ABSTRACT

As of August 16,2021,there have been 207,173,086 confirmed cases and 4,361,996 deaths due to the coronavirus disease(COVID-19),and the pandemic remains a global challenge.To date,no effective and approved drugs are available for the treatment of COVID-19.Angiotensin-converting enzyme 2(ACE2)plays a crucial role in the invasion into host cells by severe acute respiratory syndrome coronavirus 2(SARS-CoV-2),the etiological agent of COVID-19.Notably,ACE2 density is influenced by medical con-ditions,such as hypertension,or by drugs,including angiotensin-converting enzyme inhibitors(ACEIs)and angiotensin receptor blockers(ARBs),which can change the fate of SARS-CoV-2 infectivity.ACE2 is a target for these drugs and can be manipulated to limit the viral entry and replication within the cells.Different strategies aimed at blocking ACE2 with small molecules,peptides,and antibodies,or by neutralizing the virus through its competitive binding with human recombinant soluble ACE2(hrsACE2)are currently under investigation.In this article,we review the current state of knowledge that em-phasizes the need to find effective therapeutic agents against COVID-19 by exploiting ACE2 as a potential target.The increased soluble ACE2 levels and the application of hrsACE2 in patients with COVID-19 can be implemented to control the disease.It has not yet been established whether hypertension and other comorbidities,independent of age,have a direct role in COVID-19.Therefore,the use of renin-angiotensin system inhibitors,ACEls and ARBs,should not be discontinued during COVID-19 treatment.

ABSTRACT

Background: Psychological stress is an important factor for the development of irritable bowel syndrome (IBS). More and more clinical and experimental evidences are showing that IBS is a combination of irritable bowel and irritable brain. As IBS is a stress sensitive disorder, its treatment should focus on managing stress and stress-induced responses.Methods: This is a hospital based longitudinal study. 72 patients fulfilling the Rome IV criteria for irritable bowel syndrome were enrolled into the study. Perceived stress was assessed using student stress dimension questionnaire (SSDQ) while IBS severity was assessed using IBS severity scoring system (IBS-SSS). The stress interventional module (SIM) consist of specific domain targeted psychological interventions given to the patients weekly based on the stress domain mapping. The effectiveness of these interventions on reducing stress as well as IBS severity was assessed at baseline, and weeks 2, 4 and 6 follow-ups.Results: Of the studied IBS patients, most had SPS in multiple life domains while familial and interpersonal domains were affected in 78%. Also, total stress load in form of mean cumulative stress load at baseline was 451.1 which decreased significantly to 274.4 over 6 weeks. This SIM brought about a reduction in mean irritable bowel syndrome severity score from 27.5 to 20.6 in 6 weeks. This reduction in IBS severity score overtime was significant (p=0.001).Conclusions: Patients of irritable bowel syndrome show significant stress in multiple domains of life, which require proper assessment and management. Authors propose that individual-specific interventions aimed at reducing stress in all psychosocial life domains are efficacious and should be an integral part of managing IBS.

ABSTRACT

The present study was performed to study the antibiotic prophylaxis pattern in patients admitted in Surgery and Obstetrics and Gynaecology wards of a Tertiary Care Teaching hospital in North India. It was an observational, non interventional and analytical study used to evaluate the patients undergoing surgery through period of one and half years. Information concerning demographic data, type of surgery, and parameters of antibiotic therapy (selection of antibiotic, dose, route and duration of therapy as well as cost) was collected from case records of 400 patients. Third generation cephalosporins were the most commonly prescribed class of antibiotics. No consistency was noted in the timing of administration of prophylactic antimicrobials agents which were prescribed 30 minutes to 6 hours before surgery. The dose was not repeated during surgery. Majority of antibiotics were prescribed from Essential Medicines List. Surgical prophylaxis was inappropriate in terms of choice of antimicrobial agent, timing of administration as well as the total duration of prescription, in majority of the cases. Interventions are warranted to promote the development, dissemination and adoption of evidence based guidelines for antimicrobial prophylaxis.

ABSTRACT

In the present study, the preliminary phytochemical and physicochemical studies including fluorescence and thin layer chromatographic analyses of the leaves of Carallia brachiata (Lour.) Merr. (Rhizophoraceae), an Indian medicinal plant used traditionally in the treatment of a variety of human disorders, were carried out with an objective to standardize the plant/plant part (leaves) to be used either as crude drug or in the form of herbal medicine, considering the quality and safety requirements of herbal drugs as per pharmacopoeial standards and/or WHO guidelines.

ABSTRACT

Cassia tora Linn. (Caesalpinaceae) is a semi-wild annual herb grown widely in different places of south-east Asia including India, Northern Australia and Americas. This plant species is well known for having potential in traditional medicine practices for the treatment of a variety of disorders and ailments ranging from simple cough, hypertension to diabetes. Recent scientific investigation reveals its phytochemical as well as biological potential. C. tora has been proven to be medicinally effective for having antimicrobial, antiantioxidant, antihypertensive, antidiabetic and antimutagenic activities, just to name a few. This paper encompasses a comprehensive review on phytochemical and biological aspects of Cassia tora L.
