ABSTRACT

Craniopharyngioma is the most common benign intracranial tumor in children. The major post-operative complication is dysfunction of pituitary, which can result in many complicate clinical manifestations with hormonal deficiencies. Normochromic anemia has been reported as a common hematologic abnormality. However, pancytopenia is rarely reported so far. Here we describe a 21-year-old inpatient with the main complaint of nasal bleeding, who accepted craniopharyngioma surgery 9 years ago. Laboratory tests showed pancytopenia secondary to panhypopituitarism. This paper aims to increasing the awareness of this disease and accumulating clinical experiences for the clinicians.

ABSTRACT

Objective To investigate the risk factors,clinic charactertics and survival prognosis of pulmonary embolism (PE) in patients with lung cancer.Methods The clinic data of 28 lung cancer patients with PE,hospitalized in department of respiratory and critical care medicine of Tianjin Chest Hospital between June 2012 to June 2015,were retrospectively reviewed.Eleven of them were diagnosed with primary lung cancer and PE (symptomatic group),and 17 lung cancer patients were found PE accidentally (asymptomatic group).A total of 56 lung cancer patients without PE were used as control subjects (no PE group).Data of pathological types,clinical staging of lung cancer,systemic chemotherapy,white blood cell (WBC),hemoglobin (Hb),platelet (PLT),D-dimer (DD),albumin (ALB) and C-reactive protein (CRP) were analyzed by univariate analysis in the symptomatic group and asymptomatic group.Logistic regression analysis was carried out on the statistically significant indexes to observe the influencing factors.The morphology and location of the remobilization images were analyzed in lung cancer patients with PE.The median time to embolism and survival of PE patients were compared between symptomatic group and asymptomatic group.Results The proportion of adenocarcinoma,systemic chemotherapy and stage Ⅲ + Ⅳ were significantly higher in PE group than those in no PE group (P < 0.01).The ratio of white blood cells (WBC)> 11× 109/L (hyperleukocytosis) and albumin (ALB) <30 g/L and D-dimer (DD)> 0.5 mg/L were significantly higher in PE group than those of no PE group (P < 0.05).There were no statistical differences in other indicators including clinical symptoms between the two groups (P > 0.05).The logistic regression analysis showed that adenocarcinoma,chemotherapy,WBC> 11× 109/L and DD>0.5 mg/L were the risk factors of lung cancer with PE (P < 0.05).There was higher ratio of asymptomatic PE in lung cancer patients with PE.There were no significant differences in morphology and location of the remobilization images in symptomatic group.The median time of embolization was 3.6 months (95% CI:3.2-4.0) in the asymptomatic group,which was significantly earlier than that in the symptomatic group (10.5 months,95% CI:8.88-12.12,P < 0.01).The median survival time was 7.2 months (95% CI:5.86-8.56) in the asymptomatic group,which was significantly longer than that of symptomatic group (2.8 months,95% CI:2.48-3.12,P < 0.05).Conclusion Lung adenocarcinom,systemic chemotherapy,hypoproteinemia and increased D-dimer are the independent risk factors in lung cancer patients with PE.PE in lung cancer is frequently asymptomatic in the early stage.Compared to symptomatic patients,asymptomatic cases have better prognosis.

ABSTRACT

Objective:To investigate the extracellular signal-regulated kinase 5 (ERK5) and matrix metallo proteinase-9 (MMP-9) expres-sion levels in osteosarcoma tissues and their clinical significance. Methods:The ERK5 and MMP-9 expression levels in 71 specimens of osteosarcoma tissue and 40 specimens of normal bone tissue were detected by immunohistochemistry. The relationship between ERK5 and MMP-9 expression levels, their clinical characteristics, and prognosis of patients with osteosarcoma were analyzed. Results:The positive expression of ERK5 and MMP-9 in osteosarcoma tissues was 85.9%(61/71) and 74.65%(53/71), respectively, which were significantly higher than those in normal bone tissues at 12.5%(5/40) and 10.0%(4/40) (all P<0.05). The positive expression of ERK5 and MMP-9 was associated with Enneking stage and metastasis (all P<0.05). Kaplan-Meier analysis showed that the survival duration of patients with positive ERK5 and MMP-9 expression levels was shorter than those of the patients in the negative expression groups (all P<0.05). Univariate analysis of COX proportional hazards regression model revealed that tumor size, Enneking stage, metastasis, and positive ERK5 and MMP-9 expression levels are relevant to the overall survival of patients with osteosarcoma (all P<0.05). Multi-variate analysis of COX proportional hazards regression model confirmed that Enneking stage, metastasis, and positive ERK5 and MMP-9 expression levels can act as independent prognostic factors for osteosarcoma patients (all P<0.05). Conclusion:The ERK5 and MMP-9 expression levels are high in osteosarcoma tissues and are related to the clinical characteristics and prognosis of patients with osteo-sarcoma. Thus, ERK5 and MMP-9 expression levels may play important roles in osteosarcoma development and progression.

ABSTRACT

The concentration of serum adiponectin [(2.51±1.42)mg/L] was lower in the group of patients with type 2 diabetes mellitus as compared with that in normal controlgroup [(5.26±0.78)mg/L ,P<0.01]. The concentration of serum adiponectin was lower in the diabetics with macrovascular complications (MVC) [(1.38±0.77)mg/L] as compared with those without macrovascular complication [(3.66±0.91)mg/L]. The concentration of serum resistin was higher in the diabetic group as compared with that in control group[(7.07±1.11 vs 6.09±0.47)μg/L, P<0.01]. It was also higher in patients with MVC [(7.96±0.65)μg/L] compared with those without MVC [(6.10±0.43)μgL, P<0.01].

ABSTRACT

Objective Comtaring the differences of Clinical features and chronic complications between Chinese and Japanese group in newly diagnosed type 2 diabetic patients in order to search effectively preventive precaution against early diabetes. MethodsKWe adopt the systematic documents designed by Kyoto National Hospital Diabetes Center of Japan including the background factors, examination quotas relating to clinical and the criteria for judging neuropathy to manage the standard registration of diagnosed type2 diabetic patients through the way when patients come to hospital. ResultsBackground factors: Average age of Chinese group was 6 years old younger than Japanese group when newly diagnosed ( P 0.01). There was no difference in CVD and DR between two groups. Conclusion The rate of chronic complications of the two groups was higher when first diagnosed, but CHD, MAU of Chinese group were higher than Japanese group. The difference of inheritable background, BMI , HOMA, HT and dyslipidemia of two groups may lead to such results.
