ABSTRACT

<p><b>OBJECTIVE</b>To observe three-dimensional space position change of nucleus pulposus and nerve root before and after treatment of lumbar disc herniation by spinal fixed-point rotating reduction, and explore the mechanisms.</p><p><b>METHODS</b>Totally 52 patients with L5S1 lumbar disc hernation treated by spinal fixed-point rotating reduction were collected from April 2009 to June 2011. There were 33 males and 19 females with an average age of 34.6 years old (ranged, 19 to 55). Three-dimensional MRI were performed to observe relationship between nucleus pulposus and related nerve root,configuration change of spine and pelvic on coronary MRI.</p><p><b>RESULTS</b>MRI showed relationship between nucleus pulposus and related nerve root mainly located on axillary, shoulder, front and surround. Vertebral displacement disappeared, lumbocrural pain alleviated after manipulative therapy. All patients were followed up from 2 to 28 months with an average of 12 months, and no recurred. All patients recovered work. Nucleus pulposus had no change,while lumbral spinal and pelvic curve changed before and after admission.</p><p><b>CONCLUSION</b>Lumbar disc herniation combined with single (multiple) vertebral displacement,can cause biomechanical properties of nucleus puplosus and related nerve root, while spinal fixed-point rotating reduction can correct vertebral displacement, recover balance between inside and outside of spinal.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To measure position changes of lumber disk herniation before and after treatment of Feng's spinal manipulation to study the mechanism of the manipulation for lumber disk herniation.</p><p><b>METHODS</b>In 2010, 8 patients with lumber disk herniation were treated by Feng's spinal manipulation, including 3 males and 5 females who ranged in age from 14 to 53 years with a mean of 35.4 years. The patients received CT scan before and after treatment. All images were reconstructed to measure the distance between the apex of the calcified lumber disk herniation and the top of the next adjacent spinal process.</p><p><b>RESULTS</b>The position of calcified lumber disk herniation underwent change after Feng's spinal manipulation, with a slight subluxation between the two adjacent vertebrae.</p><p><b>CONCLUSION</b>If subluxation is restored,the relation between the nerve root and the hernation will undergo change, followed by subsequent change of the hernation mass, thus restoring the inner-outer balance of the spinal column system.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the aesthetic effect of wound repair with flaps.</p><p><b>METHODS</b>One thousand nine hundred and ninety-six patients with 2082 wounds hospitalized from January 2004 to December 2011. These wounds included 503 deep burn wounds, 268 pressure sores, 392 soft tissue defects caused by trauma, 479 soft tissue defects due to resection of skin cancer and mole removal, 314 soft tissue defects caused by scar excision, and 126 other wounds. Wound area ranged from 1.5 cm x 1.0 cm to 30.0 cm x 22.0 cm. Sliding flaps, expanded flaps, pedicle flaps, and free flaps were used to repair the wounds in accordance with the principle and timing of wound repair with flaps.