Chinese Acupuncture & Moxibustion ; (12): 261-266, 2022.

ABSTRACT

OBJECTIVE@#To compare the therapeutic effect between Fanzhen Jieci (warming acupuncture plus fast needling) combined with conventional acupuncture and simple conventional acupuncture on discogenic sciatica.@*METHODS@#A total of 76 patients with discogenic sciatica were randomized into a Fanzhen Jieci group and a conventional acupuncture group, 38 cases in each one. Conventional acupuncture was applied at Shenshu (BL 23), Dachangshu (BL 25), L1-L5 Jiaji (EX-B 2) and Huantiao (GB 30) on the affected side, etc. in the conventional acupuncture group. On the basis of the treatment in the conventional acupuncture group, Fanzhen Jieci was applied at L1-L5 Jiaji (EX-B 2) and Huantiao (GB 30) on the affected side in the Fanzhen Jieci group, i.e. warming acupuncture was applied at L1-L5 Jiaji (EX-B 2), and fast needling was applied at Huantiao (GB 30) on the affected side for a depth of 40-60 mm, so as to introduce a sensation of electric shock transmitting to lower limb, and then the needle was immediately withdrawn. The treatment was given once every other day, 3 times a week for 3 weeks in both groups. The visual analogue scale (VAS) score of leg and low back pain, the Oswestry disability index (ODI) score and the 36-item short form health survey (SF-36) score before and after treatment were compared between the two groups.@*RESULTS@#Compared before treatment, the VAS scores of leg and low back pain and the ODI scores after treatment were decreased in both groups (P<0.001), the changes of the VAS scores of leg and low back pain in the Fanzhen Jieci group were larger than those in the conventional acupuncture group (P<0.05). After treatment, except for the role emotional and health transition scores, the various scores of SF-36 were increased compared before treatment in the Fanzhen Jieci group (P<0.01); except for the role physical, role emotional and health transition scores, the various scores of SF-36 were increased compared before treatment in the conventional acupuncture group (P<0.01). After treatment, the physical functioning, role physical, bodily pain, mental health and general health scores of SF-36 in the Fanzhen Jieci group were higher than those in the conventional acupuncture group (P<0.05).@*CONCLUSION@#Fanzhen Jieci combined with conventional acupuncture can effectively relieve the pain and improve the mental state in patients with discogenic sciatica, its therapeutic effect is superior to simple conventional acupuncture.

ABSTRACT

Objective To explore the incidence rate of mild cognitive impairment (MCI) among the elderly which transferred to Alzheimer disease (AD) and to analyze the related influencing factors. Methods 10 urban communities were selected through stratified cluster sampling as the research sites where 361 patients with MCI were screened from 1 942 residents aged over 60 years old. Questionnaires and laboratory assays were used to collect data from subjects, including characteristics of demographic, life style, medical history, Alzheimer-associated neuronal thread protein,(AD7C-(NTP)), amyloid β-protein 42(Aβ42) and amyloid β-protein 40(Aβ40), etc. Patients with MCI were followed up for three years to determine whether they progressed to AD. Results 121 of 361 patients of MCI converted to AD, and the incidence rate of MCI to AD was 9.49% person-years. According to the results of Logistic regression analysis model, elder (80-89 years) (OR=3.651,95% CI:1.295-10.297, P<0.001),female (OR=2.603,95% CI:1.136-5.966, P<0.001), heavy drinking(OR=1.479,95% CI:1.343-1.627, P<0.001), increased ADL score (OR=1.790, 95% CI:1.443-2.220, P=0.031) and smoking (OR=1.157,95% CI:1.091-1.224, P<0.001) were the risky factors of the transition of MCI to AD. The increase of Moca score (OR=0.766,95% CI:0.681-0.861, P<0.001) was the protective factor of the transition. Conclusions We should strengthen our monitor on elder female MCI patients, and promote healthy lifestyles among the elders, encouraging them to actively participate in physical exercises and reading, in order to delay patients' transition of MCI to AD.

ABSTRACT

Objective To evaluate the efficacy of oxycodone in preventing catheter-related bladder discomfort (CRBD) during recovery from anesthesia in the patients undergoing general anesthesia.Methods A total of 155 male patients, aged 18-60 yr, weighing 46-75 kg, of American Society of Anesthesiologists physical status Ⅰ or Ⅱ , scheduled for elective spinal surgery performed under general anesthesia, were randomly divided into 3 groups: control group (group C, n =52), oxycodone group (group O, n=51), and sufentanil group (group S, n=52).After induction of anesthesia, the patients were tracheally intubated and mechanically ventilated.At 15 min before the end of surgery, oxycodone 0.07 mg/kg was injected intravenously in group O, sufentanil 0.10 μg/kg was injected intravenously in group S, and the equal volume of normal saline was given in group C.The emergence time and extubation time were recorded.Riker sedation-agitation scale (SAS) score was recorded at 5, 15 and 30 min, and 1, 2 and 4 h after extubation (T1-6).The occurrence and severity of CRBD within 4 h after surgery, and occurrence of nausea and vomiting and respiratory depression were observed and recorded.Results Compared with group C, the SAS score at T1-4 and incidence and severity of CRBD were significantly decreased, and the emergence time and extubation time were prolonged in group S, and the SAS score at T1-6 and incidence and severity of CRBD were significantly decreased (P<0.05) , and no significant change was found in emergence time and extubation time in group O (P>0.05).Compared with the group S, the SAS score at T1-4 was significantly increased, the SAS score at T5-6 and incidence and severity of CRBD were decreased, and the emergence time and extubation time were shortened in group O (P<0.05).There was no significant difference in the incidence of nausea and vomiting and respiratory depression between the three groups (P>0.05).Conclusion Oxycodone 0.07 mg/kg injected intravenously at 15 min before the end of surgery can prevent the occurrence of CRBD during recovery from anesthesia in the patients undergoing general anesthesia.

ABSTRACT

<p><b>OBJECTIVE</b>To probe the tendency of physiological and psychological change of armored vehicle drivers working continuously in hot environment.</p><p><b>METHODS</b>Ten cross-designed standard subjects were divided into control group and driver group. The driver group worked continuously in 32 degrees C approximately 38 degrees C and RH 50% approximately 80% environment. Heart rate, body temperature, systolic blood pressure and pulse pressure were synchronously and dynamically determined during work. Psychological parameters such as profile of mood states (POMS) questionnaire, serial addition/subtraction (SAS) and two-digit search (TDS) selected from neurobehavioral evaluation system (NES) before and after work were also tested.</p><p><b>RESULTS</b>The curves of the four main physiological parameters rapidly synchro-rose to a peak at (36 +/- 3) min, whereafter the curves descended smoothly. At (144 +/- 8) min, the curves climbed up to the second peak. The determination of the behavioral psychology revealed that the degree of fatigue of the drivers increased gradually when the working time prolonged, and the grip descended by great extent, excitement reduced markedly after work, but the negative mood, such as confusion and depression were increased, error of visual apperceive-operation was increased, right decreased, intelligence reduced, rapidity and delicacy of movement reduced.</p><p><b>CONCLUSION</b>The physiological parameters of armored vehicle drivers working continuously in the hot condition appeared double-peak effect, the 1st peak was at (36 +/- 3) min, and the body was in mobilizing stage. Whereafter, the body was in smooth stage. And at (144 +/- 8) min, when the 2nd peak appeared, the body then was in physiological tolerance limit stage.</p>
