ABSTRACT

Objective@#To evaluate the concordance of PD-L1 expression in various tissues using antibodies 28-8 and SP263 on their respective detection platforms.@*Methods@#Three hundred seventy four specimens of surgical resection of pulmonary diseases in the First Affiliated Hospital of Nanjing Medical University from January 1, 2012 to January 31, 2017 were collected. Totally 374 cases were tested for PD-L1 expression using the two antibodies, 28-8 and SP263, by respective detection platforms (Dako and Ventana). Finally, 336 cases were used for further evaluation, and the results were statistically analyzed for concordance.@*Results@#For non-small cell lung carcinoma (NSCLC), the positive rate of PD-L1 was 57.5% (177/308) using SP263, and 57.5% (177/308) using 28-8 antibody. The correlation coefficient was 0.97 (P<0.01). The positive rate of both benign lung diseases and paracancerous tissues was about 10.7% (3/28), and the positive concordance rate was 100.0%. The distribution of both antibodies was also relatively consistent.@*Conclusions@#The expression levels of 28-8 and SP263 antibodies in NSCLC and other tissues are relatively consistent, suggesting both antibodies may be complementary and substitute for each other, which may be useful in guiding clinical management.

ABSTRACT

Objective To analyze the occurrence of ventricular arrhythmia in old myocardial infarction (OMI) patients who combined with pathologic Q wave and fragmentation QRS (fQRS), and explore the relationship between QRS duration and ventricular arrhythmia. Methods Three hundred and thirty-five patients with OMI were examined with dynamic electrocardiogram, and they were divided into QRS duration≥110 ms group (M group, 57 cases) and QRS duration720 beats/24 h, polymorphic ventricular extrasystoles, pairing ventricular extrasystoles, non-sustained ventricular tachycardia and ventricular escape in M group were significantly higher than those in N group: 38.60%(22/57) vs. 16.55% (46/278), 22.81% (13/57) vs. 7.55% (21/278), 43.86% (25/57) vs. 20.50%(57/278), 19.30% (11/57) vs. 3.96% (11/278) and 14.04% (8/57) vs. 3.96% (11/278), and there were statistical differences (P0.05). In N group, the incidences of premature ventricular beats>720 beats/24 h in Na, Nb, Nc and Nd group were 24.73%(23/93), 13.64%(6/44), 25.81%(8/31) and 8.18%(9/110);the incidences of pairing ventricular extrasystoles were 30.11% (28/93), 15.91% (7/44), 29.03% (9/31) and 11.82% (13/110), and there were significant differences ( P720 beats/24 h of Nc group > Na group > Nb group > Nd group, and the incidence of pairing ventricular extrasystoles of Na group>Nc group>Nb group>Nd group. The incidences of polymorphic ventricular extrasystoles and non-sustained ventricular tachycardia in Ma group were significantly higher than those in Na group:34.78%(8/23) vs. 11.83%(11/93) and 30.43%(7/23) vs. 6.45%(6/93), and there were statistical differences (P 720 beats/24 h, polymorphic ventricular extrasystoles and pairing ventricular extrasystoles in Md group were significantly higher than those in Nd group:4/13 vs. 8.18%(9/110), 3/13 vs. 2.73%(3/110) and 6/13 vs. 11.82%(13/110), and there were statistical differences (P<0.05 or<0.01). Conclusions The longer QRS duration in OMI patients, the higher risk of ventricular arrhythmia. The ventricular arrhythmia risk is positively correlated with QRS duration in OMI patients with fQRS. There is still risk of ventricular arrhythmia in well recovered OMI patients (those without pathological Q wave and with fQRS), and it is positively correlated with QRS duration. There is a higher incidence of ventricular arrhythmia in OMI patients with QRS duration≥110 ms, but it is not significantly correlated with the pathological Q wave and fQRS. Simple fQRS and fQRS with pathologic Q wave in OMI patients with QRS duration<110 ms is significantly correlated with ventricular arrhythmia.

ABSTRACT

ObjectiveTo explore the relationship of the fragmented QRS (fQRS) and the fQRS time limit with ventricular arrhythmia in old myocardial infarction (OMI) patients through contrasting the incidence of ventricular arrhythmia in OMI patients whether fQRS or not and ventricular arrhythmia in different fQRS time limit.MethodsAccording to the routine electrocardiogram,321 OMI patients were divided into group A (fQRS appearance,167 cases) and group B(fQRS non-appearance,154 cases).The lead with fQRS extense was ehosen and traced another 50 mm/s electrocardiogram,and 3 consecutive fQRS time limit were measured and them average was taken in group A.According to the fQRS time limit,the patients in group A were divided into 3 groups: group X( ≤0.100 s,96 cases),group Y (0.101-0.119 s,54 cases) and group Z( ≥0.120 s,17 cases).All the patients were continuously monitored with 24 hours dynamic electrocardiogram,and the incidence of ventricular arrhythmia was analyzed.ResultsThe incidence of ventricular arrhythmia in group A [ 78.4% ( 131/167 ) ] was higher than that in group B [ 63.6%(98/154) ] (P< 0.01 ).The incidence of premature ventricular contraction(PVC) > 720/24 hours in group A [ 28.7%(48/167 ) ] was higher than that in group B[ 17.5%(27/154) ] (P < 0.05 ).The incidence of multifocal PVC,coupled PVC,nonsustained ventricular tachycardia and Lown 3-5 grades PVC was 16.2% (27/167),33.5% (56/167),12.0% (20/167),34.1% (57/167) in group A,7.8% (12/154),21.4% (33/154),4.5%(7/154),23.4%(36/154) in group B,there were significant differences between two groups (P< 0.05 ).The incidence of ventricular arrhythmia in group Z [ 100.0%( 17/17 ) ] was significantly higher than that in group Y [79.6%(43/54)] and group X [74.0%(71/96)](P< 0.05).The incidence of Lown 3-5 grades PVC in group Z[ 70.6%( 12/17 )] was significantly higher than that in group Y[ 42.6%(23/54)] and group X [ 22.9%(22/96) ],and the incidence of Lown 3-5 grades PVC in group Y was significantly higher than that in group X (P< 0.05).ConclusionsOMI patients with fQRS have higher incidence and severe degree in ventricular arrlhythmia than those without fQRS.With the fQRS time limit widened,PVC and Iown 3-5 grades PVC significantly increased.So fQRS is a new predicting index of OMI,and fQRS time limit has definite value in predicting the heart event for OMI patients.