</p><p><b>RESULTS</b>Five flaps showed venous congestion within 48 hours post-operation, 2 flaps of them improved after local massage. One flap survived after local heparin wet packing and venous bloodletting. One flap survived after emergency surgical embolectomy and bridging with saphenous vein graft. One flap showed partial necrosis and healed after skin grafting. The other flaps survived well. One thousand three hundred and twenty-one patients were followed up for 3 months to 2 years, and flaps of them were satisfactory in shape, color, and elasticity, similar to that of normal skin. Some patients underwent scar revision later with good results.</p><p><b>CONCLUSIONS</b>Application of suitable flaps in wound repair will result in quick wound healing, good function recovery, and satisfactory aesthetic effect.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To summarize and classify the frontal appearance of the philtrum in Nanjing' s young population, so as to assess the role of philtrum in the face aesthetics.</p><p><b>METHODS</b>From August 2010 to December 2011, 160 Nanjing's youth, aged from 20 to 25 years old, underwent photographic study and measurement. The aesthetic rate was graded both for philtrum and the whole face. The results were statistically analyzed.</p><p><b>RESULTS</b>The standard philtral aesthetic appearance is a slim isosceles trapezoid, with an isosceles triangle whose obtuse angle is about 140 degrees reversed at the bottom, like a necktie. The aesthetic rating was highest for philtrum in male NO. 105 and female NO. 33, while for the whole face in male NO.45 and female NO. 7. The mean data of the major parameters were as follows: the length of mouth width was (50.0 +/- 4. 3) mm (male), (47.0 +/- 4.0) mm (female); the length of right red lip was (20.2 +/- 2.4) mm (male), (19.3 +/- 2.3) mm (female); the length of left red lip was (20.2 +/- 2.2) mm (male), (18.9 +/- 1.8 ) mm (female); the length of right philtrum column was (15.5 +/- 2.0) mm (male), (13.6 +/- 2. 2) mm (female); the length of left philtrum column was (15.6 +/- 1.9) mm (male), (13.7 +/- 2.3) mm (female); the length of philtrum sulcus was (17.3 +/- 2.1) mm (male), (15.3 +/- 2.4) mm (female). The upper breadth of philtrum was (5.6 +/- 0.8) mm (male), (5.5 +/- 1.1) mm (female); the middle breadth was (9. 5 +/- 1.1) mm (male), (8.5 +/- 1.2) mm (female); the lower breadth( cupid bow width) was (13.8 +/- 1.8) mm (male), (11.8 +/- 1.6) mm (female). The right half of cupid bow was (7.5 +/- 0.9) mm (male), (6.5 +/- 0.9) mm (female); The left half of cupid bow was (7.2 +/- 0.9) mm (male), (6.1 +/- 0.8) mm (female). The philtrum angle was (141.2 +/- 8.0) degrees (male), (141.3 +/- 6.1) degrees (female). Although there is slight difference in the measurement results between two sides of philtrum column and cupid bow (P < 0.01), it is not obvious by naked eyes. Except for the upper width of philtrum and the philtrum angle (P > 0.05), the other parameters have significant sexual differences (P < 0.05).</p><p><b>CONCLUSION</b>The beautiful philtrum can not mainly decide the whole face aesthetics, which usually shows a harmony relationship between all the facial aesthetic units. The measurement results can be used as a reference for philtrum plasty.</p>

