ABSTRACT

La problemática del ruido en las ciudades toma preponderancia internacional a finales del siglo XX, y pocas veces se comentan los esfuerzos pioneros realizados por especialistas que abogaron por un ambiente sonoro saludable. Éste es el caso de Pedro Belou, un médico otorrinolaringólogo nacido en Uruguay quien realizó toda su carrera profesional en Argentina, y que se destacó como científico de renombre mundial. A Belou podríamos ubicarlo como uno de los pioneros en el Río de la Plata en advertir acerca de las enfermedades que genera en las personas la exposición al ruido en las ciudades y en las industrias durante una conferencia que dio en Buenos Aires en 1936. En este artículo se rescatan los contenidos de su ponencia por la actualidad que tienen sus argumentos, y también para contribuir a su biografía, ya que esta conferencia no se menciona en ninguna de sus biografías.


Summary: City noise became a significant issue at the global level towards the end of the 20th Century, and discussions on pioneer efforts conducted by specialists who advocated for a healthy sound environment are rare. This is the case of Pedro Belou, a Uruguayan born otolaryngologist who developed his professional career in Argentina and was a renowned scientist worldwide. Belou could be regarded as one of the first physicians in the River Plate to warn us about diseases arising from noise exposure in cities and factories, during a conference he delivered in Buenos Aires in 1936. This article brings his presentation to you given the current relevance of his argument and to contribute to his biography, since this conference has not been mentioned in any of his previous biographies.


O problema do ruído nas cidades ganha preponderância internacional no final do século XX, porém raramente se discute o pioneirismo de especialistas que defendiam um ambiente sonoro saudável, como é o caso de Pedro Belou; otorrinolaringologista nascido no Uruguai, passou toda a sua carreira profissional na Argentina, destacando-se como um cientista de renome mundial. Poderíamos colocar Belou como um dos pioneiros no Río de la Plata em alertar, em uma conferência que deu em Buenos Aires em 1936, sobre as doenças que afetam as pessoas devido à exposição ao ruído nas cidades e nas indústrias. Neste artigo o conteúdo de sua apresentação é resgatado devido não somente pela atualidade de seus argumentos, como também para contribuir com sua biografia, já que esta conferência não é mencionada em nenhuma delas.

RESUMEN Antecedentes: la inteligencia emocional (IE) es la capacidad de reconocer nuestros propios sentimientos y los de los demás, de motivarnos y de manejar adecuadamente las relaciones interpersonales. La IE se ha relacionado con muchas competencias no técnicas necesarias entre los médicos en formación. Objetivo: describir las características psicométricas de la IE en médicos residentes de Cirugía General de la Argentina y analizar los efectos que ejercen sobre ella la edad, el sexo, el ámbito de desempeño y el año de residencia. Material y métodos: estudio prospectivo, analítico de corte transversal; se utilizó la encuesta anónima y autogestionada Trait Emotional Intelligence Questionnaire Short Form (TEIQue-SF v1.5) respondida en línea durante el mes de marzo de 2020. Se aplicó la prueba de Kolmogorov-Smirnov para variables cuantitativas, pruebas T y la prueba ANOVA (IC 95%, p estadística α 0,05). Resultados: fueron respondidas 156 encuestas, de las cuales 105 (67,3%) corresponden a mujeres y 51 (32,7%) a hombres. Edad promedio: 29,02 ± 3,69 años. El promedio global de la prueba fue de 4,58 ± 0,89. El análisis ANOVA demostró que existen diferencias estadísticamente significativas de la dimensión bienestar entre los diferentes años de residencia (p = 0,002) así como en puntajes globales de IE (p = 0,0001). Conclusión: la IE es un modelo atractivo y eficaz para definir y capacitar a los futuros cirujanos generales en competencias no técnicas. Estos hallazgos son importantes para generar nuevas propuestas de formación.


