ABSTRACT

Purpose@#The purpose of this study was to identify factors influencing health-related quality of life depending on time sitting by elders living alone. @*Methods@#This was a secondary data analysis study of the 7th Korea Health and Nutrition Examination Survey (2016~2018). The participants in the study were 1,499 elderly people living alone who were 65 years of age or older. @*Results@#The average time sitting for elders living alone was 8.93 hours. If they had more than eight hours of sedentary time, their physical and psychological health conditions and quality of life were lower than those in the group that had less than 8 hours. The factors influencing quality of life were age, household income, central obesity, and subjective health status for elders with sitting-time within 8 hours (R2=.28, p<.001), and household income, economic activity, and subjective health status in elders with sitting-time over 8 hours (R2=.33, p<.001). @*Conclusion@#There were differences in factors affecting the quality of life depending on the length of sitting-time for elders living alone. Depending on the length of sitting-time, it is necessary to establish nursing interventions to promote physical and psychological health to improve the quality of life of elders living alone.

ABSTRACT

Purpose@#This study aimed to determine the effects of a 12-week metabolic syndrome BeHaS (Be Happy and Strong) program in elderly people with metabolic syndrome living alone, based on a community-based participatory research (CBPR). @*Methods@#A nonequivalent control group pre-posttest design was used, and the participants were 43 elderly people living alone (experimental group 24, control group 19). The experimental group received a one-hour program per week and two individual health consultations during 12 weeks. The control group received two sessions about the metabolic syndrome and two individual health consultations. The effects of health behavior, blood pressure, blood sugar levels, abdominal circumference, triglycerides, and self-esteem were evaluated. The data were analyzed using the independent t-test and Mann-Whitney U test. @*Results@#The health behavior with respect to the metabolic syndrome in the experimental group increased significantly (t = - 3.19, p = .002). Both diastolic blood pressure and abdominal circumference decreased in the experimental group (t = 2.00, p = .028 and t = 3.91, p < .001). No significant differences were observed between the groups in systolic blood pressure, fasting blood sugar levels, triglycerides, and self-esteem. @*Conclusion@#The 12-week metabolic syndrome BeHaS program using community resources improves the health of elderly people with metabolic syndrome living alone. Based on these findings, further studies on the effectiveness of the metabolic syndrome BeHaS program and the experiences of those who participated in the CBPR are warranted.

ABSTRACT

BACKGROUND: We evaluated new patient's satisfactory consultation time (SCT) and their willingness to pay additional costs (WPAC) for their SCT. METHODS: We surveyed medical service satisfaction, SCT, WPAC for their SCT, and payable amount to 612 new patients of single general hospital and measured their real consultation time (RCT). To compare WPAC and payable amount, we divided RCT into 4 groups (≤3 minutes, 3–5 minutes, 5–10 minutes, and >10 minutes), and SCT into 3 groups (≤5 minutes, 5–10 minutes, and >10 minutes). On the basis of WPAC, we estimated new patient's SCT. RESULTS: RCT was 6.2 minutes, SCT was 8.9 minutes, and medical service satisfaction score was 4.3 (out of 5). The number of patients having WPAC (payable group) was 381 (62.3%) and the amount was 5,853 Korean won. Their RCT and SCT were longer than non-payable group (6.4 minutes vs. 5.7 minutes, 9.3 minutes vs. 8.2 minutes). From multiple logistic regression analysis, WPAC of RCT 5–10 minutes was higher than that RCT ≤3 minutes (odds ratio=1.78). Payable amount was highest in RCT >10 minutes (6,950 Korea won) and SCT >10 minutes (7,458 Korean won). Intuitively we suggest 10 minutes as SCT, based on payable group's SCT (9.3 minutes) and cut-off time differentiating payable group with non-payable group (10 minutes). CONCLUSION: We found that new patient had WPAC for their SCT and the longer the SCT, the greater the amount. From this, we hope that current simplified new patient consultation fee calculating system should be modified combining the consultation time factor.

