 gene loci have high power of excluding probability of paternity and individual identification. They can be used in paternity testing and individual identification for forensic medicine. The gene frequencies of CSFIPO, TPOX and TP01 gene loci have significant differences between the Korean population in Mudanjiang area and those in Yanji area, but there is no difference in gene loci of D7S820, D17S317 and vWA.</p>

ABSTRACT

<p><b>OBJECTIVES</b>To examine the Epstein-Barr virus (EBV) in primary lung carcinoma tissue, and to investigate the relationship between EBV infection and tumorigenesis of lung cancer.</p><p><b>METHODS</b>Formalin-fixed and paraffin-embedded lung tissue specimens from surgically resected lung carcinoma tissues of 108 cases treated in Tanshan area from 2001 to 2006, which were confirmed further by histopathological examination after hematoxylin-eosin (HE) staining, were used to observe the EBV encoded RNA-1 (EBER1) using in situ hybridization (ISH).</p><p><b>RESULTS</b>EBER1 was detected in 36 of the 108 primary lung carcinoma cases, and in 1 of the 22 normal lung tissues. The positive rates of EBV infection in squamous cell carcinoma, adenocarcinoma, small cell carcinoma and large cell carcinoma were 35.9%, 31.6% 31.0%, 1/2, respectively. Gender, age and clinicohistopathological type were not found to have any correlation with EBER1 expression, but EBER1 expression in groups of cases with poorly and moderately differentiated carcinomas was significantly higher than those in the group of cases with well differentiated carcinoma, and the EBER1 expression in the right lung was higher than in the left lung.</p><p><b>CONCLUSIONS</b>The frequency of EBV infection in this series of patients from Tangshan area was 33.3%, the results suggest that there is a relationship between EBV infection and the occurrence of the primary lung carcinoma, EBV infection might be one of the potential causes to induce lung cancer.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the association between genetic polymorphisms of TGF-beta (TGF-beta) and susceptibility to pneumoconiosis.</p><p><b>METHODS</b>One hundred and seventeen patients with pneumoconiosis were selected as case. The control group was workers exposed to dust but without pneumoconiosis who had the same sex, nationality, and workshop or work site as case. The differences in the age and cumulative exposure time between the case and control group were not move than five years and two years, respectively. The case matched with the control according to 1:1. Polymerase chains reaction-restriction fragment length polymorphism was used to determine the frequencies of TGF-beta genes in the two groups.</p><p><b>RESULTS</b>The frequencies of this TGF-beta (-509) genotypes were CC (22.2%), CT (43.6%) and TT (34.2%) in cases, which was significantly different from the control group, respectively (OR = 1.390, P < 0.05). There was no significant difference for frequency of TGF-beta+869 genotypes and allelic between case and control (P > 0.05). The frequencies of the TGF-beta (+915) genotypes in case [GG (70.9%), GC (29.1%)] were significantly different from the control group (OR = 1.455, P < 0.05). The frequency of TGF-beta (+915) * C allele in the case and control was 14.5% and 8.5%, respectively (P < 0.05). The frequencies of carrying TGF-beta (-509) CC and (+915) GG genotypes were 12.8% and 29.9% in case and control. The frequencies of carrying TGF-beta (-509) * T and (+915) * C alleles were 9.8% and 5.1% in pneumoconiosis and control (P < 0.05).</p><p><b>CONCLUSIONS</b>TGF-beta (-509)CC genotype may be the protective factor for the pneumoconiosis. TGF-beta (+915)GC genotype may be a susceptible factor for the pneumoconiosis. The workers of carrying TGF-beta (-509) * T and (+915) * C alleles are more susceptible to pneumoconiosis.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the clinical and pathologic features of ischemic intestinal disease due to mesenteric phlebitis.</p><p><b>METHOD</b>The clinical and pathologic features of the mesenteric venous lesions in 3 patients of ischemic intestinal disease admitted during the period from 2003 to 2004 were studied.</p><p><b>RESULTS</b>All 3 patients had a clinical history of acute abdominal pain accompanying with a diffuse peritonitis. During operation, an infarcted intestinal segment was identified and was resected respectively in each patient. Histologic examination showed a lymphocytic infiltration and fibrinoid necrosis of the small to medium-sized veins, associated with mural thrombosis and infarction of the corresponding intestinal wall and mesentery. The mesenteric arteries were spared. Two-year follow up of one case showed no evidence of local recurrence or systemic vasculitis.</p><p><b>CONCLUSIONS</b>Ischemic intestinal disease due to mesenteric phlebitis is a rare entity with a pathological feature of inflammation of venous wall accompanying with the development of mural thrombosis and subsequent haemorrhagic infarction of intestine. The etiology is unknown and surgical resection of the involved intestinal segment is usually recommended.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore the relationship between the polymorphism of Interleukin-1 and the pneumoconiosis susceptibility.</p><p><b>METHODS</b>Eighty patients with silicosis and 45 with coal workers' pneumoconiosis (CWP) were selected while 125 male workers, Han nationality in the same workplace as the patients were selected as the controls. Between the patients and the control, the differences of age and cumulative length of service were less than five years and two years, respectively. The controls were exposed to dusts but did not suffer from pneumoconiosis. Moreover, the patients and the controls were paired by 1:1. DNA was extracted from leucocytes by the hydroxybenzene chloroform method. The polymerase chain reaction restriction fragment length polymorphism (PCR-RFLP) techniques and PCR were used to examine polymorphism of IL-1alpha (-889), IL-1beta (-511) and IL-1Ra (+2018) and variable number of tandem repeats (VNTR) of IL-1Ra. After the preliminary experiment, the most adaptive PCR reaction, the restriction enzyme digest and electrophoresis system were used.</p><p><b>RESULTS</b>The difference in IL-1alpha (-889) 1/2 + 2/2 between the pneumoconiosis patients and the controls was significant (P < 0.01). The result of conditional Logistic regression showed that heterozygote and allele 2 of IL-1a (-889) were risk factors of pneumoconiosis. The difference in the genotype frequencies of IL-1beta (-511) 1/2 + 2/2, IL-1Ra (+2018) 1/2 + 2/2 and IL-Ra VNTR1/2 + 2/2 between the patients and the controls were not statistically significant (P > 0.05).</p><p><b>CONCLUSION</b>IL-1alpha (-889) gene polymorphism is related to pneumoconiosis. Workers with IL-1alpha (-889) allele 2 are susceptible to the pneumoconiosis. The relationship between IL-1beta (-511), IL-1Ra (+2018), IL-1Ra VNTR genes polymorphisms and pneumoconiosis are not found.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the feasibility of monoamine neurotransmitters, serum glucose, serum glycerinate and cholesterol as objective indices for evaluating occupational stress.</p><p><b>METHODS</b>Job stressors, modifiers, job strains in 844 people with four kinds of occupation were investigated, and the concentration of monoamine neurotransmitters, glucose, glycerinate and cholesterol in blood were detected at the same time. The methods of multiple stepwise regression and covariance analysis were used to analyze the data.</p><p><b>RESULTS</b>There was close relationship between monoamine neurotransmitters and job stressors, the forecast of the equation of 5-hydroxytryptamine (5-HT), dopamine (DA) and norepinephrine (NE) was 0.7238, 0.5703, 0.4438 respectively, the critical values of them were 804.00, 226.00 and 275.00 ng/ml respectively. There was a little contribution of job stressors to the equation of glucose, glycerinate and cholesterol, the critical values were 6.40, 2.51 and 5.92 mmol/L respectively.</p><p><b>CONCLUSION</b>Monoamine neurotransmitters may be a direct objective evaluating indices. Sugar, glycerinate and cholesterol may be an indirect objective indices.</p>

