ABSTRACT

The new Coronavirus (SARS-CoV-2) appeared in China in December 2019. Since then and until April 2020 it spread worldwide affecting more than three million people. Its exponential rise is still growing all over the world, taking thousands of lives. SARS-CoV-2 is very contagious, person to person, by droplets which can generate a respiratory infection known as COVID-19. Some patients are at higher risk: Older people, those with cardiovascular disease, diabetes and hypertension are the most prone to an unfavorable outcome. Our Inflammatory Bowel Disease (IBD) patients are a special cluster, with many of them taking immunosuppressive treatment for long periods, which could pose an important risk. Scientifics societies all over the world have joined efforts to generate data, share experiences and make recommendations for good clinical management. This is a review of the available evidence, expert opinion, and proposed ways of working during the pandemic


El nuevo coronavirus (SARS-CoV-2) apareció en China en diciembre de 2019. Desde su inicio hasta abril de 2020 se ha expandido por todo el mundo, afectando a más de tres millones de personas. Su ascenso exponencial sigue creciendo, generando miles de muertes. Su contagiosidad es persona a persona por gotitas, pudiendo llegar a generar un cuadro clínico de infección respiratoria conocido como COVID-19. Algunos pacientes tienen más riesgos de tener un curso desfavorable; adultos mayores, pacientes con enfermedad cardiovascular, hipertensos y diabéticos. Nuestros pacientes con enfermedad inflamatoria intestinal son un grupo de pacientes con características particulares, muchos de ellos reciben tratamiento inmunosupresor por largos períodos, lo que pudiese suponer un riesgo específico. Las sociedades científicas de Europa y Norteamérica han realizado un esfuerzo conjunto para generar datos, compartir experiencias y dictar recomendaciones de buen manejo clínico. Esta es una revisión de la evidencia disponible, opiniones de expertos y formas de trabajo propuestos durante la pandemia.

ABSTRACT

INTRODUCCIÓN: La Insuficiencia Cardiaca corresponde a una patología cardiovascular muy frecuente en nuestra población. Causa gran número de hospitalizaciones al año, significando un gasto importante en salud. Hay clara relación de factores demográficos con el desarrollo, evolución y pronóstico de IC, siendo uno de los más aceptados la edad, y uno de los más discutidos, el sexo. El objetivo de este trabajo ha sido caracterizar a los pacientes con insuficiencia cardiaca que se hospitalizaron en el Servicio de Medicina del Hospital de Familia y Comunidad de Mulchén, Chile, durante el año 2011. MATERIAL Y MÉTODO: Se realizó un estudio retrospectivo, observacional y descriptivo, mediante la revisión de los registros oficiales de ingreso-egreso del Servicio de Medicina del Hospital de Mulchén durante el 2011, analizando variables como edad, sexo y días de hospitalización. Se incluyó un total de 162 pacientes; 84 varones (51,85 por ciento). RESULTADOS: Las edades variaron entre 38 y 96 años, con un promedio de 67 años y con un 73,45 por ciento de pacientes con 60 años o más. La duración de la hospitalización varió entre 3 y 19 días, con un promedio de11. Casi el 80 por ciento de los pacientes provino del servicio de urgencia. Los diagnósticos asociados más frecuentes fueron hipertensión/cardiopatía hipertensiva (22 por ciento), y cardiopatía coronaria (11,2 por ciento). Se registraron 12 fallecimientos (7 por ciento). El 55 por ciento de los pacientes corresponde al Fondo Nacional de Salud B. DISCUSIÓN: Los resultados obtenidos están de acuerdo a lo descrito en la literatura para la población general y aportan datos útiles para enfrentar al paciente con insuficiencia cardiaca que se atiende en el servicio.


