ABSTRACT

Objetivos: Estudiar el metabolismo intermediario basal y postprandial en pacientes con hipertensión arterial (HTA) esencial. Metodología: Se estudió a 59 personas de 45 a 72 años de edad, 29 pacientes con HTA esencial, 19 varones y 10 mujeres ya 30 personas controles normales, 18 varones y 12 mujeres, de nivel del mar, a quienes en condiciones de ayuno nocturno se determinó en sangre colesterol total (CT), HDL, triglicéridos (Tg), glucosa (G), insulina (1) y ácidos grasos no esterificados (AGNE); se calculó el colesterol VLDL, LDL, No-HDL y HOMA. Se les administró un desayuno mixto con 730 Kcal., después de lo cual se midió G, 1, Tg Y AGNE durante 6 horas. Resultados: En condiciones basales los pacientes hipertensos tuvieron valores más altos de CT, Tg, VLDL. LDL, No-HDL, glucosa y HOMA. Los hipertensos en todo el período postprandial tuvieron cifras más altas de Tg y AGNE; la glucosa e insulina estuvieron más altos entre la primera y segunda hora. Conclusiones: Se demuestra que en la HTA esencial, se producen alteraciones del metabolismo intermediario relacionadas a resistencia a la insulina y síndrome plurimetabólico, lo que deberá confirmarse en un grupo más amplio.


Objectives: To study the basal and postprandial intermediary metabolism in patients with essential high blood pressure (HBP). Methodology: A total of 59 persons, sea level dwellers, 45 to 72 years old, 29 essential HBP patients, 19 male and 10 female and 30 control subjects, 18 mal e and 12 femate were included. At fasting state glucose (G), total cholesterol (TC), HDL, triglycerides (Tg), insulin (1) and non-esterified fatty acids (NEFA) were measured in blood by conventional methods, VLDL, LDL, Non-HDL and homeostasis model assessment (HOMA) were calculated; G, 1, Tg, and NEFA were determined during 6 hours after the ingestion of a mixed meal containing 730 Kcal. Results: At fasting the HBP patients had higher CT, Tg VLDL, Non-HDL, G and HOMA than the control subjects. During the whole postprandial time HBP patients presented higher Tg and NEFA values than the controls, glucose and insulin were higher in HBP between the first and second hours. Conclusions: These results demonstrate that essential HBP patients develop intermediary metabolisrn disturbances related to insulin resistance and plurimetabolic syndrome.
