ABSTRACT

Introduction: Turnaround time (TAT) is one of the most crucial performance indicators for blood transfusion and laboratory services. It is especially crucial in transfusion services due to its seminal role as a determining factor in patient care outcomes. We examined our institution’s TAT for issuing blood units. Materials and Methods: The Department of Immunohematology and Blood Transfusion, MGM Medical College and Hospital in Navi Mumbai, Maharashtra, India, undertook this retrospective noninterventional study over 12 months from January 01, 2020 to December 31, 2020. TAT was determined using a random audit of 10% of all monthly requests at the blood center. All requests for packed red cells (PRCs) received in the blood center during the study period were included in the evaluation. All requests for other blood components such as fresh-frozen plasma, random donor platelets, and cryoprecipitates were excluded along with all reservations for PRCs. A team of investigators tracked 369 requests for packed red cells over the year, noting the turnaround time. The standard TAT was set depending on the nature of the clinical case. Any significant deviation from institutionally established TAT was investigated, and root cause analysis was done. Results: The majority of transfusion requests were routine (72%) followed by emergency (23%) and lifesaving (5%). For routine cases, the average TAT was observed at 104 minutes. For emergency cases, the average TAT was observed at 39 minutes. For lifesaving cases, the average TAT was observed at 12 minutes. The highest number of cases were categorized under routine, followed by emergency cases and lifesaving categories. Conclusion: It was observed that there were no significant variations in turnaround time in routine, emergency, or lifesaving cases. Overall, as per our blood center standards, TAT for the issue of packed red cells was observed to fall under the normal range for routine, emergency, and lifesaving. Any outliers observed during the duration of the study were mainly due to inadequate samples or patient details received at the blood center or the presence of irregular antibodies encountered during the crossmatch.

ABSTRACT

Background: Olecranon process is a large, curved eminence comprising of the proximal and posterior part of the ulna. It lies subcutaneously which makes it more vulnerable to injury. Due to intra-articular extension of fractures, anatomical reduction and early mobilization should be achieved in every case and usually managed surgically. Aims and Objectives: To access the results of reconstruction plate in fracture olecranon.Materials &Methods: This was a prospective study consisted of 25 cases of olecranon fractures which were managed by open reduction and internal fixation using 3.5mm reconstruction plate. Patients were followed up every month till 6 months. At each follow up visit clinical and radiological parameters were assessed: Final assessment was done at 6 months using the Mayo Elbow Performance Score.Result: According to the AO classification, Type A-1 � 7 cases, A-3 � 1case, B-1 � 13 cases, B-3 � 1 case, C-1 � 1 case, C-2 � 1 case, C-3 � 1 case. An adequate reduction was maintained in all fractured olecranon until union. Average radiological union time was 12 weeks in 72% cases, 15 weeks in 16% cases, 18 weeks in 8% cases and > 18 weeks in 4% cases. The results were graded as per the criteria laid by Rogers et al as excellent in 84% cases, good in 12% and unsatisfactory in 4% cases. 2 cases developed superficial infection and 1 deep infection and 1 delayed union.Conclusion: Open reduction and internal fixation of fracture of olecranon with 3.5mm reconstruction plate is based on sound biomechanical principle with a good functional outcome and a low incidence of complications

ABSTRACT

Background: Fractures of the proximal femur, including fractures affecting the pertrochanteric region, have become a public health problem. Owing to aging of the population, we have to operate on even more elderly patients who sometimes present with significant co-morbidities. Although conventional implants have given good results in stable peritrochanteric fractures, proximal femur locking compression plate (PFLCP) which has been recently introduced is better suited for comminuted, unstable pertrochanteric fractures and osteoporotic bones. In PF-LCP, bone-implantconstruct so formed is mechanically stable and fixation failure due to screw loosening in osteoporotic bone can be reduced. Materials &Methods: This prospective study was conducted in our institute and was approved by local ethical committee. Twenty-five consecutive patients of either sex with age more than 60 years (range, 60-84) having pertrochanteric fractures were operated upon with PFLCP. A.O classification was followed in the study and Singh抯 index for osteoporosis was calculated. Operating time, blood loss and any technical difficulty with the implant were recorded. Patients were followed clinically and radiologically for union at fracture site and implant-related complications for a period of 24 months. The Harris Hip Score was used to document hip functions at final follow-up. Observations: Amongst the 25 patients, the average operation time in our study was 66.60�.57 minutes. The average radiological union was 3.40�63 months (13.6 weeks). Average time for full weight bearing was 13.12�90 weeks. There were two cases of wound infection, one case of coxa vara and one case of a proximal screw back-out. There was no case of plate lift or screw breakage. There was no case of non-union. Conclusion: The PFLCP can be a feasible alternative to the treatment of peritrochanteric fractures. Treatment with PFLCP can provide good-to-excellent healing in osteoporotic bones having comminuted and unstable peritrochanteric fractures, with a limited occurrence of complications.

ABSTRACT

Microalgae are considered as a potential feedstock for producing sustainable biofuel. The high growth rate and ability to rapidly improve the strains for high lipid and carbohydrate content without competing for arable land, makes microalgae a viable option for sustainable fuel production. Five microalgae designated as MA-1, MA-2, MA-3, MA-4 and MA-5 were isolated from the waterlogged areas of villages Theri, Ghagga and Gurusar of district Muktsar, Punjab. On the basis of optical and scanning electron microscopy, these microalgal isolates were found to be round in shape, green coloured and unicellular in structure and were tentatively identified as Chlorella sp. Chlorella sp. MA-1 was found to be fast growing with maximum absorbance (A600nm), viable cell count and chlorophyll content of 3.26, 369.4×105 cfu/mL and 24.31 mg/g dry wt., respectively at 5 days after culturing (DAC) However, significantly high total soluble protein content of 11.42% was found in this isolate at 5 days after culturing. Chlorella sp. MA-3 showed significantly high total carbohydrate content of 5.10 percent at 5 days after culturing (DAC), whereas the total lipid content was found to be significantly high in Chlorella sp. MA- 2 (4.16%) at 7 DAC. Alteration in the environmental and culturing conditions can further improve the chemical constituents of these microalgal isolates to improve biofuel production.

ABSTRACT

Dentinogenesis Imperfecta and dentin dysplasia are genetic oral diseases inherited in a simple autosomal dominant mode, with high penetrance and a low mutation rate. Both of them are present with bulbous crowns, marked cervical constrictions, severe attritions, few periapical radiolucencies, and premature tooth loss. The diagnosis is based on family history, and detailed clinical examination, while genetic diagnosis may become useful in the future once sufficient disease-causing mutations have been discovered. Here, we present a case with overlapping features of both dentinogenesis imperfecta and dentin dysplasia asserting both the anomalies to be part of the same continuum of the genetic event.

ABSTRACT

A questionnaire based study to assess the prevalence of tobacco use among undergraduate health care students of Maharaja Vinayak Health Care campus in Jaipur was done on 219 students of dental, nursing, physiotherapy and occupational therapy students. Global Health professional students survey (GHPSS), survey questionnaire given by (CDC) centre for disease control Athlanta which is a standard pre-tested questionnaire for assessing prevalence of tobacco use among health care professionals around the world was used. The present study showed 25.11% prevalence of smoking and 13.70% prevalence of chewing tobacco.This reflect alarming situation and demands urgent anti tobacco & tobacco cessation measures to be adopted by health professionals, who happens to be health promoters & health role models for society.
