ABSTRACT

Objective:To explore the changing track of cancer-related fatigue in patients with lung cancer during perioperative period and analyze its influencing factors. Methods:From December 2020 to May 2021, 162 eligible patients with lung cancer were selected in the First Affiliated Hospital of Shandong First Medical University & Shandong Provincial Qianfoshan Hospital by convenient sampling method. Mental resilience and social support were investigated before operation (T0);and cancer-related fatigue was investigated before operation (T0),3 days after operation (T1), 1 week after operation (T2), 2 weeks after operation (T3) and 1 month after operation (T4), and Latent Class Growth Model was used to identify the track categories. Logistic regression analysis was used to analyze the influencing factors. Results:Three cancer-related fatigue tracks were fitted in this study, which were no fatigue group (31.7%), increased fatigue group (36.0%) and sustained fatigue group (32.3%). Univariate analysis showed that smoking history ( χ2=14.83, P<0.05), BMI ( χ2=29.67, P<0.05), hemoglobin count ( F=4.57, P<0.05), mental resilience ( F=35.53, P<0.05), social support ( F=47.53, P<0.05) were related to the track category of cancer-related fatigue. Logistic regression analysis showed that smoking history, over weight/obesity, resilience and social support were the predictors of cancer-related fatigue locus ( P<0.05). Conclusions:Perioperative cancer-related fatigue of patients with lung cancer has different change trajectories, patients with higher BMI and lower resilience are more likely to have a tendency of persistent fatigue. Patients with smoking history and lower social support are more likely to have an upward trend of fatigue. For this kind of lung cancer patients, medical staff should take timely and effective intervention measures to improve their cancer-related fatigue.

ABSTRACT

OBJECTIVE:To stud y the effects of “green removing ”processing technology of fresh fruit of Schisandra chinensis after harvested on the quality of medicinal material ,and to provide new ideas for the scientific and rational processing of Chinese medicinal material. METHODS :Fifteen fresh fruits of S. chinensis were used as samples ,with 3 samples in each group. The sample were processed preliminarily by 5 methods, such as drying at 50 ℃,drying in the sun ,drying at 50 ℃ after“green removing”processing with microwave ,drying at 50 ℃ after“green removing ”processing with blanching ,drying at 50 ℃ after “green removing ”processing with steaming. HPLC fingerprints of 15 batches of dried S. chinensis products were established and similarity evaluation was conducted according to Similarity Evaluation System of TCM Chromatographic Fingerprints (2012 edition). Cluster analysis was used to evaluate the similarity of dried S. chinensis products with different processing methods. At the same time ,HPLC method was adopted to determine the content changes of seven lignans in dried products ,such as schisandrol A , schisandrol B ,schisantherin A ,schisantherin B ,schizandrin A ,schisandrin B and schisandrin C. RESULTS :A total of 7 common peaks were obtained in the fingerprints of 15 batches of dried S. chinensis products. Except that the similarity between the chromatograms of dried samples in the sun and the control fingerprint was relatively low ,the similarities of chromatograms of dried products by other processing methods were greater than 0.900. Cluster analysis showed that 6 samples dried at 50 ℃ after“green removing”processing with microwave and dried at 50 ℃ after“green removing ”processing with blanching were grouped into the first category ;3 samples dried at 50 ℃ after“green removing”processing with steaming were grouped into the second category ;6 samples dried at 50 ℃ and dried in sun were grouped into the third category. The content determination results showed that there was no significant difference in the total content of seven lignans in the samples dried at 50 ℃ and dried in the sun (P>0.05). The total contents of seven lignans in the samples dried at 50 ℃ after“green moving ” processing with microwave ,blanching and steaming were significantly higher than those dried at 50 ℃ and dried in sun (P<0.01). CONCLUSIONS:The quality of S. chinensis samples dried after “green moving ”processing with microwave and blanching is better than those directly dried in sun and dried in oven.

