ABSTRACT

Introdução: O carcinoma basocelular é o tipo mais frequente de lesão tumoral de pele acometendo frequentemente a região nasal. Os defeitos resultantes exigem o emprego de técnicas de reconstrução nasal. A primeira cirurgia plástica descrita é a reconstrução nasal segundo o método indiano. Ao longo dos séculos sofreu diversas modificações pelos maiores nomes da especialidade, culminando no retalho frontal paramediano. Relato do Caso: Paciente do sexo masculino, 62 anos, apresentou-se no serviço de Cirurgia Plástica do Hospital Federal de Ipanema com lesão ulcerada, descamativa, ocupando toda a região lateral do nariz com três anos de evolução, sugestiva ao exame físico de tumor maligno de pele. Foi realizada exérese da lesão cutânea e imediata reconstrução com retalho frontal paramediano e posterior cirurgia de refinamento. Conclusão: Devido à sua vascularização pela artéria supratroclear e textura semelhante ao tecido nasal, o retalho frontal paramediano constitui-se como o gold standard para a correção de grandes defeitos nasais.


Introduction: Basal cell carcinoma is the most frequent type of tumoral lesion of the skin, often affecting the nasal region. The resulting defects require the use of nasal reconstruction techniques. The first plastic surgery described is the nasal reconstruction, according to the Indian method. Over the centuries, it has been the object of several modifications by the biggest names in the specialty, culminating in the paramedian frontal flap. Case Report: A 62-year-old male patient went to the Plastic Surgery Service of the Federal Hospital of Ipanema, presenting an ulcerated, scaly lesion, occupying the entire lateral region of the nose with three years of evolution, suggesting at physical examination malignant skin tumor. It was performed cutaneous lesion excision and immediate reconstruction with a paramedian frontal flap and further refinement surgery. Conclusion: Due to its vascularization by the supratrochlear artery and texture similar to the nasal tissue, the paramedian frontal flap is the gold standard for the correction of significant nasal defects.

ABSTRACT

Introdução: Avaliar os resultados da mamoplastia redutora por meio do instrumento BREAST-Q®. Métodos: Foram selecionadas 83 pacientes portadoras de hipertrofia mamária atendidas no Serviço de Cirurgia Plástica do Hospital Universitário da Universidade Federal de Juiz de Fora (HU-UFJF), conduzidas em um estudo de coorte prospectivo. Com idade superior a 18 anos, alfabetizadas, operadas no período entre julho de 2013 a agosto de 2015. Utilizou-se o questionário BREAST-Q®, módulo para mamoplastia redutora/mastopexia no pré-operatório e com seis meses de pós-operatório. Teste t pareado foi utilizado para avaliar a significância das mudanças na satisfação com as mamas, bem-estar psicossocial, bem-estar físico e bemestar sexual. Resultados: Setenta e nove pacientes (95,1%) responderam os questionários de pré e pós-operatório. Melhoras com significância estatística foram observadas nas médias das categorias presentes quando comparados pré e pós-operatório, como satisfação com as mamas, bem-estar psicossocial, bemestar físico e bem-estar sexual. Idade, peso retirado e estado nutricional não influenciaram os resultados. As complicações foram observadas em dez (12,04%) pacientes. Conclusão: O BREAST-Q® mostrou que as pacientes com hipertrofia mamária submetidas à cirurgia para redução do volume mamário tiveram uma melhora significativa na qualidade de vida em seus diversos aspectos, além de avaliar como positivo o resultado cirúrgico, a atuação da equipe médico-hospitalar e a satisfação com os mamilos no pós-operatório tardio.


Introduction: To evaluate the results of breast reduction using the BREAST-Q® instrument. Methods: This is a prospective cohort study of 83 patients with breast hypertrophy who were treated at the Plastic Surgery Service of the University Hospital of the Federal University of Juiz de Fora. The subjects were older than 18, were literate, and underwent surgery between July 2013 and August 2015. We administered the BREAST-Q® reduction mammoplasty/mastopexy questionnaire, both during the preoperative period and at 6 months after surgery. The Student's t-test was used to evaluate the significance of changes in breast satisfaction and the psychosocial, physical, and sexual well-being of the patients. Results: Seventy-nine patients (95.1%) completed the pre- and postoperative questionnaires. Significant postoperative improvements were observed in breast satisfaction and the psychosocial, physical, and sexual wellbeing. Age, amount of breast tissue removed, and nutritional status did not influence the results. Surgical complications were observed in 10 patients (12.04%). Conclusion: The BREAST-Q® showed that patients who underwent breast reduction surgery achieved a significant improvement in quality of life and were satisfied with the surgical outcome, hospital care, and nipple appearance in the late postoperative period.

ABSTRACT

Abstract Introduction: Few data can be found about cardiac arrest in the intensive care unit outside reference centers in third world countries. Objective: To study epidemiology and prognostic factors associated with cardiac arrest in the intensive care unit (ICU) in an average Brazilian center. Methods: Between June 2011 and July 2014, 302 cases of cardiac arrest in the intensive care unit were prospectively evaluated in 273 patients (age: 68.9 ± 15 years) admitted in three mixed units. Data regarding cardiac arrest and cardiopulmonary resuscitation were collected in an "Utstein style" form and epidemiologic data was prospectively obtained. Factors associated with do not resuscitate orders, return of spontaneous circulation and survival were studied using binary logistic regression. Statistical package software used was SPSS 19.0 (IBM Inc., USA). Results: Among 302 cardiac arrests, 230 (76.3%) had their initial rhythm recorded and 141 (61.3%) was in asystole, 62 (27%) in pulseless electric activity (PEA) and 27 had a shockable rhythm (11.7%). In 109 (36.1%) cases, cardiac arrest had a suspected reversible cause. Most frequent suspected cardiac arrest causes were hypotension (n=98; 32.5%), multiple (19.2%) and hypoxemia (17.5%). Sixty (19.9%) cardiac arrests had do not resuscitate orders. Prior left ventricle dysfunction was the only predictor of do not resuscitate order (OR: 3.1 [CI=1.03-9.4]; P=0.04). Among patients that received cardiopulmonary resuscitation, 59 (24.4%) achieved return of spontaneous circulation and 12 survived to discharge (5.6%). Initial shockable rhythm was the only return of spontaneous circulation predictor (OR: 24.9 (2.4-257); P=0.007) and survival (OR: 4.6 (1.4-15); P=0.01). Conclusion: Cardiopulmonary resuscitation rate was high considering ICU patients, so was mortality. Prior left ventricular dysfunction was a predictor of do not resuscitate order. Initial shockable rhythm was a predictor of return of spontaneous circulation and survival.
