ABSTRACT

Objective:To investigate the effects of preoperative endoscopic mucosal injection of carbon nanoparticle tracer and intraoperative serosa injection of carbon nanoparticle tracer on the acquisition of lymph nodes in total gastrectomy for gastric cancer.Methods:The retrospective cohort study was conducted. The clinical data of 118 patients with gastric cancer who underwent total gastrectomy in the Affiliated Tumor Hospital of Zhengzhou University between May 2017 and April 2018 were collected. There were 79 males and 39 females, aged from 26 to 81 years, with an average age of 59 years. Of 118 patients, 56 patients undergoing preoperative endoscopic mucosal injection of carbon nanoparticle tracer were divided into observation group and 62 patients undergoing intraoperative serosa injection of carbon nanoparticle tracer were divided into control group. Observation indicators: the total number of lymph node dissected, the number of positive lymph node dissected, the number of lymph node dissected at the first station and the number of lymph node dissected at the second station. Measurement data with normal distribution were represented as Mean± SD, and comparison between groups was analyzed using the independent-sample t test. Measurement data with skewed distribution were represented as M (range), and comparison between groups was analyzed using the Mann-Whitney U test. Count data were described as absolute numbers, and comparison between groups was analyzed using the chi-square test or Fisher exact probability. Comparison of ordinal data was analyzed using the rank sum test. Results:The total number of lymph node dissected, the number of positive lymph node dissected, the number of lymph node dissected at the first station, the number of lymph node dissected at the second station of the observation group were 48±16, 3(range, 0-25), 26±9, 23±7, respectively. The above indicators of the control group were 41±13, 4(range, 0-28), 25±8, 16±5, respectively. There were significant differences in the total number of lymph node dissected and the number of lymph node dissected at the second station between the two groups ( t=2.494, 6.588, P<0.05), and there was no significant difference in the number of positive lymph node dissected and the number of lymph node dissected at the first station between the two groups ( Z=0.747, t=1.689, P>0.05). Conclusions:Carbon nanoparticle labeled lymph node staining using preoperative endoscopic mucosal injection of carbon nanoparticle tracer or intraoperative serosa injection of carbon nanoparticle tracer is safe and effective in total gastrectomy for gastric cancer. Compared with intraoperative serosa injection of carbon nanoparticle tracer, preoperative endoscopic mucosal injection of carbon nanoparticle tracer can increase the total number of lymph node dissected, especially the number of lymph node dissected at the second station of gastric cancer.

ABSTRACT

BACKGROUND@#Adenocarcinoma is the most common type of lung cancer. It has been clinically evaluated that therapiestargeting against the epidermal growth factor receptor (EGFR) as the clinical standard first-line treatment. The response and outcome of EGFR-tyrosine kinase inhibitors (TKIs) in patients harboring common mutations in EGFR kinase domain (deletion in exon19 and L858R in exon 21) has been well demonstrated, but not in rare or complex mutations.@*METHODS@#A total of 150 patients that harbored rare or complex mutations in EGFR diagnosed by histopathology were included in this retrospective study. The clinical-pathological characteristics of all 150 patients as well as the response and progression-free survival (PFS) in 48 patients that received EGFR-TKIs in first/second/third line treatments weredescribed and analyzed.@*RESULTS@#Patients were divided into four groups based on the mutation types: single G719X point mutation in exon 18 (n=46, 30.7%), single L861Q point mutation in exon 21 (n=45, 30.0%), other single rare mutation (n=14, 9.3%) and complex mutations (n=45, 30.0%). The result indicated thatthere was no correlation of EGFR mutation typeswith other parameters such as gender, age, clinical stage, pathology and smoking history. For the 48 patients that received EGFR-TKIs treatment, there were no significant differencesamong 4 groups in terms of objective response rate (ORR) and disease control rate (DCR) (54.5% vs 30.0% vs 0.0% vs 35.7%, χ²=3.200, P=0.34; 90.9% vs 85.0% vs 66.7% vs 92.9%, χ²=2.162, P=0.59). The median progress-free survival (mPFS) was 11.0 months (95%CI: 4.4-17.6), and in each group of different EGFR mutation types are 15.8 months (95%CI: 9.5-22.2), 8.0 months (95%CI: 5.1-11.0), 4.9 months (95%CI: 1.4-8.4) and 23.1 months (95%CI: 15.8-30.4)(χ²=7.876, P=0.049).@*CONCLUSIONS@#The efficiency of targeting EGFR-TKIs on different types of rare or complex mutations was heterogeneous. The PFS may be better in patients that harbored complex mutations than those with single rare mutations. Further studies with larger sample size are necessary. Moreover, to discover novel therapeutic targets and develop new drugs are imminentfor those patientswith no response to the existing treatments.

