ABSTRACT

ObjectiveTo explore the effect of external diaphragm pacing therapy combined with abdominal functional electrical stimulation on respiratory function for stroke patients. MethodsFrom October, 2020 to September, 2022, 54 stroke patients were randomly divided into control group (n = 18), external diaphragm pacing group (n = 18) and combined treatment group (n = 18). All the groups received breathing training, while the external diaphragm pacing group received external diaphragm pacing therapy, and the combined treatment group received external diaphragm pacing and abdominal functional electrical stimulation therapy, for two weeks. They were measured forced vital capacity (FVC), forced expiratory volume in first second (FEV1), ratio of forced expiratory volume in first second in forced vital capacity (FEV1/FVC), peak expiratory flow (PEF), maximal inspiratory pressure (MIP) and maximal expiratory pressure (MEP) with pulmonary function instrument; measured diaphragmatic excursion (DE) and diaphragmatic thickness (DT) with ultrasound, before and after treatment. ResultsThree cases in the control group, two cases in the external diaphragm pacing group and one case in the combined treatment group dropped off. The FVC, FEV1, PEF, MIP, MEP and DE improved in all the groups (|t| > 3.366, P < 0.01) after treatment; and the FVC, FEV1, MIP and DE increased more in the combined treatment group and the external diaphragm pacing group than in the control group (P < 0.05); the FVC and FEV1 increased more in the combined treatment group than in the external diaphragm pacing group (P < 0.05). ConclusionExternal diaphragm pacing therapy may improve ventilation and inspiratory muscle strength, and increase diaphragm movement for stroke patients; while the ventilation improved more after combining with abdominal functional electrical stimulation.

ABSTRACT

Objective:To explore the relationship between constitutional types of Chinese medicine and Alzheimer's disease (AD) and to construct an early warning model for AD risk.Methods:In the established multimodal interventions to delay dementia and disability in rural China (MIND-China) study, 4 033 elderly subjects aged ≥60 years old were included. The data including demographic, underlying disease and neuropsychological data were collected.The Chinese medicine service record form for the elderly was used to assess constitutional types of Chinese medicine and to apply the NIA-AA diagnostic criteria published by the National Institute on Aging and the Alzheimer's Association in 2011 for the diagnosis of clinically likely AD. Logistic regression analysis and AD risk prediction models were constructed using R statistical software, and the final prediction results were presented using columnar plots.Results:The MIND-China cohort was dominated by the abnormal constitution (69.28%), of which Phlegm-wetness type was the most common (58.05%), followed by Yang-deficiency type (23.85%). The most constitutional type of Chinese medicine among AD patients was Phlegm-wetness type (54.35%), followed by Qi-depression type (38.04%). Multi-factorial logistic regression analysis suggested that increasing age ( β=0.101, P<0.001, OR=1.107, 95% CI=1.069-1.146) and Qi-depression type ( β=0.622, P=0.016, OR=1.862, 95% CI=1.116-3.076) were able to increase the risk of developing AD, while education ( β=-1.047, P<0.001, OR=0.351, 95% CI=0.205-0.584) was able to reduce the risk of developing AD. By using the risk score model to calculate the total risk score for each subject and plotting the receiver operating characteristic curve (ROC), the area under the ROC was 0.769 and the calibration curve showed excellent consistency between prediction and reality. Conclusion:Older adults with Qi-depression type are significantly associated with an increased likelihood of AD.

ABSTRACT

Objective:To investigate the relationship between serum total bilirubin and diabetic retinopathy (DR) in type 2 diabetes mellitus.Methods:A retrospective analysis was performed on 1676 patients with type 2 diabetes who were admitted to the Department of Endocrinology, the Third Affiliated Hospital of Anhui Medical University (Hefei First People′s Hospital) from January 2010 to December 2016. According to the fundus examination results, they were divided into diabetic retinopathy group (DR group) and the non-diabetic retinopathy group (NDR group). The clinical data of the two groups were compared and divided into Q1, Q2 and Q3 groups according to the total bilirubin trigrams to analyze the correlation between the serum total bilirubin level and the occurrence of DR.Results:The comparison between the two groups showed that the serum total bilirubin, direct bilirubin and indirect bilirubin of DR group were lower than those of NDR group ( P<0.05), and the age, disease course, systolic blood pressure, urea nitrogen and blood uric acid of DR group were higher than that of NDR group ( P<0.05). When the risk factors between the two groups were included in the binary logistic regression equation, it was found that serum total bilirubin was protective factor of DR( OR: 0.922, 95% CI: 0.893-0.952), and disease course, systolic blood pressure and urea nitrogen were independent risk factors of DR. When the serum total bilirubin level was further divided into the third quadrant, the OR value of diabetic retinopathy in the third quadrant was 0.455 (95% CI: 0.328-0.629) compared with the first quadrant, after adjusting for confounding factors. Conclusions:Serum total bilirubin is the protective factor of DR and it may be an early clinical marker to predict the occurrence of DR. For patients with low serum bilirubin, the levels of urea nitrogen and systolic blood pressure should be closely monitored, which is of great significance for the prevention and treatment of DR.

