ABSTRACT

Bariatric surgery is becoming increasingly more common for the treatment of morbid obesity. Preoperative weight loss leads to decreases in the size of the liver and the amount of intra-abdominal fat, which improve the surgical field and therefore the operative times and complication rates as well. Well-planned dietary advancement ensures proper healing from the surgery and develops life-long healthy eating habits. The recommended postoperative diet starts with soft food and gradually advances to solid food. Bariatric surgery has the potential to cause a variety of nutritional and metabolic complications, which are mostly due to the extensive surgically-induced anatomical changes incurred by the patient's gastrointestinal tract. Counseling, monitoring, and nutrient and mineral supplementation are essential for treatment, and dietary intake and eating behavior after bariatric surgery should also be changed to achieve long-lasting success. Structured nutritional counseling can help weight reduction and maintenance.

ABSTRACT

PURPOSE: This study aimed to identify the scores of postpartum depression(PPD) on the first day, 1st week, and 6th week after the delivery and to explore their related factors before and after delivery in postpartum women. METHODS: With a survey design, 293 postpartum women were recruited from a postpartum unit, Ilsin Christian hospital in Pusan via convenience sampling and were followed at 1st week and 6th week in the outpatient clinic. RESULTS: Results showed that the scores of PPD(EPDS score) were low at postpartum 1st day, 1st week and 6th week but prevalence of PPD(EPDS > or =13) was 3.1% at 1st day, 8.2% at 1st week and 7.5% at 6th week, respectively. The pre-delivery factors were experience of depression, and the post-delivery factors were baby's sex(1st day), no caregiver for baby(1st week), and no help and concern for taking care of baby from husband and family(1st day and 6th week). The greater satisfaction with becoming a mother and her life, and greater maternal attachment were related to lower level of PPD at the three time points. CONCLUSION: Regular screening for postpartum depression and supportive and informative education is needed for postpartum women visiting the outpatient clinic for follow-up.

ABSTRACT

PURPOSE: This research was aimed to explore the major subjects and research theme and to classify characteristics of the key words in recent Korean Journal of Women Health Nursing. METHOD: With survey design, whole 103 published papers during 2003-2006 were analyzed with structured analysis form; research subjects, research methodology, data analysis, and key words of the published papers were classified and extracted. Various theme was founded and classified to 9 domains. RESULT: Most of the research subjects were women. Eighty-eight percent of papers conducted quantitative research; 83% chose convenience sampling and 69% used survey design, while experimental design was 29%. Key words were categorized 9 domains: demographic, health related concept, health behavior(intervention), sex-related, life event, disease, et al. Among 9 domains, health related concept, especially psycho-social topics such as depression, anxiety, stress were mostly studied and then physiologic topics such as labor pain, fatigue, and menstrual symptoms. CONCLUSION: Most of the subjects were women and the most common domain studied was health related concepts in Korean Journal of Women Health Nursing. And key words and topics were on women's health issues. We can conclude that Korean Journal of Women Health Nursing has published by her own philosophy.

ABSTRACT

PURPOSE: The purpose of this study was to examine the effects of aquatic exercise on pain, fatigue, body composition, physical fitness and psychological variables in women with arthritis. METHOD: With a quasi-experimental design, 37 women who had arthritis were assigned into an experimental group (n=19) or a control group (n=18). Aquatic exercise consisted of one hour of exercise in water, two days per week, for six weeks and a self-help process. Measures included physiologic and psychological variables before and after the exercise. Data was analyzed with the SPSS WIN using descriptive statistics, t-test, and ANCOVA. RESULT: The mean age of the subjects was 59 years in the experimental group and 52 years in the control group. Pretest scores in outcome variables were similar in both groups except BMI and body fat(%). After controlling for age, shoulder flexibility scores in the experimental group improved more than the control group (Rt: F=10.58, p=.003; Lt: F=5.91, p= .02; Waist: F=4.95, p= .03). Depression scores in the experimental group decreased (F=12.96, p= .001), and self efficacy and quality of life improved more than the others (F=5.07, p= .03; F=6.91, p= .01). CONCLUSION: Aquatic exercise can improve waist flexibility and shows consistent findings of improved shoulder flexibility and psychological function in women with arthritis.

