ABSTRACT

Background: Quality in the health care institutions is multifaceted & multidimensional with its core element as the customer’s satisfaction. Improvement in the health care services is a long drawn relentless process which can be achieved by implementing a carefully planned Quality Management System. Laboratory services B.J. Medical College & Civil Hospital Ahmedabad, has set its Quality indicators and is monitoring it as a part of continual improvement. One of the Quality indicators is Sample rejection. The laboratory aims to reduce the sample rejections by complying with the standards & recommended guidelines. Methods: This is to monitor & analyze the Sample Rejections as a Quality Indicator for Continual improvement of the Microbiology Department. Primary data collection done before training & secondary data collection done after the training of nursing staff & Resident Doctors. Results: The sample rejection rate of the Microbiology Laboratory in the month of July 2010 was 0.31%. After the training of the nursing staff as well as resident doctors regarding proper collection & transport of samples based on scientific principles, a decreasing trend in the sample rejection rate is observed. i. e. from .31% to 0.11 % in the 13 months duration of study.Conclusion: The implementation of a systematic error-tracking system i.e. monitoring the sample rejections, analysing its trend, taking preventive & corrective actions to reduce the sample rejection rate in daily practice definitely improves the quality of the laboratory results and is a quality indicator for continual improvement of the laboratory.

ABSTRACT

Background : Bad obstetric history (BOH) implies previous unfavorable fetal outcome. Causes of BOH may be genetic, hormonal, abnormal maternal immune response and maternal infection. C.trachomatis is the most common bacterial sexually transmitted infection worldwide. Also it has been associated with adverse outcomes of pregnancy. The identification of C. trachomatis during pregnancy and its early treatment prior to delivery would therefore be an important strategy to prevent multiple complications in mother. Serology might act as a convenient tool for diagnosis. Methods:139 pregnant patients having history of BOH as study group and 30 normal pregnant as control group were tested by Indirect ELISA for detection of anti C.trachomatis IgM and IgG. Results: Anti C.trachomatis IgM was detected in 21 (15.11%) patients and anti C.trachomatis IgG was detected in 33 (23.74%) patients. Interpretation and Conclusion: Higher numbers of positive patients were found among younger age group. Statistical analysis shows that serological detection of anti C.trachomatis IgM and IgG antibodies for detection of current and past infection by C.trachomatis is significant in BOH patients.

ABSTRACT

Objecu've: On-site evaluation of laboratories with standard checklist is a first step to promote effective and consistent supervision. The present study was carried out to evaluate the impact of the RNTCP- Intermediate Reference Laboratory External Quality Assessment- On-Site Evaluation visits on quality of sputum smear microscopy services of Gujarat, India. Data of three IRL-EQA-OSE visit rounds, carried out between January 2005 and December 2010 are presented here. Materiaf and Methods: Within the Revised National Tuberculosis Control Programme EQA framework, the IRL, Ahmedabad visited all Gujarat District Tuberculosis Centres, and evaluated their sputum smear microscopy services. The study covered a cohort of 29 DTCs during each of the three IRL-EQA-OSE visits. The authors focused on section III of Annexure A to study and analyse the said impact. In order to convert qualitative data into quantitative one, the authors denoted a score of 1 to "Acceptable" (No Error) remark and 0 to "Not-Acceptable" (Error) one. Results.' Alarger degree of improvement was noted in Standard Operating Procedure practices, Disinfection practices, and Internal Quality Control practices. Many DTCs did not retrain their laboratory staff in EQA methodology. The Gujarat DTCs achieved an overall score of (8201957) 86% during the initial OSE visits which consistently improved to (842/957) 88% and (885/957) 92% during the two follow-up OSE visits along with sustenance and improvement in many important laboratory parameters. Conclusion' The co-sponsoring organisation (IRL) recognises the challenges and therefore, is committed to supporting state-level implementation of EQA through additional training, technical assistance to districts, and improving this technical guidance. By periodic IRL. EQA-OSE visits, sputum smear microscopy services can be sustained and improved at field level.