ABSTRACT

Objective To explore the clinical symptom clusters in breast cancer patients with anthracycline treatment, which could provide evidence for prevention. Methods The M.D.Anderson Symptom Inventory of Chinese version (MDASI-C) was applied to assess clinical symptoms in 506 breast cancer patients received anthracycline therapy during their 1stto 4thcycle chemotherapy.Thirteen symptoms were analyzed using main-component analysis and variance orthogonal rotation. The exploratory factor analysis was conducted to find factors value greater than 1. Results The number of symptoms with incidence rate more than 50% was 5, 6, 7 and 9 during the 1stto the 4thcycle, respectively. Fatigue, poor appetite, and nausea were the most common symptoms, and the incidence of these symptoms were 92.5% to 97.1% ,84.8% to 95.1% and 81.1% to 91.3% with the increasing cycle of chemotherapy.Three factors value greater than 1 were detected during the 1stto 2ndcycle chemotherapy by exploratory factor analysis. The cumulative variance contribution rates were 63.233% and 61.434% in the 1stand 2ndcycle, respectively. The main symptom clusters concentrate on fatigue and digestive tract symptoms, including fatigue, sleep disturbance, hypersomnia, nausea, vomit, poor appetite, dry mouth. Two factors value greater than 1 were detected during 3rdto4thcycle in chemotherapy. The cumulative variance contribution rates were 62.660% and 61.148% in the 3rdand 4thcycle, respectively. The main symptom clusters concentrate on psychological and nervous system symptoms including sadness, pain, dry mouth, numbness, hypersomnia, shortness of breath, amnesia and so on. The Cronbach α of cluster symptoms from the 1stto the 4thcycle chemotherapy was between 0.829 to 0.911. Conclusions Symptom clusters vary with the cycles of chemotherapy in breast cancer patients treated with anthracycline. Nurses should provide targeted intervention measures to improve symptom and enhance quality of life, according to specific situation.

ABSTRACT

Objective The study was aimed to explore the effect of care bundles on the management of nasal intestine for the cerebral apoplexy patients with dyspinic dysfunction. Methods A total of 102 cerebral apoplexy patients with dyspinic dysfunction, who were administrated with nasal intestine, were recruited in Qianfoshan Hospital of Shandong Province from August, 2016 to June, 2017. Random number table was applied to analyze the separate two groups, experimental group and control group, in which, the patients were administrated with care bundles and usual care. Various parameters, like one-time catheterization, complications of nasal intestine administration, hospital stay, cost and the patients' satisfaction degree, were compared between two groups. Results The success rate of once placed tube in the test group was 78.4% (40/51), which was significantly higher than that in the control group [54.9%(28/51)], and the difference was statistically significant (χ2=6.353, P<0.05). The incidence of complications of indwelling nasoenteric tube, including unplanned extubation, reflux aspiration, tube-feeding associated diarrhea, were 9.8%(5/51), 17.6%(9/51), 11.8%(6/51), respectively. All of them were significantly lower than those of the control group, which were 37.3%(19/51), 41.2%(21/51), 31.4%(16/51), respectively, indicating that there was a statistical difference (χ2= 10.679, 6.800, 5.795). The days and costs of hospitalization were (11.0 ± 2.5) d, (9645.35 ± 193.30) yuan, respectively, which were significantly lower than those of the control group [(17.0 ± 4.2) d, (12523.79 ± 190.28) yuan], indicating that there was a statistical difference (t=8.767, 10.612, P<0.01). Patients' satisfaction was 90.2%(46/51), which was significantly higher than that of the control group [68.6% (35/51)]. The difference was statistically significant (χ2=7.256, P<0.01). Conclusions The administration of care bundles for the cerebral apoplexy patients with dyspinic dysfunction can effectively increase the success rate of one-time catheterization, alleviate the complications, and therefore reduce the risk of aspiration pneumonia development. In addition, care bundles can remarkably alleviate the medical burden on the patients, shorten the hospital stays and improve the survival quality of the patients.