ABSTRACT Background: Emotional intelligence (EI) is the ability to recognize our own feelings and those of others, to motivate us and properly manage relationships. EI encompasses many non-technical skills that are important for physicians in training. Objective: The aim of this study was to describe the psychometric characteristics of EI among residents in general surgery in Argentina and to analyze the effects of age, sex, scope of practice and postgraduate year level on it. Material and methods: We conducted a prospective, cross-sectional analytic study using the Trait Emotional Intelligence Questionnaire Short Form (TEIQue-SF v1.5), a self-report and anonymous survey that was answered on-line during March 2020. The statistical analysis was performed by using the Kolmogorov-Smirnov test for quantitative variables, Student's t-test and ANOVA (95% CI, p statistic α 0.05). Results: The survey was responded by 156 physicians; 105 (67.3%) were women and 51 (32.7%) were men (mean age: 29.02 ± 3.69 years). Mean global score was 4.58 ± 0.89. ANOVA showed statistically significant differences in the well-being dimension and global scores of EI across the different postgraduate year levels (p = 0.002 and p = 0.0001, respectively). Conclusion: EI is an attractive and effective model for defining and training future general surgeons in non-technical skills. These findings are important for generating new proposals for training.

RESUMEN Objetivo: Determinar la relación existente entre el dolor osteoarticular y la depresión en adultos mayores atendidos en un hospital geriátrico de Lima-Perú. Materiales y métodos: Estudio de diseño observacional y descriptivo. Se seleccionó una muestra de 74 adultos mayores que fueron atendidos en consulta externa de un hospital geriátrico de Lima-Perú, en el 2021. Se utilizó como técnica la encuesta y como instrumento, el cuestionario. Mediante el paquete estadístico SPSS se realizó la prueba de chi cuadrado, con un nivel de significancia del 5 %. Resultados: Acerca de las características generales, el 51,35 % de los adultos mayores fueron mayores de 70 años, el 55,41 % fueron mujeres, el 60,81 % tuvo instrucción secundaria y el 86,49 % eran casados o convivientes. Respecto a las características del dolor, el 32,43 % manifestó que el dolor disminuía cuando estaba acompañado, el 20,27 % señaló que el dolor es más intenso en casa, el 81,08 % tuvo un dolor moderado y el 64,86 % experimentó dolor mayor a 6 meses. La frecuencia de la depresión en su mayoría fue leve, con el 43,24 %, seguido de la depresión establecida, con el 29,73 %. Las características del dolor osteoarticular asociados a la depresión fue el grado del dolor (p = 0,041) y el tiempo de dolor (p = 0,024). Conclusiones: El dolor osteoarticular se asocia de manera significativa con la depresión en adultos mayores atendidos en un hospital geriátrico de Lima-Perú.


ABSTRACT Objective: To determine the relationship between osteoarticular pain and depression in older adults treated in a geriatric hospital in Lima, Peru. Materials and methods: An observational and descriptive study. A sample of 74 older adults treated in the outpatient department of a geriatric hospital in Lima, Peru, in 2021 was selected. A survey was used as technique and a questionnaire as instrument. Using the IBM SPSS Statistics statistical software, a chi-square test was performed with a significance level of 5 %. Results: Concerning the older adults' general characteristics, 51.35 % were over 70 years of age, 55.41 % were women, 60.81 % had secondary education and 86.49 % were married or cohabiting. Regarding the characteristics of pain, 32.43 % stated that the pain decreased when they were not alone, 20.27 % pointed out that the pain was more intense at home, 81.08 % had moderate pain and 64.86 % had experienced pain for more than six months. As for depression, 43.24 % dealt with mild depression and 29.73 % had persistent depression. The characteristics of osteoarticular pain associated with depression were the degree of pain (p = 0.041) and duration of pain (p = 0.024). Conclusions: Osteoarticular pain was significantly associated with depression in older adults treated in a geriatric hospital in Lima, Peru.

RESUMEN Objetivo. Estimar la incidencia, mortalidad y supervivencia a cinco años por carcinoma endometrial en Manizales, para el periodo 2003-2017. Materiales y métodos. Estudio observacional, retrospectivo, de base poblacional, con alcance descriptivo. Se ajustaron tasas de incidencia y mortalidad mediante el método directo usando la población mundial Segi como referencia. Se realizó seguimiento pasivo y activo de los casos hasta completar 60 meses o hasta la fecha de cierre de seguimiento. La supervivencia fue estimada mediante funciones de Kaplan-Meier y modelos de regresión de Cox. Resultados. Se observaron 210 casos incidentes en una población de 214 928 mujeres. La edad promedio al diagnóstico fue de 61 años. El tipo histológico más frecuente fue el endometrioide, bien diferenciado. La mayoría de las pacientes estaban afiliadas al régimen contributivo y pertenecían al nivel socioeconómico medio. La tasa de incidencia ajustada por edad fue de 5,7 casos nuevos por cada 100 000 mujeres-año. Se identificaron 75 defunciones con mayor mortalidad entre los 64 y los 79 años. La supervivencia global fue de 95,1% a los 12 meses y de 83,8% a los 60 meses. Se encontraron diferencias estadísticamente significativas en la supervivencia a favor de la histología epitelial, los estadios tempranos al momento del diagnóstico y la edad al diagnóstico menor a 60 años. Conclusiones. La mortalidad es similar a la reportada en otros países de la región. En Manizales, la sobrevida al cáncer de endometrio fue mayor en pacientes con diagnóstico temprano, con edad menor de 60 años y con histología endometrioide.


