ABSTRACT

Paciente de sexo femenino, de 58 años con diagnóstico de Artritis Reumatoídea desde 2005, en tratamiento con Metotrexate y prednisona a dosis bajas, que desarrolla tuberculosis diseminada con compromiso articular (muñeca derecha y tobillo izquierdo), tuberculosis miliar y una adenitis axilar supurada. A pesar del tratamiento antituberculoso adecuado, la paciente permanece subfebril y desarrolla una espondilodiscitis tuberculosa. Fallece cuatro meses después de un shock séptico. Había sido vacunada en su infancia con el BCG, tenía una serología VIH negativa y no había resistencia bacilar al tratamiento efectuado.


A 58-year-old woman with rheumatoid arthritis since 2005, under Methotrexate and low-dose prednisone treatment, developed disseminated tuberculosis with septic arthritis of the right wrist and the left ankle, miliary tuberculosis, and axillary adenitis. Despite anti-tuberculosis treatment, she remained febrile and developed vertebral tuberculosis. She died four months later as a consequence of septic shock. She had received BCG immunization at birth, her HIV assay was negative, and bacillary resistance to treatment was not found.
