Therapeutic relationship on the web: to face or not to face? / Relação terapêutica na rede: to face or not to face?

In this age of unprecedented expansion of media and information dissemination and sharing, the use of electronic means should be reconsidered. The use of new technologies should be studied to understand how it may affect the relationship between patient and therapist during psychotherapy or psychoanalytic treatments. This study offers a critical discussion of the effect of technologies on clinical practice, and vignettes are used to describe their impact on frame, anonymity, abstinence and therapeutic neutrality. Transfer and countertransference issues resulting from these changes are also discussed. The potential benefits of new technologies in psychotherapy are appreciated, but the authors draw attention to the need to reflect about the presence of the therapist in those technologies and the preservation of the therapeutic setting, so that a satisfactory progression of the work of the dyad is ensured. This study also discusses the use of technologies in the expansion of learning and application of the therapeutic technique to overcome geographic and time barriers, among others (AU)

Na era da expansão sem precedentes dos meios de comunicação, da divulgação e compartilhamento de informações por meios eletrônicos, torna-se necessário repensar sua utilização. Os autores consideram importante compreender de que forma o uso das novas tecnologias pelos pacientes e terapeutas interfere na relação entre ambos na vigência do tratamento psicoterápico ou psicanalítico. É proposta uma discussão crítica acerca de sua influência na prática clínica, e apresentado em vinhetas o impacto no enquadre, no anonimato, abstinência e neutralidade terapêuticas. Também são abordadas questões transferenciais e contratransferenciais decorrentes dessas mudanças. Os autores consideram os benefícios potenciais das novas tecnologias na prática psicoterápica, alertando porém para a necessidade de reflexão a respeito de sua presença nas mesmas e de preservação do setting terapêutico, visando sempre a evolução satisfatória do trabalho da dupla. O ensino da psicoterapia de orientação analítica no ambulatório assistencial: público de psiquiatria do Hospital de Clínicas de Porto Alegre / Teaching analytic psychotherapy in ambulatory care: public psychiatric Hospital de Clinicas Porto Alegre

O artigo descreve o ensino da psicoterapia de orientação analítica (POA) naresidência em psiquiatria do Hospital de Clínicas de Porto Alegre (HCPA). Inicialmente,é realizada uma revisão da literatura sobre a importância do ensinoda POA nos programas de residência médica de psiquiatria, seguida dorelato da história da entrada da psicanálise no Rio Grande do Sul e da evoluçãodo ensino da POA no estado. As questões metodológicas e éticas do ensinoda POA na residência de psiquiatria do HCPA são discutidas, e o atual programa teórico e prático é apresentado. Por fim, a possibilidade da integraçãoda pesquisa com a assistência e ensino é discutida, e exemplos práticos destaintegração são apresentados, juntamente com o protocolo de avaliação e planejamentoterapêutico do programa de POA do HCPA.

Reflexões sobre psicoterapia de orientação nalítica (POA) no ambulatório assistencial/público de psiquiatria do HCPA, suas repercussões éticas e técnicas / Reflections on analytically oriented psychotherapy (POA) at the psychiatric care/public outpatient unit of HCPA – its ethical and technical repercussions

O treinamento em psicoterapia de orientação analítica (POA) nos programas de Residência Médica permanece sendo um objetivo educacional na área da Psiquiatria. Estudos demonstram a crescente necessidade de maior compreensão psicodinâmica das doenças psiquiátricas frente aos avanços biológicos da área. O Hospital de Clínicas de Porto Alegre (HCPA) vem se mantendo como espaço de formação e assistência em POA ao longo dos anos. Objetivos: O objetivo deste trabalho é trazer reflexões sobre a prática e o ensino da POA no HCPA. Métodos: Foram realizadas entrevistas com médicos psiquiatras supervisores de POA no HCPA, além de grupos de discussão e revisão bibliográfica, buscando descrever aspectos relacionados a assistência, ensino e aprendizagem da teoria e da técnica psicodinâmicas nessa instituição e em Porto Alegre. A partir disso, foram feitas reflexões relacionadas a questões éticas e técnicas do ensino de POA neste hospital. Conclusão: O ensino da POA constitui marca registrada do HCPA, acompanhando sua evolução histórica e contribuindo para o estabelecimento de uma cultura específica que caracteriza a psiquiatria gaúcha. Através de sua prática vêm-se abrindo campo para abordagens mais flexíveis, tratamentos combinados, discussões de dilemas éticos e estímulo para aplicação do método dentro da prática psiquiátrica desta instituição.

The training in psychoanalytically oriented psychotherapy (POA) in medical residency programs is still an educational objective in the area of Psychiatry. Studies demonstrate the increasing need for a broader psychodynamic understanding of the psychiatric illnesses in the context of the biological advances in the area. The Hospital de Clínicas de Porto Alegre (HCPA) remains as a place of formation and assistance in Porto Alegre, a southern city in Brazil, throughout the years. Objectives: The objective of this project is to bring in some reflections made on the practice and the education of the POA to the HCPA. Methods: Interviews with psychiatrists who are supervisors of POA in HCPA were carried out. Besides, discussion groups and bibliographical revision were made, trying to describe aspects of the assistance, education and learning of the psychodynamics theory and technique in this institution and in the city of Porto Alegre. Thus, ethical and technical issues of the education in POA in this hospital were taken into account throughout our reflections. Conclusion: The training in POA constitutes a trademark of the HCPA, following its historical evolution. It also contributes to the establishment of a specific culture that identifies the Psychiatry in Rio Grande do Sul. Through the practice of POA, more flexible approaches and combined treatments havebeen created. Moreover, discussions concerning ethical dilemmas and thestimulation for the application of these new methods in the HCPA psychiatricclinic have been taken place.