ABSTRACT

Objective Objective To analyze the torque performance of lingual brackets with slots in different directions and the influence pattern of arch wires with different sizes and materials on torque of these lingual brackets.Methods The finite element models of the anterior maxilla lingual brackets with vertical and horizontal slot were constructed,using 3 kinds of materials (stainless steel,beta-titanium,nickel-titanium) and two sizes of arch wires (0.43 mm x 0.56 mm,0.41 mm x 0.56 mm),so as to compare and analyze their mechanical properties after the application of torque load on the left upper central incisors with a rotation angle of ±20°.Results With the increase in elastic modulus of the arch wire,torque moments of lingual brackets with horizontal and vertical slot both increased gradually.Torque performance of lingual brackets was affected more by the sizes and materials of arch wires than the directions of slots.The lingual bracket with vertical slot produced 1-2 times of torque force than that with horizontal slot under the same degree of torque angle.The effect of arch wire sizes was smaller than that of arch wire materials,while the synergistic change in sizes and materials of arch wires had the largest influence on torque performance of lingual brackets.Conclusions The torque performance of lingual bracket will be influenced with its different slot directions,the sizes and materials of arch wires.In clinical application,slot directions of lingual brackets as well as sizes and materials of arch wires should be adjusted properly according to the required torque force.

ABSTRACT

BACKGROUND:In the orthodontic treatment, double wire technique is commonly used to correct the torque of anterior teeth;however, the underlying mechanical mechanisms remain unclear. OBJECTIVE:To study the torque capabilities of DamonQ system through the three-dimensional finite element method. METHODS:Three-dimensional finite element models of Damon Q brackets with different dimension arch wires were established and were grouped according to arch wire with different dimensions. The torque capabilities of Damon Q system with different dimensions and materials arch wire were analyzed. RESULTS AND CONCLUSION:The Damon Q brackets with single or double wires had the same variation of torque, torque angle curve changes under the condition of different size and material arch wires. Torque moment increased gradually with increasing torque angle. Rectangular-round double wires produced larger torque moment compared with single rectangular wire, whereas double round wires produced smal er torque moment compared with single rectangular wire. The torque moment produced by rectangular-round double wires or by single rectangular wire was approximately 2-8 times or 1-5 times more than that by double round wires. The torque moment that rectangular-round double wires produced was larger than that single wire produced in the DamonQ system. In the early clinical practice, we can use double round wires to control root movement. In the middle-term of the treatment, we can use rectangular-round double wires to control root movement as the mild forces produced by them reduce root absorption. This study lays a foundation for the application of Damon Q bracket with double wire technology in the clinic.

ABSTRACT

Objective:To analyse the difference of torque performance between lingual and labial brackets as well as the influence of the archwires of different materials with different dimension on the torque capability of lingual brackets.Methods:The 3D finite mo-del of lingual and labial brackets from the right upper canine to the left upper canine was established,all brackets were with 0.56 mm slot size.The center of the bracket slot of the left upper central incisor was used as the central point and the two brackets were rotated clockwise and counterclockwise with a rotation angle of ±20°along the horizontal axis of bracket slot.The torque load on the left up-per central incisors with different materials and dimensions of the archwires was simulated.Results:The torque force of both lingual and labial brackets increased with the increase of rotation angle and the elastic modulus of archwire.The lingual archwire generated torque force was nearly 3 -5 times more than the labial system under the same torque angle.The torque value generated with 0.48 mm ×0.64 mm archwires was greater than 0.43 mm ×0.64 mm.The lingual bracket generated torque force decreased gradually from nickel-titanium(NiTi),Beta-titanium(TMA)to stainless steel(SS)under the same dimension.Conclusion:Torque moments of lin-gual and labial brackets both increases with the increase of torque angle in each operating condition.The lingual orthodontic brackets produce more torque force than labial brackets under the same torque angle.Torque performance of lingual bracket was affected by the material and dimension of the archwires.

ABSTRACT

Objective To investigate the effects of Mailuoning on oxidative stress and ischemic brain injury. Methods A total of 126 healthy male Kunming mice were divided into sham operation (n = 18), normal saline control (n = 54) and Mailuoning (n = 54) groups. A middle cerebral artery occlusion (MCAO) model was induced Two hours after MCAO,Mailuoning injection and equivalent saline were injected via the tail vein in the Mailuoning and normal saline control groups, respectively, and then they were injected every other 24 h.Neurological score was performed, and brain water content, infarct volume, membrane potential,as well as protein oxidative stress metabolites such as 3-nitrotyrosine (3-NT), lipid oxidative stress metabolite 4-hydroxy-2-nonenal (HNE) and nucleic acid oxidative stress metabolite 8-hydroxy-2'-deoxyguanosine (8-OHdG) were detected at 12, 24 and 72 h after MCAO. Results Mailuoning injection could significantly improve the neurological function of cerebral ischemia in mice, decrease brain edema, and reduce infarct volume at different time points after cerebral ischemia Of those, it was most significant at 72 h. Mailuoning injection could reverse the decreased mitochondrial membrane potential in cerebral cortex and internal capsule, and significantly downregulate the increased 3-NT, HNE and 8-OHdG in cerebral cortex, internal capsule and serum after ischemia, of those, the effect of reducing HNE was most significant.Conclusions Mailuoning injection may effectively protect against ischemic brain injury in mice,and its mechanism is associated with inhibiting oxidative stress, particularly anti-lipid oxidation.