INTRODUCTION: Heart Failure is very frequent in our adult population. It causes a great number of hospitalizations a year and means an important expense in health. There is a clear relationship between diverse demographic factors and the development, evolution and prognosis of heart failure, being one of the accepted the age and of the discussed the sex. The objective of this study was to characterize hospitalized patients with heart failure in the Department of Medicine of the Familia y Comunidad Hospital of Mulchén, Chile, in 2011. MATERIAL AND METHOD: Retrospective, observational and descriptive study, there were checked the records of admission-discharge of the department and were selected all the patients with heart failure diagnosis at the income analyzing variables as age, sex, days and others. Included a total of 162 patients; 84 males (51.85 percent).RESULTS: The ages ranged from 38 to 96 years, with an average of 67 years, with a 73.45 percent of patients 60 years or older. The length of hospitalization ranged between 3 and 19 days, with an average of 11 days. Almost 80 percent of the patients came from to Emergency Department. The most frequent associated diagnoses were hypertension/hypertensive cardiomyopathy (22 percent) and coronary heart disease (11.2 percent. There were 12 deaths (7 percent). 55 percent of patients were affiliated to Fondo Nacional de Salud B.DISCUSSION: We conclude that the results obtained are concordant with those described in the literature for the general population and provide useful data to confront the patient with heart failure served in our service.

INTRODUCCIÓN: La Diabetes Mellitus es un conjunto de trastornos metabólicos cuya característica común es la hiperglicemia. Es importante destacar que dentro de sus complicaciones, la principal causa de muerte en el paciente diabético es cardiovascular. De este modo, un control adecuado de la patología y el manejo de los factores de riesgo asociados tienen una repercusión significativa en la morbimortalidad de este grupo de pacientes. OBJETIVO: Caracterizar clínicamente a los pacientes adultos con diabetes mellitus tipo 2 atendidos en el programa de salud cardiovascular del Consultorio del Hospital de Penco –Lirquén. MATERIAL Y MÉTODO: Estudio retrospectivo, observacional y descriptivo, revisando la última atención de los tarjetones de control crónico de los pacientes del programa de salud cardiovascular del Consultorio del Hospital de Penco-Lirquén, entre enero y diciembre del 2011, excluyéndose los adultos mayores(con edad ≥65 años). RESULTADOS: El 68,8 por ciento de los pacientes(n=86) eran mujeres. El promedio de edad fue de 52,88 años. El 58,1 por ciento presentó valores de presión arterial en rango de hipertensión.En relación a los valores de hemoglobina glicosilada, en el 63,8 por ciento fue mayor a 8. El promedio de índice de masa corporal fue de 32,4. DISCUSIÓN: La característica clínico-epidemiológica más destacada del estudio fue la mala compensación de los pacientes tanto en el control de diabetes mellitus como en los factores de riesgo asociados al desarrollo de patologías cardiovasculares, lo que requiere una intervención urgente para una mejor calidad de vida futura.


INTRODUCTION: Diabetes Mellitus is a group of metabolic disorders characterized by hyperglycemia that has increased in prevalence in the last years. Between its complications, we can find the main cause of death in diabetic patients: cardiovascular. Therefore, an adequate control and management of the associated risk factors can have a significant repercussion on the morbidity and mortality of this group of patients. OBJECTIVE: To characterize the adult patients with type 2 – diabetes mellitus, controlled at the cardiovascular health program of the Outpatient Clinic of the Penco-Lirquén Hospital, during 2011, excluding patients with 65 years old or more. MATERIAL AND METHOD: Retrospective, observational and descriptive study,by checking the last control registered in the chronic control cards of the patients of the cardiovascular health program of the Outpatient Clinic of the Penco-Lirquén Hospital, from January to December, 2011. RESULTS: 68.8 percent of the patients (n=86) were women. The average age was 52.88 years. 58.1 percent had blood pressure in hypertensive levels. The 63.8 percent had glycosylated hemoglobin above 8, and only 26 percent had normal cholesterol levels. DISCUSSION: The most important epidemiologic and clinic aspect of the study was the bad control of the patients, in the particular goals of diabetes mellitus, and in the risk factors associated with the development of cardiovascular diseases, situation that requires an urgent intervention for a better future quality of life.

ABSTRACT

Background: Inflammatory bowel disease (IBD) has a peak incidence between 15 and 25 years of age, thereby affecting women of reproductive age. Fertility rates with inactive IBD are similar to the general population, and drugs currently used, with the exception of methotrexate and thalidomide, have a good safety and efficacy profile during pregnancy. Starting a pregnancy with inactive IBD significantly reduces the potential maternal and fetal complications. Aim: To assess the evolution of pregnancy and the underlying disease in women with IBD. Patients and Methods: Retrospective and prospective study of female patients with IBD controlled in our hospital who became pregnant from January 1994 to February, 2011. Results: We followed 17patients with a total of 19 pregnancies. In two patients the onset of IBD occurred during pregnancy and from the remaining, 11 patients became pregnant during remission of IBD. Most of the patients continued the same treatment during pregnancy and the few flares that occurred were treated satisfactorily. Major complications occurred in three patients, all associated with IBD activity. Fifteen patients had full-term deliveries and the majority of the newborns had normal weight and Apgar score. None had malformations. Conclusions: Pregnancies among patients with an inactive IBD, have a good evolution. A multidisciplinary approach and patient education are invaluable to achieve these good results.