ABSTRACT

PURPOSE: This study aimed to evaluate the accuracy and adequacy of research papers reporting statistical testings for mean differences. METHODS: Original research articles utilized t-test, Chi-square test, and ANOVA were reviewed from Korean Journal of Women Health Nursing published from the year 2004 to 2006. Seventy-six papers were evaluated in accordance with formatted criteria in respect to an inclusiveness of research title, accuracy of statistical methods and presentation styles, and errors in reporting statistical outcomes. RESULTS: Research titles were quite comprehensive, however overall accuracy of statistical values and basic formats reached only 60 to 80% by items. Details of the presentation in the reporting of outcomes were not complied with the guidelines, which need careful concerns of the writers. Errors of English in table presentation were found in more than 30% of the papers. CONCLUSION: The outcome would be reflected in the submission guidelines for future writers. To reach the level comparable with internationally recognized nursing journals, concrete knowledge to apply statistical methods should be ensured in the processes of submission, reviews, and editing.

ABSTRACT

PURPOSE: The purpose of this study was to verify the effects of a Qigong prenatal education program on anxiety, depression and physical symptoms in pregnant women. METHOD: The subjects were a total of 40 pregnant women who received regular prenatal care at S hospital. Twenty people were enrolled in the Qigong prenatal education program and were assigned to the experimental group. The other 20 people who received regular prenatal care only were the control group. The Qigong prenatal education program was given to the experimental group once a week and 2 hours per session for 4 weeks. Anxiety, depression and physical symptoms were collected by a self-administered study questionnaire at the pre- and post test. RESULT: There were no significant differences in subjects' general characteristics or pretest scores of study variables at the pretest indicating both groups were homogeneous. Differential t-tests were used to test the effects of the Qigong prenatal education program on study variables. Pregnant women who received the Qigong prenatal education program had a lower level of depression than those who did not attend(t=2.23, p=.03). There were no significant differences on anxiety and physical symptoms. CONCLUSION: The Qigong prenatal education program was effective in alleviating depression during pregnancy. However, further study is needed to replicate the results with a greater sample size and to investigate the long term effects of the program on the labor and delivery process.

ABSTRACT

PURPOSE: The purpose of this study was to explore the relationships among women's total symptom distress, uncertainty and quality of life in women with endometriosis. METHOD: A total of 121 women who had been diagnosed with endometriosis was given a questionnaire. Data was collected from August 2004 to January 2005. The data was analyzed by frequency, mean, t-test, ANOVA, Pearson correlation coefficient, and multiple linear regression analysis using SPSS WIN 10.0 software. RESULT: The mean score of TSD was 3.23. 'Menstrual cramping' showed the highest score. The mean score of uncertainty, and quality of life of the subject was 56.6, and 38.3 respectively. TSD was significantly related with coffee intake and uncertainty was significantly related with marital status, economic status, alcohol intake, diagnosis after period, and family history. Quality of life was significantly related with age, and dysmenorrhea. There were significant relationships between total symptom distress and uncertainty, and quality of life. There were significant relationship between uncertainty and quality of life. CONCLUSION: Endometriosis symptoms showed a significant relationship with uncertainty and quality of life. This study will help to manage women with endometriosis.

ABSTRACT

PURPOSE: This study was done to identify degrees of maternal conflict, differences, and factors predicting conflict in mothers of toddlers. METHOD: A convenience sample of 300 mothers living in G city, Kyounggi-do whose child was between 12 to 36 months old was used. Data were collected through a questionnaire survey and analyzed using SPSS WIN 10.0. RESULTS: The average score for maternal conflict was 67.35 (SD=10.18), somewhat lower than moderate in level. 'I as a human being' was the area of the 6-sub areas with the most conflict. Mothers experiencing higher maternal conflict were those who were less satisfied with marriage, quality of life, and maternal role, and whose child was stubborn and hard to please, who were unsatisfied with baby sitters or who had to rush their sick child to hospital. Factors that were significant in predicting maternal conflict were low satisfaction with maternal role and marriage, and a child who was difficult to care for. These factors accounted for 22% of explained variance. CONCLUSION: Nurses should help mothers resolve maternal conflict through education and counseling on the maternal role, but at the same time nurses should consider relationship of the mother with her husband and also special characteristics of her child.