ABSTRACT

Objective In order to know the changes of serum pertussis IgG antibody level and the percent-age of white blood cells and lymphocytes in blood routine of children with IgG antibody positive,it is easy to diagnose and treat according to the clinical symptoms and examination results of children.Methods 485 cases of children in Shangdong Provincial Hospital were randomly enrolled as the test subjects,and pertussis anti-body IgG(PT -IgG)detection and blood cell detection were carried out.Results The levels of Pertussis IgG antibody were positive in 90 cases,the positive rate was 18.6%,pertussis IgG antibody level of children aged 8- <10 years old was the highest,2- <4 years old took the second place,10-14 years old were with the low-est levels.Among the patients with positive pertussis antibody levels,white blood cells in total were more than 20×109/L and lymphocyte percentage reached more than 60% in 19 cases,the positive rate was 21.1%,of which 6- <8 years old the highest positive rate was 10-14,the lowest.Conclusion IgG antibody of pertussis incidence is higher in children under 10 years old,the changes of white blood cells and lymphocytes mainly in children under the age of 8,the general course of the disease can be up to 2-3 months,so the need for early diagnosis and treatment,strengthen the immunity of pertussis vaccine is very necessary for children.

ABSTRACT

Objective: To observe blood levels of cystetine C (Cys C) and superoxide dismutase (SOD) in patients with obstructive sleep apnea syndromes (OSAS) related hypertension (HP). Methods: A total of 102 HP patients received polysomgography (PSG) in our hospital from 2013-01 to 2013-12 were studied. According to apnea hypopnea index (AHI), the patients were divided into 2 groups: OSAS with HP (OH) group, n=55 patients with AHI≥5 and HP group,n=47 patients with AHI0.05. Conclusion: Blood levels of Cys C in OH and HP groups were higher than Control group, Cys C in OH group was higher than HP group; SOD levels in OH and HP groups were lower than Control group, SOD in OH group was lower than HP group. The relationships between Cys C, SOD and OH severity were unclear.

ABSTRACT

Objective: To explore the effect of persistent atrial ifbrillation (AF) on circadian rhythm of blood pressure (BP) in patients with essential hypertension (EH). Methods: A total of 173 EH patients treated in Gansu Provincial Hospital from 2013-02 to 2014-01were studied. The patients were divided into 2 groups: EH group,n=88 and Persistent AF combining EH group,n=85. The baseline information was studied and the risk factors of persistent AF combining EH were investigated by multivariate logistic regression analysis. Results: Compared with EH group, the Persistent AF combining EH group showed decreased average daytime DBP, minimum daytime SBP, minimum daytime DBP and the average 24-hour DBP, while increased maximum nighttime SBP and the percentage of reverse dipper in DBP, allP0.05. Multivariate logistic regression analysis indicated that the maximum nighttime SBP was obviously related to persistent AF combining EH (OR=1.038, 95 CI 1.014-1.062,P=0.001). Conclusion: Persistent AF may incur daytime BP dropping, such change was not obviously observed for nighttime BP in EH patients.

ABSTRACT

Objective To study activation effect of a natural compound baicalein on cystic fibrosis transmembrane conductance regulator(CFTR) chloride channel.Methods A cell-based fluorescence assay was used to determine CFTR-mediate iodide influx rate/dt(mmol?L-1?s-1)] activated by baicalein(the concentrations were 0.18,0.55,1.65,5,15,44,133 and 400 ?mol?L-1).Results The Ka of flavonoid baicalein stimulating CFTR was about 16 ?mol?L-1.The half of maximal activity was reached in ten minutes and the activation disappeared in 20 min after baicalein was washed out.The activation of baicalein was not affected obviously under different concentrations of Forsklin(0,20,50 and 100 nmol?L-1)and the activation could be totally inhibited by CFTRinh-172.Conclusion Baicalein can stimulate CFTR-mediated iodide influx in a dose-dependent way and its activity manifests a rapid and reversible characteristic.It might work in both elevating CFTR protein phosphorylation and direct binding way.

ABSTRACT

OBJECTIVE To find out the status of and impact factors on bacterial concentration in clean operating room.METHODS Set a questionnaire to the staff in the operating room of 24 tertiary 3A hospitals.RESULTS The 9 basic questions got correct and accordant answers.The answers for the 5 questions which were related to the characteristic of clean operating room had biger errors and difference.CONCLUSIONS The investigatees know about the basic requirements of the detecting method for bacterial concentration,but lack for the exact cognition of special requirements in clean operating room(e.g.State standard).The sticking point of the current problem is the misunderstanding on the 5 key questions.In order to make sure the excellent air condition in the cleaning operating room,the training of correlative specialty is very important.