ABSTRACT Objective. To estimate the incidence, mortality and five-year survival of endometrial carcinoma in Manizales for the period 2003-2017. Materials and methods. Observational, retrospective, population-based study, descriptive in scope. Incidence and mortality rates were adjusted by the direct method using the Segi world population as reference. Passive and active follow-up of the cases was carried out until completing 60 months or until the follow-up closing date. Survival was estimated using Kaplan-Meier functions and Cox models. Results. 210 incident cases were observed in a population of 214.928 women. The average age at diagnosis was 61 years. The most frequent histological type was endometrioid, well differentiated. Most of the patients were affiliated to the contributory health insurance scheme and belonged to the middle socioeconomic level. The age-adjusted incidence rate was 5.7 new cases per 100,000 woman-years. Seventy-five deaths were identified, with greater mortality between 65-69 and 75-79 age groups. Overall survival was 95.1% at 12 months and 83.8% at 60 months. Statistically significant differences were found in survival in favor of epithelial histology, early stages at the time of diagnosis, and age at diagnosis less than 60 years. Conclusions. Manizales follows the global pattern of rise in the age-adjusted incidence rate. Mortality is like that reported in other countries in the region. In Manizales, endometrial cancer survival was higher in patients with early diagnosis, less than 60 years of age, and with endometrioid histology.

ABSTRACT

Introducción: La debilidad adquirida en la unidad de cuidados intensivos (DAUCI) compromete tanto a los músculos de las extremidades como el diafragma, asociándose con variables de resultado negativas. El objetivo del estudio fue describir las características epidemiológicas e incidencia de DAUCI en adultos con requerimiento de ventilación mecánica invasiva (VMI) mayor a 72 horas en un Hospital General de Agudos de la Ciudad de Buenos Aires. Método: Estudio de cohorte retrospectivo. Se incluyeron pacientes adultos con requerimiento de VMI por más de 72 horas en la Unidad de Cuidados Intensivos (UCI). Las principales variables de interés registradas fueron: incidencia de DAUCI, días de VMI, estadía y mortalidad tanto en UCI como hospitalaria. Resultados: Se incluyeron 262 pacientes de los cuales 87 (33,21%) intercurrieron con debilidad. No se establecieron diferencias estadísticamente significativas entre los pacientes con y sin DAUCI para las variables edad, motivo de ingreso a VMI, número de antecedentes personales y mortalidad tanto en UCI como hospitalaria. En el presente estudio se establecieron como factores de riesgo independiente al sexo femenino (OR: 1,98; IC 95%: 1,02 ­3,81), delirio (OR 8,4; IC 95%: 4,38-16,11) y días de VMI (OR: 1,05; IC 95%: 1,02-1,08). Conclusiones: El presente estudio nos permitió conocer la incidencia y las características epidemiológicas de los pacientes que presentan DAUCI en una UCI del sistema público de salud de Argentina. Se observó que el sexo femenino, los días de VMI y el delirio en UCI fueron factores de riesgo independiente para DAUCI.

Introduction: Intensive care unit-acquired weakness (ICUAW) affects the muscles of the limbs and diaphragm; and is associated with negative outcome variables. The purpose of this study was to describe the epidemiological characteristics and incidence of ICUAW in adults requiring invasive mechanical ventilation (IMV) for more than 72 hours in a General Hospital for Acute Diseases in the City of Buenos Aires. Method: Retrospective cohort study. We included adult patients requiring IMV for more than 72 hours in the Intensive Care Unit (ICU). The main recorded variables of interest were: incidence of ICUAW, days of IMV, length of stay and mortality both in the ICU and in the hospital. Results: 262 patients were included in the study; 87 of them (33.21%) developed weakness. No statistically significant differences were established between patients with and without ICUAW regarding the variables of age, reason for admission to IMV, medical history and mortality both in the ICU and the hospital. In this study, the variables established as independent risk factors were: female gender (OR: 1,98; 95% CI: 1.02-3.81), delirium (OR 8.4; 95% CI: 4.38-16.11) and days of IMV (OR: 1.05; 95% CI: 1.02-1.08). Conclusions: This study allowed us to know the incidence and epidemiological characteristics of patients with ICUAW in an ICU of the public health system of Argentina. It was observed that female gender, days of IMV and delirium at the ICU were independent risk factors for ICUAW.