ABSTRACT

PURPOSE: This study was to examine the effect of postpartum exercise on mental health. Mental health comprises a 9 symptom dimension of Somatization, Obsessive-Compulsive, Interpersonal Sensitivity, Depression, Anxiety, Hostility, Phobic anxiety, Paranoid Ideation, and Psychoticism. METHOD: A non-equivalent pre-test, post-test control group study was conducted. Fifty-two puerperal women who were admitted to a postpartum ward of a mother-baby clinic in Pusan were recruited; 26 women were assigned to the experimental group and 26 women to the control group. Postpartum exercise was applied to the experimental group from postpartum day 3 for a period of 8 weeks while no exercise program was applied to the control group. Mental health with SCL-90-R was measured before and after exercise. Data was analyzed using mean, x2-test, t-test, and ANOVA by SPSS 10.0. RESULT: The mental health status in the exercise group did not show a statistically significant decrease when compared to that of the control group. CONCLUSION: There is no effect of postpartum exercise on women's mental health during the first 8 weeks of the postpartum period. Further studies are needed for randomized clinical trials with a larger sample size.

ABSTRACT

PURPOSE: The purposes of this study were to examine physical health and depression by women's employment status and role satisfaction, and to identify significant predictors for women's health status. METHOD: With a cross-sectional, correlational study design, a sample of study was consisted of 181 women who were employed or not employed whose child was attending an elementary school through convenience sampling. Data were collected with a structured self-administered questionnaire and analyzed by 2-test, t-test, 2-way ANCOVA and hierarchical multiple regression analysis using SPSS program. RESULT: Employed women had poorer physical health than that of nonemployed women and women who had greater satisfaction as a parent reported better physical health and lower level of depression than who had lower satisfaction with covariates. Predictor for better physical health after controlling for covariates was being not employed, greater satisfaction with worker's role if employed, and greater satisfaction with parent's role. Women who had lower level of depression reported greater satisfaction with their social roles, but occupancy of multiple roles and role satisfaction as worker were not related to depression. CONCLUSION: Role quantity and role quality seem to be very important factors to maintain better physical and psychological well-being in women.

ABSTRACT

PURPOSE: Women who undergo gynecological surgery have moderate and severe sensation and distress of pain despite the advent of patient controlled analgesia (PCA). The purposes of this study were to describe perception of non-pharmacological therapy for postoperative pain control and examine changes of pain sensation and distress in women who had gynecological surgery. METHOD: The sample consisted of 52 women who were having gynecological surgery. Subjects who agreed to participate in the study were asked for their opinion about non-pharmacological approaches for postoperative pain control using a structured study questionnaire. Pain sensation and distress were assessed by VAS in the morning and afternoon for 2 days following the surgery. RESULT: About 50% of the subjects thought that non-pharmacological methods such as relaxation, music, massage, or meditation would be helpful for their postoperative pain control. If both pharmacological and non-pharmacological therapy were given for pain control, 96% of subjects reported it would be effective. Nurses can apply techniques of relaxation, deep breathing, meditation, and music therapy to surgical patients along with PCA. Expected sensation and distress of pain was high, but pain levels gradually decreased over time. However, subjects experienced moderate levels of pain postoperatively although they used PCA. CONCLUSION: The effect of a combined method of pharmacological and non-pharmacological approach needs to be tested if postoperative pain is to be decreased more.