ABSTRACT

Bromhidrosis is a disease presenting as malodor caused by interaction between the discharge of apocrine glands and bacteria. The main therapeutic modalities are applying topical agents, liposuction surgery, and elective surgery. Among these, elective surgery is reported to be most effective. However, the efficiency largely depends on surgical technique. Additionally, other side effects, such as hematoma and scarring, are occasionally reported. Currently, CO2 laser and 1,064 nm Nd:YAG laser therapy are used, but as the wavelength is not specific to apocrine glands, these laser therapies have certain limitations. Recently, a 1,444 nm wavelength Accusculpt(TM) laser (LutronicCorp., Seoul, Korea) has been developed which is now commonly used for facial fat plasty and laser liposuction therapy. The use of this laser for bromhidrosis therapy targeting apocrine sweat glands is currently being discussed. Still, no studies on practical clinical use and side effects of this 1,444 nm wavelength laser have been published. In this report, we treated one bromhidrosis patient with 1,444 nm wavelength Accusculpt(TM) laser therapy on one side while conventional surgery was performed on the other side using a modified Inaba's method. We compared the efficacy of this laser therapy to the surgical modality by measuring malodor severity and overall satisfaction by questionnaire. We also checked for other complications and recurrence for 12 months after the treatment. This patient was largely satisfied as it has a much shorter down time with the same therapeutic outcome. As subdermal coagulation treatment by 1,444 nm Nd:YAG laser may be less invasive but effective therapy, we would like to recommend this modality as a possible treatment option.

ABSTRACT

BACKGROUND: Beside pharmacological treatment, non-pharmacological interventions are a great deal of interest resides on ways that allow modulation of brain plasticity in the elderly. Music therapy is a potential non-pharmacological treatment for the behavioral and psychological symptoms of dementia, but a few studies reported it to be helpful. The aim of this study was to evaluate the effect of structured musical intervention therapy in patient with cognitive decline. METHODS: The subjects of the study were a total of fifty elderly with cognitive decline (K-MMSE: 21+/-3.99, CDR: 0.80+/-0.38). The musical therapy was applied to the group twice a week, fifty minutes per session for eight weeks. The data were analyzed by using chi-square and paired t-test before and after musical intervention. RESULTS: The study showed a significant reduction in depression and anxiety after musical therapy measured with short form-GDS and BDI (p<0.001). Activities daily living (ADL) markedly improved after the all session of musical interventions (p<0.001). CONCLUSION: Group music therapy is a safe and effective method for treating depression and anxiety, and also improving ADL in patients with cognitive decline.

ABSTRACT

PURPOSE: The purpose of this study was to evaluate the quality of meta-analysis regarding exercise using Assessment of Multiple Systematic Reviews (AMSTAR) as well as to compare effect size according to outcomes. METHODS: Electronic databases including the Korean Studies Information Service System (KISS), the National Assembly Library and the DBpia, HAKJISAand RISS4U for the dates 1990 to January 2014 were searched for 'meta-analysis' and 'exercise' in the fields of medical, nursing, physical therapy and physical exercise in Korea. AMSTAR was scored for quality assessment of the 33 articles included in the study. Data were analyzed using descriptive statistics, t-test, ANOVA and chi2-test. RESULTS: The mean score for AMSTAR evaluations was 4.18 (SD=1.78) and about 67% were classified at the low-quality level and 30% at the moderate-quality level. The scores of quality were statistically different by field of research, number of participants, number of databases, financial support and approval by IRB. The effect size that presented in individual studies were different by type of exercise in the applied intervention. CONCLUSION: This critical appraisal of meta-analysis published in various field that focused on exercise indicates that a guideline such as the PRISMA checklist should be strongly recommended for optimum reporting of meta-analysis across research fields.