ABSTRACT

Panic disorder (PD) pathophysiology is very heterogeneous, and the discrimination of distinct subtypes could be very useful. A subtype based on respiratory symptoms is known to constitute a specific subgroup. However, evidence to support the respiratory subtype (RS) as a distinct subgroup of PD with a well-defined phenotype remains controversial. Studies have focused on characterization of the RS based on symptoms and response to CO2. In this line, we described clinical and biological aspects focused on symptomatology and CO2 challenge tests in PD RS. The main symptoms that characterize RS are dyspnea (shortness of breath) and a choking sensation. Moreover, patients with the RS tended to be more responsive to CO2 challenge tests, which triggered more panic attacks in this subgroup. Future studies should focus on discriminating respiratory-related clusters and exploring psychophysiological and neuroimaging outcomes in order to provide robust evidence to confirm RS as a distinct subtype of PD.

RESUMEN Objetivo: Estimar la incidencia de cáncer de cuello uterino y sobrevida de pacientes con esta enfermedad según subtipo histológico y estadios en Manizales, Colombia para el periodo 2008-2012, y comparar cambios en la sobrevida con el quinquenio anterior. Materiales y métodos: A partir de datos poblacionales, se estandarizaron las tasas de incidencia según la edad para cada subtipo histológico. Se realizó un seguimiento activo y pasivo para determinar el estado vital y la causa de muerte a 60 meses. Se ajustaron funciones de Kaplan-Meier y modelos de Cox para estimar la sobrevida global y según covariables. Resultados: Se presentaron 217 casos nuevos para una incidencia media de 17,8 por 100000 mujeres-año. Hubo mayor frecuencia de estadios III-IV en pacientes de nivel socioeconómico medio y bajo. La sobrevida a cinco años fue de 68,9%; la edad (>70 años) se asoció con menor sobrevida (p<0,001). El riesgo de fallecer fue 90% mayor en pacientes con muestra histológicas no diferenciadas o desconocidas comparado con el carcinoma escamocelular (hazard ratio [HR] 1,9; IC 95%: 1,1-3,3); 1,7 veces mayor para personas mayores de 70 años (HR 2,7; IC 95%: 1,6-4,8); y tres veces mayor para estadio III (HR 4,3; IC 95%: 1,8-10,2) y siete veces mayor para estadio IV (HR 8,7; IC 95%: 3,6-20,1) en comparación con el estadio I. Conclusión: La incidencia de cáncer del cuello uterino en Manizales es similar a la global y a la continental, y fue más baja que otras ciudades colombianas con características similares. Hubo mayor frecuencia de estadios avanzados en mujeres de bajo nivel socioeconómico. La sobrevida se asoció con la edad, el subtipo histológico y la estadificación.


ABSTRACT Objective: To estimate the cervical cancer incidence and survival rates by histological subtype and stage in Manizales, Colombia during 2008-2012; and to compare the survival rate to the one from the previous five years. Materials and methods: Using population-based data, incidence rates by age were standardized for each histological subtype. Active and passive follow-up was performed to determine vital status and cause of death at 60 months. The Kaplan-Meier method and Cox models were adjusted to estimate overall survival by each covariant. Results: A total of 217 new cases were observed; with a mean incidence of 17.8 per 100,000 woman-years. Stages III-IV were more frequently observed in patients of medium and low socioeconomic status. At 5 years, the survival rate was 68.9%; ages over 70 years were associated with lower survival rates (p<0.001). Risk of death was 90% higher in patients with undifferentiated or unknown histological samples, when compared with those with squamous cell carcinoma (HR 1.9, 95% CI: 1.1-3.3). Likewise, the risk of death was 1.7 times higher for patients aged over 70 years (HR 2.7, 95% CI 1.6-4.8); and it was also found to be 3 times higher for stage III (HR 4.3, 95% CI: 1.8-10.2) and 7 times higher for stage IV (HR 8.7, 95% CI: 3.6-20.1), when compared with stage I. Conclusion: The incidence rate of cervical cancer in Manizales was found to be similar to the global and continental rates and lower than those from other Colombian cities with similar characteristics. Advanced stages were more frequent in women of low socioeconomic status. Survival was associated with age, histological subtype and staging.

