ABSTRACT

To determine the intraoperative and early postoperative complications of manual sutureless cataract extraction. Case series. Redo Eye Hospital, Rawalpindi, Pakistan, from January 2009 to December 2010. Three hundred patients of cataract through purposive non-probability sampling were selected. The patients underwent manual sutureless cataract surgery [MSCS] by single experienced surgeon and intraoperative complications were documented. The surgical technique was modified to deal with any intraoperative complications accordingly. Patients were examined on the first postoperative day and on the first postoperative week for any postoperative complications. The data was entered in Statistical Package for Social Sciences [SPSS] version 13.0 and the results were calculated in frequencies. Among the 300 cases, 81.3% surgeries went uneventful whereas 18.6% had some complication. The common intraoperative complications were superior button-hole formation in 5%; posterior capsular rent in 5% and premature entry with iris prolapse in 3% cases. Postoperatively, the commonly encountered complications were striate keratopathy in 9.6% and hyphema 9%. At first week follow-up, 4% had striate keratopathy and 0.6% had hyphema. Striate keratopathy resolved with topical medication on subsequent follow-up. A total of 9 cases [3%] underwent second surgery: 2 cases for lens matter wash, 2 cases for hyphema and 5 cases needed suturing of wound for shallow anterior chamber due to wound leak. Superior button-hole formation, posterior capsular rent and premature entry were the common intraoperative complications of MSCS whereas the common early postoperative complications were striate keratopathy and hyphema.

ABSTRACT

AIM: To find out the effect of heparin on the incidence of posterior capsular opacification after cataract surgery in children


STUDY DESIGN: Quasi Experimental Design


PLACE AND DURATION OF STUDY: REDO Eye Hospital, Rawalpindi from Jan 2008 to Jan 2011


MATERIALS AND METHODS: Twenty eyes of fifteen patients aging 4 to 10 years with uncomplicated pediatric cataracts were selected in the study. The children were operated for cataract under general anesthesia and the anterior chamber was irrigated with heparin sodium [5 IU/cc] while added to balanced salt solution [BSS Plus]. All patients received standardized postoperative treatment. All patients were followed on the first post operative day, after one week, after one month and were advised follow up at the 3rd, 6th months and one year postoperatively for the incidence of PCO


RESULTS: No fibrinous reaction or membrane or synechias formation on the pupil was observed. Posterior capsular opacification [PCO] was observed in 10 % cases after 6 months and in 15% cases after one year


CONCLUSION: After irrigating the anterior chamber with Heparin sodium in cataract surgery of children the incidence of PCOwas 15% on one year follow up

ABSTRACT

To assess the Frequency and the types of Posterior Capsular Opacification after extra capsular cataract extraction and posterior chamber intraocular lens [PMMA] implantation. Descriptive study Place and Duration of Study: Department of Ophthalmology Holy Family Hospital, Rawalpindi from May 2003 to May 2005. Two hundred patients, with age ranges from 55 to 80 years, having uncomplicated senile mature cataracts who underwent ECCE with PCIOL over a period of one year were selected for the study. At 6th Month post operatively the patients were examined for any evidence of posterior capsular opacification with its type and the results were analyzed. At 6 months follow up 70 patients [35%] developed posterior capsular opacification. Out of these 70 patients, 40 patients [57%] showed capsular fibrosis and 30 patients [43%] Elschnig pearls. Our study revealed that the occurrence of posterior capsular opacification with PMMA intra ocular lens is high leading to significant number of patients with visually disabling complication in the post operative period

ABSTRACT

To find out the effects of heparin added to the irrigating solution on anterior chamber reaction in pediatric cataract surgery. A quasi experimental study. This study was conducted in a tertiary eye care hospital from Jan 2008 to July 2010. Twenty eyes of fifteen patients aging 4 years to 10 years with uncomplicated pediatric cataract were selected in the study. All children underwent cataract extraction under general anesthesia and received anterior chamber irrigation with heparin sodium [5 ILJ/cc] during operation added to the irrigating solution of balanced salt solution [BSS Plus]. All patients received standardized postoperative treatment. All patients were followed on the first post operative day, after one week, after one month and were advised follow up at the 3[rd] and 6th months and postoperative anterior chamber reaction was documented according to modified Hogan's classification on each visit. Mild anterior chamber reaction was observed in 10 patients [50%] and moderate anterior reaction was observed in only [15%] three patients on first follow up. Anterior chamber reaction disappeared on 7[th] post operative day in all patients. Fibrin formation, anterior and posterior synechia, cyclitic and pupillary membrane formation was not observed in any patient. There was also no intraocular lens deposits or posterior capsular opacification [PCO] in any of the cases afterthe follow up of 6[th] months. Heparin sodium in the irrigating solution is safe, effective, and promising method to prevent early postoperative inflammatory reaction in pediatric cataract surgery
