ABSTRACT

ABSTRACT: This manuscript described the anatomopathological and immunohistochemical findings in a rare case of mammary fibroadenomatoid hyperplasia in a 12-month-old Holstein heifer. A yellow, multilobulated, firm 20cm x 9cm x 6.5cm mass affecting the right quarters of the udder was observed. Total mastectomy was performed. Microscopic evaluation revealed severe hyperplasia of the mammary epithelium and numerous well-differentiated and mildly pleomorphic acini. Additionally, moderate proliferation of the fibrous connective tissue and the myoepithelial cells near the proliferating acini was evident. About 50% of the proliferating epithelial cells showed positive nuclear labeling for estrogen and progesterone receptors, and approximately one-third were positive for Ki-67. In addition, the myoepithelial cells exhibited diffuse nuclear immunoreactivity for p63. Based on the clinical, morphologic, and immunohistochemical findings, a diagnosis of mammary fibroadenomatoid hyperplasia with probable influence of ovarian steroids was made.


RESUMO: Este artigo descreve os achados anatomopatológicos e imuno-histoquímicos de um caso raro de hiperplasia fibroadenomatoide mamária em uma novilha Holandesa de 12 meses. Observou-se uma massa amarelada, firme, multilobulada, medindo 20 x 9 x 6,5cm e afetando os quartos direitos do úbere. Mastectomia total foi realizada. Microscopicamente, notou-se hiperplasia acentuada do epitélio mamário e numerosos ácinos levemente pleomórficos e bem diferenciados. Proliferação moderada do tecido conjuntivo fibroso e das células mioepiteliais próximos aos ácinos proliferados era também evidente. Aproximadamente 50% das células epiteliais proliferadas mostraram marcação nuclear positiva para receptores de estrógeno e progesterona e cerca de um terço dessas células era positivo para Ki-67. Além disso, as células mioepiteliais exibiram reação nuclear e difusa para p63. Com base nos achados clínicos, morfológicos e imuno-histoquímicos, firmou-se o diagnóstico de hiperplasia fibroadenomatoide mamária com provável influência de esteroides ovarianos.

Dentre os diversos tumores que acometem bovinos, o carcinoma de células escamosas ocular é o mais frequente nessa espécie, determinando prejuízos consideráveis nos criatórios. São vários os fatores envolvidos na patogênese, porém a constante exposição à luz ultravioleta e a quantidade reduzida de pigmentação em torno dos olhos estão relacionados com a maior prevalência. A neoplasia ocorre principalmente na pálpebra inferior, membrana nictitante e a junção corneoescleral. São descritos dois casos incomuns de carcinoma de células escamosas oculares que resultaram em metástases múltiplas para linfonodo regional, glândulas salivares e pulmão em vacas criadas do Sudoeste de Goiás. Histopatologia de todos os casos revelou proliferação de blocos hipercelulares de queratinócitos neoplásicos. As células possuíam citoplasma amplo e eosinofílico, núcleo arredondado, central e basofílico, com cromatina frouxa e um a dois nucléolos grandes e proeminentes. Havia anisocariose e anisocitose acentuadas e sete a dez mitoses em dez campos de 400X. Exposição prolongada aos raios solares e despigmentação dos tecidos primários afetados foram considerados fatores predisponentes importantes.


Among the various tumor affecting cattle, ocular squamous cell carcinoma is the most common in this species, causing considerable losses to farms. Several factors involved in the pathogenesis, but constant exposure to ultraviolet light and the reduced amount of pigmentation around the eyes are related to higher prevalence. The tumor occurs mainly in the lower eyelid, the nictitating membrane and corneoescleral junction. Two unusual cases of ocular squamous cell carcinoma that resulted in multiple metastases to regional lymph node, salivary gland and lung in cows created in southwest Goiás. Histopathology exams of all cases revealed hypercellular proliferation of neoplastic keratinocytes blocks. Cells had broad and eosinophilic cytoplasm, rounded nuclei and central basophilic, with loose chromatin and one to two large and prominent nucleoli. There was marked anisocytosis and anisokaryosis seven to ten mitoses in ten 400X fields. Prolonged exposure to sunlight and depigmentation of the affected primary tissues were considered important predisposing factors.

BACKGROUND: Healthcare Associated Infections constitute an important problem in Neonatal Units and invasive devices are frequently involved. However, studies on risk factors of newborns who undergo surgical procedures are scarce. OBJECTIVE: To identify risk factors for laboratory-confirmed bloodstream infection in neonates undergoing surgical procedures. METHODS: This case-control study was conducted from January 2008 to May 2011, in a referral center. Cases were of 21 newborns who underwent surgery and presented the first episode of laboratory-confirmed bloodstream infection. Control was 42 newborns who underwent surgical procedures without notification of laboratory-confirmed bloodstream infection in the study period. Information was obtained from the database of the Hospital Infection Control Committee Notification of infections and related clinical data of patients that routinely collected by trained professionals and follow the recommendations of Agência Nacional de Vigilância Sanitária and analyzed with Statistical Package for Social Sciences. RESULTS: During the study period, 1141 patients were admitted to Neonatal Unit and 582 Healthcare Associated Infections were reported (incidence-density of 25.75 Healthcare Associated Infections/patient-days). In the comparative analysis, a higher proportion of laboratory-confirmed bloodstream infection was observed in preterm infants undergoing surgery (p = 0.03) and use of non-invasive ventilation was a protective factor (p = 0.048). Statistically significant difference was also observed for mechanical ventilation duration (p = 0.004), duration of non-invasive ventilation (p = 0.04), and parenteral nutrition duration (p = 0.003). In multivariate analysis duration of parenteral nutrition remained significantly associated with laboratory-confirmed bloodstream infection (p = 0.041).