ABSTRACT

Cycloheximide (CHX) inhibits protein synthesis in most eukaryotic cells and it is a well-known tool commonly used in biochemical research. In this paper, the antiviral spectrum of CHX against several DNA and RNA viruses have been evaluated. CHX showed strong inhibitory activities against several RNA viruses such as HIV-1, influenza viruses, coxsackie B virus, enterovirus (EV71) and several DNA viruses such as HSV and HCMV. Especially the strong inhibitory activities of CHX against coxsackie B virus and enterovirus caught our attention, since effective drugs available in clinic are limited. The SAR of CHX derivatives also has been discussed in the paper. The hydroxyl group at C-2' and carbonyl group at C-2" of CHX are essential for its antiviral activity. And modification to these groups results its derivatives' antiviral activities reduced or lost.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the pinching action to the nerve root by the lumbar disc herniation and facet or lamina (ligamentum flavum) and evaluate its clinical meaning.</p><p><b>METHODS</b>Seventy-one patients were divided into 3 groups according to the size of distance between protrusion of nuclear and facet or lamina (ligamentum flavum). The degree of the straight leg raising of the affected side (SLR) and the sagittal index (SI) of lumbar disk herniation were measured and analyzed among them.</p><p><b>RESULTS</b>There was no corelation between the affected degree of SLR and sex, age and SI of patient (P > 0.05). The coefficient between distance group and the affected degree of SLR is-0.878 7 (P < 0.01).</p><p><b>CONCLUSION</b>The degree of the surrounding compression by the protrusion of nuclear and facet or lamina (ligamentum flavum) reflects the injuried severity of the nerve root.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical effect of the treatment for sequestered type lumbar disk herniation (SLDH) with FENG's spinal manipulation.</p><p><b>METHODS</b>Forty-one patients (27 male, 14 female) were treated by FENG's spinal manipulation. The clinical results were measured by the Japanese and FENG's lumbar treatment standard and evaluated by scoring system for assessment of treatment for low back pain between pre- and post-treatment. To measure MRI sagittal index (SI) of lumbar disc herniation between pre- and post-treatment.</p><p><b>RESULTS</b>The score of subjective symptoms and clinical signs increased obviously after treatment. There was no statistical difference of SI between pre- and post-treatment (P<0.05). The outcome of treatment was satisfying.</p><p><b>CONCLUSION</b>The major pathologic change of SLDH is the single/multiple vertebral subluxation. FENG's spinal manipulation can correct the vertebral subluxation which leading to the series of clinical manifestations. The good clinical outcome may due to the alleviation of distortion and tension of dura and nerve root with the recovery of the spinal column's stability.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the incidences of urea cycle defects (UCDs) in the patients with hyperammonemia and study their etiology, clinical and laboratory features.</p><p><b>METHODS</b>In the past 7 years, 26 cases (10.2%) of UCDs were detected from 254 patients with hyperammonemia. The etiological diagnoses were made by blood amino acids analysis, urinary organic acid analysis and blood acylcarnitine profile analysis. Three patients with citrullinemia type II were further confirmed by liver pathological analysis and gene diagnosis.</p><p><b>RESULTS</b>Among 26 cases with UCDs, 15 had ornithine transcarbamylase (OTC) deficiency, 5 had citrullinemia type I, 3 had citrullinemia type II and 3 patients had arginemia. The age of onset of the patients ranged from 3 days to 13 years. Three cases (11.5%) developed hyperammonemic encephalopathy during neonatal period. Thirteen (50.0%), 7 (26.9%) and 3 (11.5%) cases developed clinical symptoms at the age of 1 to 12 months, 1 to 3 years and 6 to 13 years, respectively. Positive family history was found in 11 cases (42.3%). Among 26 patients with UCDs, 9 (34.6%) were hospitalized with the complains of seizures, psychomotor retardation, vomiting and unconsciousness, 8 (30.8%) with recurrent vomiting, headache and coma, 6 due to liver dysfunction. Intrahepatic cholestatic jaundice was found in 3 patients with citrullinemia type II. Blood ammonia ranged from 58 to 259 micromol/L on their first visit to our hospital. Twenty cases (76.9%) had liver dysfunction, 4 patients (15.4%) were diagnosed postmortem. Twenty-one patients got treatment and were followed up. Among them, 7 cases died of hyperammonemic encephalopathy or upper alimentary tract bleeding. Clinical improvement was observed in 14 cases. A boy with OTC deficiency who received a partial liver transplant from his mother showed normal general condition for two years.</p><p><b>CONCLUSIONS</b>UCDs are the most frequent causes of congenital hyperammonemia. In this study, 26 patients (10.2%) with UCDs were identified from 254 patients with hyperammonemia resulting in encephalopathy and liver dysfunction. Early diagnosis and treatment can contribute a lot to improve the prognosis of the patients. Blood ammonia assay and further etiological analysis should be considered in the differential diagnosis of neurological and hepatic abnormality.</p>

ABSTRACT

<p><b>AIM</b>To isolate and prepare chemical standards.</p><p><b>METHODS</b>Flavonol glycoside and rhamnonic acid gamma-lactone were isolated from whole plants of Lysimachia christinae Hance. with 95% ethanol and purified by silica gel column chromatography. UV, IR, MS, 1HNMR, 13CNMR, DEPT, HMQC, HMBC were used for the structural identification.</p><p><b>RESULTS</b>Two compounds were obtained. They were identified as rhamnonic acid gamma-lactone (I) and kaempferol-3-O-alpha-L-rhamnosyl (1-->2)-beta-D-xyloside (II).</p><p><b>CONCLUSION</b>Compound I was isolated from this genus for the first time. Compound II is a new compound named as lysimachiin.</p>