ABSTRACT

PURPOSE: The purpose of this study was to identify the influencing factors on the compliance of breast self-examination (BSE) among nurses who work at three general hospitals in Kyung-Nam areas. METHOD: 258 hospital nurses were included in the study. Data were collected using structured self-administered questionnaires and analyzed by descriptive statistics, t-test, x2 test and logistic regression analysis. RESULT: Nurses reported medium levels of knowledge, self efficacy and health believes about breast cancer and BSE, and 26% of the nurses performed the BSE at least once during the last 6 months. Compliers of BSE perceived significantly higher levels of self-efficacy, susceptability and health motivation, and lower level of barrier compared to non-compliers. Significant influencing factors on BSE compliance were 'experience of getting recommendation for breast self-examination', 'susceptibility', 'barrier', and 'self-efficacy' and those variables explained 22.5% of variance in compliance of BSE. CONCLUSION: Nurses, who must play as a role model for health promoting behaviors, did not have enough knowledge of breast cancer and BSE. Also, their performance rate of BSE was quite low. Thus, it is essential to provide an educational program for breast cancer and BSE to nurses in order to enhance nurses' performance rate of BSE.

ABSTRACT

PURPOSE: To examine the difference of physical discomfort and childbirth satisfaction between postpartum women with and without having taken Epidural Analgesia. METHOD: The subjects were divided into one group of 128 primipara taken Epidural Analgesia and the other of the same 70 women who were not taken it. Data were collected by questionnaires of their own physical discomfort and birth satisfaction at postpartum 1 to 2 days in OBGY hospitals, and data were analyzed using SPSS Program. RESULT: Women having taken epidural analgesia appealed higher physical discomfort than those without it in the lower limbs exercise discomfort, difficult urination, urinary retention, nausea & vomiting, whereas appeared vice versa in breast pain. Among indicators for childbirth satisfaction, women having taken epidural analgesia preferred the same delivery method later again more than those without it. CONCLUSION: It is confirmed that the method of epidural analgesia is not an absolute way to control labor pain, rather stir physical discomfort after childbirth and does not fully increase the women's childbirth satisfaction. Therefore, it is proposed that nurses should provide the pregnant women the right knowledge and information, thereby enabling them to select the useful method of childbirth to their own course of childbirth and health-recovering after the delivery.

ABSTRACT

PURPOSE: The purpose of this study was to determine rate of breastfeeding practice at 3 months and to examine predictors for breastfeeding practice. METHOD: Study subjects were 114 mothers who gave birth at one hospital in Busan area. Data regarding breastfeeding practice and potentially related factors to breastfeeding were collected through a self-administered questionaire at 1 weeks and a telephone survey at 3 months. Logistic regression was used to identifiy significant the predictors on breastfeeding practice. RESULT: Rate of breastfeeding practice was 46.5% at three months of postpartum. The significant predictors for performing breastfeeding were lower the experience of breastfeeding trouble problem(OR=.88 compared with breastfeeding women, 95% CI; 78-.99), higher first breastfeeding satisfaction (OR=1.81 compared with non-breastfeeding women, 95% CI; 1.08-3.32), higher breastfeeding confidence(OR=1.84 compared with non-breastfeeding women, 95% CI; 1.08-3.32). CONCLUSION: The findings suggest the necessity of support program for mothers during the positive postpartum period to provide correct information about breastfeeding knowledge and attitude and to teach problem-solving skills for any breastfeeding problems for highere rate of breastfeeding practice.

ABSTRACT

PURPOSE: This study was to determine the mediator or moderator role of social support in the relationship between stress and depression among family caregivers of older adults with dementia. METHOD: Sixty nine family caregivers were randomly selected from health care centers in P city and a face-to-face interview was conducted using questionnaires from January to May of 2002. Data were analyzed with descriptive statistics, Pearson correlation, and hierarchical multiple regression using SPSS program. RESULT: Family caregivers of older adults with higher dependency in ADLs and higher problematic behaviors, provided care to the older adults for a longer period of time, and perceived less social support reported higher depression. Social support showed mediating effects between stress and depression, while did not show moderating effects. Elderly dependency on ADLs and caregiving duration decreased perceived social support and decreased social support increased depression. CONCLUSION: To increase family and social support to the caregivers of more functionally impaired elderly, family education to increase emotional support and physical assistance to the caregivers and broader and flexible application of social support such as increasing accessibility to the elderly daycare service with lower price may prove beneficial.
