ABSTRACT
Paciente de sexo femenino, de 40 años de edad, con antecedente de Diabetes Mellitus tipo 1 (DM1) de larga data, con retinopatía y amaurosis en ojo izquierdo. Consulta por dolor ocular derecho asociado a desviación ocular, disminución de agudeza visual e inestabilidad de la marcha. La exploración física objetiva arreflexia, marcha atáxica, parálisis completa del III par craneal derecho y desviación exo-hiper en ojo izquierdo. Estudio imagenológico sin hallazgos. La punción lumbar presenta disociación albumino-citológica, lo que hace sospechar Síndrome de Miller Fisher (SMF). Sólo refiere el antecedente de aplicación de vacuna anti-influenza una semana previo al inicio de los síntomas. Se reporta el caso por tratarse de una patología poco frecuente y por sospecha de ser secundario a vacunación antigripal.
Female, 40 years old with long-standing type 1 diabetes mellitus with retinopathy and amaurosis of the left eye. Presents with right eye pain with ocular deviation, a lowering of visual acuity and gait instability. A physical examination revealed objective areflexia, ataxic gait, complete paralysis of the III right cranial nerve and exo-hyperdeviation of the left eye. Imaging studies were negative. Lumbar puncture showed albumin-cytological dissociation leading us to suspect Miller Fisher syndrome. Her only history was receiving the anti-influenza vaccine one week prior. We report the case as it is an infrequent pathology and because of the possibility that it was secondary to the anti-influenza vaccine.