ABSTRACT

Objective To prospectively evaluate whether DWI with mSENSE(mSENSE-DW1)can help to improve image quality and affect the calculation of lesion's ADC,compared with DWI with conventional phase encoding(conventional DWI)in patients with small HCC lesions.Methods Thirty-two patients with 47 small HCC lesions underwent single-shot spin-echo echo-planar.(SE-EPI)DWI with conventional phase encoding and mSENSE.Three b values of 300,500 and 800 s/mm~2 were used.Image quality and CNR between conventional DWI and mSENSE-DWI group were compared with different b value. Lesion's ADC were also calculated and compared.Matched-pairs Wilcoxon signed rank test and Friedman test were used to test statistical significance.Results The image quality was assessed with a five-point scale.For conventional DWI group,image quality score of 2 was assigned in 5 cases,score of 3 in 24 cases, score of 4 in remaining 3 eases with b=300 s/mm~2,score of 2 in 9 eases,score of 3 in 22 cases,score of 4 in remainingl cases with b=500 s/mm~2,score of 1 in 2 cases,score of 2 in 24 cases,score of 3 in remaining 6 cases with b=800 s/mm~2.For mSENSE-DWI group,image quality score of 3 was assigned in 4 cases,score of 4 in 25 cases,score of 5 in 3 eases with b=300 s/mm~2,score of 3 in 10 cases,score of 4 in 20 cases,score of 5 in remaining 2 cases with b=500 s/mm~2,score of 2 in 7 cases,score of 3 in 23 eases, score of 4 in remaining 2 cases with b=800 s/mm~2.Higher image quality scores were achieved at mSENSE- DWI group than conventional DWI group(Z=-5.578,-5.488,-4.796 respectively,P