ABSTRACT

Antecedentes: La apendicitis aguda (AA) es la urgencia quirúrgica más frecuente alrededor del mundo. Su diagnóstico precoz y certero es fundamental para evitar cirugías innecesarias y complicaciones asociadas a su evolución natural. Objetivo: Determinar la sensibilidad y especificidad diagnóstica de la escala RIPASA en pacientes con sospecha clínica de apendicitis aguda. Material y métodos: Estudio analítico, transversal, observacional y retrospectivo; se incluyeron todas las piezas de apendicectomías de pacientes de ambos sexos mayores de 18 años operados con diagnóstico clínico de apendicitis aguda que consultaron entre el 1º de enero y el 31 de diciembre de 2017. Como procedimiento de referencia (gold standard) diagnóstico se analizaron los hallazgos histopatológicos del apéndice extirpado. Se aplicó la escala RIPASA a cada uno de los pacientes (punto de corte de 7,5). Se realizó estadística descriptiva y pruebas diagnósticas (sensibilidad, especificidad, valor predictivo positivo [VPP], valor predictivo negativo [VPN]). Resultados: Se incluyeron 68 apendicectomías. Cincuenta y siete muestras (83,82%) fueron positivas para apendicitis aguda. El promedio de edad fue 43,32 años. De los 38 hombres, 32 (84,21%) tuvieron diagnóstico histopatológico de AA. De las 30 mujeres, 25 (83,33%) tuvieron AA. Para la escala RIPASA, 54 pacientes tuvieron un resultado verdadero positivo (punto de corte ≥7,5 e histología positiva para AA) resultando con una sensibilidad del 93%, especificidad del 40%, VPP del 90%, VPN del 50%, razón de verosimilitud positiva de 1,55 y razón de verosimilitud negativa de 0,175. Conclusión: El puntaje (score) RIPASA ha demostrado una excelente sensibilidad y especificidad para el diagnóstico clínico de AA de manera rápida, simple y no invasiva.


Background: Acute appendicitis (AA) is the most common surgical emergency worldwide. Its correct and early diagnosis is essential to avoid unnecessary surgeries and complications associated with its natural history. Objective: The aim of this study was to determine the diagnostic sensitivity and specificity of the RIPASA score in patients with suspected AA. Material and methods: This analytical, cross-sectional, observational and retrospective study included appendectomy specimens of patients of both sexes > 18 years operated on with clinical diagnosis of AA between January 1 and December 31, 2017. The histopathological examination of the appendectomy specimens was considered the gold standard diagnostic test. A score of 7.5 for the RIPASA score was chosen as cut-off value. Sensitivity, specificity, positive predictive value (PPV) and negative predictive value (NPV) were calculated. Results: A total of 68 appendectomies were included; 57 (83.82%) specimens were positive for acute appendicitis. Mean age was 43.32 years. The histological diagnosis of AA was present in 32 (84.21%) men and in 25 (83.33%) women. For the RIPASA score, 54 patients had a true positive result (cut-off point ≥ 7.5 and positive histology for AA) with a sensitivity of 93%, specificity of 40%, PPV of 90%, NPV of 50%, positive likelihood ratio of 1.55 and negative likelihood ratio of 0.175. Conclusion: The RIPASA score has demonstrated excellent sensitivity and specificity for the clinical diagnosis of AA in a fast, simple and non-invasive fashion.

ABSTRACT

ABSTRACT Wide-field angiography enables assessing peripheral areas with better quality and gives greater deep focus, which improves the image periphery. Some studies have proposed the usefulness of these angiographic systems in inflammatory diseases of the retina. However, few studies have evaluated this technique in Eales disease. We present a case series in which 5 eyes of 3 patients with Eales disease were evaluated by using retinal fluorescein angiography with 30º, 50º, and 150º lenses in a laser-scanning ophthalmoscope. These cases highlight the usefulness of wide-field fluorescein angiography in the diagnosis and follow-up of peripheral ischemic retinal areas in Eales disease, which enables better follow-up than possible with conventional fluorescein angiography images.


