ABSTRACT

Although nivolumab shows survival benefits for patients with advanced gastric cancer (AGC), predictive biomarkers for nivolumab treatment in AGC remain unclear, especially in the case of peritoneal metastases. This study investigated the clinical significance of the prognostic nutrition index (PNI), reflecting the host nutritional status and immunity, in AGC patients undergoing nivolumab monotherapy. This study retrospectively analyzed 53 AGC patients who received nivolumab between October 2017 and February 2021. Among them, 35 patients with peritoneal metastases were reviewed to investigate the relationship between the PNI and oncological outcomes. The PNI was calculated as 10×serum albumin level (g/dl)+0.005×total lymphocyte count (per mm3 ) at the first administration of nivolumab. With a median follow-up duration of 2.0 (0.3-13.5) months, the median overall survival (OS) was 2.0 months. The overall response and disease-control rates were 0.0% and 20.0%, respectively. Among the 35 patients, 13 patients were identified as a high-PNI group. In the univariate analysis, the high-PNI group showed a significantly longer PFS and OS than the low-PNI group. In the multivariate analysis, the high-PNI was independently associated with a longer PFS (p=0.021) and OS (p=0.022). The PNI can be useful for predicting PFS and OS in AGC patients with peritoneal metastases. However, further studies are required to validate these results in AGC and new strategies are needed to improve the outcome for AGC patients with peritoneal metastases.

ABSTRACT

OBJECTIVE: bjective: By identifying the unknown substance responsible for binding with nebulin SH3 domain within the sarcomeric Z-line, we tried to find out Z-line structure which plays an important role on muscle contraction and maintenance of muscle funtion. METHOD: First, the bait plasmid was made by binding the DNA binding domain of Gal4 protein of yeast and the SH3 domain. Second, library plasmid was made by binding activation domain and human skeletal cDNA library. Then, the base sequence of the clone, produced by combining the two proteins expressed by transgenically converted plasmid in yeast, was analyzed. RESULT: We screened out six true positive clones and analyzed the base sequence of the two of six clones. We identified them to be alpha-actinin2. CONCLUSION: We can theorize that Neublin SH3 domain and alpha-actinin2 plays a vital role for the integration of Z-line. Thus, this is an important data in further studying muscle functions, mechanisms, and muscular disease as well.

ABSTRACT

To compare the results of the chemonucleolysis and the automated percutaneous lumbar discectomy(APLD) for symptomatic lumbar disc herniation, retrospective study of 19 chemonucleolysis and 20 APLD was done from July, 1984 to February, 1993. Number of male patients was 23 and female 16. The patients ranged in age from 14 to 56 years. L4-5 was the most commonly involved level comprising 73.6%, and 2 level involvement was 20.5%. Clinical results were evaluated according to Mcnab s criteria. Excellent and Good result were 31.6% and 47.3% in chemonucleolysis with an average follow up period of 6.2 years, and those were 35% and 45% respectively in APLD with an average follow up period of 18.7 months. Among 19 chemonucleolysis, there were loss of disc height in 8, and lateral recess stenosis in 1. Among 20 APLD, there were loss of disc height in 2, and nerve root irritation symptoms in 2. Chemonucleolysis and APLD have a low morbidity, can be performed under the local anesthesia and are simple, rapid and less traumatic procedure.