ABSTRACT

Background: The better treatment modalities for bleeding esophageal varices have improved the prognosis of cirrhosis. Aim: To inquire about diagnostic and treatment modalities for esophageal bleeding in Chile. Material and methods: An enquiry about diagnosis and treatment of esophageal bleeding was designed and electronically sent to public and private health institutions that could admit patients and were located in cides with more than 100,000 inhabitants. Results: The enquiry was answered by 31 of 35 public and 17 of 19 private health institutionis that were consulted. Emergency endoscopy was available in 6 of 27 public and in the 16 private institutionis that had an emergency room. Rubber band ligation was available in 16 public (52 percent) and in all private institutions. Cyanoacrylate injections were done in 10 public (32 percent) and 11 (65 percent) private institutions. No public institution installed transjugular intrahepatic portosystemic shunts, but 8 had occasional access to this technique. This procedure was done in 7 (41 percent) private institutions and all had access to it. Surgical treatment was feasible in 20 public (65 percent) and all private institutions. Primary prophylaxis was done in 18 public (58 percent) and 14 private (82 percent) institutions. Secondary prophylaxis was carried out in 26 public (84 percent) and 16 private (94 percent) institutions. Conclusions: Public health institutions have poor access to adequate diagnostic and treatment methods for esophageal bleeding. The primary and secondary prophylaxis of esophageal varices must be improved in both types of institutions.

ABSTRACT

We report a 39 years old woman with Ulcerative Colitis unresponsive to medical treatment. A total colectomy with Brooke ileostomy was performed, as the first operation. After six weeks she developed a peri-ileostomic Pyoderma Gangrenosum. She was treated by a multidisciplinary team, and after seven months with local treatment and systemic medication, the skin healed. Her bowel transit was reconstructed later with an ileoanal pouch and she was reinserted into her previous job (Rev Méd Chile 2004: 132: 747-9).

ABSTRACT

Three endoscopic systematic biopsies were obtained from 261 patients showing chronic gastritis. Histopathologic features of chronic gastritis were graded from 0 to 3 points according to the Sydney System. In addition, an extension and grading histopathologic score was applied. This score was obtained from the sum of qualified grades for each feature in all three samples. Inflammation, activity, atrophy and intestinal metaplasia were predominantly grade 1 and H pylori density was predominantly grade 2. Only 2,6 percent of the sections whith out atrophy showed intestinal metaplasia, while 79,3 percent of the sections depicting grade 2-3 intestinal metaplasia showed moderate to severe atrophy. Inflammation was more severe in antral lesser curvature and the more severe atrophy was present in the antrum than in the corpus mucosa. Sydney System and extension and grading histopathologic score showed more extensive activity in patients older than 45 years. A lower histopathologic score of H pylori was seen in these patients. The presence of H pylori was directly correlated with inflammation severity and inversely with atrophy. These results, in accordance with data shown in the literature, suggest that the Sydney System and extension and grading histopathologic score can be applied to compare chronic gastritis features in different groups of patients

ABSTRACT

Three gastric mucosal biopsies were obtained from 300 patients showing a normal upper digestive tract endoscopy. Histologically in 9 percent of the patients the biopsies were normal; in 87 percent showed a common-type chronic gastritis and in 4 percent showed a reactive (chemical or reflux-type) gastritis. Helicobacter pylori was present in 25.9 percent of the patients without gastritis, in 33.3 percent of the patients with reactive gastritis and in 87.7 percent of those with common-type gastritis. In 19.9 percent of the patients with common-type chronic gastritis there was intestinal metaplasia, consisting of type I metaplasia in 14.1 percent, type II in 3.1 percent and type III metaplasia in 2.3 percent. The association of type III intestinal metaplasia with the other forms of metaplasia, its lower frequency and its tendency to be present in older patients supports the hipothesis that type III incomplete colonic metaplasia represents a more advanced stage than complete and incomplete small bowel metaplasia of the gastric mucosa