ABSTRACT

PURPOSE: The purpose of this study was to evaluate the effects of clinical practice and simulation-based practice for obstetrical nursing in terms of self-efficacy, practice satisfaction, and practice stress. METHODS: A non-equivalent control group post test design was used. The participants were 171 junior nursing students, 103 in the experimental group, and 68 in the control group. Simulation-based practice consisted of nursing assessment during labor, nursing assessment and intervention for normal and high risk mother, nursing education for mother and family during labor, and open lab. The experimental group participated in simulation-based practice for two days and the control group participated in clinical practice at delivery room for two weeks. RESULTS: The experimental group showed higher level of self-efficacy (p=.043), practice satisfaction (p<.001) and practice stress (p=.003) compared to the control group. CONCLUSION: Simulation-based practice is an effective learning method for obstetrical nursing and can be used as the alternative for clinical practice. However, stress management strategies are needed for simulation-based practice.

ABSTRACT

PURPOSE: The purpose of this study was to determine the effects of the BeHaS exercise program on balance and walking ability in middle-aged women with knee osteoarthritis. METHOD: Participants were 36 patients (experimental group: 18, control group: 18) with knee osteoarthritis. Women with knee osteoarthritis in the experimental group participated in the BeHaS exercise program for 65 to 90 minutes per session twice a week for 12 weeks. Data were collected by a structured interview and measurement tool from September 1, 2005 to March 30, 2006. Descriptive statistics, such as frequency, percentages, means, SD and t-test, Chi square test were used with the SPSS Win 12.0 to analyze the data. RESULTS: There was a significant difference between the two groups for dynamic balance, but differences in balance and walking ability were only partially supported. CONCLUSION: These findings indicate that the BeHaS exercise program could be a good nursing intervention for health maintenance and promotion of patients with knee osteoarthritis. Further research is needed in order to confirm these findings in elderly people with osteoarthritis.

ABSTRACT

PURPOSE: The purpose of the study was to identify effects of the BeHaS program on self esteem, fatigue and anxiety in postoperative care patients with breast cancer who had surgery with, chemotherapy, radiotherapy, with or without current hormone therapy. METHODS: This study was a 2-group quasi-experimental research study with a pre and post test design. Fifty-four patients with breast cancer were assigned to the experimental (n=29) or control group (n=25). The BeHaS program which strengthens self esteem through support over a 90-minute period, consists of theme activity (30 minutes), education (15 minutes), group support (15 minutes) and exercise (30 minutes). The experimental group participated in the program once a week for 10 weeks, but the control group was not involved. Data were gathered from October to December, 2008 using a questionnaire with measures of self esteem, fatigue and anxiety. Data were analyzed using t-test with SPSS Win 12.0 to identify differences between the groups. RESULTS: Self esteem was significantly increased (p= .001) and fatigue significantly decreased (p= .013) in the experimental group. But there was no significant difference in anxiety (p= .868). CONCLUSION: These results suggest that the BeHaS program for patients with breast cancer had beneficial effects on self esteem and fatigue.

ABSTRACT

PURPOSE: The purpose of this study was to identify the effects of a BeHaS exercise program on pain, health status, body weight in middle-aged women who have osteoarthritis in their knees. METHOD: The participants in this study were eighteen women whose mean age was 56.22years and who had osteoarthritis in the knees. The BeHaS exercise program consists of approximately one and half hour of exercise, two days per week, for twelve weeks. Data were gathered from September 2005 to December 2005 using a questionnaire and measuring VAS for pain, KWOMAC for health status, body weight for scale. Data were analyzed with the SPSS Win 11.0 using frequency and paired t-test for differences between pre-exercise and post-exercise. RESULTS: The results of this study are as follows: Scores for pain on post test were statistically lower than pre test(t=4.202, p=0.001), health status was statistically improved after exercise program (t=3.479, p=0.002). But body weight did not change(t=-.774, p=0.222). CONCLUSION: From these results, it is concluded that the BeHaS exercise program can be effective in decreasing pain and increasing health status in middle-aged women with knee osteoarthritis.