RESUMO A angiografia de campo amplo permite avaliar áreas periféricas com melhor qualidade e proporciona maior foco profundo, melhorando a imagem da periferia. Alguns estudos têm proposto a utilidade desses sistemas angiográficos nas doenças inflamatórias da retina. No entanto, poucos estudos avaliaram esta técnica na doença de Eales. Apresentamos uma série de casos em que 5 olhos de 3 pacientes com doença de Eales foram avaliados usando angiografia de fluoresceína da retina com lentes de 30º, 50º e 150º em um oftalmoscópio de varredura a laser. Esses casos destacam a utilidade da angiografia com fluoresceína de campo amplo no diagnóstico e no acompanhamento das áreas isquêmicas periféricas da retina na doença de Eales, permitindo um melhor acompanhamento do o possível com imagens por angiofluoresceinografia convencional.

ABSTRACT

Appendicitis is the inflammation of the vermiform appendix. Acute appendictis is the most common surgical emergency, and the most frequent cause of abdominal pain. It is considered that between 7 to 12% of the world population will develop the disease in some ciurcumstances of their lifes, being more frequent during the second and third decades. The aim of this study was to evaluate the comlications derivated from the delay in the anagement of the patients with this complaint. Usually, the signs and symptoms are frequently typical and the diagnosis quickly made. However, the classic syndrome may not be evident and the correct diagnosis may be obscure, in whom a delay in diagnosis can result in a lifebreatening situation. Inflammation of the appendix usually results from the obstruction of the appendiceal lumen. The diagnosis of appendicitis depends primarily on the history and physical findings, but confirmation of the diagnosis usually requires appropriate laboratory, radiographic or endoscopic studies. Pain is usually the first simptom. vomiting usually develops. Acute appendicitis should always be included in the differential diagnosis of abdominal pain. Low-grade fewer is usual. The most characteristic sign is point tenderness at Mc-Burney's pint, which is located one third of the distance from the anterior-superior iliac spine to the umbilicus.We present a retrospective analysis, descriptive and observational. In our study the Alvarado score was empliye. The complications observed are detailed, and the results obtained are commented

ABSTRACT Plants are considered among the main sources of biologically active chemicals. The species Solidago chilensis Meyen, Asteraceae, is native to the southern parts of South America, where the aerial parts of the plant are commonly used for the treatment of inflammatory conditions. However, the effects of S. chilensis on human cancer cells remain to be elucidated. In this study, we evaluated the antiproliferative effects of the hydroalcoholic and dichloromethane extracts of S. chilensis, as well as their chemical constituents quercitrin and solidagenone against the five human tumor cell lines in vitro. The dichloromethane extract showed a promisor antiproliferative effects in vitro, especially against glioma cell line. Besides, the hydroalcoholic extract and quercitrin were inactive. The diterpene solidagenone showed highly potent antiproliferative effects against breast (MCF-7), kidney (786-0), and prostate cancer (PC-3) cells (total growth inhibition: TGI < 6.25 µg/ml). Solidagenone meets the theoretical physico-chemical criteria for bioavailability of drugs, according to the "Rule of Five" and, by theorical studies, the observed biological effects were probably related to the interaction of the molecule with nuclear receptors and as an enzymatic inhibitor. This study contributes to chemical study and to the identification of antiproliferative molecules in S. chilensis.

Los lipomas retroperitoneales son infrecuentes y suelen representar el 2.9% de los tumores primarios de origen en retroperitoneo1 . Pueden alcanzar gran tamaño y el diagnóstico diferencial debe hacer con el liposarcoma bien diferenciado, aunque es muy difícil de realizar en el preoperatorio2 . Se presenta el caso de un paciente con lipoma retroperitoneal cuyo diagnóstico se realizó por tomografía axial computada (TAC) y que fue extirpado en su totalidad.


Retroperitoneal lipomas are infrequent and can represent a 2.9 % of the primary tumors originated in the retroperitoneum. They can reach a great size and the differential diagnosis must be performed with the well differentiated liposarcoma, even though it is very difficult to establish it during the preoperative time. We describe herein a case of a patient with a retroperitoneal lipoma. The diagnosis was established with a computed axial tomography (CAT) and it was totally surgically removed.