ABSTRACT

Objective To investigate the current prevention status of hospital acquired venous thromboembolism(VTE) in Shandong province, to further improve the prevention and control of VTE system, and provide reference for prevention of hospital care to improve the quality of VTE. Methods An electronic questionnaire survey was conducted among nurses in 81 hospitals of Shandong province. The questionnaire mainly included three parts:basic information, the current prevention status of hospital acquired VTE and the training needs of nurses. A total of 3 766 valid questionnaires were recovered. Results 59.26%(48/81)hospitals established multi-discipinary team to prevent VTE,76.00%(38/50)of the tertiary hospitals established multi-discipinary team, which was higher than 32.26% (10/31) of the secondary hospitals, the difference was statistically significant (χ2=15.166, P=0.000). The VTE risk assessment scale had been routinely used to assess the risk of VTE in hospitalized patients,80.37%(352/438) tertiary hospital departments implemented grading nursing care while 71.43%(175/245) secondary hospitals departments implemented grading nursing care, the difference was statistically significant(χ2=7.120, P=0.008). Each level hospital mechanical preventive equipment configuration rate was less than 70%.Training needs of nurses:92.11%(3 469/3 766)nurses believed that it was necessary to organize the VTE related knowledge training for nurses, but there were significant differences in the training of nurses with different professional titles (Z=-12.607, P=0.000). 48.04%(1 149/2 392) nurses with junior grade professional titles were not trained. Conclusions The hospital attaches great importance to the prevention of hospital acquired VTE,but the construction level of VTE prevention system is unevenness, and primary hospitals should be further reinforced. The risk assessment scale selection lacks of uniform standards, mechanical preventive equipment allocation rate should to be further improved. And the training of primary nurses should be further strengthened.

ABSTRACT

Objective:To observe the effect of Yizhi cream on vascular dementia in rats. Methods:The rat model of vascular de-mentia was established by the double thread ligation of bilateral common carotid artery. The rats in each dose group were intragastrically given Yizhi cream at high, medium and low dose for 30 days. Morris water maze was used to observe the effect of Yizhi cream on learn-ing and memory impairment in rats. The biochemical method was used to observe the levels of acetylcholine (Ach) and nitric oxide (NO),and the activity of nitric oxide synthase(NOS) in serum was measured. The brain index was calculated to evaluate the state of cerebral edema. HE staining was used to observe the pathological changes of brain tissue. Results: Compared with that in the model group, the escape latency in each Yizhi cream group was significantly shortened(P<0.05),the number of sites found in 3 min signif-icantly increased (P<0.05),the Ach content significantly increased (P<0.05),the NO content and NOS activity significantly de-creased (P<0.05),the brain index was significantly lowered (P<0.05), and the brain histopathology was significantly improved. Conclusion:Yizhi cream can significantly improve the learning and memory ability of vascular dementia rats, and notably improve the cerebral edema and brain histopathology in the rats with vascular dementia. The mechanism of action is related to reducing the serum NO content and NOS activity, enhancing the ability of oxidative damage prevention, reducing brain injury, increasing the Ach content and enhancing the function of cholinergic nerve.

ABSTRACT

Objective To optimize the prescription ofBaichanting Tablets by the central composite design-response surface methodology. Methods The doses of MCC, cCMC-Na, SiO2, and magnesium stearate were set as investigation factors; disintegration time and moisture rate were set as indexes. Linear equation quadratic polynomial described mathematic relationship of disintegration time and moisture rate with other four influence factors. Response surface was described according to the optimal mathematic models; the optimal prescription was chosen; predictive analysis was conducted.ResultsThe relationship of disintegration time and moisture rate with other four influence factors could not be described by linear equation. When quadratic polynomial matching was used, correlation coefficients were 0.837 9 and 0.923 1, with relatively high reliability. Optimal prescription contained 30.6%MCC, 10%cCMC-Na, 0.30%SiO2, and 0.10% magnesium stearate. The theoretical value and predicted value deviations of the disintegration time limit and moisture absorption rate were within 5%.Conclusion The predictability of the established model is good. Application of central composite design-response surface methodology can precisely optimize the prescription ofBaichanting Tablets.

