ABSTRACT

Population aging has social, economic and political consequences. Most family caregivers prefer to care for their family member older person with chronic disease at home. Despite traditional culture within Iranian families, in some cases, hospitalization of the elderly in nursing home is inevitable, and this affects the old person and his/her family. The aim of this study was to explain how Iranian family cargivers give up caring their older person with chronic condition at home. A grounded theory approach was used to conduct the study. The study setting included four nursing homes under the auspices of Iran Welfare Organization. Fourteen participants were recruited through purposive sampling. Data were collected from December 2010 to March 2011 by Semi-structured interviews lasting about 17 to 95 minutes [average 52 minutes]. Constant comparative analysis was used to analyze the data. Three main categories appeared at the end of the analysis: [going out of the road of usual life], [challenge of meeting older person, family and caregivers care needs], and [the appearance of inconstancy in the family]. They explained exclusively how family caregivers of old people give up caregiving. Health care providers are recommended to become familiar with challenges of family caregivers in taking care of older person with chronic disease at home, and then organize their supportive and consulting actions according to family situations in order to improve the life quality of older person and family caregivers

ABSTRACT

Moral distress, a major issue in nursing, affects nurses in all healthcare sections. The existence of such distress and its consequent job dissatisfaction may pose a threat to nurses' physical and mental health, as well as their quality of life; and hinder their accomplishment of individual and social goals. The aim of this study was to determine the relationship between the moral distress and job satisfaction In nurses. This cross-sectional correlational study was conducted on 210 nurses employed in the selected healthcare centers of Tehran University of Medical Sciences in 2009. Data were collected using a questionnaire consisting demographic characteristics, the Corley's moral distress scale, and the Minnesota Satisfaction Questionnaire. Data were analyzed using descriptive statistics and independent t-test, analysis of variance, Wilcoxon test and Pearson's correlation coefficient in the SPSS v.14. According to the findings of the study, the mean of the moral distress was 1.77 out of four; and the mean of the job satisfaction score was 3.17 out of 5; indicating a moderate job satisfaction among the nurses. A significant relationship was observed between the moral distress and nurses' job satisfaction in the present study, indicating that by decreasing moral distress, job satisfaction increases. Although numerous factors govern nurses' job satisfaction, the observed relationship between moral distress and job satisfaction indicates that identifying and limiting factors influencing these distresses can lead to improved job satisfaction for nurses

ABSTRACT

Evidence based practice [EBP] is one of the main professional competencies for health care professionals and a priority for medicine and nursing curriculum as well. EBP leads to improve effective and efficient care and patient outcomes. Nurse educators have responsibility to teach the future nurses, and an opportunity to promote patient outcomes. Therefore, the aim of this study was to describe nurse educators' knowledge and attitude on EBP. This was a descriptive study conducted in nursing faculties of two major universities of medical sciences affiliated to Ministry of Health and Medical Sciences in Tehran, Iran. Data were gathered using a three section questionnaire. Content and face validity was further enhanced by submitting it to nursing research and education experts. Statistical analysis was carried out using SPSS 11 software. According the results, nursing faculties' knowledge of EBP was mainly moderate [47.1%]. Significant statistical relationship was found between the level of knowledge with education and teaching experience in different nursing programs. Nurses generally held positive attitudes toward EBP [88.6%] and there was no statistical significant relationship with demographic variables. Nursing educators are in a position to influence nursing research in clinical practice in the future. Therefore, it is critical to achieve implementation of EBP and be a change agent for a paradigm shift toward EBP.

ABSTRACT

Considering patients' rights has a long history in medicine. Continued attention to this phenomenon has risen in medical sciences literature particularly during the last few decades and has caused many discussions in health care delivery systems. Despite Iranian Muslim culture's fullness of humanistic lessons and the challenges by the government, the concept of patients' rights has not received its proper situation in Iranian health care delivery system yet. However, establishment of "Iranian patient bill of rights" by the Ministry of Health and Medical Sciences in recent years, has figured an encouraging vision in this area. It should be mentioned that Iranian patients' bill of rights is a perceived translation of world's contemporary's patients' bill of rights, which has been formulated by some compassionate medical policy makers who are highly concerned about patients' rights, but it's most important defect is that none of the stakeholders had been involved in the process of formulating this document. In addition there are some items in this bill of rights that are not fit with our socio-economic and cultural background. On the other had there are some problems in our health care context, that this bill can not respond them properly. This article has analyzed the Iranian patients' bill of rights, and is followed by providing some key concepts which should be considered for promoting patients' rights practice in Iran