ABSTRACT Celtis iguanaea (Jacq.) Sarg., Cannabaceae, is popularly used in the treatment of diabetes mellitus. However, chemical and pharmacological investigations are lacking. In this study, we investigated the effects of the hydroalcoholic extract from C. iguanaea on markers of cardiovascular diseases and the glucose metabolism in cholesterol-fed rats. Therefore, hypercholesterolemic rats (1% cholesterol) were orally treated with C. iguanaea extract (C-150, CI-300, or CI-600 mg/kg) or simvastatin (4 mg/kg) (n = 6) once a day (30 days) with a hypercholesterolemic diet. A control group (C) was given saline. C. iguanaea extract showed significant decreases in serum levels of total cholesterol, LDL-cholesterol, HMG-CoA-reductase, interleukin-1 and 6, TNF-α and IFN-γ when compared to group C (p < 0.001). Hypoglycemic effects were observed along with a decrease of the activity of sucrase (CI-600), maltase (CI-150, CI-300), and an increase in muscle glycogen levels (CI-300). Antioxidant effects were observed in plasma by the decrease of TBARS and increase of nonprotein thiols levels (CI-600). The histopathological analysis showed a significant decrease in the liver fat area for C. iguanaea extract compared to group C (p < 0.001). Our results suggest that the biological effects of C. iguanaea extract could be related to the flavonoids that possibly exert antioxidant, enzymatic inhibitory, and insulin-mimetic effects.

Abstract Natural products are a major source of drugs for the treatment of cancer. The species Alpinia zerumbet (Pers.) B.L. Burtt & R.M. Sm, Zingiberaceae, is widely distributed in Brazil where it is known as "colônia". The leaves are commonly used in the treatment of hypertension and dyspepsia, however, the effects of A. zerumbet extracts and isolated substances on human cancer cells remain to be elucidated. This study was designed to identify the chemical constituents of hydroalcoholic and dichloromethane extracts from A. zerumbet leaves and to investigate their in vitro antiproliferative activity. The isolated phytochemicals included kaempferol, dihydro-5,6-dehydrokavain, 5,6-dehydrokavain, and pinostrobin. The hydroalcoholic extract inhibited cellular proliferation only at high concentrations, while the dichloromethane extract showed a moderate antiproliferative effect against leukemia and lung tumor cell lines. 5,6-Dehydrokavain showed potent cytostatic activity against glioblastoma cells and a moderate effect on all other tumor cell lines. Pinostrobin showed potent activity against leukemia and breast tumor cell lines and moderate cytostatic effect against ovarian cell. Furthermore, this is the first report on the isolation of kaempferol and pinostrobin from A. zerumbet leaves. Moreover, the purification process described in this study was effective. These results suggest that A. zerumbet leaves are a promising source of anticancer compounds.

RESUMEN Los tumores quísticos primarios de la órbita, causados por infestaciones parasitarias como el Equinococcus granulosus son raros. Al ser el Perú zona endémica para este parásito, presentamos el caso de una niña de 10 años de edad, proveniente de una zona rural de Huancavelica, con proptosis axial irreductible del globo ocular derecho, de curso progresivo, con pérdida de la agudeza visual e imágenes tomográficas de la órbita en relación a masa tumoral de apariencia quística. Los exámenes serológicos fueron negativos, la paciente fue sometida a resección quirúrgica con resultado histopatológico de membranas anhistas correspondientes a quiste hidatídico. Al año de seguimiento no se evidencia recurrencia local ni sistémica. En conclusión, ante una tumoración orbitaria quística en pacientes pediátricos, se debe de considerar como diagnóstico diferencial la hidatidosis orbitaria, sobre todo si proceden de zonas endémicas.


ABSTRACT Primary orbital cystic tumors caused by Echinococcus granulosus are rare. As this parasite is endemic in Peru, we report the case of a 10-year-old girl from a rural area of Huancavelica, who presented with progressive and irreducible axial proptosis of the right eye and loss of vision. Orbital tomographic images demonstrated a tumor mass with a cystic appearance. Serologic tests were negative. The patient underwent surgical resection, and histopathologic analysis revealed anhistic membranes, corresponding to hydatid cysts. During one year of follow-up, no evidence of local or systemic recurrence was observed. In conclusion, orbital hydatidosis should be considered in the differential diagnosis of a cystic orbital tumor in pediatric patients, especially if the patient is from an endemic area.