ObjectiveTo study the neuron protection mechanism of Uncariae Ramulus Cum Uncis extract for the MPTP-induced PD mice.MethodsC57BL/6 mice were randomly divided into control group, model group, Uncariae Ramulus Cum Uncis extract group and madopar group, and injected with MPTP in abdominal cavity. Behavior test was used to detect grabbing capacity and body movement coordination capacity: three biochemical indexes (SOD, MDA, GSH-Px) were detected by biochemical process and the activity of two immune enzyme-linked indexes (IL-1β, IL-6) were detected by enzymelinked immunosorbent assay.ResultsCompared with the control group, the autonomic activity numbers of mice increased and climbing pole time decreased in the model group (P<0.05), enzymatic activity of SOD and GSH-Px decreased significantly and the contents of MDA, IL-1β and IL-6 increased obviously (P<0.01). Compared with model group, the autonomic activity numbers of mice increased and climbing pole time decreased in the Uncariae Ramulus Cum Uncis extract group and madopar group (P<0.05). Enzymatic activity of SOD and GSH-Px increased and the contents of MDA and IL-1β and IL-6 decreased (P<0.05,P<0.01).ConclusionUncariae Ramulus Cum Uncis extract can reduce neuronic apoptosis PD mice, whose therapeutic action may be realized through eliminating oxygen free radicals, improving oxidation resistance and reducing inflammatory reactions.

ABSTRACT

BACKGROUNDThe regimen of cyclophosphamide, doxorubicin, vincristine, and prednisolone (CHOP) is an efficient treatment of non-Hodgkin's lymphoma (NHL). This study aimed to assess the efficacy and toxicity of dose-adjusted CHOP alone or in combination with rituximab (R-CHOP) by examining the stem cell mobilization in NHL patients. Factors affecting the collection of CD34+ cells were also explored.

METHODSOur retrospective study included 39 patients eligible for autologous stem cell transplantation: 14 patients who expressed CD20 and were financially eligible received R-CHOP for autologous peripheral blood stem cell (APBSC) mobilization; the remaining 25 patients received CHOP.

RESULTSThe median CD34+ cell yield was 7.01×10(6) cells/kg body weight (range 1.49-28.39×10(6) cells/kg body weight), with only two patients failing to meet the target CD34+ cell harvest of ≥2.0×10(6) cells/kg body weight. The median number of apheresis procedures per patient was 1 (range 1-3). The APBSC mobilization yield of the CHOP group appeared to be higher than that of the R-CHOP group (P=0.005), whereas the success rate was similar between groups. R-CHOP elevated the complete response (CR) rate in B cell lymphoma patients as compared with CHOP (P=0.01). No significant differences in toxicity or engraftment were observed between the two groups.

CONCLUSIONThe present study demonstrated that dose-adjusted CHOP chemotherapy effectively mobilized APBSCs in NHL patients and that the addition of rituximab to dose-adjusted CHOP chemotherapy elevated the CR rate for patients with B-cell lymphoma.

ABSTRACT

OBJECTIVE@#To investigate the effect of genistein on cell proliferation and apoptosis in human laryngeal carcinoma cell line Hep-2.@*METHOD@#Cell Counting Kit-8 (CCK-8) assay was used to measure the 50% inhibiting concentration (IC50) value of genistein; cell apoptosis rate and the distribution changes of cell cycle were determined with flow cytometry assay after treatment by gensitein. The morphological changes of tumor cells were evaluated by inverted phase contrast mircroscopy.@*RESULT@#The IC50 of geniste responses to Hep-2 cells for 24 h was 23.64 µg/ml. The apoptotic rates of Hep-2 cells treated by genistein for 24 h were 22.40% ± 1. 65% (at 12 µg/ml genistein) and 30.64% ± 2.94% (at 24 µg/ml genistein) respectively, significantly statistical differences were foundbetween above threated groups and the control group (P < 0.05); the apoptotic rates of Hep-2 cells treated by genistein for 48 h were 30.55% ± 0.72%(at 12 µg/ml genistein) and 48.69% ± 1.06% (at 24 µg/ml genistein) respectively, significantly statistical differences were found between above threated groups and the control group (P < 0.05). When Hep-2 cells exposed to the same concentration of genistein for 24 h, 48 h respectively, the difference in apoptotic rate was statistically significant.@*CONCLUSION@#Genistein inhibited Hep-2 cells growth obviously, meanwhile it could induced apoptosis of Hep-2 cells, the apoptotic rate was increasing with the increase of the time and dose of genistein.
